INTRODUCTION/PROBLEM: Property damage incidents, workplace injuries, and safety programs designed to prevent them, are expensive aspects of doing business in contemporary industry. The National Safety Council (2002) estimated that workplace injuries cost $146.6 billion per year. Because companies are resource limited, optimizing intervention strategies to decrease incidents with less costly programs can contribute to improved productivity. METHOD: Systematic data collection methods were employed and the forecasting ability of a time-lag relationship between interventions and incident rates was studied using various statistical methods (an intervention is not expected to have an immediate nor infinitely lasting effect on the incident rate). RESULTS/SUMMARY: As a follow up to the initial work, researchers developed two models designed to forecast incident rates. One is based on past incident rate performance and the other on the configuration and level of effort applied to the safety and health program. Researchers compared actual incident performance to the prediction capability of each model over 18 months in the forestry operations at an electricity distribution company and found the models to allow accurate prediction of incident rates. IMPACT ON INDUSTRY: These models potentially have powerful implications as a business-planning tool for human resource allocation and for designing an optimized safety and health intervention program to minimize incidents. Depending on the mathematical relationship, one can determine what interventions, where and how much to apply them, and when to increase or reduce human resource input as determined by the forecasted performance.  相似文献

Promoting occupational safety and health in Hong Kong,Special Administrative Region of China is an important and ongoing mission. As the major organization with statutory responsibilities,the Occupational Safety and Health Council understand the importance to strengthen and cultivate our safety culture. It is widely believed that numbers of occupational related diseases and injuries could be prevented with the improvement of the awareness and attitudes of the employees and the public. Therefore,a comprehensive and in-depth study to monitor the occupational health and safety level and status of the community and working population is needed. Objectives: Our Council has developed the Occupational Safety Culture Index ( OSCI) to measure the current level of community and workplace safety and health awareness,knowledge and attitude. Benchmarking measures of the key safety performance indicators are to be derived thereof. Methods: A territory-wide random telephone survey was conducted to assess the community and employees’awareness,attitude and knowledge in 2008. A structured questionnaire was designed with the content validity and reliability assessed before the survey administration. A series of quality control approaches were also applied to assure the quality of the fieldwork and the reliability of the data. Results: 1,531 eligible participants’data were collected and computed into 2 types of composite indices,Occupational Safety Culture Index ( Community) ( OSCIC) and Occupational Safety Culture Index (Workplace) (OSCIW) . With the maximum score of index at 100,the overall score of OSCIC is 66. 9 and the OSCIW is 61. 3 in Hong Kong. Achievements: OSCI served as an effective management tool to measure the safety culture in Hong Kong. With a representative sample and high quality study control and validated assessment approaches,the OSCI and the sub-indices are reliable indicators to assess the effectiveness of safety culture enhancement strategy and the OSH intervention measures.  相似文献

This paper compares calculated results for two-phase flows through safety relief valves and pipes using the TPHEM, CCFLOW and RRERSP computer programs. These studies were conducted to locate errors in the programs as well as to further our understanding of how each program worked. For most low-to-moderate viscosity flow examples, and for the frozen flow examples, the program results agreed despite differences in the calculation methods. Current thinking is that the TPHEM computer program gets better results for high viscosity flows through safety relief valves (nozzles) and for pipe down flows. This is because this program successively iterates nozzle flows to achieve results consistent with the choke pressure, temperature and quality and successively iterates pipe flows to achieve results consistent with the pipe length. Safety relief valve flows from the RRERSP computer program are reduced by the inlet pipe non-recoverable pressure loss. This effect is significant for several cases.  相似文献

Introduction: FOCUS, the Fire Service Organizational Culture of Safety survey, has evolved from a research to practice enterprise within the United States fire and rescue service. The FOCUS tool was developed through a FEMA Assistance to Firefighters Research & Development grant. Then it moved to practice in the field. To date over 35,000 firefighters have participated. A current FEMA Fire Prevention & Safety grant can support FOCUS assessment in up to 1,000 fire departments, with the potential of nearly 120,000 respondents. With each funding cycle, the goal of the FOCUS program is to grow and measure its research to practice impact. Methods: We describe how FOCUS safety culture results are disseminated to fire service stakeholders. By utilizing customized reports and a training curriculum we demonstrate how FOCUS is moving research to practice by: (1) illustrating how survey results can be delivered effectively to practitioners, (2) providing examples of how fire departments are using results, and (3) sharing the reactions of the fire service to the FOCUS instrument, reports, and our flagship data training curriculum – Culture Camp. Results' Conclusions: Qualitative and quantitative data are analyzed to demonstrate the impact and acceptance of the FOCUS report and Culture Camps. Stakeholders reflect on the report and the experience of having quantitative safety culture data. Culture Camps are evaluated qualitatively and quantitatively using a matching game exercise, pre/post-test, a fire department teach back, and a Qualtrics evaluation. Practical Applications: Traditionally, the fire service has focused on reducing negative safety outcomes. FOCUS is helping shift their attention further upstream in the prevention pathway through the measurement of important organizational outcomes. The research to practice evolution of the FOCUS program may hold utility for other occupational groups when considering how to steadily move occupational health and safety research to practice in the field for measurable impact.  相似文献

