Introduction: Safe production is a sustainable approach to managing an organization’s operations that considers the interests of both management and workers as salient stakeholders in a productive and safe workplace. A supportive culture enacts values versus only espousing them. These values-in-action are beliefs shared by both management and workers that align what should happen in performing organizational routines to be safe and be productive with what actually is done. However, the operations and safety management literature provides little guidance on which values-in-action are most important to safe production and how they work together to create a supportive culture. Method: The researchers conducted exploratory case studies in 10 manufacturing plants of 9 firms. The researchers compared plant managers’ top-down perspectives on safety in the performance of work and workers’ bottom-up experiences of the safety climate and their rates of injury on the job. Each case study used data collected from interviewing multiple managers, the administration of a climate survey to workers and the examination of the plant’s injury rates over time as reported to its third party health and safety insurer. Results: The researchers found that plants with four values-in-action —a commitment to safety, discipline, prevention and participation—were capable of safe production, while plants without those values were neither safe nor productive. Where culture and climate aligned lower rates of injury were experienced. Discussion and conclusion: The four value-in-actions must all be present and work together in a self-reinforcing manner to engage workers and managers in achieving safe production. Practical application: Managers of both operations and safety functions do impact safety outcomes such as reducing injuries by creating a participatory environment that encourage learning that improves both safety and production routines.  相似文献

INTRODUCTION: Focusing on people and organizations, this paper aims to contribute to offshore safety assessment by proposing a methodology to model causal relationships. METHOD: The methodology is proposed in a general sense that it will be capable of accommodating modeling of multiple risk factors considered in offshore operations and will have the ability to deal with different types of data that may come from different resources. Reason's "Swiss cheese" model is used to form a generic offshore safety assessment framework, and Bayesian Network (BN) is tailored to fit into the framework to construct a causal relationship model. The proposed framework uses a five-level-structure model to address latent failures within the causal sequence of events. The five levels include Root causes level, Trigger events level, Incidents level, Accidents level, and Consequences level. To analyze and model a specified offshore installation safety, a BN model was established following the guideline of the proposed five-level framework. A range of events was specified, and the related prior and conditional probabilities regarding the BN model were assigned based on the inherent characteristics of each event. RESULTS: This paper shows that Reason's "Swiss cheese" model and BN can be jointly used in offshore safety assessment. On the one hand, the five-level conceptual model is enhanced by BNs that are capable of providing graphical demonstration of inter-relationships as well as calculating numerical values of occurrence likelihood for each failure event. Bayesian inference mechanism also makes it possible to monitor how a safety situation changes when information flow travel forwards and backwards within the networks. On the other hand, BN modeling relies heavily on experts' personal experiences and is therefore highly domain specific. IMPACT ON INDUSTRY: "Swiss cheese" model is such a theoretic framework that it is based on solid behavioral theory and therefore can be used to provide industry with a roadmap for BN modeling and implications. A case study of the collision risk between a Floating Production, Storage and Offloading (FPSO) unit and authorized vessels caused by human and organizational factors (HOFs) during operations is used to illustrate an industrial application of the proposed methodology.  相似文献

Airports represent highly complex organisations, incorporating such interdependent operations as airlines, ground transport, flight services, ground services, refueling, maintenance, customer services, catering, administration and security. Airports, and especially International Airports, must ensure that their operations are conducted in a safe and efficient manner, as the consequence of any error or failure during operations has the potential for catastrophic outcomes. The international governing body for air transportation, ICAO, requires that airports must implement a Safety Management System as a means of ensuring safe operations and eliminating or reducing the likelihood of low frequency/high consequence incidents. This research project sought to determine the extent to which the implementation of a Safety Management System (SMS) influenced the attitudes of airport employees toward unsafe acts. The hypothesis tested was that the implementation of an SMS into an airport will result in an improvement in attitudes toward safety. A Safety Culture Survey was utilized to measure these attitudes. Two International Airports were chosen to measure the extent to which the introduction of an SMS at one airport would influence the safety attitude and culture of those employees. Sharjah International Airport, UAE, was used as the experimental group, as it did not have a formal SMS in operation, with another English-speaking International Airport (the Second Airport) being used as a control group, as it already had an SMS in operation. A Safety Culture Survey was used as the pre and post-test measure over a 12-month period to determine the extent of influence of the introduction of the SMS at Sharjah Airport. The results of Phase 1 of the survey (June 2008) were compared against the results of Phase 2 of the survey (June 2009). The average score reported by participants at Sharjah Airport increased significantly from pre-test measure to post-test measure in relation to communication, safety rules, supportive environment, personal risk appreciation, work environment, and involvement. At the same time, the average score for personal priorities decreased significantly from Phases 1 to 2. Results indicate that participants at Sharjah Airport recorded a significant positive shift in attitude to the safety factors covered in the Safety Culture Survey, whilst at the same time responses from the Second Airport showed no such shift in attitude. The Second Airport showed neither decline nor improvement in responses. Whilst some methodological issues were identified, the results were of sufficient strength as to conclude that the outcomes reported support the hypothesis that the introduction of an SMS at Sharjah Airport has effected positive changes not observed at the Second Airport. Recommendations for ongoing research were made to further explore the strength of relationship between SMS and safety attitudes, as well as the relationship between safe attitude and safe behaviour.  相似文献

