The growth of legal regulations in occupational safety during the 70's alienated to some extent top management from health and safety issues; safety was regarded as an activity of safety professionals and labour inspection. This lack of commitment to safety management led to reduced activity and awareness. However, both research and practice indicate that the role of top management is of crucial importance for achieving results in safety.Having realized the problem and the lack of appropriate training to meet the demands of top management, a program called “One hour for safety management” has been developed. The training is arranged in an “action” setting, during the formal meeting of the executive board of the company. The program consists of four main parts: (1) Principles of modern safety management, (2) The company's accident insurance system, the interdependency between compensation costs and premiums, (3) The safety situation of the company; accidents, premiums and losses, hazards, safety program, and (4) Setting the safety targets and action plans required. More recently, a short audit process has been developed to reveal the important sources of loss and the state of affairs in the company's loss control systems. A special software package includes essential handouts of the safety management meetings complemented by self-auditing tools.The approach initiated or revived top management interest in and awareness of safety in several companies. The success of the meeting seems to depend on several factors, e.g. consultation skills, safety expertise, style of presentation, corporate culture and co-operation in the preparation of the session. The factors that contribute to an effective training for top management should further be analyzed, and the impact of the management training on accident rates is a task for evaluation studies in future.  相似文献

The effectiveness of various forms of caution signs was evaluated using saccadic eye movements and reaction time as the dependent variables. Both dependent measures show that caution signs requiring specific positive actions are processed more slowly and less efficiently than signs requiring avoidance behaviors. Additionally, the data show that changing the order of the “consequences” and “instructions” elements of the caution signs did not influence the number of saccades, the speed, or accuracy of interpretation. The research shows that saccadic eye movement may be an effective method for evaluating the design of caution signs.  相似文献

This research examined whether trainee demographics and pre‐training competence predicted participation in voluntary diversity training. Results indicate that demographic variables had no impact on interest in training (Study One) or on actual training participation (Study Two). However, pre‐training competence levels had a positive effect on both outcomes. More competent trainees expressed more interest in additional training (Study One) and were more likely to attend a voluntary training session (Study Two). The authors suggest that trainees with low competence in the diversity domain are unaware of their low competence levels and therefore are not motivated to participate in training programs designed to increase diversity competence. Training research is often criticized for being atheoretical. Further, the research has typically ignored the role of the trainer. We present a model that applies attribution theory to the training process with a focus on the attributions that trainers make for trainee behavior and their implications for training effectiveness. The impact of trainees own attributions is also discussed. The model indicates how characteristics of both trainers and trainees can influence the attributional processes. We know that the systems which are trusted by the users are more often used, especially in a risky situation where they need to delegate control, but we still ignore largely the factors which improve trust in the systems. Our issue here was to explore whether the way we present the system to the users will have an effect on their confidence in it.In this study, we had nine subjects using for the first time a cruise control system on open road; before, we present the system to them in three different ways: (i) a function-oriented written presentation (G1), (ii) a use-oriented written presentation, “augmented” with difficult situations (G2) and (iii) a use-oriented film presentation (3). They evaluate their trust in the system on scales before the whole experiment, after the presentation and after the real use. At the end, they also have self-confrontation interviews, where they see the video of their driving and describe their activity, strategies and feelings. We then develop quantitative and qualitative analysis of trust, linked with specific situations of action.Our results indicate that the presentation of instructions lowers the evaluation of trust (and of efficiency) that conductors have a priori; they had constructed an a priori representation of a CCS that is rather idealistic, and realised, after reading the instructions, and above all after having watched a film, that its use is not so obvious as they had previously thought There is thenceforth a drop in trust that nevertheless goes up again after use of the system during driving We remark, from qualitative analyses of use experience of the regulator in real driving conditions, that this drop in trust in the system does not inhibit subjects in their use, and in particular, for subjects who have watched a film of projection into use They know more of the functions of the system in driving conditions, they produce less distorted reconstruction of the functioning, and they have a deeper level of understanding of the system.  相似文献

