事故报告、事故分析与事故管理是企业安全文化的核心要素之一。作为现代生产安全事故的管理,强调变事故后处理为事故前预防,通过对事故进行深入的剖析,找出导致其发生的深层次原因,进而制定出有针对性的整改措施。     

从事故 看统计

据统计，2002年1～11月全国共发生各类事故980689起，死亡124581人，同比分别上升5.8％和4.6％。其中，一次死亡3～9人重大事故2463起，死亡9708人，同比分别上升7.1％和2.9％。2002年一次死亡10人以上特大事故125起，死亡2289人，同比分别下降10.7％和10.5％，其中一次死亡30人以上特别重大事故11起，死亡593人，同比分别下降31.3％和16.0％。全国工矿企业1～11月共发生伤亡事故12018起，死亡13093人，同比分别上升9.9％和8.9％。其中，一次死亡3～9人重大事故564起，死亡2334人，同比分别下降1.1％和6.4％；一次死亡10人以上特大事故60起…     

完善事故快报制度

为适应新形势下安全生产监督管理工作的需要，及时掌握和统计全省伤亡事故情况，组织协调伤亡事故的抢救和调查处理工作，去年以来，我局进一步强化了各项规章制度建设，完善了事故快报制度，逐步实现了伤亡事故统计工作规范化和制度化。依据《企业职工伤亡事故报告处理规定》和《辽宁省职工因工伤亡事故处理条例》，制定了《辽宁省职工因工伤亡事故报告暂行规定》，要求各级安全生产监督管理部门接到生产经营单位发生一次重伤3人以上和死亡事故报告后，必须在一小时内用电话或传真逐级转报至省安全生产监督管理部门。对一次死亡3人以上的重…     

从龙年事故频发谈起

龙年,在中国老百姓眼里是吉祥之年,但在某种意义上讲又是令人生畏之年,2000年的龙年,在中华大地上,安全生产形势的确是触目惊心：3月初,从江西省萍乡发生33人死亡的爆竹作坊爆炸特大事故开始,到年终为止其间重特大事故接连不断。据某报统计,仅3至10月,全国就发生一次 死亡30人以上的特大事故10余起。     

三起锅炉严重缺水事故的教训

2004年1月至9月中旬，辽中县连续发生了3起锅炉严重缺水事故，事故发生的原因令人深思，希望引以为戒。     

三起与隔离刀闸有关的事故

接地保护线烧伤人一、事故经过1994年4月6日下午3时许,某厂671变电站运行值班员接班后,312油开关大修负责人提出申请要结束检修工作,而值班长临时提出要试合一下312油开关上方的3121隔离刀闸,检查该刀闸贴合情况。于是,值班长在没有拆开312油开关与3121隔离刀闸之间的接地保护线的情况下,擅自摘下了3121隔离刀闸操作把柄上的“已接地”警告牌和挂锁,进行合闸操作。突然“轰”的一声巨响,强烈的弧光迎面扑向蹲在312油开关前的大修负责人和实习值班员,2人被弧光严重灼伤。二、原因分析本来3121隔离刀闸高出人头约2米,而且有铁柜遮挡,其弧光不…     

企业应制订的事故应急救援预案

近年来，我国企业不断发生重特大生产安全事故。对事故救援的过程暴露出企业应对重大事故的不足，这个问题已引起了社会的关注。生产企业应尽快制订本单位的事故应急救援预案，加强对危险源和危险部位的管理和检查，使生产过程中的风险降到最低程度。

Incident reduction and risk migration

In 2002 an incident trend in Air Traffic Management in a European Centre was analysed from a Human Factors perspective, and a single solution was developed, which stopped the incidents occurring. Three years later a new incident trend appeared, which upon analysis appeared to be a more complex version of the former pattern. This required a more comprehensive analysis as well as a more co-ordinated and systemic approach to reduction. In the end, nine recommendations were made, of which more than half were implemented. The incidents stopped. This case study is used to highlight the issue of risk migration in the context of incident analysis and reduction.

Incident investigation of mono-nitro toluene still explosion

At one of the nitro-toluene facilities, an o-nitro toluene still was in standby mode. During the standby mode feed to the still had been stopped, the column was not de-inventoried, steam flow to the reboiler was not shut off and the condenser was on total reflux. After 22 days of standby mode, steam was shut off for 5 days for annual plant maintenance. Eight days after the re-start of steam, the still exploded, hurling debris as far as one mile. The internal packing was expelled from the column and landed on the ground burning. The incident was investigated to establish the contributing factors leading to explosion. Accelerating Rate Calorimetery was extensively used to determine the onset temperature for exothermic activity in the presence/absence of a number of materials to establish the underlying cause of the accident. It was found that foulant collected in the column over a period of years accelerated the ’runaway reaction’.

石油钻井工业事故统计分析

石油钻井工业每年发生大量的事故,造成人员伤亡,环境污染和经济损失。本文通过对中东某采油区13年间3008起各类事故进行统计分析,找出不同因素影响下钻井事故发生的分布规律,为安全管理提供依据。统计结果表明,在炎热的季节,事故发生较为集中,在气候宜人的季节,事故相对较少;斋月所在的月份,事故数量明显增多,与斋月相邻的两个月事故数量明显减少;各类事故按其严重程度呈现金字塔形分布;手脚受伤在LTI中占据相当高的比例;HSE管理质量和承包商的变更对事故的分布有显著的影响。     

Dispersion of the vapour cloud in the Buncefield Incident

Dispersion of the flammable vapour cloud in the 2005 Buncefield Incident is examined. Footage from security cameras around the site is analysed and the results from Computational Fluid Dynamics (CFD) simulations of the vapour dispersion are presented. It is shown that the shape of the terrain and the presence of obstacles significantly affected the dispersion of vapour from the overflowing tank. The CFD model is shown to produce similar qualitative behaviour to that observed in the incident, both in terms of the arrival time of the vapour cloud and its final depth.     

The development of a prototype behavioral marker system for US Navy officers of the deck

The officer of the deck (OOD) of a US Navy ship is in charge of the safe and proper operation of the ship, and accountable to the commanding officer for every event that occurs during his or her OOD watch. This paper discusses the development of a prototype behavioral marker system to evaluate, and provide feedback on, the nontechnical (cognitive, social, and personal resource) skills of OODs.An initial set of 17 categories of nontechnical skills were identified from a literature review. A focus group with four qualified OODs used the skills identified from the literature review to develop an initial taxonomy of five categories, each with two or three corresponding behavioral elements. This taxonomy was then used to classify 149 statements concerned with the nontechnical skills of OODs collected from 16 critical incident interviews. After three iterations of adaptations to the taxonomy, two independent raters were able to reach acceptable levels of reliability in using the taxonomy to classify the statements. Although further development work is required, it is suggested that the prototype behavioral marker system has implications for improving safety and performance on military and civilian ships.