PROBLEM: Mining in the United States remains one of the most hazardous industries, despite significant reductions in fatal injury rates over the last century. Coal mine fatality rates, for example, have dropped almost a thousand-fold since their peak in 1908. While incidence rates are very important indicators, lost worktime measures offer an alternative metric for evaluating job safety and health performance. The first objective of this study examined the distributions and summary statistics of all injuries reported to the Mine Safety and Health Administration from 1983 through 2004. Over the period studied (1983-2004), there were 31,515,368 lost workdays associated with mining injuries, for an equivalent of 5,700 person-years lost annually. The second objective addressed the problem of comparing safety program performance in mines for situations where denominator data were lacking. By examining the consequences of injuries, comparisons can be made between disparate operations without the need for denominators. Total risk in the form of lost workday sums can help to distinguish between lower- and higher-risk operations or time periods. METHOD: Our method was to use a beta distribution to model the losses and to compare underground coal mining to underground metal/nonmetal mining from 2000 to 2004. RESULTS: Our results showed the probability of an injury having 10 or more lost workdays was 0.52 for coal mine cases versus 0.35 for metal/nonmetal mine cases. In addition, a comparison of injuries involving continuous mining machines over 2001-2002 versus 2003-2004 showed that the ratio of average losses in the later period to those in the earlier period was approximately 1.08, suggesting increasing risks for such operations. DISCUSSION: This denominator-free safety measure will help the mining industry more effectively identify higher-risk operations and more realistically evaluate their safety improvement programs. IMPACT ON INDUSTRY: Attention to a variety of metrics concerning the performance of a job safety and health program will enhance industry's ability to manage these programs and reduce risk.  相似文献

Over the years, a number of high-profile laboratory accidents involving severe injuries, fatalities, and economic losses have been reported, prompting a significant increase in efforts towards laboratory safety. However, the dominant safety measures rely excessively on add-on safeguards such as sprinklers and respirators and pay little attention to reducing the hazardous factors at their sources. This study introduced the inherent safety concept to minimize laboratory hazards and developed a dedicated implementation tool called Generic Laboratory Safety Metric (GLSM). The Traditional Laboratory Safety Checklist (TLSC) was first used to represent the safety indicators, and then the Precedence Chart (PC) and Bayesian Networks (BN) methods were used to reconcile the safety indicators to develop the GLSM. The developed GLSM was subsequently demonstrated through a case study of a university laboratory. The results revealed that the safety level increased from 2.44 to 3.52 after the risk-based inherently safer retrofitting, thus creating laboratory conditions with a relatively satisfactory safety level. This work presented a set of generic solutions to laboratory retrofitting towards inherent safety with a novel GLSM as the implementation tool. The proposed GLSM would contribute to risk quantification and identification of key risk factors for assigning targeted and fundamental safety measures to achieve inherently safer laboratories.  相似文献

This article reviews the procedures and results of 28 different programs at nine work settings that increased employees' use of vehicle safety belts from a minimum gain of 12% over baseline to a maximum gain of 285% . A total of 244,543 vehicle observations were made to assess shoulder belt use before. during, and after the various programs. Several program evaluations included follow-up observations from 6 months to 18 months after the safety belt campaign ended. Four basic types of employee programs were compared with regard to both immediate and long-term impact: (a) direct and immediate rewards, (b) direct and delayed rewards, (c) indirect and delayed rewards, and (d) awareness/commitment strategies that involved no extrinsic rewards. Each program was practical for its particular location and substantially increased the use of safety belts by targeted employees. Safety belt use declined markedly when the programs were terminated, but postprogram levels rarely got as low as preprogram baseline. Consistent with theories of intrinsic motivation and minimal justification, the amount of residual impact was greater for those programs that did not involve extrinsic rewards. Five conclusions are particularly important and provocative: (a) Safety belt use at corporations and institutions can be prominently increased by practical cost-effective procedures; (b) significant residual effects of safety belt promotion remain long after program termination; (c) gains in safety belt use can be increased with intermittent programs; (d) further research is needed to determine optimal scheduling of various program strategies for response maintenance and generalization; and (e) the advantages and disadvantages of using extrinsic rewards compared to no rewards for safety belt promotion require additional study.  相似文献

