Abstract:
At about 10 o'clock on a summer morning in 1987, an industrial filter used to purify an electrolytic plating solution exploded at a printed wiring board manufacturing plant. An investigation team used the failure modes and effects analysis technique to identify potential failures that could have caused the explosion and to reconstruct the most probable sequence of events that led to the accident. This paper describes the investigation and its results.
