This paper concerns a procedure to quantify the risk of occupational accidents based on Fuzzy Logic approach, named Fuzzy Application Procedure (FAP). FAP was developed as a support tool, after the method based on fuzzy logic was developed and validated. The methodology and FAP support tool were found to be able to quantitatively assess the risk of occupational accidents for different industrial and site activities and to identify the most efficient intervention measures that can be taken to reduce risks. The application of FAP to two Italian industrial plant allowed the validation of the procedure. It can be seen how FAP should be considered easy to use for any type of company, with the only requirement of having a sufficient and homogeneous number of accidents recorded, so as to be able to correctly tune the reference parameters of the fuzzy methodology. 相似文献

为评价某区域或某设备潜在的毒气泄漏事故场景的风险水平,以对不同的风险等级进行风险控制和安全规划,详细介绍了工业事故风险评估方法ARAMIS所采用的风险严重度指数法.首先基于事故频率和后果的风险矩阵选取毒气泄漏事故场景;然后运用毒气当量浓度计算任意暴露时间下不同风险严重度等级所对应的特征距离,并根据同一风险等级内风险严重度指数与距离的线性关系计算任意点的风险严重度指数;最后应用1个实例分析了考虑所研究区域的风向概率后对风险严重度指数的真实影响,便于工厂或企业识别不同的风险等级,进行不同场景下风险水平的对比性研究,为其安全设计及风险分析提供了一种新的评估方法. 相似文献

PROBLEM: Prior research has demonstrated that suspended/revoked drivers pose a significant traffic risk, but until now little has been known about whether, and if so how, this risk varies as a function of the reason for suspension/revocation. METHOD: This study classifies suspended/revoked drivers into subgroups based on their reason for suspension/revocation, and then develops demographic and driving record profiles for each group. Separate driving record profiles are developed for the following traffic safety indicators, measured 3 years prior to the suspension/revocation action: (a) total crashes, (b) fatal/injury crashes, (c) total traffic convictions, and (d) total incidents (crashes + convictions). RESULTS: The findings clearly show that: (a) suspended/revoked drivers are a heterogeneous group, both demographically and in their driving behavior; (b) some suspended drivers, such as those suspended/revoked for a non-driving offense, have low traffic risks that are not much higher than those of validly-licensed drivers, and; (c) all suspended groups have elevated crash and conviction rates, compared to validly-licensed drivers. DISCUSSION: The implications of these findings for current laws and policies targeting suspended/revoked drivers are discussed, and recommendations for improving these laws/policies are presented. 为提高我国城市燃气风险管理水平,帮助风险管理者科学地分配维护资源,探讨提出了我国城市燃气事故生命损失风险可接受标准;采用AIR指标法确定了个人风险可接受标准范围为(2.397 3×10-7,4.794 7×10-7);运用F-N曲线法结合ALARP原则,确定了社会风险可接受标准,最大可接受风险的截距为4.794 7×10-7,可忽略风险的截距为4.794 7×10-8;利用生活质量指数推导模型,计算了达到城市燃气事故可忽略风险水平的最优安全投入成本;基于风险动态原则,分析提出了风险可接受标准的更新办法。研究结果表明:我国城市燃气行业可接受风险水平低于煤矿、大坝、化工等危险行业的可接受风险水平;虽然我国城市燃气事故死亡率逐年降低,但要达到可忽略的风险水平,每年还需大量安全资金投入。 相似文献

