This paper presents a new approach to the casualty treatment problem following a large-scale disaster, based on a mathematical model of how a regional health-care system responds to an earthquake event. The numbers and locations of casualties rescued alive, the scale of pre-hospital care, the post-earthquake hospital capacity, and the transport system are inputs to the model. The model simulates the movement of casualties from the stricken areas to hospitals. It predicts the number of casualties that die as well as other statistics about the health-care system response, such as waiting time before treatment. The model can be run with varying input assumptions to simulate alternative disaster response strategies. Preliminary runs demonstrate the potential of the model as a tool for planning and training.  相似文献

This paper examines the collapse of a five-storey building in Nairobi, Kenya, on 23 January 2006. It draws on reports from local authorities and on debriefings by Israel's Home Front Command (HFC), including information on injury distribution, rescue techniques, and the mode of operation. Most of the 117 people found under the structure were evacuated on the first day to a public hospital, which was overwhelmed by the incident. HFC forces arrived 23 hours after the disaster. At that stage, two people were still buried under the building and special techniques (tunnelling and scalping) were required to secure their evacuation. The two people quickly recovered after a short stay in hospital. Local technology is the preferred option during such events because time is crucial. International cooperation is required when this technology is not available. All of the hospitals in the disaster area, including private facilities, should participate in treating casualties.  相似文献

In recent times Pakistan's biggest city, Karachi, has witnessed numerous terrorist attacks. The city does not have an emergency response system and only one of the three public sector hospitals has a trauma centre. We describe the pattern of injuries and management of two terror-related mass casualty incidents involving suicide bombers in a developing nation with limited resources. The first incident occurred in May 2002 with 36 casualties, of whom 13 (36%) died immediately and 11 (30.5%) died at the primary receiving hospitals. The second incident was targeted against the local population in May 2004. The blast resulted in 104 casualties, of which 14 (13.46%) died at the site. All patients had their initial assessment and treatment based on Advanced Trauma and Life Support principles and documented on a trauma form.  相似文献

Hazard vulnerability analysis (HVA) is used to risk‐stratify potential threats, measure the probability of those threats, and guide disaster preparedness. The primary objective of this project was to analyse the level of disaster preparedness in public hospitals in the Emirate of Abu Dhabi, utilising the HVA tool in collaboration with the Disaster Medicine Section at Harvard Medical School. The secondary objective was to review each facility's disaster plan and make recommendations based on the HVA findings. Based on the review, this article makes eight observations, including on the need for more accurate data; better hazard assessment capabilities; enhanced decontamination capacities; and the development of hospital‐specific emergency management programmes, a hospital incident command system, and a centralised, dedicated regional disaster coordination centre. With this project, HVAs were conducted successfully for the first time in health care facilities in Abu Dhabi. This study thus serves as another successful example of multidisciplinary emergency preparedness processes.  相似文献

From a review of the literature, it can be concluded that it is always advantageous and usually necessary to distribute the casualties of a disaster to several hosp-hospitals rather than allowing 1 hospital, the nearest to the scene, to bear the whole load. The potential of these hospitals would be enhanced if their preparedness and planning included anticipation of their functioning as part of a framework of the entire capacity of the medical services of an entire small country, or a region of a larger one. For the smooth functioning of this framework, central co-ordination and control of the distribution of the load, the movements within it, the strengthening of its various component parts is necessary and services other than medical (transport, communications, etc.) are essential. The establishment of a multihospital system for disaster situations for a whole region is suggested. This would integrate all medical facilities and services of the region, as well as promoting additional services, first horizontally integrated and later vertically, and would have a central governing body with full authority to prepare the system for the calamity and to execute the implementation of the plan when disaster occurs. This body may find it useful to base some of its decision in the planning as well as the implementing stage, on an assumed population of casualties that is characteristic for the region and on implications that may be drawn from the population.  相似文献

