Decision-making involving risk of accidental loss occurs in a variety of settings. The constraints of the decision settings have a strong impact on decision-making. Different decision settings may thus call for different approaches to decision support. The purpose of this paper is to propose a contingency model of decision-making involving risk of accidental loss. Based on two dimensions, (a) proximity to hazard and (b) level of authority, I identify five types of decision settings: (1) operations, (2) business management, (3) administrative and technical support functions, (4) political arenas, and (5) crisis handling. Each setting is characterised in terms of dominant constraints, dominant decision criteria, and representative decision modes.Decision-making is constrained and influenced by previous decisions, and decisions may interact in the way they influence the risk of accidental loss. The following set of concepts may help us identify ways in which safety may be affected by interactions between decisions: (1) distributed decision-making and local optimization, (2) meta-decisions, (3) absorption of uncertainty, and (4) normalization of deviance.Advice for improving decision-making can be derived from the proposed model by identifying possible problems related to each type of decision setting and each pattern of interaction between decisions. The possible problems may be used as a basis for identifying relevant functions of decision aids and to propose specific decision aids. A similar analysis may be performed with regard to the ways in which decisions may interact in their impact on safety.  相似文献

Advances in computer technology and artificial intelligence are providing powerful capabilities for developing intelligent decision aids (IDAs) to support operating teams in managing complex systems. Early system designs, which acted as independent computer advisors, constrained human decision making, were brittle and encountered problems of user acceptance. As a result, there has been a shift towards making IDAs more cooperative in assisting users to make decisions under stress. Although there is a growing body of research on how to design IDAs as collaborate team players, this article provides additional insights by focusing on the competencies manifested by efficient teams in adapting to stress. To this extent, a survey of how teams adapt their decision-making strategies, cooperation patterns and team structure has provided a good basis for proposing design principles for collaborative IDAs. The article concludes with some challenges for further developments in information technology and research needs in the area of teamwork under stress.  相似文献

In production plants, monitoring and maintaining industrial processes and emergency shutdowns are not straightforward tasks due to the large number of events and alarms which are triggered during the plant shutdown process. It is also vitally important to provide decision support to stakeholders for efficient and effective monitoring and maintenance of production process. This paper presents a novel framework and design to enhance maintenance decisions based on the knowledge gathered through the process of monitoring. This monitoring process is based on signals which are triggered during the plant safety shutdown process. We have designed and implemented a framework using an ontology and business rules to define the logical structure and operation of the petroleum plant with the objective of monitoring the cause and effect of the petroleum plant shutdown process. To enhance maintenance decisions, we have extended the ontology and the framework to ensure that decision makers have sufficient information to make the right decision at the right time. The proposed extended framework is designed, implemented and evaluated using an example petroleum production plant as a case study.  相似文献

We present a general methodology for developing environmental emergency decision support systems (EEDSS) based on an Artificial Neural Network (ANN). We highlight the method for developing the system using an illustrative example of an unexpected atmospheric accident with an ANN prototype system for a district in Shanghai. The network architecture of the ANN is introduced. Then the development process and key technologies are addressed. The procedures for matching the environmental emergency decision support characteristics are as follows: (1) digitization (coding) of case information and emergency measures, in which the information of cases are divided into the input attributes and decision-making information, and standardized and digitized through the Feature Evaluation (FE) method and the Intensity Hierarchical (IH) method, respectively; (2) construction of environmental emergency ANN, in which Gradient Descent with Momentum and Adaptive Learning Rate (GDMALR) method (traingdx function), a modified back-propagation algorithm, is employed to do training; and (3) translation (decoding) of decision-making information, in which output data of ANN is interpreted into practical contingency measures with Translation Based on Conventional Import Ratios (TBCIR) method. The training features, time, errors, accuracy, and input attribute weights of the prototype system are analyzed. The usage of the prototype system is demonstrated through a hypothetical case. This article encounters the challenge of ANN’s own lack of training samples. We discuss to the concept of integrating Case-Based Reasoning (CBR), Genetic Algorithm (GA), and ANN to overcome this difficulty and form a technology system for generating useful decision support information for environmental emergency response.  相似文献

