This article presents an analysis of results of 1035 serious and 341 minor accidents recorded by Poland's National Labour Inspectorate (PIP) in 2005–2011, in view of their prevention by means of additional safety measures applied by machinery users. Since the analysis aimed at formulating principles for the application of technical safety measures, the analysed accidents should bear additional attributes: the type of machine operation, technical safety measures and the type of events causing injuries. The analysis proved that the executed tasks and injury-causing events were closely connected and there was a relation between casualty events and technical safety measures. In the case of tasks consisting of manual feeding and collecting materials, the injuries usually occur because of the rotating motion of tools or crushing due to a closing motion. Numerous accidents also happened in the course of supporting actions, like removing pollutants, correcting material position, cleaning, etc.  相似文献

Mobility is a matter of great importance in daily life: However, it also causes costs and involves accident risks. To make mobility safer and reduce accident risks, a scientifically based road safety management is needed. Within such a safety management system, a concert of adequate and efficient strategies, tools and measures is developed and implemented. To ensure that the chosen means are efficient they should be derived from research evidence. Secondly, research is also needed to regularly monitor the impact of road safety management tools, serving as a “controlling instrument” for the appropriateness of safety management efforts. This article explains the main strategic aspects of safety management in Germany and illustrates it exemplarily on the basis of two recently implemented road safety measures.  相似文献

国外大量研究表明,安全氛围调查在改进企业的安全行为方面有积极的作用,为组织制订未来的安全政策提供依据。本研究通过查阅文献、国外类似的诊断调查工具及软件,根据所提出的理论假设,采用线性结构方程建模的方法,构建了本研究的LISREL理论模型,并设计了相应的调查问卷和量表。从所建立的安全氛围的三个构面(安全管理、安全认知、安全态度)入手探讨企业的安全氛围对企业的安全行为(两个构面:安全执行、安全处理)的影响。本研究采用问卷调查的方式采集数据,将所得资料用SPSS11.0和LISREL8.3软件来分析处理和检验研究假设,研究结果为实证单位改进企业的安全行为提供了帮助。  相似文献

Although municipalities clearly deal with a broad range of safety issues, little is known about their safety management practices and their usage of safety management tools across sectors. In order to open up the municipal safety management toolbox, this study explores which tools that are being used in Swedish municipalities and by which municipal sector. Our results indicate that tools such as risk inventory, safety rounds and risk analysis are broadly used across most sectors, while usage of other tools is more restricted to certain sectors. Legislative requirements, tool characteristics and actual needs are assumed to influence the usage of tools. Further research in this field is needed for better adaption of safety management tools to municipal needs.  相似文献

在建设工程项目中,为提高安全水平,需要营造良好的安全氛围。安全氛围是抽象化概念,这就要求项目管理团队首先对安全管理进行概念化。研究目的在于探索项目管理人员的概念技能构成维度,分析维度间关系以及对安全管理水平提升的作用机理。因此,运用问卷调查收集相关数据,然后对有效数据进行因子分析,得到概念技能对于建设工程项目安全至关重要的两个维度:项目愿景,项目范围与整合。基于两个重要维度,初步建立理论模型并采用SEM分析论证概念技能对建设工程安全管理的作用机理。研究结果表明,概念技能可以帮助项目管理人员实施安全管理工作,安全管理工作的顺利实施又会提升现场的安全氛围。  相似文献

Exploratory analysis of the safety climate and safety behavior relationship   总被引:13,自引:0,他引:13  
Problem: Safety climate refers to the degree to which employees believe true priority is given to organizational safety performance, and its measurement is thought to provide an “early warning” of potential safety system failure(s). However, researchers have struggled over the last 25 years to find empirical evidence to demonstrate actual links between safety climate and safety performance.Method: A safety climate measure was distributed to manufacturing employees at the beginning of a behavioral safety initiative and redistributed one year later.Results: Multiple regression analysis demonstrated that perceptions of the importance of safety training were predictive of actual levels of safety behavior. The results also demonstrate that the magnitude of change in perceptual safety climate scores will not necessarily match actual changes (r=0.56, n.s.) in employee's safety behavior.Discussion: This study obtained empirical links between safety climate scores and actual safety behavior. Confirming and contradicting findings within the extant safety climate literature, the results strongly suggest that the hypothesized climate-behavior-accident path is not as clear cut as commonly assumed.Summary: A statistical link between safety climate perceptions and safety behavior will be obtained when sufficient behavioral data is collected.Impact on Industry: The study further supports the use of safety climate measures as useful diagnostic tools in ascertaining employee's perceptions of the way that safety is being operationalized.  相似文献