A major chemical company established a formal incident investigation and reporting system several years ago. The original system focused heavily on worker-related injuries, illnesses, and near-misses and was used primarily to track statistics reportable to the Occupational Safety and Health Administration (OSHA). This Occupational Injury and Illness (OII) approach has been recognized to be an ineffective tool for measuring, predicting, and preventing process safety incidents. The Center for Chemical Process Safety (CCPS) recently published guidelines on how to establish safety metrics for the measurement and reduction of process safety incidents. The process safety metrics approach relies upon leading and lagging metrics to improve organization process safety. This paper is a case study of the analysis of one organization’s incident database, which represents approximately five years of data from over a dozen facilities. The aim of this investigation was to extract useful process safety metrics from the incident investigation and reporting system, which would be pertinent to the types of process units and process functions at these facilities. This paper will discuss the approach taken to extract process incident information from an OII-based database and present the difficulties of performing an analysis on such a database. This paper provides guidance on how to migrate an existing OII-based reporting system to a program that includes process safety metrics in accordance with industry best practices.  相似文献

The U.S. National Institute for Occupational Safety and Health (NIOSH) conducts research to improve and protect the health and safety of workers. This paper describes the experience of the NIOSH Construction Program with two recent program planning initiatives intended to improve the program: (a) an independent external review of work over the past decade and (b) the development of strategic goals organized into a “National Construction Agenda” to guide a decade of future work. These goals, developed with input from construction industry stakeholders and researchers, are a part of the NIOSH National Occupational Research Agenda (NORA) initiative. The NORA goals are intended to provide an ambitious set of goals for all construction stakeholders to work together on. Both efforts relate to insuring the relevance and impact of research, reflecting an emerging policy perspective that research programs should be judged not just by the quality and quantity of science produced, but by the industry impact and tangible benefit resulting from the research. This paper describes how views on research planning have evolved to incorporate lessons learned about how research leads to improved safety and health for workers. It also describes the process used to develop the goals and the resulting strategic and intermediate goals that comprise the National Construction Agenda.  相似文献

Problem: Safety management literature generally categorizes key performance indicators (KPIs) as either leading or lagging. Traditional lagging indicators are measures related to negative safety incidents, such as injuries, while leading indicators are used to predict (and therefore can be used to prevent) the likelihood of future negative safety incidents. Recent theory suggests that traditional lagging indicators also possess characteristics of leading indicators, and vice versa, however empirical evidence is limited. Method: The current research investigated the temporal relationships among establishment-level injuries, near misses, and fatal events using injury and employment data from a sample of 24,910 mining establishments over a 12-year period. Results: While controlling for employee hours worked, establishment-level reported injuries and near misses were associated with of future fatal events across the sample of mines and over the time period studied. Fatal events were also associated with increases in future reported near misses, providing evidence of a cyclic relationship between them. Discussion: These findings challenge the strict categorization of injuries, near misses, and fatal events as lagging indicators. Practical applications: Understanding the KPIs that should be used to manage organizational safety, and how they can be used, is of critical practical importance. The results of the current study suggest that, depending on several considerations, metrics tied to negative safety incidents may be used to anticipate, and possibly prevent, future negative safety events.  相似文献

INTRODUCTION: While smoke alarm installation programs can help prevent residential fire injuries, the costs of running these programs are not well understood. METHOD: We conducted a retrospective cost analysis of a smoke alarm installation program in 12 funded communities across four states. Costs included financial and economic resources needed for training, canvassing, installing, and following-up, within four cost categories: (a) personnel, (b) transportation, (c) facility, and (d) supplies. RESULTS: Local cost per completed home visit averaged 214.54 dollars, with an average local cost per alarm installed of 115.02 dollars. Combined state and local cost per alarm installed across all four states averaged 132.15 dollars. For every 1% increase in alarm installation, costs per alarm decrease by 1.32 dollars. CONCLUSIONS: As more smoke alarms are installed, the average installation cost per alarm decreases. By demonstrating effective economies of scale, this study suggests that smoke alarm programs can be implemented efficiently and receive positive economic returns on investment.  相似文献