Due to rapid industrialization, with high population density and constraints of land, it is expected that level of risks arising from the hazardous industries will increase in India in the coming decades. However, 30 years after the Bhopal accident (1984), except a few discrete regulations, there is as yet no integrated system for assessing and managing risks arising out of these hazardous industries in India. The gravity of aspects related to the management of industrial risk still remains crucially important. In particular, there is no standard guideline on risk analysis methodology, acceptability or tolerability criteria, nor is there an accident database or a risk reduction strategy for the areas where risk levels are already high. On top of this, there are technical and legislative gaps in the institutional framework to implement any of the above mentioned issues. With the backdrop of the Bhopal gas tragedy, the objective of this paper is therefore to evaluate the effectiveness of a comprehensive risk assessment framework for the emerging economy of India, in order to control and/or to reduce the risk level that exists. Most of the available risk management methods are not directly applicable to academic research laboratories. One solution to systematically perform risk analyses in this environment is the Laboratory Assessment and Risk Analysis (LARA) method. This method was developed to allow untrained personnel to identify of possible risks and rank them according to their importance. The purpose of this study was to find out, if this method can be used as a holistic risk management technique in different environments, and which are the differences when comparing the results to other, well established risk analysis techniques. The risk analyses were performed at two European universities and for various procedures. The results show, that the LARA procedure is more easily performable than the other methods and gives comparably adequate results. Being applicable by non-experts, this holistic risk analysis method for research laboratories can help to reduce the accident rate in the academic environment. 相似文献

PROBLEM: Research suggests that people who engage in risk-taking behaviors often hold specific beliefs that can mitigate or reduce their perceptions of risk associated with those behaviors. METHOD: A scale was developed (Speeding Risk Belief Scale (SRBS)) to assess beliefs about speeding-related risk and predict self-reported speeding in a random-digit telephone survey of 800 South Australian drivers between the ages of 16 and 50. RESULTS: The scale was internally consistent, and path analyses showed it to be associated with self-reported speeding, both directly and indirectly through participants' estimates of speeding-related risk. DISCUSSION: Origins of risk-mitigating beliefs and the extent to which they may be causally linked with speeding are discussed, and recommendations are made for future research. IMPACT ON INDUSTRY: This research has strong implications for the conduct of countermeasure campaigns that disseminate information on speeding-related risk. 相似文献

Reducing accident occurrence in petrochemical plants is crucial, thus appropriately allocating management resources to safety investment is a vital issue for corporate management as international competition intensifies. Understanding the priority of safety investment in a rational way helps achieve this objective.In this study, we targeted an acrylonitrile plant. First, Dow Chemical's Fire and Explosion Index (F&EI) identified the reaction process as having the greatest physical risk. We evaluated the severity of accidents in the reaction process using the Process Safety Metrics advocated by the Center for Chemical Process Safety (CCPS); however, this index does not express damages a company actually experience. To solve this problem, we proposed a new metric that adds indirect cost to CCPS metrics. We adopted fault tree analysis (FTA) as a risk assessment method. In identifying top events and basic events, we attempted to improve the completeness of risk identification by considering accidents from the past, actual plant operation and equipment characteristics, natural disasters, and cyber-attacks and terrorist attacks. Consequently, we identified the top events with high priority in handling because of serious accidents as fire/explosion outside the reactor, fire/explosion inside the reactor, and reactor destruction. The new CCPS evaluation index proposed in this study found that fire and explosion outside the reactor has the highest severity. We considered the creation of the fault tree (FT) diagram of the top event, estimating the occurrence probability, and identifying the risk reduction part and capital investment aimed at risk reduction. As an economically feasible selection method for risk reduction investment, using the difference in loss amounts before and after safety investments indicated investment priority. 相似文献

PROBLEM: Road traffic injury is the leading cause of death among adolescents in high-income countries. Researchers attribute this threat to driver risk taking, which driver education (DE) attempts to reduce. Many North American authorities grant DE graduates earlier access to unsupervised driving despite no evidence of this being a safety benefit. This theoretical article examines risk taking and DE in relation to an apparent mobility bias (MB) in policymaking. METHOD: The MB is defined, the history and sources of driver risk taking are examined, and the failure of DE to reduce collision risk is analyzed in relation to a potential MB in licensing policies. DISCUSSION: The author argues that DE's failure to reduce adolescent collision risk is associated with a MB that has produced insufficient research into DE programs and that influences public policymakers to grant earlier licensure to DE graduates. Recommendations are made regarding future research on DE and risk taking, coordinated improvements to DE and driver licensing, and a plan to reduce collision risk by encouraging parental supervision after adolescent licensure. IMPACT ON INDUSTRY: Research on adolescent driver risk taking would have direct applications in DE curricula development, driver's license evaluation criteria, graduated licensing (GDL) policies, as well as other aspects of human factor research into the crash-risk problem. 相似文献