This paper reviews principal concepts, tools, and metrics for risk management and Inherently Safer Design (ISD) during the conceptual stage of process design. Even though there has been a profusion of papers regarding ISD, the targeted audience has typically been safety engineers, not process engineers. Thus, the goal of this paper is to enable process engineers to use all the available design degrees of freedom to mitigate risk early enough in the design process. Mainly, this paper analyzes ISD and inherent safety assessment tools (ISATs) from the perspective of inclusion in conceptual process design. The paper also highlights the need to consider safety as a major component of process sustainability. In this paper, 73 ISATs were selected, and these tools were categorized into three groups: hazard-based inherent safety assessment tools (H-ISATs) for 22 tools, risk-based inherent safety assessment tools (R-ISATs) for 33 tools, and cost-optimal inherent safety assessment tools (CO-ISATs) for 18 tools. This paper also introduces an integrated framework for coordinating the conventional process design workflow with safety analysis at various levels of detail.  相似文献

In order to obtain political interest in road safety problems and to learn from other countries’ ‘good practices’, it is often helpful to compare one’s own safety situation with that of other countries. In a number of projects tools have been developed for such comparisons. These tools range from simple ratings of countries on their safety outcomes, such as the annual number of fatalities per capita or per kilometre driven by (motor)vehicles to more comprehensive comparisons.These comparisons not only show differences in safety between countries, but to a certain extent also explain such differences in terms of their safety background and measures taken. Finally, tools have been defined to support road safety policy makers in developing possible safety measures or actions. Procedures for such complex safety comparisons have been developed and tested in several so-called SUNflower studies.This promising approach can be further developed into standard procedures for safety comparisons between all countries in the European Union, and other countries worldwide. This paper wishes to outline the development of such standards for the benchmarking of road safety and safety trends as well as procedures for quantifying safety performances of countries.Starting point of this conceptual framework is the so-called SUNflower-pyramid in which three types of indicators are distinguished. The first one of these, the road safety performance indicator, is called an outcome indicator and is based on the number of killed and injured road users. The second indicator type indicates the quality of the implementation of road safety policies: the implementation performance indicators. The third type of indicator indicates the quality of response in policy documents to improve road safety (policy performance indicator). The three types of indicators are embedded in a policy context: the structure and culture of a country, which are considered as background variables.This paper sets out to describe the framework for the development of a comprehensive set of indicators to benchmark road safety performances of countries or of sub-national jurisdictions. The paper also discusses the advantages and disadvantages of combining such indicators and if combined, how to aggregate how different indicators in one composite performance index. It is argued to group countries in different classes with more or less comparable countries. Different procedures are used for this grouping. The results are promising and it is recommended to work with classes of countries.  相似文献

We propose an initial set of six system safety archetypes that model common dynamic organizational behaviours that often lead to accidents. The archetypes help clarify why safety-related decisions do not always result in the desired behaviour, and how independent decisions in different pans of the organization can combine to have a negative, and often unforeseen, impact on safety. Examples from the NASA manned space program and general industry are used to illustrate the archetypes. As accident analysis and investigation tools, the archetypes can be used to develop dynamic models that describe the systemic and organizational factors contributing to accidents. As risk analysis tools, the archetypes can be used to improve understanding of the ways that risk can arise in complex socio-technical systems.  相似文献