Solid major accident prevention management is characterized by efficient and effective risk assessments. As a means of addressing the efficiency aspect, decision support analysis software is becoming increasingly available. This paper discusses the results of a survey of decision support tools for investigating (internal and external) major hazards in the chemical industry. The most significant features, such as the usability and the functionality of the toolkits are discussed. Toolkit characteristics are analyzed in the light of the different major risk decision process stages. Consequently, valuable supportive information for company decision makers purchasing such software is given. Furthermore, conclusions are drawn and recommendations are formulated for establishing priorities for future risk toolkits developments.  相似文献

Operational personnel in complex hazardous industries are regularly called upon to make decisions that balance the production and safety objectives of their organisation. Regulations and industry standards focus on defining and complying with operating limits of various kinds as the primary method of achieving the right balance. Such limits remove the need in many cases for in-the-moment judgements about safety and production priorities.Focussing solely on the compliance with a pre-defined envelope underestimates the direct contribution to safety from operational managers based on their professional judgement. Research in a chemical plant, a nuclear power station and an air navigation service provider has identified a “line in the sand” approach taken by experienced operating personnel when abnormal situations arise. In developing these situation-specific self-imposed limits, operational managers focus on the status of safety barriers, rather than considering risk from first principles. This approach could form the basis of a new procedure, which lays out the process to be followed in determining how best to proceed (similar to job safety analysis or permit to work). This would acknowledge that such decisions are necessary, assist operational personnel in making better choices and open such decisions to appropriate audit and scrutiny.  相似文献

This study used the manufacturing process in the petrochemical industries as an example and developed a dedicated maintenance programme and executive information system (EIS) for this industry. The software for EIS was established on a CMMS platform, with logical and extractive analysis used to store the information in a KPI databank. The system developed can provide plant managers and engineers with a complete summary of information and keep them updated regarding the present status of their maintenance efforts. The objective of this study was to establish a management system for maintaining knowledge in the petrochemical industries, such as the management of standard operating procedures (SOPs), historical records and the analysis of data for the facility. To design the software, a review of petrochemical facility was purposed to enhance the maintenance efforts and facilitate the decision-making process. The main functions of the system include asset reliability analysis, failure analysis and maintenance benefit cost analysis. For the petrochemical industry, the impact of safety and environment caused by equipment malfunction is more substantial than that of other industries. If executives can manage essential points effectively and make decisions according to a key performance index, risks to safety and environment, which result from equipment malfunction, can be decreased and safety can be enhanced for petrochemical refineries.  相似文献

Many incident reports and much research literature have concluded that incident rates in high-hazard industries can be reduced by enhancing the knowledge and information sources available to workers. However, a sparsity of published research reporting workers' perspectives on the value of information and knowledge sources in different operational situations potentially limits the effectiveness of interventions aimed at improving human responses to process safety performance. In response, this work seeks to understand how frontline oil and gas industry workers, responsible for managing process safety and other risks in their hazardous workplaces, rely on their own knowledge, the knowledge of others and information provided to manage different operating scenarios. A set of open and closed-ended survey questions were distributed to workers at several upstream and downstream oil and gas companies. Statistical analysis of 133 responses revealed interesting insights relating to participants’ experience levels, decision making practices and perceptions on the importance of various information and knowledge sources in different operating circumstances. The data revealed differences in the sources of knowledge relied upon by experienced and less experienced workers. Experienced workers rely on their own knowledge in both routine and potentially high-impact scenarios. Less experienced workers also rely on their own knowledge for routine decisions however they reported seeking out external knowledge and information in potentially high-impact scenarios. The data suggests that interventions to improve safety by enhancing the availability of knowledge and information sources are less likely to affect experienced workers and alternate strategies might be needed. However, such interventions might be effective if they are focused on supporting newer workers.  相似文献