The European iNTeg-Risk project is a large-scale integrated project aimed at improving the management of emerging risks related to new technologies in European industry. The project aims to build a new management paradigm for emerging risks as a set of principles supported by a common language, agreed tools and methods, and key performance indicators, all integrated into a single framework. It is using a number of Emerging Risk Representative Applications (ERRAs), or case studies, to inform the development of the framework; one of which concerns the carbon capture and storage (CCS) process.This paper describes the iNTeg-Risk CCS ERRA. Relevant hazards and properties of carbon dioxide are described and the emerging risks from CCS are discussed. Three new tools have been developed or trialled within the ERRA. These are: the DyPASI methodology for taking account of atypical (not usually identified) events during hazard identification; a methodology for including the time dimension in a risk assessment; and life-cycle approaches for risk management and communication. For CCS, the risk assessment needs to include both short-term potential accidents from capture, transport or injection, as well as very long-term risks from storage. Knowledge gaps which are generic to emerging risks are also identified.  相似文献

The NIOSH Construction Program worked with industry stakeholders to develop a National Occupational Safety and Health Construction Agenda to target future research and activities. The Program and its partners are also cognizant that new developments can emerge over time and that research can play an important role in helping to understand and address these emerging issues. Examples of emerging issues relevant to construction safety and health are described. These include: (a) climate change and energy considerations; (b) green construction developments and opportunities; (c) new materials; (d) changes in industry structure and practice; (e) workforce developments and disparities; (f) injury underreporting and cost and risk shifting; and (g) increased interest in addressing root causes. Responding to emerging issues while maintaining a focus on fundamental longstanding issues represents an ongoing challenge for researchers and industry organizations. Additional research to understand the diffusion and adoption of research by the industry is also needed. Research accomplished to date provides a strong foundation for addressing future industry needs and trends.  相似文献

IntroductionYoung workers (aged 15–24) suffer disproportionately from workplace injuries, with a nonfatal injury rate estimated to be two times higher than among workers age 25 or over. These workers make up approximately 9% of the U.S. workforce and studies have shown that nearly 80% of high school students work at some point during high school. Although young worker injuries are a pressing public health problem, the critical knowledge and skills needed to prepare youth for safe and healthy work are missing from most frameworks used to prepare the emerging U.S. workforce.MethodsA framework of foundational workplace safety and health knowledge and skills (the NIOSH 8 Core Competencies) was developed based on the Health Belief Model (HBM).ResultsThe proposed NIOSH Core Competencies utilize the HBM to provide a framework for foundational workplace safety and health knowledge and skills. An examination of how these competencies and the HBM apply to actions that workers take to protect themselves is provided. The social and physical environments that influence these actions are also discussed.ConclusionsThe NIOSH 8 Core Competencies, grounded in one of the most widely used health behavior theories, fill a critical gap in preparing the emerging U.S. workforce to be cognizant of workplace risks.Practical applicationsIntegration of the NIOSH 8 Core Competencies into school curricula is one way to ensure that every young person has the foundational workplace safety and health knowledge and skills to participate in, and benefit from, safe and healthy work.  相似文献