航空安全信息是民航实施安全风险控制和事故预防的基础,对航空安全水平的提高有重要的作用。笔者总结了民航安全信息的基本构成和来源,及其在安全风险识别、管理决策方面的功用;结合中国民航安全管理状况及行业发展趋势,分析了当前中国民航安全信息管理方面存在的主要问题,指出当前民航安全信息存在着数量不足、质量不高、交流共享困难、分析利用不充分等问题,不利于基于数据驱动的安全管理机制的建立和向规章符合性基础上的绩效管理的转变。针对安全信息管理的这些问题,文章建议民航局通过改善安全信息管理政策、建立航空企业安全信息共享平台、建立国家航空安全信息分析中心、开发安全信息分析利用工具等措施来改善安全信息的收集、分析利用和共享,为提高行业安全管理水平服务。  相似文献

The lack of plant-specific reliability data for probabilistic safety assessments is occasionally used for discrediting and doubting the validity of such analyses. On the other hand, analyses are often performed without even discussing the applicability of generic reliability data. In an attempt of clarification the impact of several sets of reliability data stemming from different sources on the explosion frequency of two processes with exothermal reactions (one of them is analyzed with plant-specific data) are quoted. As a further case study the reactor cooling system of one of the processes is analyzed. Additionally, a procedure using frequency and probability ranges is employed for comparison. The results show agreement within their respective uncertainty bounds; the identification of key components for safety is not hampered by data differences. The superiority of plant-specific data, which should of course be acquired, cannot be doubted. Nevertheless, improving the safety of a plant is possible using probabilistic safety analyses even with data which do not stem from the plant under investigation.  相似文献

In recent years, investigations into major incidents often highlight poor safety culture as one of the key causal factors. These investigations are often assisted by causal analysis tools that help to ensure that the investigation and the information captured are systematic. However, current causal analysis tools are not designed to analyse dynamic complexity of major incidents and safety culture, which arises from the interactions between actors and the temporal and spatial gaps between actions and consequences. This is because most causal analysis tools model events and causal factors linearly. In contrast, systems thinking, a discipline of seeing systems holistically, emphasises the circular nature of complex systems, i.e. cause and effect are not distinguishable. This paper proposes that traditional causal analysis tools and investigation should be enhanced with the use of systems thinking tools.One of the systems thinking tools that is particularly useful in analysing major incidents and safety culture is causal loop diagrams. The diagrams can be used to explain the systemic structure sustaining a safety culture and identify effective interventions to improve the safety culture and prevent a recurrence of a major incident. The paper demonstrates the use of systems thinking and causal loop diagrams through a case study on Bellevue hazardous waste fire in Western Australia. The case study shows how different actors in the system, each acting in reaction to pressures that they are facing, produced and sustained a poor safety culture that was a major contributory factor to the fire in 2001.  相似文献

According to official statistics, a large percentage of crashes in Portugal are reported on urban roads. For instance, from 2004 to 2007, about 70% of all injury accidents and 43% of the fatalities occurred inside urban agglomerations. This important safety problem has also been observed on the urban network of Lisbon. Understanding this significant problem, the Government of the Portuguese Republic via its research grant agency – The Foundation for Science and Technology – funded a project whose primary objective consists of developing tools that would help estimating the safety performance of various components of the urban highway system in Lisbon. This paper documents one component of the safety tools that were developed and describes the steps that were taken to develop predictive models for estimating the safety performance of signalized and unsignalized intersections of Lisbon. Several crash predictive models were developed using the Poisson-gamma modeling framework. Two types of models were estimated: flow-only and models with covariates. They were estimated using crash and other related data collected at 44 three-legged and 50 four-legged intersections for the years 2004–2007, inclusively. It was found that some highway geometric design characteristics were associated with the crashes occurring at urban three- and four-legged intersections in Lisbon.  相似文献

This study reports an attempt to improve safety in an industrial plant through the use of applied behavior analysis. The behavioral safety performance of all employees of the plant was observed and recorded with a behavior observation instrument consisting of 35 specifically identified key behaviors. The applied behavior analysis package consisted of training, goal setting, and feedback, used with a withdrawal design. The results confirm the applicability of a behavioral approach to safety. Implications for measurement and evaluation of safety are also discussed.  相似文献