PROBLEM: After decreasing to a historic low in 1997, motorcycle crash-related fatalities are increasing. Although causes remain unclear, motorcycle rider education and licensing play key roles in reducing motorcycle crashes and injuries. Yet, little is known about what constitutes effective rider training and licensing. This study develops a model of best practices in motorcycle rider education and licensing and combines primary and secondary data to identify states that most closely adhere to this model. Evidence on the validity of the model is also examined. METHOD: States were rated along three areas of best practices: (a) program administration; (b) rider education; and (c) licensing based on 2001 data collected for a National Highway Traffic Safety Administration (NHTSA)-sponsored study. RESULTS: Results indicate wide variation in states' adherence to best practices; several states meet most, others very few. When the areas of best practices are considered separately, a state tends to behave similarly on all three. Initial evidence supports the validity of the model, with high best practices states having the lowest rates of motorcycle fatalities. IMPACT ON TRAFFIC SAFETY: As motorcycle-related crashes increase and state and federal support for rider education programs diminishes, it is critical that states identify deficiencies in their program and learn from successful states about efficient, cost-effective strategies for increasing best practices in motorcycle rider education and licensing.  相似文献

Abstract

Objective: There are a plethora of child bicycle safety education programs worldwide. However, the content and durations vary widely from program to program and no gold standard has been established. The main objective of this project was to create an inventory of youth bicycle safety education programs and their content, approaches, and age and developmental considerations.

Methods: Methods used to gather program information included web and online database searches, extraction from personal lists, and a widely distributed online survey. A total of 89 programs met inclusion criteria and were reviewed (78 from the United States and 11 from other countries).

Results: Over half (56.2%) of the included programs were not explicitly age specific, meaning that the content was not appropriately designated by age and developmental cut points. Many programs had target age groups, but these were often very wide age ranges (e.g., all elementary school–age children) resulting in content too advanced for the youngest children. Only 12.4% of included programs had published program evaluations and their scientific rigor varied widely. Main results from this inventory revealed a weak evidence base for program effectiveness due to lack of program evaluations. Results also highlighted a need for age, development, and skill-level considerations in youth bicycle education programs.

Conclusions: This study provides a comprehensive list of program components and approaches that can be used as a foundation upon which child bicycle education standards can be evaluated and built, with the long-term goal of increased childhood safety.  相似文献

BACKGROUND: Safety teams have become a popular means to recognize and prevent injuries in the workplace. In fact, organizations, such as OSHA, NIOSH, NIEHS, DOE, and the Ohio BWC, not only encourage safety teams, but have implemented them in their organizations. However, safety teams may not be legal as defined by NLRB Act Sections 2(5) and 8(a)(2). OBJECTIVE: To determine whether safety teams are illegal labor organizations. METHODS: A review of government regulations, safety programs, and safety teams was performed to create a framework of what types of safety programs/regulations are requested and/or required by a variety of government agencies. Next, these requirements were compared to existing case law on illegal labor organizations as defined by the NLRB. RESULTS: Most safety teams and programs, even those implemented by the Federal Government, are illegal labor organizations as defined by NLRB Act Sections 2(5) and 8(a)(2). CONCLUSIONS: The Federal Government needs to amend existing legislation and develop a revision of the NLRB Act that would allow safety teams to function legally in organizations without having to meet intrusive, constricting guidelines that may inhibit the value of safety teams. IMPACT ON INDUSTRY: Organizations must be aware that safety teams may be valuable in reducing job-related accidents and injuries but they may be illegal labor organizations.  相似文献

Safety climate, the objective measurement of attitudes and perceptions toward Occupational Health & Safety (OH&S) issues, has been largely ignored and measures such as lost time and frequency rate have been used to determine the efficacy of OH&S programs. Yet, it is clear that measuring the precursors of accidents identified in a safety climate analysis provides a powerful proactive management tool. This paper reports on safety climate factors found in clerical and service organizations in Australia. Concurrent validity studies are reported. In contradistinction to published studies, it is argued that safety climate factors are not stable across organizations. The significance of this for planning and assessing the efficacy of OH&S programs is discussed. It is concluded that although safety climate factors are not stable, the safety climate factor pattern identified in organizations provides a powerful management tool for designing OH&S programs.  相似文献

