Traditional approaches on the prevention of accidents/injuries in mines reached its limit of effectiveness in improving safety performance and a fresh approach is utmost required. Behavioral safety analysis has been identified as an effective alternative in many industries. This paper is therefore sought to examine the role of behavioral factors on the occurrence of mine accidents and injuries through a case study. Data were collected from two neighboring underground coalmines operating under a large public sector organization of India. High–low plots and t-test were done to explore the differences between behavioral characteristics of accident involved (case) and non-involved (control) workers. How these differences could cause accidents/injuries in mines was estimated through structural equation modeling. The case study results show that accident group of workers (cases) are more job dissatisfied, negatively affected, and highly risk taking compared to the non-accident group of workers (controls). The accident model path analysis shows that negative affectivity, job dissatisfaction, and risk taking behaviors predict an increased number of injuries in mines. Apart from direct influences to work injuries, negative affectivity and job dissatisfaction make workers to take more risks and behave unsafely. These findings contribute to the design of safety programs including safety training, which should be behaviorally motivated. Mine safety management of the case study mines should outskirt their age old belief that accidents/injuries are due to hazardous nature of mining and only engineering control and regulatory monitoring are sufficient for improving safety of the mines. The multivariate analysis also shows that experience bears no relationships with work injury indicating that a less experienced worker is equally likely to be injured as an experienced worker. It implies that experience though helps workers in understanding the physical hazards, however, avoiding the imminent danger is much more behavioral. The variables negative affectivity, job dissatisfaction, and risk taking behaviors are therefore crucial in avoiding accident/injuries in mines.  相似文献

在现有基于风险的土地使用安全规划方法的基础上,通过引入潜在生命损失这一综合风险评价指标,建立了基于移动危险源潜在生命损失的危险化学品仓储区规划选址模型和基于固定危险源潜在生命损失的仓储区安全布局优化模型。在此基础上,提出了危险化学品仓储区规划选址和安全布局优化方法,并通过案例分析验证了该方法的适用性和可行性。采用该规划选址和安全布局优化方法可提高区域土地使用安全规划的效率,为进行城市重大危险源的规划选址和安全布局提供了新的思路。  相似文献

Ethylene treaters are widely used in the petrochemical industry to remove impurities from ethylene feedstock imported from pipeline networks or storage caverns. The safety concerns of dense phase ethylene treaters due to the reactive and highly flammable nature of ethylene are well known and studied. Under certain conditions, ethylene may self-polymerize and decompose violently with heat release. Under other conditions, ethylene will auto-refrigerate, generating cold liquids that may cause potential brittle fracture hazards. Therefore, dense phase ethylene treaters present design challenges with the unique combination of high temperature decomposition and cold temperature brittle fracture hazards.Due to these safety concerns, it is important to select the appropriate engineering design options for dense phase ethylene treaters and the associated regeneration facilities. Totally automated treater regeneration systems add complexity and instrument maintenance requirements while manually operated systems rely heavily on operator training and procedures. Unfortunately, little or no information or design guidance is available from published research findings in the literature on the evaluation and risk assessment of current industrial design options and practices for dense phase ethylene treaters.This paper presents a systematic risk assessment method to evaluate the engineering design and safe operation options for dense phase ethylene treaters. The proposed risk assessment method integrates human factors task analysis into the traditional HAZOP, LOPA and fault tree analysis to allow evaluation of automated, manual and hybrid approaches with a goal of selecting and optimizing design options to ensure plant safety. This approach provides a realistic assessment of the operational risk and allows identification of fit-for-purpose risk reduction. Applying this systematic risk assessment approach, a simpler and more cost effective design solution can be justified, thereby avoiding the need for a high integrity protective system.  相似文献

旨在为煤矿安全风险预控管理提供一种适用的风险评价模型或方法。研究过程中,介绍了人工神经网络自适应共振理论的ART-2算法;在安全系统工程理论及相关研究基础上,结合调研分析建立了风险评价指标体系;选取山西9家煤矿作为研究样本进行实证研究。该算法仿真识别结果与煤矿实际安全风险情况一致性程度达到77.78%,表明针对煤矿安全风险预控管理过程中的安全风险评价,ART-2神经网络具有较好的适用性。  相似文献

INTRODUCTION: Safety and security share numerous attributes. The author, who heads the (Security) Vulnerability Assessment Team at Los Alamos National Laboratory, therefore argues that techniques used to optimize security might be useful for optimizing safety. OPTIMIZING SECURITY: There are three main ways to attempt to improve security-security surveys, risk assessment (or "design basis threat"), and vulnerability assessments. The latter is usually the most effective. SAFETY ANALOGS: Vulnerability assessment techniques used to improve security can be applied to safety analysis--even though safety is not ordinarily viewed as having malicious adversaries (other than hazards involving deliberate sabotage). Thinking like a malicious adversary can nevertheless have benefits in identifying safety vulnerabilities. SUGGESTIONS: The attributes of an effective safety vulnerability assessment are discussed, and recommendations are offered for how such an adversarial assessment might work. CONCLUSION: A safety vulnerability assessment can potentially provide new insights, a fresh and vivid perspective on safety hazards, and increased safety awareness.  相似文献

