At around 06.00 on Sunday 11th December 2005, a vapour cloud explosion occurred at Buncefield Oil Storage Depot, Hemel Hempstead, Hertfordshire, UK, generating significant blast pressures. However, as a storage site, the Buncefield terminal had very little pipework congestion and at first sight would not have been considered as having much potential for a vapour cloud explosion. As a consequence, one of the actions of the Buncefield Major Incident Investigation Board (BMIIB) was to initiate a review of the possible causes of the severe explosion on the site. This review was then extended to a Joint Industry Project, Phase 1 of which has offered an explanation of the cause of the explosion. The conclusions are summarized along with reference to relevant experimental studies, illustrating how the elements of the explanation were already known. The implications of the incident for the assessment of vapour cloud explosion hazards will be discussed, both in terms of understanding worst case consequences and the use of risk based approaches. 相似文献

The construction industry is plagued by occupational risky situations and poor working conditions. Occupational risk assessment (ORA) on workplace sites is the first and key step to achieve adequate safety levels, particularly to support decision-making in safety programs. Most construction safety efforts are applied informally under the premise that simply allocating more resources to safety management will improve safety on site. Moreover, there are many traditional methods to address ORA, but few have been adapted and validated for use in the construction industry, particularly in the design stage, for which traditional approaches do not give adequate answers. This paper presents a state-of-the-art on ORA traditional methods, for the construction industry, discussing their limitations and pointing advantages of using fuzzy sets approaches to deal with ill-defined situations. 相似文献

Relevant safety issues are associated with hazardous materials transportation, especially when transport routes cross populated areas. On March 6th, 2015, a passenger train collided with the last rail car of a freight train in Tilburg, the Netherlands. The last car contained 50 t of liquefied 1,3-butadiene. As a result of the collision, the last car showed deformation; a small leakage occurred but fortunately with no relevant consequences. However, extremely severe consequences could have happened, such as in the rail accident that occurred in Viareggio, Italy in 2009. In this work, the case of Tilburg was firstly outlined and explored by qualitative methods, in order to identify possible realistic final scenarios that could have happened. Second, the potential consequences of the identified scenarios were estimated through conventional integral model for physical effects evaluation. Comparison with the Viareggio case was also shown in order to support the discussion of the results obtained. Finally, lessons learned after the incident, policy making considerations, and indications for the risk mitigation of hazardous materials transportation are given. 相似文献

There exist many perspectives on risk, and traditionally some of the perspectives have been seen as representing completely different frameworks, making the exchange of ideas and results difficult. Much of the existing discussions on risk perspectives have in our view lacked a sufficient level of precision on the fundamental ideas of risk assessments and management. For example, there is more than one line of thinking in risk analysis and assessment and mixing all approaches into one gives a rather meaningless discussion. In this presentation we summarise and categorise some of the common perspectives on risk, including an approach integrating aspects of technical and economic risk analyses, as well as social scientists’ perspectives on risk. For the different perspectives we clarify the meaning of key concepts such as risk and uncertainty. Special focus is placed on the different perspectives’ impact on decision-making. Implementation of the ALARP principle is used as an example to illustrate the differences. 相似文献

The aim of this paper is to provide a simple methodological tool for the incorporation of human factors in the process of risk assessment. This attempt takes place through a simplified quantification scheme including various affecting factors for intentional and unintentional human behavior in risky situations. This scheme is simple and autonomous and it can be applied in a “bow-tie” analysis. It produces numerical coefficients to adjust “nominal” estimated probabilities in order to take the impact of human factors into account. Although the whole approach lacks theoretical accuracy, it remains simple and proper for risk assessment in small scale workplaces. The study reports the results of a follow-up evaluation, conducted in 1994, of the impact of a training program for female unskilled metal workers that was implemented at the end of the 1970s. The program was designed to promote occupational skills development, and was first evaluated in 1979. On both occasions of evaluation the investment in training was examined from two perspectives, those of the training participants and management. Data were collected through semistructured personal interviews with key members of personnel management and through a group conversation session with a majority of the original course attendees. Supplementary background information was obtained from documents and records maintained by the organization. At some points, there was a convergence of views on the significance of the training program for occupational skills development, at others a divergence. These convergences and divergences are finally discussed and the findings of the study are related to other research on this particular kind of intervention. 相似文献