In this research, a framework combining lean manufacturing principles and fuzzy bow-tie analyses is used to assess process risks in chemical industry. Lean manufacturing tools and techniques are widely used for eliminating wastes in manufacturing environments. The five principles of lean (identify value, map the value stream, create flow, establish pull, and seek perfection) are utilized in the risk assessment process. Lean tools such as Fishbone Diagram, and Failure Mode and Effect Analysis (FMEA) are used for risk analysis and mitigation. Lean principles and tools are combined with bow-tie analysis for effective risk assessment process. The uncertainty inherent with the risks is handled using fuzzy logic principles. A case study from a chemical process industry is provided. Main risks and risk factors are identified and analyzed by the risk management team. Fuzzy estimates are obtained for the risk factors and bow-tie analysis is used to calculate the aggregated risk probability and impact. The risks are prioritized using risk priority matrix and mitigation strategies are selected based on FMEA. Results showed that the proposed framework can effectively improve the risk management process in the chemical industry. 相似文献

Operating several assets has resulted in more complexity and so occurrence of some major accidents in the refining industries. The process operations risk factors including failure frequency and the consequence components like employees' safety and environment impacts, operation downtime, direct and indirect cost of operations and maintenance, and mean time to repair should be considered in the analysis of these major accidents in any refinery. Considering all of these factors, the risk based maintenance (RBM) as a proper risk assessment methodology minimizes the risk resulting from asset failures. But, one of the main engineering problems in risk modeling of the complex industries like refineries is uncertainty due to the lack of information. This paper proposes a model for the risk of the process operations in the oil and gas refineries. The fuzzy logic system (FLS) was proposed for risk modeling. The merit of using fuzzy model is to overcome the uncertainty of the RBM components. This approach also can be accounted as a benchmark for future failures. A unified risk number would be obtained to show how the criticality of units is. The case study of a gas plant in an oil refinery is performed to illustrate the application of the proposed model and a comparison between the results of both traditional RBM and fuzzy method is made.For the case study, 26 asset failures were identified. The fuzzy risk results show that 3 failures have semi-critical level and other 23 failures are non-critical. In both traditional and fuzzy RBM methods, some condenser failures had the highest risk number and some pumps were prioritized to have the lowest risk level. The unit with unified risk number less than 40 is in the non-critical conditions. Proposed methodology is also applicable to other industries dealing with process operations risks. 相似文献

This industry along with fertilizer plants, petrochemical plants, etc., which handle hazardous chemicals, are potential targets for deliberate actions by terrorists, criminals and disgruntled employees. The process industries face different levels of threats. It is imperative to analyze the entire threat scenario before taking steps to counter it, otherwise each and every threat will have to be treated as most severe, thus resulting in a huge and wasteful expenditure.The Security Risk Factor Table (SRFT) and a Stepped Matrix Procedure (SMP) have been developed to assess the security risk of oil and gas industry as well as the other chemical process industries. While the SRFT deals with the effects of individual threats, the SMP deals with the cascading/domino effects which a lone, low probability event can cause. This paper provides a risk assessment method of sheltering in-place for high-pressure natural gas wells with hydrogen sulphide. In this paper, the shelter-in-place risk is estimated by integrating the health consequences of an individual taking one kind of emergency response to the emergency orders of sheltering in place from the emergency decision makers and the probability of the corresponding emergency response action. The probability of the corresponding emergency response action in the proposed method is estimated through the accident probability analysis and the probability analysis of taking a certain response action. The health consequence estimation is based on air exchange rate test of the shelter buildings as well as accident consequence calculation. The evaluation of shelter-in-place risks based on “as low as reasonably practicable (ALARP)” guidelines was employed to provide suggestions for emergency management under both normal conditions and off normal conditions. A case study of risk assessment of sheltering in the local residential houses in Xuanhan County of Sichuan Province, China was taken as an example to illustrate the proposed risk assessment process of shelter-in-place and its application in the decision-making process for emergency management. 相似文献