The key objective of this paper is the presentation of a new risk assessment tool for underground coal mines based on a simplified semi-quantitative estimation and assessment method.In order to determine the risk of explosion of any work process or activity in underground coal mines it is necessary to assess the risk. The proposed method is based on a Risk Index obtained as a product of three factors: frequency of each individual scenario Pucm, associated severity consequences Cucm and exposure time to explosive atmospheres Eucm. The influence of exposure time is usually not taken into account up to now. Moreover, the exposure to explosive atmospheres may affect factors of hazardous event probability as much as its consequences. There are many definitions of exposure to explosive atmospheres but in the case of underground coal mines the exposure is defined as frequency risk of firedamp and coal dust. The risk estimation and risk assessment are based on the developed of a risk matrix.The proposed methodology allows not only the estimation of the explosion risk but also gives an approach to decide if the proposal investment is well-justified or not in order to improve safety.  相似文献

Introduction: Underground mining is considered one of the most hazardous industries and is often associated with serious work-related fatalities; this paper addresses job-related hazards and associated risks. Method: A risk assessment approach is proposed (Pythagorean fuzzy environment) and a case study is carried out in an underground copper and zinc mine. Results: Results of the study demonstrate that hazards can be categorized into different risk levels via compromised solutions of the fuzzy approach. Conclusion: The study provides a theoretical contribution by suggesting a Pythagorean fuzzy numbers-based VlseKriterijumska Optimizacija I Kompromisno Resenje (PFVIKOR) approach. Moreover, it contributes to improving overall safety levels of underground mining by considering and advising on the potential hazards of risk management. Practical applications: The proposed approach will improve the existing safety risk assessment mechanism in underground copper and zinc mining.  相似文献

PROBLEM: Tragic marine and offshore accidents have caused serious consequences including loss of lives, loss of property, and damage of the environment. METHOD: A proactive, risk-based "goal setting" regime is introduced to the marine and offshore industries to increase the level of safety. DISCUSSION: To maximize marine and offshore safety, risks need to be modeled and safety-based decisions need to be made in a logical and confident way. Risk modeling and decision-making tools need to be developed and applied in a practical environment. SUMMARY: This paper describes both the offshore safety case approach and formal safety assessment of ships in detail with particular reference to the design aspects. The current practices and the latest development in safety assessment in both the marine and offshore industries are described. The relationship between the offshore safety case approach and formal ship safety assessment is described and discussed. Three examples are used to demonstrate both the offshore safety case approach and formal ship safety assessment. The study of risk criteria in marine and offshore safety assessment is carried out. The recommendations on further work required are given. IMPACT ON INDUSTRY: This paper gives safety engineers in the marine and offshore industries an overview of the offshore safety case approach and formal ship safety assessment. Introduction: In safety management (SM), it is important to make an effective safety decision based on the reliable and sufficient safety-related information. However, many SM failures in organizations occur for a lack of the necessary safety-related information for safety decision-making. Since facts are the important basis and foundation for decision-making, more efforts to seek the best evidence relevant to a particular SM problem would lead to a more effective SM solution. Therefore, the new paradigm for decision-making named “evidence-based practice (EBP)” can hold important implications for SM, because it uses the current best evidence for effective decision-making. Methods: Based on a systematic review of existing SM approaches and an analysis of reasons why we need new SM approaches, we created a new SM approach called evidence-based safety (EBS) management by introducing evidence-based practice into SM. Results: It was necessary to create new SM approaches. A new SM approach called EBS was put forward, and the basic questions of EBS such as its definition and core were analyzed in detail. Moreover, the determinants of EBS included manager's attitudes towards EBS; evidence-based consciousness in SM; evidence sources; technical support; EBS human resources; organizational culture; and individual attributes. Conclusions: EBS is a new and effective approach to teaching the practice of SM. Of course, further research on EBS should be carried out to make EBS a reality. Practical applications: Our work can provide a new and effective idea and method to teach the practice of SM. Specifically, EBS proposed in our study can help safety professionals make an effective safety decision based on a firm foundation of high-grade evidence.  相似文献

Occupational accidents are stochastic events. The moment of their occurrence cannot be predicted. However, their rate of occurrence may be reduced, albeit not to zero. Considerable effort has been invested into this objective. This led to decreasing numbers of accidents over the years, a tendency which seems, however, to have come to a standstill since approximately 1998. The potential of presently used methods like check lists or HAZOP-based approaches for evaluating and producing recommendations for reducing occupational hazards has apparently been exhausted. Hence, further improvement of the injury rates is expected only from the use of in-depth analysis methods. Therefore, fault tree analysis is applied to occupational safety. This leads to the formulation of the hazard base model and its representation by means of an occupational hazard tree. This tree serves as an example for the assessment of occupational hazards of work places and activities. The characteristic feature of the occupational hazard tree is the decomposition of the undesired event ‘injury’ into basic events. These are qualified verbally using the expressions ‘certain/permanently’, ‘impossible/never’, ‘probable/frequently’, ‘possible/occasionally’, ‘improbable/rarely’. The procedure is implemented in the program GAP (HazardAnalysisProgram), which enables one to generate and evaluate occupational hazard trees. A global verbal assessment of a workplace or an activity is then obtained. On its basis weaknesses of the occupational safety concept are identified, appropriate countermeasures are devised, they are prioritized and their efficiency is valuated. The procedure was successfully tested by analyzing two accidents and assessing a workplace at a major industrial firm.  相似文献

