An individual method cannot achieve the optimum risk-assessment result in the worksites, and future perspectives should focus on the parallel application of a deterministic approach with a stochastic approach. In particular, the risk analysis and assessment techniques of the deterministic (DET) approach are classified into three main categories: (a) the qualitative, (b) the quantitative, and (c) the hybrid techniques (qualitative-quantitative, semi-quantitative). Furthermore, the stochastic (STO) approach includes the classic statistical approach (CSA) and the accident forecasting modeling (AFM). The objective of this paper is triple: (a) the presentation and classification of the main risk analysis and risk assessment methods and techniques of the deterministic approach and the stochastic approach as well, (b) the development and presentation of a new alternative risk assessment framework (called as STODET) including a stochastic and a deterministic process, and (c) the application of STODET in the Greek Public Power Corporation (PPC) by using occupational accidents that have been recorded, during the 17-year period of 1993-2009. In particular, the STODET application proves that required actions (or suppressive measures) are essential and must be taken in a medium-term period (1 working year) for abolishing the hazard sources.  相似文献

Behavior and lifestyle characteristics of male Kuwaiti drivers   总被引:1,自引:0,他引:1  
Introduction: The high traffic accident risk among young drivers is a well-known and well-documented fact in most countries. Lifestyle has proven to affect driving behavior as well as accident risk. This study covers the lifestyle component of the problems related to young male Kuwaiti drivers’ accident risk. Methods: The purpose of the study is to measure the relationship between lifestyle and accident risk. Lifestyle is measured through a questionnaire, where 302 male Kuwaiti drivers (mean age = 28 years; range 25-35 years) answer 39 questions related to behavioral and social factors, road conditions, police enforcement, and life satisfaction. They also report their involvement in accidents and traffic violations. Results: The questionnaire's validity and reliability (Cronbach's alpha = 0.7) were achieved. Principal component analysis reduced the 39 items on the questionnaire to 5 factors. Inadequate police enforcement is strongly correlated (r = 0.862) to accident risk and traffic violations and is thus considered the best predictor of traffic accidents in Kuwait. Impact on Industry: As driving-related incidents (on-the-job and off-the-job) are a significant source of fatalities and lost-work-days, the study points to the importance of considering cultural factors in the design of comprehensive safety programs for industry.  相似文献

This paper concerns a procedure to quantify the risk of occupational accidents based on Fuzzy Logic approach, named Fuzzy Application Procedure (FAP). FAP was developed as a support tool, after the method based on fuzzy logic was developed and validated. The methodology and FAP support tool were found to be able to quantitatively assess the risk of occupational accidents for different industrial and site activities and to identify the most efficient intervention measures that can be taken to reduce risks. The application of FAP to two Italian industrial plant allowed the validation of the procedure. It can be seen how FAP should be considered easy to use for any type of company, with the only requirement of having a sufficient and homogeneous number of accidents recorded, so as to be able to correctly tune the reference parameters of the fuzzy methodology.  相似文献

Although it has been estimated that as many as 80% of all occupational accidents have human errors as a cause, no risk assessment tools incorporating human-related elements have been developed for small companies. Human error probability (HEP) and human error analysis (HEA) have been used for large-scale, safety-critical industries for last three decades, but these tools are not suitable for smaller, more general industries that comprise the majority of accident settings.Here, we describe and verify a risk assessment tool that includes human-related elements for small companies. The tool expands on traditional risk assessment methods, such as matrix, risk graph and numerical scoring method, by adding human-related elements. The tool is easy-to-use in occupational environments, and includes assessments of human behavior and potentially outdated machinery at work place.  相似文献

A novel method for dynamic risk assessment of oil spill accidents based on numerical simulation was presented in this paper. The dynamic risk assessment model was developed consisting of a comprehensive list of caused consequences like environmental damage, asset loss, health impact and social effect as well as emergency actions preventing these losses. Contributing events in the Mater Logic Diagram (MLD) of the dynamic risk assessment model were valued based on the simulated evolvement of oil contaminants under spill scenario on quasi-static fluid, which was obtained by coupling an oil spill model with hydrodynamic module in Jialing river of Three Gorges Reservoir in China after the impoundment of the reservoir to 175 m water level in 2010. Calculated result of dynamic risk as grade IV indicated that the assessed oil spill was not as catastrophic as we thought because of the slow transport of oil fractions on water surface and absence of dispersed oil in water body due to the quasi-static fluid, very gentle wind and effective emergency actions, as well as by the reason that rare agriculture or industrial crops exist in spill adjacent area.  相似文献

