Over the last few decades, the concept of inherent occupational health has gained increasing attention to reduce occupational hazards that may adversely impact workers’ health. In order to assess occupational hazards in the chemical process, different inherent occupational health assessment methods have been developed at the early stages of process development and design. The methods in the order of process information availability – ranging from the detailed piping and instrumentation diagrams to a simple sketch of process concepts are the: occupational health index (OHI), health quotient index (HQI) and inherent occupational health index (IOHI). This paper proposes systematic heuristic frameworks to assist process designers and engineers in assessing and reducing inherent occupational health hazards or risks based on process information availability. Strategies for reducing health hazards or risks in the OHI, HQI and IOHI methods based on inherently safer design (ISD) keywords of minimization, substitution, moderation and simplification are included in this study. It is worth mentioning that the proposed frameworks act as guidelines for design engineers in systematically selecting the appropriate index and methodology to assess and reduce health hazards/risks based on the availability of the process information. A case study is solved to illustrate the proposed framework.  相似文献

Many incident reports and much research literature have concluded that incident rates in high-hazard industries can be reduced by enhancing the knowledge and information sources available to workers. However, a sparsity of published research reporting workers' perspectives on the value of information and knowledge sources in different operational situations potentially limits the effectiveness of interventions aimed at improving human responses to process safety performance. In response, this work seeks to understand how frontline oil and gas industry workers, responsible for managing process safety and other risks in their hazardous workplaces, rely on their own knowledge, the knowledge of others and information provided to manage different operating scenarios. A set of open and closed-ended survey questions were distributed to workers at several upstream and downstream oil and gas companies. Statistical analysis of 133 responses revealed interesting insights relating to participants’ experience levels, decision making practices and perceptions on the importance of various information and knowledge sources in different operating circumstances. The data revealed differences in the sources of knowledge relied upon by experienced and less experienced workers. Experienced workers rely on their own knowledge in both routine and potentially high-impact scenarios. Less experienced workers also rely on their own knowledge for routine decisions however they reported seeking out external knowledge and information in potentially high-impact scenarios. The data suggests that interventions to improve safety by enhancing the availability of knowledge and information sources are less likely to affect experienced workers and alternate strategies might be needed. However, such interventions might be effective if they are focused on supporting newer workers.  相似文献

The risks of landing overrun (LDOR – LanDing OverRun), Take-off Overrun (TOOR – Take-Off OverRun) and landing undershoot (LDUS – LanDing UnderShoot) are dependent on multiple factors related to operating conditions. These include wind, runway surface conditions, landing or take-off distances required, the presence of obstacles, runway distance available, the existence and dimensions of runway safety areas.In this paper we propose risk models for runway overrun and landing undershoot, using a probabilistic approach. These models are supported by historical data on accidents in the area around the runway and will enable us to determine if the risk level is acceptable or whether action must be taken to mitigate such risks at a given airport. Furthermore, these models permit comparison of the results of different risk mitigation actions in terms of operational risk and safety.The principal advantage of this method is the high quality results obtained for a limited investment in terms of time, computing power and data. As such the method is extremely practical and easy to apply in aerodrome planning, development and operation.  相似文献

Inappropriate decisions are often regarded as causes of major accidents in the process industries. To improve the quality of decisions, it is important to make the right information available at the right time. The objective of this work is to investigate what types of risk information is needed for risk-related decisions in various decision-making processes. A framework is proposed to facilitate future research for easing information deficiency. In this paper, risk information is examined through common decision-making processes, and is identified serving to 1) detect and characterize risk-related decision problems, 2) indicate the severity and urgency of decisions, 3) state requirements and constraints of workable solutions, 4) represent attributes for comparing and evaluating solutions, and 5) act as rules to maintain safety or control risk. These usages of risk information in different decision problems imply the large diversity in information needs for decision-making. An adaptive information support is thus suggested to provide targeted risk information to specific decision-makers for effective and efficient decision-making in accident prevention in the process industries.  相似文献

The objective of this paper is to discuss the benefits and risks of providing drinkers with tools that allow them to estimate their blood alcohol concentration (BAC), and to examine the field usability of one commercially available tool. Drinking and driving laws are specified in terms of the driver's BAC, and there is concern that the absence of a method for drivers to accurately estimate their BAC level limits their ability to determine whether they can drive legally. A number of devices that provide a method for the individual to estimate or measure their BAC have been developed. Although some of these devices--such as the "know your limit" (KYL) cards--have been widely distributed, their effectiveness in encouraging good driving decisions have rarely been tested. This article describes a pilot study on the field usability of the Guardian Angel (GA) personal alcohol test in a field setting. The GA test analyzes saliva samples from drinking and indicates under which BAC category they fall (.00 -.04;.04 -.08;.08+). The research examined whether drinkers could, in natural drinking environments, correctly administer and interpret the test results. The methodology involves sampling drinkers on a weekend night on and around the grounds of a large West Coast university as they traveled between off-campus parties, bars, and their dorms. They were asked to assess their own intoxication and impairment, then self-administer and interpret the Guardian Angel test. After interpreting the test, participants were asked to reassess their intoxication and impairment levels, and were given a breath BAC test using a calibrated unit. The results revealed that although the majority of drinkers' were able to administer the GA test, their interpretations did not correspond with actual BACs. The interpretations of the GA test produced false-negatives, underestimating actual BACs. Drinkers perceived themselves to be less intoxicated, on average, after interpreting the GA test results. In conclusion, this research addresses potential pros and cons of providing BAC information to drinkers. It underscores the importance of testing BAC estimation tools under field conditions and the potential risks associated with tests that do not produce accurate results.  相似文献