In 2008 the authors developed a methodology for assessing underground coal mines for high risk for major-hazard events. It focused on major hazard-related violations of safety standards associated with high-risk conditions. Later using the same stratified pilot sample of 31 mines, injury measures and MSHA citation data were combined into a Safe Performance Index (SPI). Using 2009 data, the database was expanded to 107 mines, which is a 30% sampling of all underground coal mines. The SPI was used to assess the relative safety-related risk of mines, including by mine-size category. The methodology can be used to assist companies, the Mine Safety and Health Administration, or state agencies in targeting mines with high risk for serious injuries and elevated citations for remediation of their violation and/or injury experience.  相似文献

Process industries involve handling of hazardous substances which on release may potentially cause catastrophic consequences in terms of assets lost, human fatalities or injuries and loss of public confidence of the company. In spite of using endless end-of-the-pipe safety systems, tragic accidents such as BP Texas City refinery still occur. One of the main reasons of such rare but catastrophic events is lack of effective monitoring and modelling approaches that provide early warnings and help to prevent such event. To develop a predictive model one has to rely on past occurrence data, as such events are rare, enough data are usually not available to better understand and model such behavior. In such situations, it is advisable to use near misses and incident data to predict system performance and estimate accident likelihood. This paper is an attempt to demonstrate testing and validation of one such approach, dynamic risk assessment, using data from the BP Texas City refinery incident.Dynamic risk assessment is a novel approach which integrates Bayesian failure updating mechanism with the consequence assessment. The implementation of this methodology to the BP Texas City incident proves that the approach has the ability to learn from near misses, incident, past accidents and predict event occurrence likelihood in the next time interval.  相似文献

Risk assessment constitutes a critical phase of the safety management process. It basically consists of evaluating the risks involved in the execution of working activities, so as to provide the managers with information suitable to address intervention measures. The most important activity in risk assessment is the risk evaluation task but, despite its importance, national and international regulations have never formalized a standard methodology for addressing it, leaving companies complete freedom to adopt the approach they consider appropriate. As a consequence, companies generally settle on the use of simple methodologies which frequently are not able to emphasize all the main aspects affecting risk in the workplace and to produce a precise ranking of hazardous activities. In this paper, a new methodology for risk evaluation is then proposed with the aim to overcome limitations of the classical approaches. In particular, new factors are introduced to take into account effects of human behavior and environment on risk level, other than the classical injury magnitude and occurrence probability of an accident. The proposed methodology also integrates an estimative approach based on the fuzzy logic theory, which permits more coherence in the evaluation process, producing a very suitable final rank of hazardous activities. Finally, an actual case study is reported to show the capabilities of the new risk evaluation model.  相似文献

A retrospective study on severity analysis of Indian coal mines accidents for 100 years was done considering fatal and serious accidents and the resulting causalities. The impact of recommendations based on safety conferences and committees over the years was studied. An event evaluation algorithm (EEA) was developed for this purpose. The hazard rate functions and cumulative risk functions for major hazards in Indian coal mines were developed. The study evaluated the status of safety level as well as the scope of improvement for Indian coal mines safety.  相似文献

As conceptualized and put forward by Kletz, avoiding hazard at first instance is always desirable than trying to protect or manage it. However, there could be certain hazards which may not be possible to be totally eliminated and thus shall be analyzed and assessed in order to either reduce the probability of occurrence or limit the severity of consequences. Trevor's methodologies on HAZOP and hazard analysis (HAZAN) for process hazards are quite straightforward where the hazard is considered as a point source unlike hazmat transportation which is a moving source of hazard. The authors applied the HAZAN techniques to the transportation risk assessment of three different United Nations classes of hazardous materials (hazmats) through important industrial corridors of Surat district in western India that helped local authorities not only to decide on the minimum risk routes to regulate hazmat traffic but also to prepare an effective evacuation and emergency response strategies.  相似文献

Hydrogen is considered an excellent clean fuel with potential applications in several fields. There are serious safety concerns associated with the hydrogen process. These concerns need to be thoroughly understood and addressed to ensure its safe operation. To better understand the safety challenges of hydrogen use, application, and process, it is essential to undertake a detailed risk analysis. This can be achieved by performing detailed consequence modellings and assessing risk using the computational fluid dynamics (CFD) approach. This study comprehensively reviews and analyses safety challenges related to hydrogen, focusing on hydrogen storage, transmission, and application processes. Range of release and dispersion scenarios are investigated to analyse associated hazards. Approaches to quantitative risk assessment are also briefly discussed.  相似文献
