计算机仿真事故响应中的班组绩效影响因子研究   总被引:1,自引:1,他引:0  
在核电站应急事件的工况下,用计算机仿真的人的班组行为模型(IDAC模型)可以预计主控室操作班组人员(决策者、操作者和咨询者组成)的失误概率,为了量化事件过程中人的认知心理活动发生的机理,采用绩效影响因子(PIF)计算班组人员发生失误的概率。讨论PIF的定义、分类以及量化值的确定,描述人的认知心理活动的失误机理与PIF的关系,预计班组失误亊件发生的概率。对50个PIFs中的一些重要因子进行描述,每组中的PIF都是独立于其他组的,但是不同组中的PIF可能存在从属关系。试图基于心理学观察以及各种人的可靠性分析方法的影响路径,合理有效地选择PIF,以实现动态概率风险评价中的人误事件发生概率的量化。  相似文献

在复杂工业系统中 ,人因可靠性分析 (HRA)是预防和减少人因失误的有效方法。人在复杂工业系统中的生产活动 ,往往是由组织中班组成员集体完成的 ,完整的HRA必须充分考虑班组人误的产生。班组人误的产生有其自身的规律 ,如何合理地定义人员行为形成因子 (PSFs)是班组人因失误分析的难点 ,也是班组人因失误分析的重要手段 ,被广泛应用在核电厂 ,航空和造船工业领域的事故分析中。笔者详细分析和探讨了班组人因失误的定义、产生过程及相关的人的行为形成因子 ,以期能使大规模工业系统中的人因失误分析更加合理和完善。  相似文献

在探讨第一代人的可靠性分析 (HRA)方法的局限性之后 ,笔者进而探讨第二代HRA方法的模型 ,建立在多种学科 (认知心理学、行为科学、可靠性工程等 )相互结合的基础上 ,又着重研究产生人的行为 /绩效的情景环境以及它们是如何影响人的行为 /动作的 ,并与工业系统的运行经验和现场或模拟机获得的信息紧密结合。重点介绍了以“失误分析技术”(ATHEANA)和“认知可靠性和失误分析方法”(CREAM)为代表的第二代HRA方法。它们是当前HRA领域中的具有应用和发展前景的新框架思想。  相似文献

人因可靠性分析 (HRA)已成为概率安全分析 (PSA)必不可少的内容 ,事故前人因事件可靠性分析对有效预防维修、调校工作中的人因失误有着重要作用 ,是人因可靠性分析的重要组成部分 ,对PSA最终计算结果有重要影响。笔者结合核电站人因可靠性分析的实际需求 ,运用了以THERP为主的人因失误概率评价方法 ,创建了事故前人因事件分析的基本程序、方法及分析文档模式 ,表述了程序化的事故前人因事件分析模式 ,为我国核电站事故前人因可靠性分析提供了完整和有效的分析方法 ,并有效用于秦山核电站的PSA。  相似文献

To address human error in system reliability, Human Reliability Analysis (HRA) is an essential issue. Human Error Assessment and Reduction Technique (HEART) as a rather straightforward technique for HRA has successfully been used in many areas to predict human error probability (HEP). However, knowledge acquisition of experts during assessed proportion of affect (APOA) calculation is subjected to vagueness and ambiguity. To overcome this challenge, in this paper Intuitionistic Fuzzy (IF) set due to their advantage to represent more fuzzy information than a classical fuzzy set adopted through APOA calculation. To demonstrate this hybrid approach short for, IF-HEART, the furnace start-up operation is handled, since analysis shows that most of explosions and losses occur during furnace start-ups operation. Further, a sensitivity analysis is carried out to approve the proposed integrated approach. In addition to its academic contribution, the results of the paper enable to improve the overall safety level of a furnace by taking into account potential human error.  相似文献