Limitations of current risk assessment methods to foresee emerging risks: Towards a new methodology?
The objective of this work-in-progress is to investigate the potentialities but also the limitations of traditional risks analysis tools especially in the context of emerging technologies and develop a method facilitating the early detection of scenarios of accidents. This is certainly a challenge particularly for new industrial fields since, in this case, very little or no lesson from past accidents is available. It is believed that such situations cannot be conveniently treated using traditional risk assessment methods (HAZOP, FMEA, …) and typical examples are given. The reason is that those methods rely heavily on past accidents and are therefore “trapped” in them so that they are largely “inductive”. In terms of foreseeing the future, the shortcomings of inductive methods are recalled. The possibility to imagine the future with very little clues is then discussed on the ground of theoretical consideration and a way to do so is proposed (abduction, serendipity). Then on the basis of the observation of how the experts work and how discoveries are made, a potential new methodology is outlined. 相似文献

The use of a new computer-automated tool TORAP (TOol for Rapid risk Assessment in Petroleum refinery and petrochemical industries) is demonstrated through a rapid and quantitative risk assessment of a typical petroleum refinery. The package has been applied for an appraisal of the risks of accidents (fires, explosions, toxic release) posed by different units of the refinery, and to identify steps to prevent/manage accidents. The studies reveal that TORAP enables a user to quickly focus on the accidents likely to occur, and enables forecasting the nature and impacts of such accidents. This information is directly utilisable in identifying “soft” spots and in taking appropriate remedial measures to prevent or control accidents. The special attributes of TORAP are: (a) a wide range of applications—achieved by incorporating models capable of handling all types of industrial fires and explosions, (b) sophistication—brought about by including state-of-the-art models developed by these authors and others, (c) user-friendliness—achieved by incorporating on-line help, graphics, carefully formatted output, and, above all, an automatic module with which even a lay user can conduct risk assessment. The entire package, especially its automatic module, is supported by an extensive knowledge-base built into the software. 相似文献

Assessment of the risk of exposure to a “dangerous dose” (DD) is the basis of the UK Health and Safety Executive's HSE's current risk assessments for land-use planning (LUP). Some years ago, a hybrid approach using both DD and “significant likelihood of death” (SLOD) was proposed as an improvement, but was not adopted.Here, an alternative, weighted multiple threshold approach, provisionally titled “total risk of death” (TROD), is described. TROD improves the comparability of assessed risks from diverse hazards. This is achieved by first performing assessments for more than one threshold of consequence (such as DD assessment and SLOD assessment). The predicted risk for each threshold is then combined into a single risk value (at a specified location) by weighting the contributions to risk according to the predicted consequences for each threshold.This paper makes the case, in principle, for using TROD and illustrates how TROD values are constructed.TROD overcomes some of the objections that have barred progress to more widespread use of risk assessment, it is more comparable between different installations and hazards than DD, it is more sensitive than SLOD and more adaptable than probits (which can introduce a false sense of precision). It could support more direct comparison with other risks (e.g. everyday risks and transport risks) in the future.The appropriate “weightings” for addition of risks predicted for different consequence thresholds (contributing to TROD) are discussed here. A three-threshold scheme for evaluation of TROD is described. The thresholds are DD (assumed to approximate to a dose leading to ~1% fatal consequences or LD1), LD10, and SLOD (~LD50).TROD has been used in HSE sponsored research and in HSE's exploration of societal risks. 相似文献

On July 31, 2014, at around 23:57, several huge explosions occurred that lasted for 2 h in Kaohsiung City, Taiwan. As a result of a gas leak from a ruptured underground pipeline, the catastrophic incident destroyed more than 6 km of roads, killed 32 people, injured 321 people, and damaged 3259 buildings. Pipeline explosions have been reported as a repeatedly occurring problem, indicating that (1) complex systems are difficult to manage and control, and (2) humans are unable to effectively learn from experiences of accidents. Initial analyses results reveal that root causes of this incident were a combination of a series of complex chain reactions, which eventually led to propylene leakage and explosion. This is a systematic problem, which can hardly be investigated or analyzed by traditional research approaches. Based on the investigation reports and “systems thinking” method, this study develops causal loop diagrams for the Kaohsiung gas explosion to explore the root causes of the disaster. The research results indicate that (1) this pipeline explosion incident was the result of the chain reactions and was the output of a complex system; (2) the mental model of “production first” and “experience gap” were the root causes of the disaster; and (3) to achieve a higher safety standard, continuous education to improve the mental model of “safety first and safety over production” are essential. The findings of this study may contribute toward the improvement of the standard operating procedure for disaster management and preventing similar incidents in the future. 相似文献

