Ethics are concerned with distinguishing between what actions are “right” and “wrong” and what values are “good” and “bad”, etc. and there is a long academic tradition in discussing ethics and ethical theories. Risk acceptance criteria, on the other hand, distinguish between levels of risks that are acceptable and levels that are intolerable. In some sense, one may say that risk acceptance criteria distinguishes between “good” and “bad” systems and activities with regards to the risk they expose the society or elements of a society to and there is thus an obvious link between ethics and risk acceptance criteria or to risk management at large. However, there are few references in the literature that explores this link, and in this paper, the ethical foundation of fundamental principles of risk acceptance criteria will be elaborated upon.This paper considers some important principles for establishing risk acceptance criteria for safety critical systems and activities. The various principles and the philosophies behind them might at first sight seem contradictory and exclusive, but it is demonstrated how they may coexist in one and the same regulatory regime; They may complement each other in order to achieve the overall safety objectives of society. Then, some brief considerations of the ethical foundations for the principles will be given and some relevant examples of actual risk acceptance criteria will be given from the maritime industries. However, it is believed that the principles and discussions are of general interest and apply to all areas of technical risk and to safety regulations in a broader perspective.  相似文献

This article describes optimization strategy of SMCCM from perspective of utility of coal miner with the method of empirical research. Four elements of SMCCM are put forward which consist of “employee safety knowledge”, “equipment safety knowledge”, “environment safety knowledge” and “rule safety knowledge”. Eight elements of utility are put forward which consist of “benefit satisfaction”, “promotion satisfaction”, “job satisfaction”, “enterprise satisfaction”, “management satisfaction”, “colleague satisfaction”, “relationship satisfaction” and “communication satisfaction”. Then hypotheses of influence path between SMCCM and utility are proposed. JCIA is chosen as the sample to carry out survey, which questionnaire is designed based on the likert six-point scale to avoid the middle tendency of the test samples. Data analysis is done from following angles: analysis of the structure and utility of test samples, variables analysis of SMCCM and utility, model analysis of interaction between SMCCM and utility. And then results are obtained from the analysis of elements of SMCCM and elements of utility. Conclusions: first, “rule safety knowledge” should be strengthened above all; second, “job satisfaction” should be paid more attention to; third, coal miner’s benefit should be concerned specially.  相似文献

According to the risk management characteristics and the actual needs of safety production in coal mine, we thoroughly analyze the system of risk management method in coal mine and implement it in Geting Coal Mine. The system manages and controls the potential accident risks, hazard sources and human behavior risks. On this basis, the system of workers’ safety behavior control technology in coal mine is further studied, the “three disobeying” is classified and managed, the “three disobeying” database and safety countermeasures database are established, and the application software - the system of risk management and safety countermeasures optimization in coal mine based on B/S mode is developed and applied, which uses intranet to analyze and supervise the “three disobeying”, publish early-warning information, optimize management and control countermeasures; at the same time, the important prompting messages can be automatically sent to the mobile phones of relevant managers and the person in charge through public communication system in order to improve the real time capability and effectiveness of unsafe behavior control. The technological system and application software implemented in Geting Coal Mine has achieved good results.  相似文献

In this theory-driven literature review we examine how leadership and emerging research in positive organizational behaviour (POB) may inform our understanding of human mechanisms that affect safety outcomes. According to authentic leadership theory, leader self-awareness and self-regulation processes are vital mechanisms in the leader-follower exchange. From emerging research on authentic leadership, we propose that production management values, attitudes, and behaviour are linked to safety climate and safety outcomes in safety critical organizations (SCOs). According to recent developments in management theory, “psychological capital” is comprised of four distinct aspects that can be linked to desirable organizational outcomes and sustained high quality performance in individual workers. From this we offer a research model and five research propositions implicating that authentic leadership directly affects safety outcomes via promoting positive safety climate perceptions. Furthermore, we propose a second path where psychological capital mediates the relationship between authentic leadership, safety climate and safety outcomes in SCOs.  相似文献