Energy production for oil companies requires high-performance equipment. However, after a certain period of use, this equipment could be damaged, causing numerous problems and defects, impeding production, and leading to incidents that may cause material and human losses. Therefore, it is necessary to master the performance parameters of each equipment, which are key indicators of its functionality and can indicate the possibility of reusing it after inspection and diagnosis. However, it is worth noting that the studied system is a vertical cylindrical furnace in Algeria's refinery. This paper describes a new hybrid approach, a methodology that evaluates system reliability by combining two studies. The first is based on the surety of the system's functioning through an analysis with a fault tree to identify the risks, followed by a quantitative analysis using a Bayesian method. The second study follows the thermal calculation of the equipment's performance parameters, namely: the thermal efficiency (η), heat transfer coefficient (h), and heat absorbed quantity (Q), using heat balance equations to quantify the heat absorbed quantity and identify the fluid flow regime inside the tube. The calculation considers three cases: The design case, the failure case, and the after renovation case. The originality of this work appears when the collected findings from the second study are fed into the first study's Bayesian Network (BN) to analyse the system's reliability using the calculated parameters. Then, BN is used to calculate the Probabilistic Importance Factors (PIFs) in order to identify the most impactful system components. Finally, a prediction analysis was conducted. The latter reveals that one of the furnace's major faults is the fumes accumulation inside the combustion chamber. Corrosion, overheating, coke formation inside the tubes, dumper deformation, and burner tip clogging all contribute to this fault.  相似文献

Evacuation from underground coal mine in emergency as soon as possible makes the difference between life and death. Human factors have an important impact on a successful evacuation, but literature review shows that there is a lack of consideration of human error risk during coal mine emergency evacuation in China. To address the above problems, in this paper, we established a framework for human error risk analysis of coal mine emergency evacuation, consisting of scenario and task analysis, risk assessment and risk reduction. A general evacuation procedure which is applicable for different causes is detailed through the scenario and task analysis. A new method based on expert judgment, named OGI-Model, is proposed to evaluate the reliability of human safety barrier. In this new approach, human safety barrier is divided into three sub-barriers, i.e., organization safety sub-barrier (OSSB), group safety sub-barrier (GSSB), and individual safety sub-barrier (ISSB). Each sub-barrier consists of a series of concrete measures against specific evacuation actions. An example is provided in this paper to demonstrate the use of this framework and its effectiveness.  相似文献

对已有人因可靠性分析模型进行归纳总结,结合核电厂运行人员可靠性研究实际,对人因分类、人因建模、数据采样及人因数据库完善等方面存在的问题进行分析,并提出可能的解决办法或研究方向:在人因分类方式上,要加强因素之间的细化程度,以降低评价的模糊性;在人因量化方面,要重点考虑其所依赖的HRA模型、数学依据、结果精确度等3个方面的影响;对待数据采集的问题,规范性数据采集平台的构建是确保数据一致性的关键措施,并有利于不同HRA模型之间接口相容性问题的解决;人因数据库管理系统的智能决策功能是研发设计的趋势。  相似文献

人的可靠性分析 (HRA)是人机工程、安全科学极为关注的问题。近年来 ,该领域的理论研究和应用技术发展很快 ,尤其将认知科学与人的失误的研究相结合 ,从失误模型到计算方法都更加与现实情景相吻合。笔者介绍了HRA的发展历史和相关的第一代HRA计算方法及其特点 ,并且就HRA在我国核电领域中的应用状况和前景进行了探讨 ,重点介绍了国际原子能机构赞助的中国核电厂操纵员可靠性研究项目 ,其成果对于进一步完善安全管理和提高我国核电厂人员可靠性提供了科学的依据和借鉴经验。  相似文献

This article addresses methodological issues of the human reliability analysis (HRA) in the context of probabilistic safety studies. Several conventional HRA techniques, more often used for the evaluation of the human error probabilities (HEPs), have been classified. A taxonomy of human actions, failure events, and related factors is outlined in order to distinguish action phases, human behavior types and incorrect outputs (errors of omission or commission), error types (slips, lapses, and mistakes), and performance-shaping factors (PSFs) influencing the human performance. A tree is proposed to facilitate the selection of a specific method for the evaluation of human reliability with regard to attributes of the situation analyzed. A software system based on the expert system technology to facilitate and document PSA and HRA is outlined. At the end of the article some research challenges in the domain are discussed.  相似文献

介绍和评析了人误分析历史上有重要影响的几种人误原因因素分类方法:传统人因分类法、信息处理分类法和认知系统工程分类法.基于认知可靠性及失误分析方法(CREAM)的人误原因因素分类,运用层次分析法(AHP)基本原理,建立了大规模复杂人-机系统人误原因因素层次结构模型及相应的AHP程序,并以JCO公司超临界事故为实例进行了分析.分析和应用结果表明,本文所建立的人误原因因素层次结构模型及AHP程序能够辨识出大规模复杂人-机系统中人误的主要原因因素,进而寻找出最优预防方案,对预防和减少此类人误事故的发生有积极意义.  相似文献