In many situations, and particularly in legal matters, decisions may be based on whether some possibility is judged to be “more probable than not”, as for example, whether a warning, had it been present, would “more probably than not” have prevented an injury. The probability of a warning accomplishing its purpose of preventing injury can, in principle, be computed based on probability calculus. However, this paper proposes that: (1) such computations must use “fuzzy” probability statements which as a result produce a range of weighted possibilities for the final outcome and, moreover; (2) the critical statistic for determining whether the outcome is “more probable than not” is the median rather than the mean of the possible outcomes. This paper briefly outlines the mathematics for such computations and provides several examples. The paper basically treats fuzzy probabilities as standard “random variables” restricted to the range of [0,1].For brevity, this paper only treats the multiplication of two or more fuzzy probabilities. However, this is adequate for evaluation of a two stage “read and heed”, or of longer multiple step linear sequences and is indicative of the general approach. This paper describes two studies conducted to test selected warning variables for their influence on the use of personal protective equipment. Both of the studies were field demonstrations; one conducted in a recreational setting and the other as part of a consumer product scenario. Each of the studies involved manipulations that could be practically implemented in many warning situations. Both studies showed that cost strongly influenced the use of personal protective equipment. “Cost” was shown to include parameters of time and difficulty with obtaining the equipment. The results indicate that a small increase in cost can devastate equipment use rates. Additional factors that were shown to positively influence equipment use under at least some circumstances include: (1) the addition of “specific consequence” information, including accident frequency and severity data, to standard warning signs/labels; and (2) product labels requiring physical manipulation by the consumer for product use.  相似文献

Thought self-leadership involves employee self-influence through cognitive strategies that focus on individual self-dialogue, mental imagery, beliefs and assumptions, and thought patterns. A training intervention-based field study with a control group was undertaken to empirically examine the applicability of thought self-leadership in an organizational setting (of bankruptcy financial status), and the potential for cognitions to be self-controlled. Results suggested that individuals who received the thought self-leadership training experienced increased mental performance, positive affect (enthusiasm), job satisfaction, and decreased negative affect (nervousness) relative to those not receiving the training. Additionally, the trainees reported a strong and positive reaction to the training. Finally, those who received the training experienced enhanced perceptions of self-efficacy and more optimistic perceptions of the organization's bankruptcy condition than those not receiving the training.  相似文献

Introducing safety devices does not always reduce the frequency of accidents. Operators adjust their response to technological improvements. Protection mechanisms may be used to support “unsafe” working practices so that the net risk of failure remains unchanged or may even rise. Regulatory bodies have reacted against this risk compensation. Rules and procedures have been imposed upon the day-to-day operation of protection equipment. Unfortunately, these constraints often fail to preserve the safety of an application. Inattention, fatigue, poor training and willful neglect can lead to rules and guidelines being ignored. In this paper alternative means of reducing threats posed by risk compensation are explored. In particular, it is argued that designers must exploit an integrated approach to the development of high-risk applications. By this we mean that both human factors and systems engineering must be recruited to support the development of protection equipment.  相似文献

The risk estimation presumably is the most crucial part of the entire procedure of assessing hazards/unsafe situations in the work, and especially in the industries’ and constructions’ worksites, where the working conditions are unstable. We can consider the risk as a quantity, which can be estimated and expressed by a mathematical relation, under the help of real accidents’ data. The aim of this work is quadruplicate: (a) the development, elaboration and explanation of two new quantitative risk-assessment techniques, (b) the improvement of specific points of other scientific works, as far as concerns quantitative risk estimation, (c) the application of these techniques on an industrial productive procedure (as a case study) and (d) the comparison of their outcome risk-estimation results. Particularly, we develop and analyze the theoretical background of the two techniques, which we call as “proportional technique” and “decision matrix technique”, and apply them on an aluminum extrusion industry's worksite, which is situated in Greece, by using real data of potential sources of hazards, recorded by safety managers, during the 5.5-year time period of 1999–2004. Comparing the results of the two quantitative risk-assessment techniques, we infer that they are compatible. Therefore, the most important hazard source in the aluminum industry is the “squeezing and hits by dropping objects (transported by derricks)”, and imposes that immediate suppressive measures must be taken place to abolish the danger source.  相似文献

Although the diffusion of its storage and transport under liquefied conditions, nowadays it is common to have methane in gaseous form in several industrial applications. This leads to safety implications to be considered: hazards are linked to both the high-pressure at which the gas is kept and to its flammability. Scenarios where flammable jets impact an obstacle are of paramount importance because of their possible occurrence. Following a numerical approach, literature shows up that their assessment can be reliably performed by means of only Computational Fluid Dynamics tools. However, despite the improvements of computing power, Computational Fluid Dynamics costs still limit its use in daily risk analysts’ activities. Therefore, considering an accidental jet-obstacle scenario of industrial interest, the present work investigates how a pipe rack can influence the development of a high-pressure methane jet. Based on a Computational Fluid Dynamics analysis, main achievements of this work are a simple criterion able to identify the situations where the pipe rack does not influence the high-pressure methane jet behavior, therefore allowing to identify the scenarios where simpler models can be used (i.e., analytical correlations known for the free jet situation), and, if present, a simple analytical relationship that roughly predicts the influence of the pipe rack without the need of performing complex Computational Fluid Dynamics simulations.  相似文献

