Hydrocarbon leaks on offshore installations may result in severe consequences to personnel, to the environment and to assets. In order to prevent such leaks, it is crucial to understand their root causes. The objective of this paper is to study the circumstances of hydrocarbon leaks on the Norwegian continental shelf (NCS). In the study, all reported hydrocarbon leaks from process inventories on all offshore installations on the NCS, with an initial leak rate higher than 0.1 kg/s in the period 2008–2014, have been considered. This includes 78 hydrocarbon leaks, of which about 60% have occurred during manual intervention on normally pressurized systems. The dominating activity when leaks occur is preventive maintenance. A significant fraction of the leaks occur during the preparation for maintenance; such a preparation is typically carried out during the night shift. About half of the leaks are associated with wellhead area and manifolds, separation and compression systems. A substantial fraction of the leaks can be associated with verification faults, dominated by the failure to comply with procedural requirements that are needed to carry out independent verification.  相似文献

Operational safety is receiving more and more attention in the Norwegian offshore industry. Almost two thirds of all leaks on offshore installations in the period 2001–2005, according to the Risk Level Project by the Petroleum Safety Authority in Norway, resulted from manual operations and interventions, as well as shut-down and start-up. The intention with the Risk OMT (risk modelling – integration of organisational, human and technical factors) program has been to develop more representative models for calculation of leak frequencies as a function of the volume of manual operations and interventions. In the Risk OMT project a generic risk model has been developed and is adapted to use for specific failure scenarios. The model considers the operational barriers in event trees and fault trees, as well as risk influencing factors that determine the basic event probabilities in the fault trees. The full model, which applies Bayesian belief networks, is presented more thoroughly in a separate paper. This paper presents the evaluation of the model. The model has been evaluated through some case studies, and one important aspect is the evaluation of the importance of each risk influencing factor. In addition some risk-reducing measures have been proposed, and the paper presents how the effect of these measures has been evaluated by using the model. Finally, possible applications and recommendations for further work are discussed.  相似文献

Major hazards risk indicators are proposed for offshore installations, based on what has been used by the Petroleum Safety Authority Norway for the Risk Level approach in the Norwegian offshore petroleum industry. Since 2002 also leading indicators are used, in the sense that indicators for barrier performance are included together with the lagging indicators. There are individual indicators for a number of barrier functions, including fire and gas detection, a number of safety valves, active fire protection and mustering of personnel. The performance of the leading indicators during several years of data collection as well as lessons learned from the project are discussed. This is followed by a review of how indicators may be used by individual companies, and how the lessons may be utilised by individual companies to develop risk indicators for individual installations as well as company wide indicators.  相似文献

Hydrocarbon (HC) leaks are important initiating events for major accidents in the oil and gas industry. This study explores the extent to which a safety climate indicator from a survey on working conditions undertaken in an oil and gas company (n = 2188) can be used as a leading and/or lagging indicator in relation to HC leaks on 28 offshore installations. It was found that more negative safety climate scores were associated with increasing numbers of HC leaks over a 12-month period following the survey. The safety climate indicator explained more of the variance in HC leaks than technical indicators. HC leaks in the 12-month period preceding the survey did also correlate significantly with the safety climate indicator. More HC leaks during this period were associated with worse scores on the safety climate indicator. Thus, the results support that the safety climate measure could serve as leading and lagging indicator for HC leaks. The results and their possible implications are discussed.  相似文献

The purpose of this paper is to systematically analyse a typical planning process in the offshore industry from the perspective of causes of major accidents, with the ultimate aim of identifying factors that affect the risk for major accidents occurring. We first study and describe a typical planning process for offshore oil and gas operations in Norway. Then we analyse a number of theories of major accidents, to see how the different theories and their explanations of causes and contributing factors can be of relevance for future plans and planning processes. Finally, we review accident investigations to search for evidence of how weaknesses in planning processes can contribute to major accidents through the above identified factors. Also, we try to identify any additional factors that have not been recognised through the theoretical review. This provides empirical support for the theoretical basis. Thirteen factors which directly or indirectly can influence the planning process causing a major accident potential are identified. These are exemplified through a review of investigation reports. The paper suggests that planning process should focus more on increasing quality in the plans at an early phase, with examples from incidents, and illustrate the relation between planning quality and potential for major accidents.  相似文献