Problem: Safety management programs (SMPs) are designed to mitigate risk of workplace injuries and create a safe working climate. The purpose of this project was to evaluate the relationship between contractors’ SMPs and workers’ perceived safety climate and safety behaviors among small and medium-sized construction subcontractors. Methods: Subcontractor SMP scores on 18 organizational and project-level safety items were coded from subcontractors’ written safety programs and interviews. Workers completed surveys to report perceptions of their contractor’s safety climate and the safety behaviors of coworkers, crews, and themselves. The associations between SMP scores and safety climate and behavior scales were examined using Spearman correlation and hierarchical linear regression models (HLM). Results: Among 78 subcontractors working on large commercial construction projects, we found striking differences in SMP scores between small, medium, and large subcontractors (p < 0.001), related to a number of specific safety management practices. We observed only weak relationships between SMP scales and safety climate scores reported by 746 workers of these subcontractors (β = 0.09, p = 0.04 by HLM). We saw no differences in worker reported safety climate and safety behaviors by contractor size. Discussion: SMP only weakly predicted safety climate scales of subcontractors, yet there were large differences in the quality and content of SMPs by size of employers. Summary: Future work should determine the best way to measure safety performance of construction companies and determine the factors that can lead to improved safety performance of construction firms. Practical applications: Our simple assessment of common elements of safety management programs used document review and interviews with knowledgeable representatives. These methods identified specific safety management practices that differed between large and small employers. In order to improve construction safety, it is important to understand how best to measure safety performance in construction companies to gain knowledge for creating safer work environments.  相似文献

PROBLEM: Safety in the construction industry is a major issue in Hong Kong, representing about 46% of all occupational injuries in 1998. This study explored linear and curvilinear relations between age and safety performance (accident rates and occupational injuries), as well as safety attitudes, in construction workers in Hong Kong. METHOD: A Chinese version of the Safety Attitude Questionnaire (SAQ by Donald & Canter) was developed and administered to a sample of Chinese construction workers (N=374, 366 males, 8 females) from 27 construction sites. RESULTS: Accident rates were not related to age. Occupational injuries were related to age in a curvilinear manner, with injuries at first increasing with age, then decreasing. Two safety attitude scales were related to age with older workers exhibiting more positive attitudes to safety. If age and tenure are controlled, some attitude scales are predictors of safety performance. IMPACT ON INDUSTRY: Management/supervisors, team leaders, and workers are all responsible for safety, and any negative bias toward older construction workers is unfounded.  相似文献

Employer commitment is a key factor in an effective safety program, yet limited research has focused on the safety priorities of retail store managers. To address this, the U.S. National Institute for Occupational Safety and Health recruited 4 experienced ergonomists, who met and interviewed 9 retailers in different parts of the eastern USA. The reports from the 9 interviews were used to document the hazards facing retailers and the interventions they attempted. Those interviewed were managers/owners of establishments that ranged from a small bakery with 11 employees to a supermarket with 85 or more employees. The main hazards across all establishments included overexertion, contact-with-objects, and falls-to-the-same-level. We also compared the retailers’ perceptions of safety hazards with injuries from actual hazards as supplied by the U.S. Bureau of Labor Statistics. This report provides insight into the retailers ‘ perceptions of safety hazards as well as their commitment to the prevention of workplace injuries.  相似文献

Legislation regarding seat belt use in Canada is a provincial/territorial responsibility. Each of the 13 jurisdictions has enacted legislation and set the penalties regarding non-use of seat belts and appropriate child restraint systems. The federal government regulates occupant restraint systems and child restraints. In addition, Transport Canada (TC) gathers annual survey data on the use of seat belts and child restraints on Canadian roads and provides research support. National coordination toward the Canadian Road Safety Vision 2010 goal of a 40% reduction in fatalities and serious injuries related to non-belt/child restraint use and a 95% restraint usage rate is provided by the Canadian Council of Motor Transport Administrators' (CCMTA) National Occupant Restraint Program (NORP). The paper examines the history of legislation, provincial/territorial penalties, NORP, and TC involvement in promoting the use of occupant restraints.  相似文献

Problem

Safety culture relates to injuries and safety incidents in organizations, but is difficult to asses and measure. We describe a preliminary test of assessing an organization's safety culture by examining employee interpretations of organizational safety artifacts (safety signs).

Method

We collected data in three organizations using a new safety culture assessment tool that we label the Safety Artifact Interpretation (SAI) scale; we then crossed these data with safety climate and leadership evaluations.

Results

SAI were interpreted by employees in accordance with two conceptually distinct themes that are salient in the literature on organizational safety culture: safety compliance and commitment to safety. A significant correlation exists between SAI scores and the organizational safety climate. A similar (though insignificant) relationship was observed between SAI scores and leadership ratings.

Impact on industry

Employee perceptions and interpretations of safety artifacts can facilitate assessments of safety culture and can ultimately lead to understanding of and improvements in the level of organizational safety.  相似文献