The focus of this work is on prediction of human error probabilities during the process of emergency musters on offshore oil and gas production platforms. Due to a lack of human error databases, and in particular human error data for offshore platform musters, an expert judgment technique, the Success Likelihood Index Methodology (SLIM), was adopted as a vehicle to predict human error probabilities. Three muster scenarios of varying severity (man overboard, gas release, and fire and explosion) were studied in detail. A panel of 24 judges active in the offshore oil and gas industry provided data for both the weighting and rating of six performance shaping factors. These data were subsequently processed by means of SLIM to calculate the probability of success for 18 muster actions ranging from point of muster initiator to the final actions in the temporary safe refuge (TSR). The six performance shaping factors considered in this work were stress, complexity, training, experience, event factors and atmospheric factors.  相似文献

提出一种贝叶斯网络的人因可靠性评价(HRABN)方法,其中的每个因子对应于贝叶斯网络中的节点,该方法可对人因可靠性作定量分析和定性分析。在定性分析上,节点的因果关系(HRA中的因子关系)及需要改进的薄弱节点都直观地显示在层次图中;在定量分析方面,对节点因子后验概率的推断通过HRA中的先验信息(包含仿真数据、现场操作及专家知识等)和最新信息得到。如果人因可靠性贝叶斯网络中的每个节点的先验概率分布和后验概率分布都已知,模型的可信性就可通过贝叶斯因子进行定量验证。贝叶斯网络扩展性好,当有新的节点因子需要考虑时,只需要补充对应的节点;笔者的方法也能很好地应用在不同行业的HRA。  相似文献

为有效改善桥式起重机驾驶员的操作水平,通过分析和确定这种起重机的工作流程,建立驾驶员基本动作序列。采用第二代人因可靠性分析(HRA)模型,按正常工作状态与紧急工作状态2种情况,对驾驶员操作可靠度进行全面、定量的分析与研究。结果表明,驾驶员操作可靠度定量研究中,重要的是要考虑起重机机型和基本操作动作序列、工作状态、动作时间、人机工效、作业环境等复杂因素。建立驾驶室人机工效定量分析与评价机制,加强驾驶员专业教育培训及考核制度、改善工作环境等技术措施,从根本上提高驾驶员操作可靠度等级,才能符合起重机械安全的发展趋势。  相似文献

长期以来,海洋石油领域风险研究主要集中在对设施的潜在失效概率和后果研究方面,而对于人员可靠性及风险管理方面的研究较弱。然而,据统计资料显示,海上设施在其生命周期内的设计、建造和生产作业等各个阶段中发生的事故大多与人为错误和组织错误(HOE)有关联。在海洋工程领域,开展人员可靠性分析(HRA)的研究工作具有重要意义。根据海上作业的特殊性及轮班制度的特点,推导出轮班风险的定量分析公式,据此计算出每个工作日轮班风险的相对大小,然后结合我国海上作业的实际情况,对影响轮班风险的可变因素进行了敏感性分析,为风险控制与管理提供参考。  相似文献

In recent years, several accidents in confined spaces have threatened the safety of staff and property in industries. A statistical analysis of 120 fatal accidents involving confined space operations in China from 2008 to 2018 was conducted in this study. The causes and characteristics of confined space operations accidents (CSOAs) were summarized. Focusing on the impact of human factors on CSOAs, the HFACS-MCS (HFACS-Modified Confined Space Accident) model, which consists of 5 levels and 21 factors, was established based on HFACS (Human Factors Analysis and Classification System). The chi-square (χ2) test and the OR analysis were implemented to analyze the statistically significant correlations between adjacent levels in the model. Finally, three human influence paths in CSOAs were derived. The results show that inadequate safety culture, organizational process vulnerability, inadequate supervision, supervisory violations, decision errors, and operational violations are the principal causes of the accidents. Besides, hurried and ad hoc rescue often caused more casualties. The targeted pre-accident prevention and post-accident emergency rescue countermeasures are proposed to prevent the recurrence of CSOAs and secondary accidents.  相似文献