Risk screening instruments can be very useful for evaluating hazards and establishing priorities for risk reduction. Further, these instruments can provide educational value to field staff when they are distributed to individuals outside of traditional process safety roles. This paper describes a retro-synthesis technique whereby risk screening instruments can be readily generated for these types of hazard evaluation and priority setting activities. Further, by utilizing a retro-synthesis approach, data gathered at the field level can often be drawn upon at a later point if a more detailed risk assessment is required. The retro-synthesis approach involves working backwards from traditional risk functions (relationships) so as to define the predominant elements for incorporation within the risk screening instrument. Compared to conventional techniques, the retro-synthesis approach utilizes deductive reasoning techniques in a manner similar to Fault Tree Analysis (FTA). However, the directional relationships that are established between causes and consequences become inverted.For example, risk can be defined as a function of “likelihood” and “severity”, where “severity” may be a function of the “energy released” and the “exposure group”. Similarly, the “energy released” is often a function of the “properties of the material involved” and the “amount of material released”. Through identifying these relationships, “risk” in this example can be characterized as a function of “likelihood”, “exposure group”, “properties of the material involved”, and “amount of material released”. Further, by highlighting the various influential parameters, the relationships between each of these parameters, as related to risk, can also be understood. Once the various terms and functions are defined, screening categories can then be developed to quantify each of the terms so as to allow for an approximation of the relative risk involved for the given scenarios.This approach benefits from being simple to develop, maintaining alignment with the mathematics required for more detailed quantitative risk assessments, and generating screening categories which can be given to field staff. Through careful construction of the screening categories field staff can then rapidly, and in a reproducible manner, screen a large number of scenarios so as to identify those situations which represent priority hazards.  相似文献

Learning from incidents is considered a very important source for learning and improving safety in the process industries. However, the effectiveness of learning from reported incidents can often be questioned. Therefore, there is a need to be able to evaluate the effectiveness of learning from incidents, and for that purpose we need methods and tools. In this paper, a method is described for evaluating the effectiveness of learning, based on the idea of “level of learning” of the lessons learned. The level of learning is expressed in terms of how broadly the lesson learned is applied geographically, how much organizational learning is involved and how long-lasting the effect of learning is. In the 6-step method, the incidents reported in a typical incident learning system are evaluated both for the actual and the potential level of learning in a semi-quantitative way with different tools. The method was applied in six process industries on a large number of incidents. The method was found to be very useful and to give insights of aspects that influence the learning from incidents.  相似文献

Aims

A sociological and anthropological view of culture was used to investigate how work culture, independent of “safety culture”, may affect safety in the workplace. We explored how work cultures of nurses and physical/occupational therapists (PT/OTs) in two acute care hospitals are related to the adoption of patient lifting devices.

Methods

Focus groups were conducted between 2006 and 2009, seven with nurse staff (n = 39) and two with PT/OT staff (n = 17), to explore issues concerning a Minimal Manual Lift Environment policy, initiated in 2004, and subsequent use of patient lift equipment. Audio recordings of the sessions were transcribed; text data were analyzed using N6-QSR. Cultural facilitators and barriers to the adoption of patient lift equipment were examined.

Results

Data revealed cultural similarities and differences between these healthcare professions. Both displayed a “patient first” approach to care-giving which may promote lift device use for patients’ benefits, not necessarily for staff safety. Also, the implied purpose of patient lifting devices clashes with the nurses’ cultural emphasis on compassion, and with PT/OTs’ cultural emphasis on independence except when use increases patients’ independence.

Conclusions

Cultural expressions regarding the nature of care-giving among healthcare professionals may affect the propensity to adopt safety measures in complex ways. The workers’ understanding of the purpose of their work, and acceptable means of conducting it, should be understood before implementing safety interventions. The utilization of lift assist teams, who are not socialized into the cultures of nursing or PT/OT, may be one means of circumventing cultural barriers to lift equipment use.  相似文献