A novel method for dynamic risk assessment of oil spill accidents based on numerical simulation was presented in this paper. The dynamic risk assessment model was developed consisting of a comprehensive list of caused consequences like environmental damage, asset loss, health impact and social effect as well as emergency actions preventing these losses. Contributing events in the Mater Logic Diagram (MLD) of the dynamic risk assessment model were valued based on the simulated evolvement of oil contaminants under spill scenario on quasi-static fluid, which was obtained by coupling an oil spill model with hydrodynamic module in Jialing river of Three Gorges Reservoir in China after the impoundment of the reservoir to 175 m water level in 2010. Calculated result of dynamic risk as grade IV indicated that the assessed oil spill was not as catastrophic as we thought because of the slow transport of oil fractions on water surface and absence of dispersed oil in water body due to the quasi-static fluid, very gentle wind and effective emergency actions, as well as by the reason that rare agriculture or industrial crops exist in spill adjacent area. 相似文献

The liquid fuel safety issues on fuel storage, transportation and processing have gained most attention because of the high fire risk. In this paper, some 0# diesel pool fire experiments with different diameters (0.2–1 m) were conducted with initial fuel thicknesses of 2 cm and 4 cm, respectively, to obtain liquid fuel combustion characteristics. Some key parameters including mass burning rate, flame height and the flame radiative heat flux, associated with fire risk, were investigated and determined. Subsequently, a detail quantitative risk assessment framework for 0# diesel pool fire is proposed based on the 0# diesel burning characteristics. In the framework, the probability of personal dead and the facility failure are calculated by the vulnerability models, respectively. In the end, 10 special tank fire scenarios were selected to show the whole risk calculation process. The tank diameter and the distance to pool fires were paid more attention in the cases. The safety distances in the cases are provided for the persons and nearby facilities, respectively. The paper enriches the basic experimental data and the provided framework is useful to the management of 0# diesel tank areas. 相似文献

A previous study has shown that the useful visual field deteriorates in a simulated road traffic situation as a function of the driver’s age and of the vehicle’s speed under monotonous conditions [Rogé, J., Pébayle, T., Lambilliotte, E., Spitzenstetter, F., Giselbrecht, D., Muzet, A., 2004. Influence of age, speed and duration of monotonous driving task in traffic on the driver’s useful visual field. Vision Research 44 (23), 2737–2744]. The aim of this new experiment is to study the effects of traffic density and age on the useful visual field of the driver during a simulated driving task with controlled traffic characteristics (speed, number of cars) for all participants. In total, 10 young drivers (m = 28.2 years) and 10 older drivers (m = 51.2 years) followed a car in road traffic at an average speed of 126 km h−1 during two 2 h sessions corresponding to two conditions of traffic (light traffic, with five vehicles around the participant; and heavy traffic, with nine vehicles). While following this vehicle, the driver had to detect changes in the colour of a signal located in the central part of his or her visual field and a signal that appeared at different eccentricities on the rear lights of other vehicles in the traffic. Analysis of the data indicated that age interacted with the location of the peripheral signal and density of traffic interacted with the duration of driving. The implications of these results are discussed in terms of road safety and in terms of models of deterioration of the useful visual field (general interference and tunnel vision). 相似文献

Fire is the most prevalent accident in natural gas facilities. In order to assess the risk of fire in a gas processing plant, a fault tree analysis (FTA) and event tree analysis (ETA) has been developed in this paper. By utilizing FTA and ETA, the paths leading to an outcome event would be visually demonstrated. The framework was applied to a case study of processing plant in South Pars gas complex. All major underlying causes of fire accident in a gas processing facility determined through a process hazard analysis (PHA). Fuzzy logic has been employed to derive likelihood of basic events in FTA from uncertain opinion of experts. The outcome events in event tree has been simulated by computer model to evaluate their severity. In the proposed methodology the calculated risk has the unit of cost per year which allows the decision makers to discern the benefit of their investment in safety measures and risk mitigation. 相似文献