地震预警系统建设是降低地震灾害的一个重要途径,可以有效地减少人员伤亡和财产损失,但地震预警系统目前还处于发展阶段,应用中存在一定风险。设计了若干个关于地震预警系统应用的社会调查问卷,以通过调查结果定性地评估地震预警系统产生的社会影响,统计公众对预警系统的认识和需求,分析预警信息发布对公众的影响和公众对地震漏报、误报等风险的接受程度,以及地震预警系统相关的法律问题。调查结果显示,我国公众对地震预警系统的期望程度及有效性评价均很高,但也存在着一些问题,如对地震预警相关知识了解较少,对紧急事件应变能力较差,对预警盲区和漏报等风险接受程度较低等。最后,结合国外地震预警系统的研究成果和应用现状,提出了相关建议及应对措施,希望能对我国预警系统的建设提供参考作用。  相似文献

每年地震都会给社会造成巨大人员伤亡,如果震后能迅速预知地震人员伤亡数量,这对挽救生命损失是十分有意义的。统计了近10年来中国大陆数10次强震、中强震灾害损失,根据影响伤亡的主要因素,用线性回归分析法,建立地震人员伤亡快速评估模型,并对模型进行验证,使之可用于地震预警。  相似文献

Toxicological mass disasters have occurred frequently in past years and constitute a permanent threat in urban areas. From the standpoint of hospital planning, special consideration is required to treat a large number of poisoned casualties in a relatively short period. Several unique medical aspects characterize toxicological mass disasters: casualties present a single disease entity with many "borderline" cases, most medical personnel are unfamiliar with the problem and casualties present a potential contamination hazard to the hospital. A hospital deployment scheme is presented recommending Decontamination, Triage and simple Treatment Algorithms to meet the medical and organizational challenge of such a mass casualty situation. A further specific deployment scheme for treatment of organophosphorus agents poisoning is described to illustrate the principles presented.  相似文献

An evaluation of the socioeconomic consequences of earthquakes is an essential part of the development of risk reduction and disaster management plans. However, these variables are not normally addressed sufficiently after strong earthquakes; researchers and relevant stakeholders focus primarily on the physical damage and casualties. The importance of the socioeconomic consequences of seismic events became clearer in Iran after the Bam earthquake on 26 December 2003, as demonstrated by the formulation and approval of various laws and ordinances. This paper reviews the country's regulatory framework in the light of the socioeconomic aspects of two major and destructive earthquakes: in Manjil–Rudbar in 1990, and in Bam in 2003. The results take the form of recommendations and practical strategies for incorporating the socioeconomic dimensions of earthquakes in disaster risk management planning. The results presented here can be applied in other countries with similar conditions to those of Iran in order to improve public preparedness and risk reduction.  相似文献

以汶川8.0级地震为例,对新《防震减灾法》进行了解读,新《防震减灾法》在旧法的基础上,主要完善了防震减灾规划制度、建设工程抗震设防制度,增加了对学校医院等重要建筑和农村民居、乡村公共设施的抗震设防要求的规定,强化了防震减灾群测群防制度以加强对地震灾害的预防;完善了地震应急救援机制;进一步规范了地震灾后过渡性安置和恢复重建制度;加强了政府在防震减灾工作中的领导和协调作用。同时文中也指出了新《防震减灾法》存在的不足。  相似文献

China's disaster management system contains no law‐based presidential disaster declarations; however, the national leader's instructions (pishi in Chinese) play a similar role to disaster declarations, which increase the intensity of disaster relief. This raises the question of what affects presidential disaster instructions within an authoritarian regime. This research shows that China's disaster politics depend on a crisis threshold system for operation and that the public and social features of disasters are at the core of this system. China's political cycle has no significant impact on disaster politics. A change in the emergency management system has a significant bearing on presidential disaster instructions, reflecting the strong influence of the concept of rule of law and benefiting the sustainable development of the emergency management system. In terms of disaster politics research, unlocking the black box of China's disaster politics and increasing the number of comparative political studies will benefit the development of empirical and theoretical study.  相似文献