Although safety management is known to be vital to construction projects, very few studies have solicited views from construction practitioners about their perceptions of which safety management practices (SMPs) are important to construction projects and related to project performance. An empirical study was undertaken in Hong Kong in order to shed more light on this topic. In the study, the importance levels of 15 popular SMPs and five project performance criteria were rated by 232 respondents. An exploratory factor analysis was conducted, and three SMP categories - information, process, and committees - were extracted. Of these three categories, safety management process was perceived by the construction practitioners as being the most important, followed by safety management information and committees. Moreover, the effect of the three SMP categories on a composite project performance variable was tested using hierarchical regression analysis. Results indicate that the “information” and “committees” categories were associated with project performance positively and significantly. One of the major conclusions of the study is that the construction industry has paid relatively less attention to safety management committees, which were empirically analyzed as having a strong perceived impact on project performance. In order to improve project performance, construction companies should promote the criticality of safety management committees.  相似文献

Introduction: To be consistently profitable, a construction company must complete projects in scope, on schedule, and on budget. At the same time, the nature of the often high-risk work performed by construction companies can result in high accident rates. Clients and other stakeholders are placing increasing pressure on companies to decrease those accident rates. Clients routinely demand copies of safety plans and evidence of past results at the “pre-qualification” or “request for proposal” stages of the procurement process. Are high accident rates and the associated costs just a part of business? Findings: Companies that deliver on scope, schedule, and budget have a competitive advantage. Is it possible for projects with low accident rates to use it as a competitive advantage? Is the value added by safety just a temporary or parity issue, or does a successful safety program offer significant advantage to the company and the client? Impact on Industry: This article concludes that in the case of a high-risk industry, such as the construction industry, an organization with a successful safety program can promote safety performance as a sustainable competitive advantage. Introduction

Falls prevention evidence has changed and evolved over time with positive and negative studies revealing that a “one-size fits all” approach is not the solution. Care must be taken to critically appraise the evidence and the potential applicability of that evidence to the specific hospital setting.

Method

A narrative account of the evolution of research evidence in this field is first presented. How this evidence should be applied in clinical practice is challenging, with a lack of translational evidence for the hospital setting we draw on broader theory of translating knowledge to action.

Conclusions

The journey should begin with formation of a management and engagement committee. A review of existing practices and the difference between existing practice and evidence-based practice should be undertaken to identify the “evidence-practice gap.” Engagement with staff is recommended to inform a plan for practice change. Plans for resourcing, targeting, and evaluating these strategies should also be undertaken.

Impact on Industry

This paper will assist hospitals to identify and implement evidence based falls prevention strategies leading to an improvement in patient safety.  相似文献

Operational personnel in complex hazardous industries are regularly called upon to make decisions that balance the production and safety objectives of their organisation. Regulations and industry standards focus on defining and complying with operating limits of various kinds as the primary method of achieving the right balance. Such limits remove the need in many cases for in-the-moment judgements about safety and production priorities.Focussing solely on the compliance with a pre-defined envelope underestimates the direct contribution to safety from operational managers based on their professional judgement. Research in a chemical plant, a nuclear power station and an air navigation service provider has identified a “line in the sand” approach taken by experienced operating personnel when abnormal situations arise. In developing these situation-specific self-imposed limits, operational managers focus on the status of safety barriers, rather than considering risk from first principles. This approach could form the basis of a new procedure, which lays out the process to be followed in determining how best to proceed (similar to job safety analysis or permit to work). This would acknowledge that such decisions are necessary, assist operational personnel in making better choices and open such decisions to appropriate audit and scrutiny.  相似文献

This paper delves into the relationship between safety and constructivism. In the past 30 years, the constructivist discourse has become very popular but also controversial as it challenges some key categories associated with modernity, such as reason, objectivity, truth and reality. In the safety literature, several works advocate its use. This paper has three objectives. The first one is to reveal the existence of a constructivist discourse in the field of safety. It does this by bringing together scattered pieces of works from different authors who endorse and apply to various topics a position labelled as constructivist. However, and that is the paper’s second objective; it demonstrates that there is not only one constructivism, but several. In order to ground this contention, the paper proceeds with a multidisciplinary and historical approach. It is then argued that it is more appropriate not to conflate this diversity of constructivisms. The paper looks for a solution to this problem by providing a classification based on two groups of parameters: mild/strong and cognitive/social, defining four types. This step serves the third objective which consists in initiating a multifaceted constructivist program in safety composed of heterogeneous but related empirical and theoretical areas of investigations.  相似文献