Obtaining knowledge about factors affecting health, safety and environment (HSE) is of major interest to the petroleum industry, but there is currently a severe shortage of relevant studies. The aim of this study was to examine the relative influence of offshore installation (local working environment) and company belonging on employees’ opinions concerning occupational health and safety. We analyzed data from a safety climate survey answered by 4479 Norwegian offshore petroleum employees in 2005 on the dimensions “Safety prioritisation”, “Safety management and involvement”, “Safety versus production”, “Individual motivation”, “System comprehension” and “Competence” using one way analysis of variance (ANOVA), effect size and mixed model. The companies differed significantly for “Safety prioritisation”, “Safety versus production”, “Individual motivation”, “System comprehension” and “Competence”. The local offshore installation explained more of the safety climate than the company they were employed in or worked for did.  相似文献

This paper provides an overview of the concepts of “risk” and “safety-integrity” in relation to safety-related electrical/electronic/programmable electronic systems. The paper is an abridged version of Annex A of the emerging International Electrotechnical Commission (IEC) Standard; “Functional safety of electrical/electronic/programmable electronic systems”. Although based on Annex A, the authors have deviated in a few instances, from the strict wording of Annex A in order to more properly represent their own views. Where this occurs, a note in the text has been added to alert the reader of the deviation. This paper examines the effect of aviation safety education on passenger cabin safety awareness in knowledge, attitude, and behavior (KAB). The educational value of KAB has been addressed in various fields such as AIDS prevention and campus safety. KAB has been applied in aviation safety related areas, especially in training, such as crew resource management training, pilot training, and mechanic training. However, KAB has not been implemented in aviation passenger education in cabin safety. This paper uses the construct of “aviation safety education”, which is intended to influence airline passenger knowledge of, attitude toward, and behavior about cabin safety awareness. Passenger surveys were done at two Taiwan airports. The results show that aviation safety education positively affects airline passenger cabin safety knowledge, attitude, and behavior. We recommend safety education that involves accurate instruction about emergency equipment procedures, situational awareness, emergency responses, and relevant cabin-safety regulations.Our findings indicated that an increase in cabin safety knowledge positively affected airline passenger behavior, which supported knowledge–behavior consistency. We also found that a positive attitude toward cabin safety positively affected airline passenger behavior, which supported attitude–behavior consistency. The hypothesis that cabin safety knowledge positively affected passenger attitude – knowledge–attitude consistency was not supported.  相似文献

Introduction

Research in human error has provided useful tools for designing procedures, training, and intelligent interfaces that trap errors at an early stage. However, this “error prevention” policy may not be entirely successful because human errors will inevitably occur. This requires that the error management process (e.g., detection, diagnosis and correction) must also be supported. Research has focused almost exclusively on error detection; little is known about error recovery, especially in the context of safety critical systems. The aim of this paper is to develop a research framework that integrates error recovery strategies employed by experienced practitioners in handling their own errors.

Method and Results

A control theoretic model of human performance was used to integrate error recovery strategies assembled from reviews of the literature, analyses of near misses from aviation and command & control domains, and observations of abnormal situations training at air traffic control facilities. The method of system dynamics has been used to analyze and compare error recovery strategies in terms of patterns of interaction, system affordances, and types of recovery plans. System dynamics offer a promising basis for studying the nature of error recovery management in the context of team interactions and system characteristics.