Continuous production processes on North Sea installations necessitate extended work schedules; 2-week offshore tours (alternating with shore breaks), 12 h shifts and rapid day/night shift changes are inherent features of offshore work. These intensive rosters, worked in a demanding physical and psychosocial environment, are potential sources of fatigue and impaired performance among offshore personnel. This article focuses on offshore working time arrangements, and presents a systematic review of studies which examine offshore day/night shift patterns in relation to operational safety and individual health risks. Of the 53 studies retrieved, 24 met the review criteria.Field study findings are generally consistent in showing that sleep, alertness and performance are relatively stable across day-shift tours; initial night shifts are adversely affected by circadian disruption, but full physiological and psychological adaptation occurs within 5–6 days; re-adaptation to day shifts is slower, and varies widely across individuals; the offshore environment is conducive to night-shift adaptation, but interventions to facilitate re-adaptation have proved only modestly effective. Analyses of survey data and accident/sickness records identify offshore night work as a risk factor for impaired sleep, health problems, and injuries, but little is known about the long-term health effects of different offshore shift rotations.In conclusion, research methodology and findings, and working time issues of current concern to the offshore oil/gas industry, are discussed. Aspects of offshore work schedules that have been not been widely studied (e.g. overtime, irregular work patterns) are also highlighted, and research areas that would merit further attention are noted.  相似文献

In their regulations, the Petroleum Safety Authority Norway (PSA) states that the companies responsible for oil and gas exploration and production activities in Norway shall record and investigate accidents and serious incidents. This paper introduces a general model for the accident investigation process in the Norwegian petroleum industry, and presents some results derived from a comparative study of causal factors identified in offshore accident investigation reports from 2002 to 2006. The paper also offers a discussion of the improvement potential in the use of investigation methods. Finally, a discussion of future challenges in accident investigations is provided.  相似文献

When a major hazard occurs on an installation, evacuation, escape, and rescue (EER) operations play a vital role in safeguarding the lives of personnel. There have been several major offshore accidents where most of the crew has been killed during EER operations. The major hazards and EER operations can be divided into three categories; depending on the hazard, time pressure and the risk influencing factors (RIFs). The RIFs are categorized into human elements, the installation and hazards. A step by step evacuation sequence is illustrated. The escape and evacuation sequence from the Deepwater Horizon offshore drilling platform is reviewed based on testimonies from the survivors. Although no casualties were reported as a result of the EER operations from the Deepwater Horizon, the number of survivors offers a limited insight into the level of success of the EER operations. Several technical and non-technical improvements are suggested to improve EER operations. There is need for a comprehensive analysis of the systems used for the rescue of personnel at sea, life rafts and lifeboats in the Gulf of Mexico.  相似文献

对1985-2005年中国城市水源地突发污染事件进行了统计分析,以从总体上揭示这些事件的发生规律及其对城市水源地和供水安全的危害.采用文献检索方法,通过对1985-2005年<中国环境报>、<人民日报>、新华网等报刊和网络的检索,从发生日期、地点、污染物种类、事件简况4个方面统计中国城市水源地突发污染事件,经整理和筛选后共列出102起.这些突发污染事件的分析结果表明:1)中国城市水源地突发污染事件总体上呈数量逐年增多,危害增大的趋势;2)化学品和污水是主要污染物;3)河道交通事故和工厂泄漏事故是主要风险源;4)突发污染事件一般都造成了比较严重的经济社会影响,但缺乏相应的应急管理机制和应急部门;5)3个典型突发污染事件造成重大经济社会损失并引发社会高度关注,凸现了构建中国城市水源地突发污染事件应急机制的重要性和迫切性.  相似文献