Several major accidents caused by metal dusts were recorded in the past few years. For instance, in 2011, three accidents caused by iron dust killed five workers at the Hoeganaes Corp. facility in Gallatin, Tennessee (USA). In order to prevent such accidents, a dynamic approach to risk management was defined in this study. The method is able to take into account new risk notions and early warnings and to systematically update the related risk. It may be applied not only in the design phase of a system, but also throughout the system lifetime as a support to a more precise and robust decision making process. The synergy of two specific techniques for hazard identification and risk assessment was obtained: the Dynamic Procedure for Atypical Scenarios Identification (DyPASI) and the Dynamic Risk Assessment (DRA) methods. To demonstrate its effectiveness, this approach was applied to the analysis of Gallatin metal dust accidents. The application allowed collecting a number of risk notions related to the plant, equipment and materials used. The analysis of risk notions by means of this dynamic approach could have led to enhanced hazard identification and dynamic real-time risk assessment. However, the approach described is effective only if associated to a proper safety culture, in order to produce an appropriate and robust decision making response to emerging risk issues.  相似文献

The objective of this paper is to investigate causes and incidence rates of 115 fatalities between 2000 and 2010 in Turkish shipyards and to propose mitigation measures. The fatality rate in Turkish shipbuilding industry is unacceptably high, being 3.5 times the average of all industry groups. Classification of fatal occupational accidents revealed five major fatality reasons for the shipbuilding industry; falling from higher elevation to a lower level, exposed to electric shock, fire and/or explosion, being struck by or struck against objects, and caught in between. The highest number of cases of fatal accidents was found among welders, blasters, painters and substructure workers. Statistically analyzed data showed that working on Mondays and Saturdays have special impact on the fatality probability. Also, the number of fatalities is highest in working temperatures above average 25 °C, which covers from June through September. Strategies recommended to minimize the fatalities at shipyards are discussed. Furthermore a risk framework of fatal occupational accidents were drawn so that when working conditions are within the high risk region, extra precautions can be taken in order to minimize the occurrence of accidents and fatalities.  相似文献

Offshore oil production is one of the most important human productive activities. There are many risks associated with the process of constructing a subsea well, pumping oil to the platform, and transporting it to refineries via underwater pipes or oil tankers. All actions performed by workers in those operations are influenced by specific working conditions, involving the use of complex systems. Contextual factors such as high noise, low and high temperatures and hazardous chemicals are considered to be contributors to unsafe human actions in accident analysis and also give a basis for assessing human factors in safety analysis. Some failure modes are particularly dangerous and can result in severe accidents and damage to humans, the environment and material assets. Fires and explosions on oil rigs are some of the most devastating types of offshore accidents and can result in long-term consequences. The most typical root causes related to accidents include equipment failure, human error, environmental factors, work organization, training and, communication, among others. The principal objective of this study is to propose a methodological framework to identify the factors that affect the performance of operators of an offshore unit for oil processing and treatment. In this phase, an ergonomics approach based on operators' work analysis is used as a supporting tool. After identification of factors that affect the performance of operators, a decision-making model based on AHP (analytic hierarchy process) is applied to rank and weight the principal performance shaping factors (PSFs) that influence safe operations. The next step involves the use of the SHELLO model to group the main PSFs in elements named software, hardware, environment, liveware and organization. In the last phase, a relevant accident that occurred aboard a floating production storage and offloading (FPSO) vessel is analyzed. The allocation process of the factors that affect the operator's performance in risk assessment was developed through fuzzy logic and the ISO 17776 standard.  相似文献