Crash fault determination is one of the most critical issues in applications of quasi-induced exposure. Traditionally, the driver citation issued by the investigating police officer is the primary source to assign responsibility for motor vehicle crashes. Such citations are based on the “evidence” or observation of a moving violation (such as engaged hazardous actions) in combination with non-moving violations (such as suspended driver license) prior to the crash. The objective here is to identify the contributing factors that may lead to driver citations in two-vehicle crashes in addition to the hazardous action. Multivariate binary logistic regression modeling is employed to explore the behavior of the investigating police officer in terms of issuing citation at the crash scene. A series of explanatory parameters including roadway characteristics, environmental factors, and driver and vehicle attributes is assessed. The results show that whether the crash type was a hit-and-run, alcohol and illegal drug use, driver gender, driver age, and injury severity all appear to have significant impacts on the investigating officer’s decision-making. Specific examples are given to demonstrate how two factors hit-and-run and drinking status can skew the exposure estimates in the context of quasi-induced exposure. The findings will help to serve as a basis to select appropriate parameters in assigning crash responsibility in quasi-induced exposure applications; and we make recommendations to modify existing crash database for better safety research in the future.  相似文献

Safety and safe are terms closely related to risk, but do they extend beyond the realm of risk? Is safety/safe just the antonym of risk, or the same as acceptable risk? This issue has been given due attention in the literature and in this paper we explore how different perspectives of risk affect the relationship between safety/safe and risk. If risk is defined as an expected value or as a measure of the probability and severity of adverse effects, the conclusions would be different than if the essential component of risk is uncertainty. For risk understood as uncertainty about and severity of the consequences of an activity we argue that safe means acceptable risk. Three dimensions of safety are discussed; events and consequences (harm), probability and uncertainty.  相似文献

The improvement of safety in the process industries is related to assessment and reduction of risk in a cost-effective manner. This paper addresses the trade-off between risk and cost related to standby safety systems. An age-dependent unavailability model that integrates the effects of the test and maintenance (T&M) activities as well as component ageing is developed and represents the basis for calculating risk. The repair “same-as-new” process is considered regarding the T&M activities. Costs are expressed as a function of the selected risk measure. The time-averaged function of the selected risk measure is obtained from probabilistic safety assessment, i.e. the fault tree analysis. This function is further extended with inclusion of additional parameters related to T&M activities as well as ageing parameters related to component ageing. In that sense, a new model of system unavailability, incorporating component ageing and T&M costs, is presented. The testing strategy is also addressed. Sequential and staggered testings are compared. The developed approach is applied on a standard safety system in nuclear power plant although the method is applicable to standby safety systems that are tested and maintained in other industries as well. The results show that the risk-informed surveillance requirements differ from existing ones in technical specifications, which are deterministically based. Moreover, the presented approach achieves a significant reduction in system unavailability over a relatively small increase of total T&M costs.  相似文献

This article aims to demonstrate the need for changing the methods with which accidents are analyzed, if we truly wish to use what we uncover from them to learn and enrich our knowledge base of organizational management. The goal is to relinquish the broadly adopted and rather simplistic paradigm that accepts the search for human error and unsafe acts performed by workers, and produces “guilt diagnostics”. Instead, we use a systemic accident analysis methodology, based on the sociotechnical principle of understanding the real operating conditions in which accidents take place. In order to demonstrate the benefits of the theoretical framework, we compare the analyses of an Anhydrous Ammonia gas leakage accident in a fish processing plant using the traditional accident analysis model based on unsafe acts and the proposed systemic approach. The results favor the latter since it tends to be more reliable and offering useful recommendations to safety management processes, thus helping to prevent accidents, especially in complex systems.  相似文献