The U.S. National Institute for Occupational Safety and Health (NIOSH) conducts research to improve and protect the health and safety of workers. This paper describes the experience of the NIOSH Construction Program with two recent program planning initiatives intended to improve the program: (a) an independent external review of work over the past decade and (b) the development of strategic goals organized into a “National Construction Agenda” to guide a decade of future work. These goals, developed with input from construction industry stakeholders and researchers, are a part of the NIOSH National Occupational Research Agenda (NORA) initiative. The NORA goals are intended to provide an ambitious set of goals for all construction stakeholders to work together on. Both efforts relate to insuring the relevance and impact of research, reflecting an emerging policy perspective that research programs should be judged not just by the quality and quantity of science produced, but by the industry impact and tangible benefit resulting from the research. This paper describes how views on research planning have evolved to incorporate lessons learned about how research leads to improved safety and health for workers. It also describes the process used to develop the goals and the resulting strategic and intermediate goals that comprise the National Construction Agenda.  相似文献

Introduction: Cargo Tank Trucks (CTTs) are a primary surface transportation carrier of hazardous materials (hazmat) in the United States and CTT rollover crashes are the leading cause of injuries and fatalities from hazmat transportation incidents. CTTs are susceptible to rollover crashes because of their size, distribution of weight, a higher center of gravity, and the surging and sloshing of liquid cargo during transportation. This study identified and quantified the effects of various factors on the probability of rollover and release of hazmat in traffic crashes where a CTT was involved. Method: Bayesian Model Averaging (BMA)-based logistic regression models were estimated with rollover and hazmat release as the binary response variables, and crash, truck, roadway, environment, and driver characteristics as the explanatory variables. 2010–2016 police-reported CTT-involved crash data from Nebraska and Kansas was utilized. Receiver Operating Characteristic (ROC) curves confirmed appropriateness of the modeling approach for inference and prediction on the crash dataset. Results: CTTs are more likely to rollover in crashes while turning and changing lanes relative to going straight; side impacts (side collisions) and severe crosswinds increased the likelihood of rollovers; tractor and semi-trailer body style decreased the probability of rollover, while truck tractors are more prone to rollovers; collisions with fixed objects and higher posted speeds increased the rollover probability; rollovers and intersection crash locations increased the likelihood of hazmat release. Conclusions: The findings can assist stakeholders (policy-makers, private shippers, and CTT drivers) in restricting CTTs’ operations for safety; scheduling, routing, and fleet planning; and low-level decision-making (e.g., emergency stopping or local routing). Practical Applications: This study identified and quantified the effects of different factors on the conditional probability of rollover and release of hazmat in CTT-involved crashes. The findings may assist stakeholders in decision-making towards safe operations of CTTs for transportation of hazmat.  相似文献

The development of an effective safety culture is essential to promote safe operations. Previous studies have either identified the characteristics of effective safety culture analytically, inferring them from signs and symbols derived from working practices, or have restricted the study of the development of safety culture to workers within an organisation. This paper describes a large-scale survey-based study in which the factors influencing the evolution of safety culture are identified empirically and, drawing upon open systems theory, are also extended beyond the bounds of the organisation. Three major determinants of safety culture are identified: safety concerns, influences and actions. Sub-components within each of these categories are also identified and the relationship between them is hypothesised.  相似文献

The ability to perceive hazards has been suggested as being an important factor in determining safety risk. By measuring the ability to perceive hazard, at-risk workers could be identified and provided with additional training. This can reduce knowledge-based human errors and thereby help to avoid accidents. This paper proposes the use of fuzzy signal detection theory (FSDT), which combines fuzzy logic and conventional signal detection theory (SDT) to measure hazard perception. The flexibility of the proposed method allows multiple forms of qualitative and quantitative information to be used, including expert opinions and workers’ responses when quantitative data are lacking or when only qualitative aspects are known. Details have been provided on the basic postulates, formulas, and steps involved in FSDT analysis. A real-world example demonstrates the feasibility of the proposed framework. The results of a case study show that the participating workers had a medium level of hazard perception. This means that these workers were sensitive to the difference between hazard and safety. But there were still opportunities for improvement. Of the workers surveyed, five workers expressed conservative views regarding risk while the other workers expressed risky views. FSDT increases the applicability of conventional SDT analysis to many real-world settings, where the definition of a signal is less dichotomous than in a controlled laboratory environment.  相似文献