Modern process plants are complex engineering systems. While thorough reviews of system safeguards are performed, catastrophic events continue to occur, often unfolding in unforeseen ways. Success in process safety demands safe processes, and understanding rare, high consequence events is central to the traditional process safety approach. This philosophy is common to all high-hazard industries, offering the potential for sharing approaches, experience, and lessons learned. The problem, however, is that people (and organizations and entire industries) who fear failure (atychiphobia) sometimes obsess about failure so much that they miss opportunities to succeed.This paper examines selected risk management practices in the power generation and aerospace industries and how those practices have led to improved performance. Risk informed decision making (RIDM) has had widespread application in the nuclear and aerospace industries, and is undergoing enhancements to become a key framework for risk management. Additionally, rather than focusing on avoidance of loss, there are emerging approaches supporting achievement of success. This approach provides a more direct link of risk to business and operational objectives, but does challenge conventional risk approaches founded in a loss prevention-centric view. The paper reflects upon risk informed decision making and success modeling, and suggests how these methods may be applied in the field of process safety. Specific examples are drawn from the defense in depth approach from the nuclear power industry and mission success concepts developed for NASA.  相似文献

利用信息技术研究灾害问题成为研究热点之一 ,但在小城镇防灾减灾方面仍缺少应有的关注和重视。笔者将地理信息系统 (GIS)和决策支持系统 (DSS)集成应用于开发“基于GIS的小城镇基础设施防灾减灾决策支持系统” ,简要介绍了该系统研制目标和原则、系统结构框架及各子系统的功能 ,解决了系统实现过程中的一系列关键技术 :快速、高效构建小城镇基础空间数据库及三维可视化、应急预案及历史灾情库的电子化及各类灾害分析模拟的专业模型等。实践证明 ,基于基础设施、灾害源、防护保卫目标、应急救援力量的可视化与数字化 ,该系统能够实现对其进行科学管理、对各单一灾种的模拟分析以及应急决策支持 ,有效提高小城镇政府的应急指挥决策水平 ,增强其灾害应急反应能力。  相似文献

This paper provides a classification and discussion of the empirical cases presented in this special issue. The cases are classified according to the dimensions “proximity to hazard” and “level of authority”. The majority of cases concern decision-making at the blunt end, i.e. removed in space and causal distance from the sources of hazard. The papers show that many decision processes at the blunt end are characterized by various forms of bounded rationality, but there is one notable exception. Normative evaluations of the goodness of the observed decisions processes are absent in several papers. This suggests that there may be a need to clarify the normative basis for research-based advice on decision-making.  相似文献

Selection problems are common in process engineering. In most cases, it is necessary to rank alternatives based on multiple criteria (e.g., cost, safety, environmental impact), which are often conflicting. In addition, some criteria may be fundamentally difficult to quantify due to data scarcity, in which case subjective assessments need to be used as a proxy. Decision analysis tools such as the analytic hierarchy process (AHP) are useful to ensure decision-making is done rationally. In this work, we propose a fuzzy AHP variant, wherein pairwise comparison of decision elements by domain experts is expressed with triangular fuzzy numbers. This approach allows the degree of confidence of the expert to be quantified explicitly; it also allows inconsistencies in judgment to be reconciled within the bounds of the fuzzy numbers to generate reasonable values for the weighting factors. We demonstrate the methodology on three case studies, involving the comparison of different types of chlor-alkali electrolytic cells, CO2 capture techniques in cement plants and wastewater treatment options for municipal wastewater.  相似文献