In studies of occupational risks, severity, which is a component of the estimation of every risk, appears as a multifaceted entity assessable according to numerous criteria. A method of measuring the degree of severity of the consequences of potentially dangerous events would be of undeniable value to organisations seeking to improve their understanding of the complexity of such events. The need to control severity is highlighted by scientifically acquired improvements in the understanding of occupational risks, by certain new regulatory obligations in Europe, and by some requirements in the financial management of organisations. We put forward a statistical way of integrating several constituent elements of severity and hence of determining a relevant, synthetic, one-dimensional index. This is achieved by means of principal component analysis (PCA), which is used here to calculate a resultant severity, as in some physical measurements. We also investigate how severity may be statistically modelled, with the aim of contributing to the quantitative assessment of occupational risks. The choice of parametric models is detailed and illustrated by the search for a suitable model for workplace accidents in an organisational setting. The practical value of modelling severity is two-fold. First, one is able to study the distribution of the numerical values of severity over a continuum (a theoretically infinite numerical set) rather than through a limited number of arbitrarily defined categories. Second, with a generally applicable parametric model, one can estimate the law of probability of a measurement of severity in a particular situation, notably recent or new. Lastly, the statistical concept of risk curve is defined and discussed. The goal is to incorporate the severity component into the risk assessment in the form of a probability law, thus circumventing the difficulties associated with an analysis of scenarios.  相似文献

Hazard identification and risk assessment are key aspects in process plant design. They are often applied in the final stages of the process at whatever the cost, unless financial constraints are imposed. However, a much better solution would be to introduce risk analysis earlier by including it in earlier stages of the design process, such as when the cost of a plant and the cost of any accidents that may occur are estimated. In this paper, an optimization methodology is proposed, in which both cost and risk (with a deterministic approach) are taken into account, to improve on the current situation. If a decision variable is chosen, an objective function will be established that makes it possible to analyze variations in overall costs, including the cost of the investment and the cost of accidents. This leads to an optimum situation in which costs are kept to a minimum. Of course, this optimization is subject to constraints, the greatest of which is the fact that risk must not exceed tolerated threshold levels. The procedure is explained and two examples, one involving a toxic release and the other a BLEVE/fireball, are used to illustrate it.  相似文献

The paper focuses on risk sources under no legislative pressure in the field of prevention of major accidents. Despite this, they can represent significant sources of risk of accidents.The aim of the paper is to present the results of the risk assessment associated with the operation of enterprises not regulated by the SEVESO III Directive (the so-called subliminal enterprises), to provide information on possible operational problems and to verify the applicability of recognized risk analysis methods for these specific sources of risk. Last but not least, its purpose is to point out that subliminal enterprises, due to their location close to residential areas or areas with a high concentration of population, pose a serious risk to the population.The paper summarizes the results of the quantitative risk assessment of a specific enterprise not included in the Seveso Directive – a filling station. Filling stations are frequently located in built-up areas with a dense coefficient of habitability. Due to their number, location (e.g. close to residential areas), frequency of occurrence of persons in the area and handling of dangerous substances during normal operation, they can have negative or even tragic consequences to the life and health of the population.Due to the non-existent risk assessment methodology for enterprises with subliminal quantities of dangerous substances and the lack of a systematic search for risk sources, a risk assessment procedure for these companies is designed.  相似文献

Fleet, work-related or occupational road safety has the potential to make a major contribution to reducing both human harm and asset damage. To date, however, there would appear to be a dearth of well researched and evaluated case studies in the public domain in this area.This paper provides an on-going, documented, real-world case study of work-related road safety within an Australian company that has sought to manage, monitor and improve its road safety performance over the last 5 years through a range of proactive, fleet manager, insurance and risk-led initiatives.Roche Australia has successfully implemented a Company Motor Vehicle Safety Program with support from its fleet insurers and risk management advisors. Four key initiatives are set out covering: (1) Driver risk assessment, monitoring and improvement. (2) Policy development and communications. (3) Process and outcomes evaluation. (4) Continuous review and refinement of policies, processes, programs and future developments.The program has led to a range of process and performance-based outcomes. Typical barriers to success in work-related road safety have been identified and overcome. Almost 100% risk assessment and improvement process compliance by drivers has been achieved. A predictive relationship between risk assessment outcomes and collision history has been identified. Reductions in insurance claims, collision costs and claims ratio have been sustained. A detailed project plan is in place to continue the program for the next 3 years.The paper concludes that with effective management, a well developed work-related road safety program can result in performance improvements within the initiating organisation. Several limitations in the case study methodology, as well as areas for further work and lessons for policy makers, researchers and practitioners are also identified.  相似文献