International companies investing in emerging markets need to address the management of safety under varying conditions. The paper presents an evaluation of the approaches taken by an international hydropower company in two different construction projects, one in India and one in the Philippines. In the first project, the company had to intervene as a reaction to poor safety and overall performance. Based on previous experiences, the company selected a proactive approach in the second project, involving use of the contracting process to ensure adequate conditions from the start of construction and onwards. The paper accounts for the safety performance results in the two projects and interprets them in relationship to external and internal influencing factors in the respective project. It is concluded that a proactive approach has the potential of delivering satisfactory safety results at moderate costs for follow-up. A reactive approach risks exposing the company to ethical and reputation risks due to a poor safety performance before the corrective actions have taken full effect. It is also resource demanding. Results demonstrate the feasibility of turning around a project’s safety performance also when preconditions are challenging. 相似文献

Nowadays, pipelines have been extensively used for transporting oil and gas for long distances. Therefore, their risk assessment could help to identify the associated hazards and take necessary actions to eliminate or reduce the risk. In the present research, an artificial neural network (ANN) and a fuzzy inference system (FIS) were used to prepare a new model for pipeline risk assessment with higher accuracy. To reach this objective, the Muhlbauer method, as a common method for oil and gas pipeline risk assessment, was used for determining important and influential factors in the pipeline performance. Mamdani fuzzy model was developed in Matlab software by considering expert knowledge. The outcomes of this model were used to develop an ANN. To verify the developed model, the inter-phase shore pipe of phase 9–10 refinery in the South Pars Gas field was considered as a case study. The results showed that the proposed model gives a higher level of accuracy, precision, and reliability in terms of pipe risk assessment. 相似文献

The starting point for this article is the need to develop empirical insights about contemporary societal risk and safety management practice and executive structures. In order to facilitate insights about societal risk and safety management in a Scandinavian welfare context we use Sweden and its local governmental level (municipal) as an empirical frame in this paper. The aim for this article is to analyse how a variety of risk and safety management tasks are divided within the Swedish municipalities. The objectives are to frame the current directions for internal allocations of risk and safety issues by providing an empirically based executive typology and to contemplate the implications and future research needs that arise from that management pattern.The analysis is based on statistical analysis of information from a web-survey with chief officials (n = 1283) with responsibilities for different municipal functions and sectors. In this study the responses to one of the survey questions are used for statistical analysis. The analyzed question focused on the degree that the respondents estimated that their administrative sector or function handled a selection of risk and safety management assignments (n = 45). A factor analysis was applied to identify patterns in the dataset. The analysis resulted in an eight factor solution with a high degree of explained variance (74.3%). The results provide an elementary contribution to the understanding of the current societal risks and safety management directions. 相似文献

RAMP I is a screening tool developed to support practitioners in screening for work-related musculoskeletal disorder risk factors related to manual handling. RAMP I, which is part of the RAMP tool, is based on research-based studies combined with expert group judgments. More than 80 practitioners participated in the development of RAMP I. The tool consists of dichotomous assessment items grouped into seven categories. Acceptable reliability was found for a majority of the assessment items for 15 practitioners who were given 1?h of training. The usability evaluation points to RAMP I being usable for screening for musculoskeletal disorder risk factors, i.e., usable for assessing risks, being usable as a decision base, having clear results and that the time needed for an assessment is acceptable. This article discusses the strengths and weaknesses of various kinds of standardization, when applied to the field of safety management. Recently, there are signs that organizations operating in high risk environments take further steps towards standardization. On the positive side, standardization has the potential to enhance the predictability of normal operations as well as facilitating the transfer of lessons learnt across organizational contexts. On the negative side, standardization is by definition a strategy for dealing with known hazards and accident scenarios. We discuss how too strong an emphasis on standardization can involve unintended negative consequences for organizations’ crisis-handling capabilities. 相似文献

Obtaining knowledge about factors affecting health, safety and environment (HSE) is of major interest to the petroleum industry, but there is currently a severe shortage of relevant studies. The aim of this study was to examine the relative influence of offshore installation (local working environment) and company belonging on employees’ opinions concerning occupational health and safety. We analyzed data from a safety climate survey answered by 4479 Norwegian offshore petroleum employees in 2005 on the dimensions “Safety prioritisation”, “Safety management and involvement”, “Safety versus production”, “Individual motivation”, “System comprehension” and “Competence” using one way analysis of variance (ANOVA), effect size and mixed model. The companies differed significantly for “Safety prioritisation”, “Safety versus production”, “Individual motivation”, “System comprehension” and “Competence”. The local offshore installation explained more of the safety climate than the company they were employed in or worked for did. 相似文献