Current available risk analysis techniques are well adapted to industry needs since they were developed for its purpose. All hazards present in industry are also met in research/academia, although quantities of some hazardous substances are smaller. Still, because of its characteristics e.g., high turnover of collaborators, rapid reorientation of research programs, freedom of research, equipment often in development stage, difficulty to obtain accidents statistics, not well described processes, etc., research/academia milieu is an environment whose risks are difficult to assess by available risk analysis techniques. In the present paper, a new methodology, Laboratory Assessment and Risk Analysis – LARA, for research and/or complex environment is proposed. When multiple hazards are analyzed, the result of assessment is a risk ranking calculated using a Lab Criticity Index – LCI, providing identification of critical areas and prioritization of safety actions. LCI is conceived through two approaches: the Risk Priority Number – RPN and the Analytic Hierarchy Process – AHP. It is suggested to estimate risk as a combination of severity, probability, detectability, worsening factors and research specificities.  相似文献

Pedestrians are involved in traffic accidents due to many reasons. It is generally thought that personal background of pedestrians has an effect on their involvement rate in the road traffic accidents. Identifying these characteristics would lead to a better understanding of pedestrian accident pattern so that the resources in the field of education, engineering, and enforcement could be used in better ways. This study attempts to test the hypothesis mentioned earlier. The investigated personal background includes the following characteristics: gender, type, age, nationality, and educational background. The data was reduced from vast number of pedestrian injury accident reports in the Kingdom of Bahrain. The actual accident records were categorized according to these characteristics and compared to their exposure risk. It was assumed that the exposure risk, which is the expected number of accidents for each category of the pedestrians, was in proportion to their presence in the pedestrian population. Another study was carried out in parallel to observe the pedestrian characteristics in Bahrain. The results of the two studies were analyzed statistically using Chi-square method to compare the actual to the expected accident frequencies. The whole Kingdom of Bahrain population statistics were used wherever the information on the pedestrian population was not available. The findings revealed that personal characteristics considered in this study have significant influence on pedestrian’s involvement in traffic accidents. The results also showed that pedestrians with the following characteristics were probably showing risk to exposure to accidents more than other categories: male, young (0–12 years) and old (50 years and over), non-local, and those with low educational background.  相似文献

This article analyses, using Bayesian networks, the circumstances surrounding workplace tasks performed using auxiliary equipment (ladders, scaffolding, etc.) that may result in falls. The information source was a survey of employees working at a height. We were able to determine the usefulness of this approach – innovative in the accident research field – in identifying the causes that have the greatest bearing on accidents involving auxiliary equipment: in these cases, the adoption of incorrect postures during work and a worker’s inadequate knowledge of safety regulations. Likewise, the duration of tasks was also associated with both these variables, and therefore, with the accident rate. Bayesian networks also enable dependency relationships to be established between the different causes of accidents. This information – which is not usually furnished by conventional statistical methods applied in the field of labour risk prevention – allow a causality model to be defined for workplace accidents in a more realistic way. With this statistic tool, the expert is also provided with useful information that can be input to a management model for labour risk prevention.  相似文献

Much research within risk has investigated risk perception and how risk behaviour is understood by the public. One goal of risk research is to understand how people perceive and interpret risk to facilitate safe behaviour. This is seen as important for many different reasons, one being because policy measures might be more effective when they address and understand individual differences in beliefs and perceptions of a target group (Steg, L., Sievers, I., 2000. Cultural theory and individual perceptions of environmental risks. Environment and Behavior 32 (2), 250–269). The main aim of this study was to investigate what is most important in regard to safety priorities. Three personality assets – anxiety, excitement-seeking and trust – were first examined. Further factors were driver optimism, worrying about transport risks, willingness to pay to increase safety, and negative attitudes toward traffic rules (as a driver). The results are based upon two questionnaire surveys carried out among a representative sample of the Norwegian public in 2004. The results showed that worry was the most important predictor of safety priorities. In addition, negative attitudes towards rules were also found to have an impact on priority. The proposed model explained 44% of the variance of safety priority. This knowledge gives additional information to improve the success of interventions because it will develop the ability to target those who consider safety to be of low priority and guide them to modify their attitudes. Despite recent major chemical process accidents in Japan, the top management teams of firms still avoid taking costly risk reduction measures because of their low perceived impact on firm performance. The disclosure of information on accident risks might motivate managers to enhance workplace safety because of the subsequent evaluation of firms by investors in stock markets. If the disclosed risk information is newly available for investors, firms with a high risk of accidents would receive a poor evaluation by stock markets and thus managers would take risk reduction measures to prevent stock prices from declining. In this study, we conduct an event study analysis to examine whether accident risk information is already reflected in stock prices, using data on the Japanese chemical industry. The results of our event study show that the estimated cumulative average abnormal returns of firms' stocks are significantly negative after severe accidents actually occurred. This finding implies that risk information is not already reflected in the stock prices of Japanese chemical firms and that the disclosure of accident risk information has the potential to motivate the top management teams of firms to reduce their firms’ accident risk.  相似文献