Highly exothermic materials have caused many serious accidents involving storage and transportation, due to being thermally reactive. The safe storage and management of these materials is still a critical problem in many countries. Our aim was to study the thermal hazard of thermal reactive materials, such as a propellant, by employing differential scanning calorimetry (DSC) non-isothermal tests and isothermal tests, and then comparing the kinetic parameters by isothermal and non-isothermal of kinetics. The chosen approach was to obtain reliable thermal decomposition by a safe and effective method, which acquired the kinetic and safety parameters of storage conditions that could be applied as highly exothermic materials' reduction of loss prevention and energy potential for safer design during storage transport and processing operations.  相似文献

The paper examines the level of awareness of stakeholders in adopting Inherently Safer Design (ISD) principles in the Project Life Cycle Management (PLCM) of South African Energy utility projects. It seeks to understand whether stakeholders in the latter stages of the Project Life Cycle are competent to make design change decisions on these projects. ISD principles are essentially useful for reducing risks and as such, safety experts have acknowledged it as an excellent approach in the design process. However, there are no known efforts to date that attempt to integrate ISD concepts into PLCM. This paper seeks to fill this gap. This study, through the review of extant literature establishes that ISD principles can be used in project procurement, and adopts a quantitative survey approach in obtaining information from stakeholders in the South African utility industry. Findings reveal that the principles of inherent safety are permeating into the management of South African utility projects but the level of awareness and its adoption are below optimal levels. It also emerged that there is a divergent awareness of ISD strategies amongst PLCM stakeholders and that the design engineers are better informed about the ISD approach of eliminating risks and hazards in the industrial systems studied than other stakeholders. The findings also indicate that the level of awareness of stakeholders of ISD principles is greater within earlier project phases. Based on these findings, the paper concludes that the hazards witnessed within the project execution and finalization phases could be as a result of the low levels of awareness, divergent views and lower use of ISD strategies by PLCM stakeholders. It is suggested that the level of awareness of the principles, concepts, basics and benefits of integrating ISD into PLCM be raised amongst stakeholders functioning within utility industry project procurement in South Africa and that design changes be limited to the earlier phases of utility project procurement.  相似文献

SESAR, the ‘Single European Sky Air traffic Research’ program, envisages radical changes for European Air Traffic Management (ATM). It integrates and implements new technologies and information processing. This paper examines the safety decision-making in the implementation of SESAR projects. SESAR poses new safety problems because it adopts new paradigms for ATM safety – what lessons are there from environmental, nuclear and defense modeling? These disciplines have also had to confront the limitations of modeling the rates of rare and damaging – even catastrophic – events. A major conceptual change in SESAR is that of automated separation assurance systems. Some existing responsibilities transfer from the controller – either to the pilot or to computer systems – in a progressively phased approach. The major problem for SESAR safety validation is that mixed equipage/operations within a common airspace potentially generate new and different safety issues regarding the validation of safety predictions. A potential way forward uses high-fidelity Human In The Loop Simulations (HITLS) to generate confidence in the resilience of the ATM system. The focus changes from proving safety, i.e. through traditional kinds of validation processes, to extensive resilience testing using these simulations. The aim would be to test how resilient the system is to seeded errors, penetration testing, and crash/stress testing. This would be a high cost process because of the large investments required and the need for long sequences of testing. However, these demanding processes can provide ‘justified belief’ to the decision-maker that the changed ATM system is acceptably safe.  相似文献

针对大型浮顶罐区火灾事故风险特征,通过对火灾场景辨识及其火灾风险影响因素分析,结合预防初期火灾场景风险要素和消防系统有效性,提出了罐区火灾事故现实风险的评估框架及确定方法。在此基础上,构建了基于火灾场景的全过程风险防范体系,包括库区评估规划、罐体本质安全化设计、库区火灾预防、罐区火灾扑救、园区火灾事故管理等5个方面,并给出相应的对策措施。大型浮顶罐区全过程火灾风险防范是一个持续改进的结构化过程风险管理体系,有助于实现罐区火灾风险的过程安全管理,进而提高仓储型园区的整体消防安全保障能力和风险防控水平。  相似文献