地震预报是人们讨论的一个热门话题。当前国际上普遍认同的一个基本事实是 ,地震预报水平远远未达到实用化的阶段。无庸置疑的是 ,作出发布短临预报的决策 ,是防震减灾工作中一项难度最大的工作。它长期困扰着各国地震工作部门和政府行政部门。公开发布地震预报 ,主要考虑因素有 :短临地震预报能力 ,作短临预报需付出的代价 ,以及地震预报公开发布后将会取得的实际收益。对以上三个因素权衡得失 ,趋利避害 ,注重实际效果。如果我们将这三个因素用预报效益来联系 ,那么 ,预报效益则等于预报实际取得的收益与发布地震预报必须付出的代价之比。若比值大于 1,则可公开发布 ;如比值小于 1,则不宜公开发布 ;当比值接近 1时 ,需同时考虑预报意见的信度这个因素。我国不同地区由于其经济发达程度、人口密度、国土财富率相差甚大 ,因此在用上述三个因素作是否公开发布地震预报的抉择上显然其结论是不同的。本文认为 :1 5 5级以下地震 ,东部和西部直接经济损失 (人民币 )约为 8亿元和 2亿元 ,经济上公开发布代价远大于当地所取得的实际收益 ,人员伤亡不超过 10人。因此无论东、西部均不宜公开发布地震预报 ,我们将 5 5级地震定为公开发布预报的震级下限。2 对于 5 5级至 6 5级地震 ,我国东部地区的直接经济损  相似文献

Abstract

This paper examines the relationship between household evacuation decisions and official emergency management practices in light of recent increases in the availability and diversity of hurricane-related information. While we focus on Hurricane Floyd in South Carolina, we incorporate findings of our longitudinal research effort covering the last four years and six post-1995 hurricane threats to the state. While only 64% of residents in the mandatory evacuation zone complied with the Hurricane Floyd evacuation order, over 80% agreed that calling an evacuation was an appropriate precautionary response given the uncertainties of the storm. Longitudinal surveys indicate that Horry County residents have developed a fairly robust strategy in making evacuation decisions. This “hurricane savvy” population depends more heavily on individuals' assessments of risks than on official orders. Individual assessment practices differ from official orders in that greater weight is given to household circumstances and preferences, the diligent monitoring of a variety of information sources, and the incorporation of past experiences into the decision-making process. Surveys indicate differences between the general public and officials in terms of priorities and preferences about hurricane evacuations. The public demands more information about the hurricane threat. Officials place more emphasis on planning evacuation routes and public safety measures.  相似文献

This reflection, based on a keynote address to celebrate the fortieth anniversary of Disasters on 14 September 2017, begins by considering some of the prevailing assumptions that existed at the time of the journal's creation. Next is a summary of significant milestones during the past 40 years, covering major global trends, changing disaster impacts, and key developments in disaster risk management. Contrasting approaches in the first and fourth decade of the journal's history are then followed by examples of changes in terminology in the disaster field as an indication of shifting values. The paper goes on to explore the context of 2017, with observations of problem areas such as the loss of knowledge, failures to tackle the root causes of disasters, shortfalls in international assistance, and some negative attitudes. It closes by evaluating some current positive occurrences, including a reduction in casualties, sharing of experience, technological advances, and dramatic improvements in disaster warning.  相似文献

Epidemiological procedures can be organised under disaster conditions by means of a simple surveillance system and with few personnel.
In the aftermath of the 4 February 1976 earthquake in Guatemala, an information system was organized by which the requisite information for decision-making was obtained with adequate speed and promptness.
The initial epidemiologic informution was based on reports collected during the early days on symptoms observed at hospitals and health centers and in localities and villages in the stricken area.
At a second, post-emergency stage a more elaborate surveillance system was instituted to provide guidance in the investigntwn of outbreaks, evaluate the health activities and establish basic criteria for preventive and control measures.  相似文献