INTRODUCTION: Safety hazards are unavoidable in many work environments. Employees must be both productive and safe, however, conflicting safety and production demands can negatively affect safety, production, or both. The employee's perception of the compatibility of management's safety and production expectations is a possible predictor of such consequences. This paper defines "safety-production compatibility" and describes how measures of safety-production compatibility, as well as safety pressure and production pressure, were developed. METHOD: We used LISREL structural equation modeling to test the influences of safety-production compatibility, safety pressure, and production pressure on safe work behavior and interference with performing other work tasks. The 239 study participants were workers employed in diverse but hazardous occupations. RESULTS: Pressure to work safely was positively associated with safe work behavior. The perceived compatibility of safety and production demands positively influenced safe work behavior and reduced the interference of safety hazards performing other tasks. Safety-production compatibility was also found to mediate the relationship between trust in management and safe work behavior. CONCLUSIONS: The results of this field study suggest increased compatibility, and thus less conflict, between safety and production demands influences safe work behavior and the interference of safety hazards with performing other work tasks. More broadly, the worker's reaction to multiple work demands is a safety and performance influence. IMPACT ON INDUSTRY: Safety management efforts that focus only on the hazards fail to eliminate many accidents because accidents arise from many factors including technology, safety climate, social influences, production, and safety demands. This study suggests that workers differ in their perception of the compatibility of safety and production demands. These differences will show up in safe work behavior, influencing the effectiveness of safety management efforts and the trust workers have in management's concern for safety.  相似文献

PROBLEM: Tragic marine and offshore accidents have caused serious consequences including loss of lives, loss of property, and damage of the environment. METHOD: A proactive, risk-based "goal setting" regime is introduced to the marine and offshore industries to increase the level of safety. DISCUSSION: To maximize marine and offshore safety, risks need to be modeled and safety-based decisions need to be made in a logical and confident way. Risk modeling and decision-making tools need to be developed and applied in a practical environment. SUMMARY: This paper describes both the offshore safety case approach and formal safety assessment of ships in detail with particular reference to the design aspects. The current practices and the latest development in safety assessment in both the marine and offshore industries are described. The relationship between the offshore safety case approach and formal ship safety assessment is described and discussed. Three examples are used to demonstrate both the offshore safety case approach and formal ship safety assessment. The study of risk criteria in marine and offshore safety assessment is carried out. The recommendations on further work required are given. IMPACT ON INDUSTRY: This paper gives safety engineers in the marine and offshore industries an overview of the offshore safety case approach and formal ship safety assessment. 根据飞行员执行新的或特殊任务的需要,在研制飞行安全评估系统时,设置了针对性评估模块.运用层析分析法(AHP)确定了针对性评估模块与一般性评估模块的权重关系.根据飞行安全理论确定了一般性评估简捷方法的因素、要素及量化数据;根据层析分析法、基元事件分析法确定了针对性评估模块的结构和因素的权重,采用基元事件分析法、飞行法规分析和调查研究相结合的方法,对针对性评估的要素进行了量化.最后针对具体事例做了评估计算.研究表明,当针对性评估的安全度小于90%时,应引起足够的警觉,采取必要的措施.  相似文献

HAZOP analysis is a process hazard analysis method that has been widely applied both within and outside the chemical processing industries. This paper presents a design method for a process safety data management program for petrochemical plants based on HAZOP analysis and demonstrates the steps of application involved in building a process safety data management system for an ethylene oxide/ethylene glycol production plant. Firstly, the production data files and relevant documents of the plants should be classified and stored in the program database as reference documents and treatment schemes for coping with abnormal situations should be collected and summarized as guidance documents. Secondly, the HAZOP analysis method is employed to identify all the dangerous deviations possibly existing in the production process of the ethylene oxide/ethylene glycol plant. Then, the relationships among the deviations, the reference documents and the guidance documents should be considered and evaluated. Finally, each dangerous deviation will be given a corresponding reference document and guidance document. The reference documents and guidance documents stored in the expert system can be utilized to help operators solve the corresponding technical problems and cope with abnormal situations. The process safety data management program will contribute to the identification, analysis and resolution of operation problems. When an abnormal situation occurs, according to the deviations exhibited in the system, the necessary reference documents and guidance documents will be quickly consulted by the operators, and an appropriate decision will be made to address the abnormal situation. Therefore, by using the process safety data management program, plant security and human safety in the petrochemical industries will be improved.  相似文献