In Dutch external safety policy, the acceptance of risk for the population in areas surrounding hazardous substances establishments is based on a limit value for individual risk (IR). Additionally, changes to societal risk (SR) must be justified. A specific software program (SAFETI-NL) with the associated Reference Manual Bevi Risk Assessments (RIVM, 2009) is legally required for the calculation of IR and SR. This prescribed “Bevi calculation method” forms the basis for decisions with important consequences for industry, land use planning and the protection of citizens. It is important that the outcome of calculations made with the prescribed method can be relied upon when making decisions about land use planning that affects both industry and population. This is the subject of this paper.The prescribed calculation method has been evaluated by performing a case study. The evaluation focussed on risk modelling of a Boiling Liquid Expanding Vapour Explosion (BLEVE) at an LPG filling station, an incident type that plays a significant role in Dutch external safety. The risk modelling of the BLEVE with the prescribed calculation method was found to have a number of serious deficiencies. It is concluded that the prescribed calculation method yields no reliable perspective on the safety of production, use and storage of hazardous substances, nor of possibilities to increase safety.Decision making should not only depend on quantification of IR and SR. Improving the safety-relevance of the prescribed calculation method requires an increase of the number of dimensions of the outcome of risk calculations in order to make feedback possible. It is recommended to incorporate additional, safety-relevant information into planning and decision-making processes. It is envisaged that a more far-reaching change of Dutch QRA practice is needed (medium to long term). In this context, a number of interesting elements have been noticed in decision-making procedures in other EU Member States.  相似文献

The problem of less and/or even lack of information and uncertainty in modeling and decision making plays a key role in many engineering problems; so that, it results in designers and engineers could not reach to sure solutions for the problems under consideration. In this paper, an application of the fuzzy logic for modeling the uncertainty involved in the problem of pipeline risk assessment is developed. For achieving the aim, relative risk score (RRS) methodology, one of the most popular techniques in pipeline risk assessment, is integrated with fuzzy logic. The proposed model is performed on fuzzy logic toolbox of MATLAB® using Mamdani algorithm based on experts' knowledge. A typical case study is implemented and a comparison between the classical risk assessment approach and the proposed model is made. The results demonstrate that the proposed model provides more accurate, precise, sure results; so that, it can be taken into account as an intelligent risk assessment tool in different engineering problems.  相似文献

Management of safety, and barriers in particular, includes using information expressing performance, i.e. use of safety performance indicators. For this information to be useful, the indicators should demonstrate adequate quality. In other words, they should satisfy some predefined set of quality criteria. Without showing adequate quality, the indicators are generally unable to provide sufficient support for barrier management, which could result in poor decisions. In this article, the use of the SMART criteria is considered to assess the quality of safety performance indicators in process industries. SMART being an acronym for ‘specificity’, ‘measurability’ or ‘manageability’, ‘achievability’, ‘relevancy’ and ‘time-based’, covering five key aspects and criteria for assessing the quality of an indicator. A discussion on whether the indicators are able to demonstrate adequate quality by satisfying these criteria has been conducted. The finding is that all of the SMART criteria should be satisfied for a safety performance indicator to demonstrate acceptable quality and to be regarded as useful to support barrier management decision-making. However, it has also been observed that including the ‘M’ criterion in the assessment of quality is not needed. When all the other criteria are satisfied there is no way the conclusions could be misleading as a result of measurability or manageability aspects. Hence, for safety performance indicator quality, only four of the criteria are assessed and suggested for such situations to shorten the acronym to ‘STAR’. A key safety indicator used in downstream process facilities, i.e. ‘dangerous fluid overfilling events’, motivated from the 2005 Texas City refinery accident, is used to illustrate the situation. The indicator is also applied to another incident, the Buncefield oil storage depot's accident in 2005, to provide a broader context for using it. The findings in this article could also be applied beyond the context studied. This means that, despite focusing on safety indicators in the process industries, the findings are considered as relevant and applicable to other types of performance indicators and to other energy industries.  相似文献