Impact on industry

The proposed taxonomy of error recovery strategies can help human factors and safety experts to develop resilient system designs and training solutions for managing human errors in unforeseen situations; it may also help incident investigators to explore why people's actions and assessments were not corrected at the time.  相似文献

Many incident reports and much research literature have concluded that incident rates in high-hazard industries can be reduced by enhancing the knowledge and information sources available to workers. However, a sparsity of published research reporting workers' perspectives on the value of information and knowledge sources in different operational situations potentially limits the effectiveness of interventions aimed at improving human responses to process safety performance. In response, this work seeks to understand how frontline oil and gas industry workers, responsible for managing process safety and other risks in their hazardous workplaces, rely on their own knowledge, the knowledge of others and information provided to manage different operating scenarios. A set of open and closed-ended survey questions were distributed to workers at several upstream and downstream oil and gas companies. Statistical analysis of 133 responses revealed interesting insights relating to participants’ experience levels, decision making practices and perceptions on the importance of various information and knowledge sources in different operating circumstances. The data revealed differences in the sources of knowledge relied upon by experienced and less experienced workers. Experienced workers rely on their own knowledge in both routine and potentially high-impact scenarios. Less experienced workers also rely on their own knowledge for routine decisions however they reported seeking out external knowledge and information in potentially high-impact scenarios. The data suggests that interventions to improve safety by enhancing the availability of knowledge and information sources are less likely to affect experienced workers and alternate strategies might be needed. However, such interventions might be effective if they are focused on supporting newer workers.  相似文献

Mechanical friction and impacts is still today a main cause of ignition of explosive atmospheres (ATEX) in the industry and this trend seems to be stable in time. This situation certainly results from a significant gap of knowledge in the underlying mechanisms so that the parameters to play on are not precisely identified. In this programme of European dimensions, the process of degradation of the mechanical energy into heat during friction and impacts have been studied.

An extensive experimental programme is presented to this end. The mechanisms of dissipation of the mechanical energy into heat during friction has been studied with rubbing machines in which a slider equipped with temperature sensors rubs against a rotating wheel. For impacts, a new device has been developed using a special “air driven cannon” to propel a projectile accurately up to 50 m/s onto an inclined target. A very significant effort has been reserved to the investigation of the ignition mechanisms, not only for ATEX but also for dust accumulations.

Some “simple” modelling is proposed on purpose of practical applications. For frictional situations, a critical rubbing power is calculated without any limitations about any lower boundary concerning the rubbing velocity. For “impacts”, the relevant parameter for ignition is not the kinetic energy of the projectile but its velocity and the nature of the materials.  相似文献

This paper addresses the development of training in team-level decision making to lead to improvements in safety and performance. A one-day Workshop was developed around tactical decision games (TDGs) for use by action teams, i.e. involving a team of multi-disciplinary experts, in the oil and gas drilling industry. Three TDGs were conducted to help team members to rehearse how they would respond to potential challenging situations, focusing in particular on team skills, such as decision making, situation awareness and communication. TDGs have previously been presented as a useful training technique to improve decision making and have been introduced in many high hazard industries [Crichton, M., Flin, R., Rattray, W.A., 2000. Training decision makers – tactical decision games. Journal of Contingencies and Crisis Management 8(4), 208–217]. Building on TDGs for individuals, in a team-based TDG, team members discuss the scenario and reach a team-based solution, allowing team members to recognise the potential differing perspectives brought to the solution by team members from different disciplines. Specific team skills aspects raised during the TDG scenario were then generalised to the wider project, and their impact on overall team performance, safety, and effectiveness. Members of a deepwater exploration wells team (n = 30) attended the Workshop and reported that the use of TDGs in the Workshop was valuable in defining and illustrating team skills and their influence on team performance, and helping to prepare better for their forthcoming operations.  相似文献

The primary objective of this study was to investigate the relationship of work stress, consisting of role stressors and job insecurity, with safety compliance at work. A secondary objective was to test for the possible moderating effect of individual employees’ coping behaviour between experienced work stress and job insecurity, and their safety compliance. A cross-sectional survey design was used (n?=?771). An electronic survey, with a biographical questionnaire and scales on role conflict, role ambiguity, role overload, job insecurity, coping and safety compliance at work, was administered. The results indicated that specific aspects of work stress, notably role conflict, role ambiguity and quantitative job insecurity, and of coping, namely an avoidance style and changing the situation, were important in understanding safety compliance. A moderating effect of avoidance coping was also found.  相似文献

Despite the popularity of diversity training in corporate America, a lack of systematic evaluation has left managers with little guidance on how to design effective diversity training programmes. In this research, we examine how training group composition and trainee experience interact to influence the effects of diversity training on cognitive, affective, and behavioral outcomes. Results indicate that trainees with prior experience with diversity training responded most positively to training groups homogeneous with respect to racioethnicity and nationality; trainees without prior experience with diversity training were generally unaffected by training group composition. The implications of these findings for the design of diversity training programmes in organizations and future research on diversity training are discussed.