Introduction: Occupational Health and Safety (OHS) stakeholders rarely factor the cost of workplace accidents into prevention spending decisions. The lack of consideration of this key information is related to the fact that the scientific literature has failed to provide OHS stakeholders with a cost-calculation tool that is both sufficiently accurate and does not require a data-collection stage ill-suited to the time constraints of workplace decision-makers. Method: This study reviews the recent literature to identify key elements that should foster the use of indirect-cost calculation methods by decision makers. Results: A “local” approach currently appears to be the best method for calculating indirect costs of workplace incidents or accidents in comparison to “bottom-up” or “top-down” approach. This paper discusses four criteria that this type of approach must satisfy to be compatible with the time constraints and accuracy demands of OHS stakeholders in organizations. In addition, four bases for the development of a new indirect cost estimation model are presented and discussed. Impact on industry: this study provides bases and criteria to help the development of indirect-cost calculation models better suited to workplace use than those currently available.  相似文献

The safety status of a dynamic mechanical system is determined by its historical, current and future states together. Therefore the safety assessment process of such system should have dynamic and diachronic characteristics, which helps to track the dynamic states of system and predict future probable danger in advance. In order to overcome the disadvantages of traditional static safety assessment approaches, the results from which are often delayed and prone to produce false alarms, an adaptive online safety assessment method is proposed in this paper, which consists of two steps. A dynamic adaptive weighting method is first introduced and an aggregation scheme based on “3-D” time perspective is further presented to integrate system’s historical, current and future safety performance in a unit framework, considering both of assessment and pre-warning functions. The proposed method is able to track and predict the safety status of system dynamically and discover the potential fault in time. Its feasibility and benefits are investigated with a field case study of gas turbine compressor system, which validates that the proposed method improves the accuracy of safety assessment in dynamic conditions, and finally helps to restrain the fault symptom by proactive maintenance successfully.  相似文献

The U.S. National Institute for Occupational Safety and Health (NIOSH) conducts research to improve and protect the health and safety of workers. This paper describes the experience of the NIOSH Construction Program with two recent program planning initiatives intended to improve the program: (a) an independent external review of work over the past decade and (b) the development of strategic goals organized into a “National Construction Agenda” to guide a decade of future work. These goals, developed with input from construction industry stakeholders and researchers, are a part of the NIOSH National Occupational Research Agenda (NORA) initiative. The NORA goals are intended to provide an ambitious set of goals for all construction stakeholders to work together on. Both efforts relate to insuring the relevance and impact of research, reflecting an emerging policy perspective that research programs should be judged not just by the quality and quantity of science produced, but by the industry impact and tangible benefit resulting from the research. This paper describes how views on research planning have evolved to incorporate lessons learned about how research leads to improved safety and health for workers. It also describes the process used to develop the goals and the resulting strategic and intermediate goals that comprise the National Construction Agenda.  相似文献

Objective

To determine and validate patterns of seat belt use and attitudes of taxi drivers on wearing a seat belt following national and provincial seat belt legislation in 2004-2005. Design: Roadside daylight seat belt observation and interview survey methods were used, as well as observations from inside taxis during routine trips and a taxi driver focus group. The setting was Nanjing, Jiangsu Province, PR China in April of 2006 and 2007.

Main outcome measures

Prevalence of seat belt use and attitudes to wearing a seat belt were determined, as were vehicle and driver characteristics, and comparisons with other motor-vehicle driver's seat belt use and attitudes.

Results

Taxi drivers interviewed were predominantly male and aged 30-39 years. They spent more hours per week in their vehicles and had more driving experience than other drivers. Over half (56.2%) of taxi drivers interviewed reported that they always wore seat belts, while observation of taxi drivers showed lower wearing rates (i.e., roadside observation was 43.8%, and observation from inside taxis was 36.2%). Belt tampering was a practice of 12-15% of taxi drivers. “Fine avoidance, safety, high speed and long trips” were given as important reasons for wearing and “feeling trapped and uncomfortable” for not wearing. Seat belt reminder signs in taxis were common (82.6% of taxis), but did not appear to impact on driver seat belt use.

Conclusion

The four research methods found taxi drivers to have consistently low “correct wearing” rates.

Impact on industry

As in several other countries, taxi drivers are particularly resistant to seat belt use. Innovative strategies, including occupational health and safety approaches, may be required to achieve increased levels of seat belt use.  相似文献