Operating several assets has resulted in more complexity and so occurrence of some major accidents in the refining industries. The process operations risk factors including failure frequency and the consequence components like employees' safety and environment impacts, operation downtime, direct and indirect cost of operations and maintenance, and mean time to repair should be considered in the analysis of these major accidents in any refinery. Considering all of these factors, the risk based maintenance (RBM) as a proper risk assessment methodology minimizes the risk resulting from asset failures. But, one of the main engineering problems in risk modeling of the complex industries like refineries is uncertainty due to the lack of information. This paper proposes a model for the risk of the process operations in the oil and gas refineries. The fuzzy logic system (FLS) was proposed for risk modeling. The merit of using fuzzy model is to overcome the uncertainty of the RBM components. This approach also can be accounted as a benchmark for future failures. A unified risk number would be obtained to show how the criticality of units is. The case study of a gas plant in an oil refinery is performed to illustrate the application of the proposed model and a comparison between the results of both traditional RBM and fuzzy method is made.For the case study, 26 asset failures were identified. The fuzzy risk results show that 3 failures have semi-critical level and other 23 failures are non-critical. In both traditional and fuzzy RBM methods, some condenser failures had the highest risk number and some pumps were prioritized to have the lowest risk level. The unit with unified risk number less than 40 is in the non-critical conditions. Proposed methodology is also applicable to other industries dealing with process operations risks.  相似文献

Problem

This paper aims at examining the occupational accident rate in Spain in the olive oil mill industry. These mills produce olive oil by physical or mechanical, but not chemical procedures. Although Spain is the leading olive oil producer in the world with 1,200,000 tons/year and over 25% of the world olive farming area, the occupational accident rate in this sector has been little studied in the relevant scientific literature.

Method

Our study analyses all occupational accidents which occurred during 2004-2009, with or without sick leave, at 90 Andalusian olive oil mills covered by one of the biggest state insurance companies for accidents at work and occupational diseases (within the Spanish Social Security system).

Results and discussion

Slips or trips are the most frequent cause of occupational accidents in this sector, accounting for 14.86% of the cases. In second position, incorrectly coordinated bodily movements account for almost 13.51% of the accidents. Falls from height, 12.2% of the cases, are also statistically noteworthy. As for the seriousness of the injuries, entrapments are particularly important, since in 2.70% of the cases they result in the traumatic amputation of a part of the body. The analysis of accidents without sick leave reveals that a high number of incidents are caused by contact of the skin and eyes with hazardous substances (20% of the cases) and 10% of them involve superficial injuries.

Impact on the industry

This paper provides a clear and updated image of the accident rate in Spanish olive oil mills and can be a useful tool for the design and adequate adjustment of the management systems implemented in these plants to guarantee a satisfactory level of occupational health and safety by means of more efficient planning and monitoring of measures intended to reduce professional risks and improve working conditions.

Recommendations

Given not only the high incidence of falls on the same level and the potential seriousness of falls from different levels, but also the small, round fruit involved, with a high oil content and a hard round stone, it is extremely important to design and implement safe working procedures and specifically train both operatives and managers in charge of the tidiness, cleanliness and regular control of the premises and operations. The analysis of the accidents not requiring sick leave (mostly cases of contact with hazardous substances on the skin or eyes, or superficial injuries) clearly reveals that the use of suitable personal protective equipment and the relevant training are crucial issues.  相似文献

Land use planning (LUP) around industrial sites at risk of major accidents requires the application of sound approaches in the selection of credible accident scenarios. In fact, the ‘technical’ phase of LUP is based on the identification and assessment of relevant accident scenarios. An improper choice of scenarios may critically affect both the ‘technical’ phase of risk assessment and the following ‘policy’ phase concerning decision making on land-use restrictions and/or licensing. The present study introduces a procedure aimed at the systematic identification of reference accident scenarios to be used in the gathering of technical data on potential major accidents, which is a necessary step for LUP around Seveso sites. Possible accident scenarios are generated by an improved version of the MIMAH methodology (Methodology for the Identification of Major Accident Hazards). The accident scenarios are then assessed for LUP relevance considering severity, frequency and time scale criteria. The influence of prevention and mitigation barriers is also taken into account. Two applications are used to demonstrate the proposed procedure. In both case-studies, the proposed methodology proved successful in producing consistent sets of reference scenarios.  相似文献