Wind turbines are a green energy source that Taiwan has been endeavouring to develop in the past five years. Excellent onshore wind farms have already been developed; however, offshore development is inevitable because of Taiwan's limited land area. With regard to energy sustainability, operation and trouble-free maintenance of wind farms are essential. If damage to fan components can be mostly prevented, the operating cost and maintenance time would be considerably decreased; thus, remarkable economic benefits could be achieved. An appropriate coating can help a structure resist the stress of the working environment and improve its durability, resulting in lower maintenance cost and less staff being required to perform on-site repairs. Diatomaceous earth (DE) has been widely used to produce composite porous materials. In this study, epoxy resin and titanium dioxide were mixed with DE to increase the hydrophobicity of the surface of coatings and provide them photocatalytic self-cleaning ability. The ionic liquid 1-butyl-3-methylimidazolium hexafluorophosphate was also added to the coatings to prevent the corrosion of coated metallic material. This study employed thermal analysis to investigate the thermal stability of the coatings and the corrosion resistance of coated iron blocks. Finally, a self-made quartz container was connected to a volatile organic compound detector, and sun exposure was simulated using ultraviolet light. The results confirmed that the modified coating retained its functionality under the harsh working environment and can be applied to the surfaces of large machinery used in offshore wind turbine towers.  相似文献

Deepwater drilling is one of the high-risk operations in the oil and gas sector due to large uncertainties and extreme operating conditions. In the last few decades Managed Pressure Drilling Operations (MPD) and Underbalanced Drilling (UBD) have become increasingly used as alternatives to conventional drilling operations such as Overbalanced Drilling (OVD) technology. These newer techniques provide several advantages however the blowout risk during these operations is still not fully understood. Blowout is regarded as one of the most catastrophic events in offshore drilling operations; therefore implementation and maintenance of safety measures is essential to maintain risk below the acceptance criteria. This study is aimed at applying the Bayesian Network (BN) to conduct a dynamic safety analysis of deepwater MPD and UBD operations. It investigates different risk factors associated with MPD and UBD technologies, which could lead to a blowout accident. Blowout accident scenarios are investigated and the BNs are developed for MPD and UBD technologies in order to predict the probability of blowout occurrence. The main objective of this paper is to understand MPD and UBD technologies, to identify hazardous events during MPD and UBD operations, to perform failure analysis (modelling) of blowout events and to evaluate plus compare risk. Importance factor analysis in drilling operations is performed to assess contribution of each root cause to the potential accident; the results show that UBD has a higher occurrence probability of kick and blowout compared to MPD technology. The Rotating Control Devices (RCD) failure in MPD technology and increase in flow-through annulus in UBD technology are the most critical situations for kick and blowout.  相似文献

The stated and revealed travel behavior of a sample of 60 rural drivers aged 54-92 years provided a basis to explore the potential effectiveness of two common driver's license restrictions aimed at older drivers: time of day and road class. The potential utility and impact of these restrictions have not been explored with revealed data for jurisdictions with a large population of rural older drivers where automobile dependence is high. Data were drawn from a multiday Global Positioning System-based travel diary survey of rural older drivers in New Brunswick, Canada. Revealed travel data showed that over 50 percent of the rural drivers in the sample did not drive after dark, and 40 percent drove less than 1 percent of their total surveyed kilometers on major highways, higher rates than from participant-stated responses. The proportion of participants taking night trips and traveling on major highways decreased with age. The majority of trips taken after dark by all participants had a rural destination. The average daily kilometers driven on major highways by men and women aged 75 years and older was nearly identical (1.79 km/day). These exposure considerations suggest that restricting night travel and major highway travel for the oldest rural drivers (75 years and older) may have limited utility given that the majority of participants did not drive in these situations, and for those who did, most of their trips were in rural areas where enforcement could be expected to be limited. A better approach may be to encourage increased self-regulation through training, age-friendly upgrades to transportation infrastructure to help rural older drivers stay driving safely as long as possible, and the development of appropriate rural alternatives to help a driver transition to nondriver.  相似文献

The demands and constraints of the offshore working environment can have adverse effects on health, particularly the quality of sleep. Perceived risk and safety are significant psychological stress factors which may interfere with and deteriorate the sleep quality of offshore personnel. The present study explores the relationship between risk perception, safety climate and sleep quality. Data were collected under the auspices of the Petroleum Safety Authorities in Norway using a cross-sectional design. A total of 9601 offshore workers from 52 offshore installations on the Norwegian continental shelf participated in the study. Our findings indicate that both risk perception and safety climate are significantly related to sleep quality. The results of the present study suggest that risk perception and safety climate not only are important aspects of safety performance in the offshore industry, but also have an impact on sleep quality.  相似文献