This paper attempts to evaluate the impact of risk factors from the container security initiative (CSI) on the maritime supply chain in Taiwan, employs a loss exposure matrix to identify the severity and frequency of security risk, and uses a bowtie diagram to investigate appropriate risk management strategies to deal with maritime security risks.This paper’s findings consist of the following: (1) the leading categories of CSI risk factors are operational risk, physical risk and financial risk; (2) apart from two security risks comprising high-risk level, the majority of risk exposure is at the medium level; risk management strategies include loss prevention, loss reduction, avoidance, and non-insurance transfer; and (3) all maritime security regulations and measures should consider the balance among cost, time, security, efficiency and competition; failure to do so may lead to additional burdens for maritime supply chain service providers.  相似文献

Introduction

Police records are the most common source of data used to estimate motor-vehicle collision risks, understand causal or contributing factors, and evaluate the efficacy of interventions. The literature notes concerns about this information citing discrepancies between police reports and other sources of injury occurrence and severity data. The primary objective of the analysis was to assess the adequacy of police reports for an examination of weather-related injury collision risk.

Method

Analyses of relative risk were carried out using both police records and comprehensive insurance claim data for Winnipeg, Canada over the period 1999-2001.

Results and conclusions

Both data sets yielded very similar results—precipitation substantially increases the risk of injury collision (police records: RR 1.76, CI 1.55-2.00; insurance: RR 1.80, CI 1.62-1.99) and risk of injury (police records, RR 1.74, CI 1.55-1.96; insurance, RR 1.69, CI 1.55-1.85) relative to corresponding dry weather control periods. Both rainfall and snowfall were associated with large increases in collisions and injuries.

Impact on Industry

While relative risks are almost identical, over 64% more injury collisions and 74% more injuries were identified using the insurance data, which is an important difference for evaluating absolute risk and exposure.  相似文献

This paper reports the first investigation of risk perception by workers on offshore oil and gas installations on the UK Continental Shelf, following changes in offshore safety legislation in the wake of the Piper Alpha disaster in 1988. The Offshore Safety Case regulations (Health and Safety Executive, 1992, A Guide to the Offshore Installations (Safety Case) Regulations) put the onus on the operator to identify the major hazards and to reduce the risks to As Low As is Reasonably Practicable (ALARP). The regulations specifically state that Quantitative Risk Assessments (QRA) must be used when preparing the Safety Case. However, people do not use QRA when making everyday judgements about risk; they make subjective judgements known as risk perceptions, which are influenced by a number of different factors. This study was designed to complement the extensive QRA calculations that have already been carried out in the development of Safety Cases. The aim was to measure subjective risk perception in offshore personnel and examine how this relates to the more objective risk data available, namely accident records and QRA calculations. This paper describes the Offshore Risk Perception Questionnaire developed to collect the data and reports on UK offshore workers' perceptions of the risks associated with major and minor hazards, work tasks and other activities aboard production platforms.  相似文献

The purpose of the present research is to collect information about accidents and incidents that have occurred at fuel ethanol facilities from 1998 to October 2014, and to keep complete unified records of them in a database. The developed database contains general information about the accident or incident, its sequence, mitigation measures, its causes and consequences for humans, environment and for the plant. Until now, this information is not available. The work consisted in gathering information from different documental sources and subsequent organization in a database. It complements the previous work made for biodiesel industry and fills the existing gap in the field of ethanol. Knowledge about this information enables us to manage plant risks, since the accidents that are more likely to occur and the main sources of risk can be easily identified. Also, it makes it possible to exchange information with interested third parties. Statistical analysis shows that accident frequency has an oscillatory behavior, rising in the last year. Fire is the most common type of accident, while equipment mechanical failure is the main cause of accident. Partial material loss has been identified as the most common consequence. Finally, some conclusions are obtained concerning to the importance of having an updated and complete accident and incident database.  相似文献

This paper presents a novel quantitative risk analysis process for urban natural gas pipeline networks using geographical information systems (GIS). The process incorporates an assessment of failure rates of integrated pipeline networks, a quantitative analysis model of accident consequences, and assessments of individual and societal risks. Firstly, the failure rates of the pipeline network are calculated using empirical formulas influenced by parameters such as external interference, corrosion, construction defects, and ground movements. Secondly, the impacts of accidents due to gas leakage, diffusion, fires, and explosions are analyzed by calculating the area influenced by poisoning, burns, and deaths. Lastly, based on the previous analyses, individual risks and social risks are calculated. The application of GIS technology helps strengthen the quantitative risk analysis (QRA) model and allows construction of a QRA system for urban gas pipeline networks that can aid pipeline management staff in demarcating high risk areas requiring more frequent inspections.  相似文献