PROBLEM: A number of studies have found that use of in-car phones by drivers can interfere with the cognitive processing necessary for making appropriate and timely vehicle control decisions. However, the specific linkage between communication-based distraction and unsafe decision-making has not been sufficiently explored. METHOD: In a closed-course driving experiment, 39 subjects were exposed to approximately 100 gaps each in a circulating traffic stream of eight vehicles on an instrumented test track that was wet about half the time. The subjects were at the controls of an instrumented car, which was oriented in a typical left-turn configuration (traffic-crossing situation in North America) and with parking brake on and the transmission in neutral. The subjects were instructed to press on the accelerator pedal when they felt that a gap was safe to accept. Their performances were monitored and incentives were provided for balancing safe decision-making with expeditious completion of the task. For half of the gap exposures (randomly assigned), each subject was required to listen and respond to a complex verbal message. RESULTS: When not distracted, the subjects' gap acceptance judgment was found to be significantly influenced by their age, the gap size, the speed of the trailing vehicle, the level of "indecision," and the condition of the track surface. However, when distracted, the subjects did not factor pavement surface condition into the decision process. On wet pavement, the subjects were judged to have initiated twice the level of potential collisions when distracted by the messages that they did when not distracted. DISCUSSION: Listening/responding to verbal messages may reduce the capacity of drivers to process adequately all the important information necessary for safe decision-making. The effects of the messages in our study seemed to cause the subjects to misjudge gap size and speed information when operating under the additional disadvantage of adverse pavement condition. SUMMARY: Attention to complex messages while making decisions about turning through gaps in an on-coming vehicle stream was associated with significantly increased unsafe decision making by subjects in our experiment when the additional complexity of wet surface condition was introduced. IMPACT ON INDUSTRY: While the results reflected a somewhat artificial situation where the measure was signaled intention to act rather than the act itself, nevertheless, they do strongly suggest a scenario in which mental distraction could contribute to crash risk. With the rapid proliferation of telematics in the vehicle market, even with the laudable objectives represented by the Intelligent Transportation Systems initiative, there is a danger of the primary task of the driver being subordinated to a perceived need to enhance information flow to/from the external "world." Industry and governments need to work together to ensure that apparently desirable in-vehicle communication improvements do not compromise safety.  相似文献

The growing scale and complexity of process industries have brought safety, health, and environmental issues to the forefront. As a result, proactive risk reduction strategies (RRSs) are commonly employed to address these issues by reducing the frequency or mitigating the consequences of potential incidents. Among these strategies, inherent safety, which is a proactive measure of loss prevention and risk management, is considered to be the most effective method. This review aims to provide a comprehensive analysis of RRSs for achieving inherency, as well as techniques for evaluating the performance of inherent safety, health, and environmental aspects. Background information is presented, including the development and implementation of the inherently safer process design, as well as the approaches for achieving inherently healthier and environmentally friendlier processes. Subsequently, the execution approaches and practical applications of other RRSs are discussed to highlight the distinctiveness and benefits of inherent safety. Next, this study examined the characteristics of inherency assessment tools (IATs) based on available information at different process stages. Furthermore, the evaluation methods and historical development of IATs are investigated from the perspectives of safety, occupational health, and environmental considerations, followed by a statistical analysis of IATs. It is concluded that the no-chemical hazards-based IATs have not been extensively studied yet, which may improve the safety level of process plants from the perspective of comprehensive inherency risk reduction. As a way forward, future research opportunities are proposed to promote the implementation of greater optimized risk management.  相似文献

围绕基于模板的规划、组织决策协调与基于Agent系统的协调机制以及马尔可夫决策规划等应急决策的关键理论与方法,对国内外相关研究进行分析与总结;提出以决策理论规划为应急决策研究的主要建模和分析框架;采用逻辑程序与规划相结合的思想,研究基于应急预案模板的应急决策规划方法;基于应急处置任务的时间、资源约束关系,研究应急处置任务的多Agent马尔可夫决策建模及求解方法。基于决策理论规划的应急决策理论可以很好地用于应急决策的多阶段动态过程建模,而且能够利用预案模板降低模型求解的难度。上述研究完善和丰富了应急决策的理论和方法,为科学的应急决策实践以及应急决策支持系统提供了一种理论和方法。  相似文献