In the past, the chemical industry in Japan has been the cause of a number of major industrial accidents. Subsequent to each accident, specific lessons have been learned. These lessons learned have been implemented in terms of safety education of the employees and/or safety measures of the equipment and facilities resulting in a rapid decrease of corresponding accident frequencies. In this paper, we summarized both recent and past major accidents caused by chemical substances in fixed installations in Japan. Case studies show that runaway reactions are among the main causes of major accident occurrences in the chemical process industry in Japan. A recent fatal poisoning accident caused by H2S gas generated during maintenance work again highlights the necessity of adequate safety management in a chemical factory. Therefore, even if hazard evaluation of chemical substances and chemical processes is necessary to prevent runaway reactions, human error is also an important factor contributing to reaction hazards [Wakakura, M. Overpressure is a major hazard in the process industry with the potential to lead to a major accident. Pressure Safety Valves (PSVs) are often used as the last layer of protection against such a hazard and require regular recertification in order to be dependable. The valve safely vents gas from a vessel when the pressure becomes excessive. It is often the practice in industry to apply one or two years as the normal recertification interval of PSV. However, experience from the Norwegian oil and gas industry is that the recertification process several times have caused leaks of gas. The process thus represents a certain risk in itself and the question is then whether the recertification intervals presently being used actually are optimal from a risk point of view? The objective of this paper is to look into this problem, applying typical data from an oil and gas installation. An optimal recertification interval will be calculated based on minimization of risk to personnel.  相似文献

The offshore industry in the North Sea has gone from a very high risk level for employees some 30–40 years ago, to a low level. The paper reviews the statistics briefly and discusses the implications of the low level. There has from the beginning of the operations been strong focus on emergency preparedness, due to the remoteness of the operations mostly far from shore. But there will always have to be a balance between the focus on preventative safety efforts and the extent of emergency preparedness provisions. This is discussed in detail, based on an extensive evaluation of the adequacy of emergency preparedness provisions recently performed. The main findings are presented, and the robustness and validity of the approach is discussed, together with implications of the rare major accidents.  相似文献

This paper presents a technique for detecting and locating leaks in a single viscoelastic pipe, by means of transient analysis. The system studied is a reservoir-pipe-valve structure. The viscoelastic behavior of the pipe wall material is modeled by a generalized KelvinVoigt model. To determine the leak location, the mathematical formulation has been solved by the method of characteristics. The approach by the method of characteristics is often chosen because it is based on the concept of acoustic wave propagation which is the main mechanism of all transient events considered. The presence of the two leaks in a pipe partially reflects pressure waves initiated by the sudden closure of a downstream shut-off valve. These waves affect the shape and the amplitude of the time-history-pressure. The computed results describe the influence of the presence of two leaks on pressure time-history and the effect of leaks locations and sizes on the pressure signal behavior. The effect of the pipe wall viscoelasticity on the two leaks detection and sizing is also discussed. The leaks discharges are determined by resolving two independent equations derived from literatures and based on transient analysis. The friction and leaks depths effects on two leaks locations and sizing are involved.  相似文献

High pressure jet fires pose a serious hazard to offshore installations operated by the oil and gas industry as demonstrated by the Piper Alpha incident. Following the Piper Alpha incident a major initiative by the offshore oil and gas industry operators led to the production of Interim Guidance Notes which provided guidance to operators on how to assess jet fire hazards. However, many areas of uncertainty were identified where no data was available. Areas of particular concern identified in the Interim Guidance Notes were two-phase jet fires, the effect of confinement on jet fires and their behaviour with water deluge. Since that time a considerable body of experimental research has been undertaken. Based on this recent data, this paper reassesses jet fire hazards in an offshore environment and provides updated guidance on the hazards they pose, including tabulated data and simple calculation techniques for predicting jet fire hazards.  相似文献

Petroleum exploration and production in the Barents Sea is a controversial topic. The Goliat field outside the northern coast of Norway will be the first offshore oil development in this region, with planned production start in 2013–2014. Avoiding major accidents at Goliat is critical; not only to reduce the risks to human lives and the environment, but also to gain political acceptance. Providing early warnings of major accidents for Goliat is one of the main objectives of the research project ‘Building Safety’. The objective of this paper is to describe the development of early warnings in the form of indicators. In addition, the paper includes an overview of current status of early warnings of accidents in other major hazard industries; the nuclear power industry, the chemical process industry, and aviation. Experiences from these industries, including lessons learned from recent major accidents, have been used as important input to the development of early warning indicators.  相似文献