The core aim of the study was to gain insight into the cross-country differences in traffic risk perception and driving behaviour and also how culture and cultural differences may influence perceived risk and risk behaviour by comparing a sample of the Russian population with a sample of the Norwegian population. A new measurement instrument aimed at measuring culture as symbol exchange was applied. Self-completion questionnaire surveys were carried out among representative samples of the Norwegian (n = 247) and the Russian (n = 299) population aged 18 years and over. The results showed that culture defined as symbol exchange was weakly associated with risk perception. It is suggested that research carried out to date on the role of culture in risk research may have focused on criterion variables which are not very relevant. However, while traffic safety culture does not seem to be important for risk perception, this study shows that it seems to be relevant for drivers’ risk behaviour and thus it is still relevant and important to focus on traffic culture in risk research despite the fact that culture does not predict perceived risk. As symbol exchange, the newly developed measure of traffic safety culture is capable of predicting drivers’ risk behaviour in traffic and is therefore a valid predictor of traffic safety.  相似文献

Each year more people die from work-related diseases than are killed in industrial accidents. Therefore it is essential to evaluate occupational health aspect during the process design. Early evaluation of safety, health, and environmental (SHE) performance is advantageous, since the opportunities to make the process inherently benign are greater and the cost therefore lower. The methods for occupational health assessments need to be tailored to specific design stages, since the data availability is changing as the design proceeds. In this paper, an index-based method called the Occupational Health Index (OHI) is presented for the basic engineering stage. The OHI is the final of the three methods in series proposed for health assessment in development and design stages. The OHI is based on the information available in piping and instrumentation diagrams (PIDs) and the plot plan. Four health aspects are considered; chronic inhalation risks to noncarcinogens and carcinogens, acute inhalation risk, and dermal/eye risk. The index is demonstrated on separation system of a toluene hydrodealkylation process. The assessment results allow the level of occupational health risks to be evaluated, the sources of exposures be detected, and corrective actions taken in a focused way.  相似文献

The objective of this work is to determine and study, analyze and elaborate, classify and categorize the main risk analysis and risk-assessment methods and techniques by reviewing the scientific literature. The paper consists of two parts: a) the investigation, presentation and elaboration of the main risk-assessment methodologies and b) the statistical analysis, classification, and comparative study of the corresponding scientific papers published by six representative scientific journals of Elsevier B.V. covering the decade 2000-2009. The scientific literature reviewing showed that the risk analysis and assessment techniques are classified into three main categories: (a) the qualitative, (b) the quantitative, and (c) the hybrid techniques (qualitative-quantitative, semi-quantitative). The qualitative techniques are based both on analytical estimation processes, and on the safety managers-engineers ability. According to quantitative techniques, the risk can be considered as a quantity, which can be estimated and expressed by a mathematical relation, under the help of real accidents’ data recorded in a work site. The hybrid techniques, present a great complexity due to their ad hoc character that prevents a wide spreading. The statistical analysis shows that the quantitative methods present the highest relative frequency (65.63%) while the qualitative a lower one (27.68%). Furthermore the hybrid methods remain constantly at a very low level (6.70%) during the entire processing period.  相似文献

区域职业病危害风险是一个地区特定时期内职业病与职业病危害事故发生的可能性与严重程度的综合反映,科学、客观地评价一个区域总体的职业病危害风险状况,对于科学制定有关政策、确定重点监管区域并合理分配职业病危害控制资源等都具有重要意义。在分析区域职业病危害风险影响有关因素并建立量化评定方法的基础上,研究遴选提出了用于区域职业病危害风险综合评价的指标,构建了区域综合职业病危害风险评价模型,确立了区域职业病危害风险分级标准,并给出了区域职业病危害风险管理的初步建议。  相似文献
