An increase in the number of accidents in the process industries and the concomitant damage potential is a cause of concern in many countries. In order to control the alarming risk posed by these industries, the United States government has asked each manufacturing facility to carry out a worst-case disaster study and to develop alternatives to control this high risk. Other developed and developing countries such as Canada and India have taken similar measures.Recently Khan and Abbasi (J. Loss Prevent. Process Ind. (2001a) in press) have proposed a maximum credible accident analysis with a maximum credible accident scenario approach, which scores over a worst-case scenario approach for being realistic and reliable. In another effort, Khan and Abbasi (J. Hazard. Mater. (2001b) in press) have developed an efficient and effective algorithm for probabilistic fault tree analysis. These two approaches have been combined to yield a new methodology for a more realistic, reliable, and efficient safety evaluation and the design of risk control measures. The methodology is named SCAP: Safety, Credible Accident, Probabilistic fault tree analysis. The methodology is comprised of four steps of which the last step is a feedback loop. This paper recapitulates this methodology and demonstrates its application to ethylene oxide (EO) plants. The application of SCAP to EO plants identifies five units as risky and needing more safety measures. Further, this study recommends safety measures and demonstrates through SCAP that their implementation lower the risk to an acceptable level.  相似文献

The Abnormal Situation Management® Consortium1 funded a study to investigate procedural execution failures during abnormal situations. The study team analyzed 20 publically available and 12 corporate confidential incident reports using the TapRoot® methodology to identify root causes associated with procedural execution failures. The main finding from this investigation was the majority of the procedural execution failures (57%) across these 32 incident reports were associated with abnormal situations. Specific recommendations include potential information to capture from plant incident to better understand the sources of procedural execution failures and improve use of procedures in abnormal situations.  相似文献

Information of occupational accidents is not standardized worldwide. Especially, developing countries do not have reliable information on their occupational accidents due to lack of proper recording and notification systems. The number of accidents is under-reported but figures are still used as a baseline for occupational safety work. In this paper global estimates of occupational accidents are presented for 175 countries. These estimates are based on figures from selected countries in eight different regions. Global estimates help to compare different countries and regions to each other to detect improvements in safety and safety work. In 1998 the average estimated number of fatal occupational accidents was 350 000 and there were 264 million non-fatal accidents. Global estimates are needed to guide national policies and decision-making.  相似文献

Work-related accidents are considered as a major health problem worldwide. The aim of present study was to determine the epidemiological pattern of occupational accidents among workers insured by the Iranian Social Security Organization (ISSO) between 2001 and 2005.Subjects consisted of all workers and drivers who had a work-related accident during 2001–2005 and were registered in the Iranian Social Security Organization (ISSO) database according to the inspection reports. An ordinal logistic regression model (proportional odds regression model) was used to assess the concurrent effects of independent variables on accident outcomes.Overall, 86,437 work-related accidents were investigated. The accidents were more frequent in metal workplaces and electrical industries, respectively. More than half of the accidents were due to incautious activities. Workers’ age (age at the time of accident) (OR = 0.99, CI: 0.989–0.994), gender (OR = 1.3, CI: 1.191–1.683), marital status (OR = 1.25, CI: 1.143–1.675) as well as accident setting (OR = 1.88, CI: 1.728–1.975) had significant effect on accident outcomes. Pattern of occupational injuries in Iran was consistent with the global pattern for accident outcomes.  相似文献

Experimental research shows that using mobile phones while driving leads to impaired driving, and it has been suggested that this driving impairment to a large extent is a result of cognitive, rather than physical, distractions. This notion is partly supported by empirical data showing that use of hands-free phones is associated with impaired driving in much the same way as use of hand-held phones. In the present study, accident risk when using hand-held and hands-free phones was investigated in a sample of 4307 drivers who were involved in accidents in 2007. In addition, data from a similar survey from 1997 (N = 5007) were used in order to get more observations. Relative risk was estimated using “quasi-induced exposure” in multiple-vehicle accidents. Results from the two surveys combined showed a significant increase in accident risk for hand-held mobiles and for hand-held and hands-free phones together. A non-significant tendency towards increased risk for hands-free mobiles was also detected. However, analyses of data from 2007 separately did not result in statistically significant relative risk estimates for any of the mobile types. Hand-held users were more inclined to attribute the accident to mobile phone use than were hands-free users.  相似文献

Siting regulations and industrial standards for liquefied natural gas (LNG) terminals are evolving along different paths within Europe and the United States (U.S.). The 49 Code of Federal Regulations (CFR), Part 190 continues to delineate the United States process to adopt and revise safety regulations pertaining to LNG terminals and peak shaving plant sitting.1 Embodied in these regulations are rich legal and regulatory traditions that are unique to the U.S. perspective. For example, the public is encouraged to petition existing regulations and to comment on regulatory proposals. Litigation within the U.S. court system is another means by which industry and the public may seek regulatory change. This approach promotes public involvement in governmental oversight and creates a distinctive obligation and accountability for U.S. regulatory agencies, which uniquely shape technical, safety, risk mitigation, and societal risk perspectives for siting LNG terminals.European traditions shape siting regulations for LNG terminals as well. Though American siting guidance includes references to the National Fire Protection Association’s NFPA 59A and 49 CFR, Part 193, European developers also apply the guidance within EN 1473 – a risk-based case-by-case analysis directive.2, 3 and 4 The NFPA 59A standard is applied for a basis to examine property line spacing as they pertain or may relate to off-site hazard impacts. The European approach applies the assessment and suitability of code compliance and the application of accepted engineering practices. In addition the approach incorporates the application of empirical risk assessments and computational modeling to reach an understanding of risk exposures. European policies set limits on the population’s cumulative exposure to facility risks and require LNG facility developers not to exceed established risk criteria.This paper describes how the U.S. and Europe site LNG terminals, identifies key differences in their risk-based approaches, and explains why these differences exist. This discussion also examines historical precedents that have influenced regulations and approval processes for siting LNG terminals within each continent.  相似文献

Conventional hazard evaluation techniques such as what-if checklist and hazard and operability (HAZOP) studies are often used to recognise potential hazards and recommend possible solutions. They are used to reduce any potential incidents in the process plant to as low as reasonably practicable (ALARP) level. Nevertheless, the suggested risk reduction alternatives merely focus on added passive and active safety systems rather than preventing or minimising the inherent hazards at source through application of inherently safer design (ISD) concept. One of the attributed reasons could be the shortage of techniques or tools to support implementation of the concept. Thus, this paper proposes a qualitative methodology that integrates ISD concept with hazard review technique to identify inherent hazards and generate ISD options at early stage of design as proactive measures to produce inherently safer plant. A modified theory of inventive problem solving (TRIZ) hazard review method is used in this work to identify inherent hazards, whereby an extended inherent safety heuristics tool is developed based on established ISD principles to create potential ISD options. The developed method namely Qualitative Assessment for Inherently Safer Design (QAISD) could be applied during preliminary design stage and the information required to apply the method would be based on common process and safety database of the studied process. However, user experiences and understanding of inherent safety concept are crucial for effective utilisation of the QAISD. This qualitative methodology is applied to a typical batch reactor of toluene nitration as a case study. The results show several ISD strategies that could be considered at early stage of design in order to prevent and minimise the potential of thermal runaway in the nitration process.  相似文献

RationaleThe phenomenon of repeat work-related accidents has not been sufficiently studied.ObjectivesTo evaluate the role of work-related factors in the occurrence of repeat accidents in Italy, using economic activity and the size of the enterprise as proxies and basing the analysis on available administrative data.MethodsThe data, provided by the Italian Workers’ Compensation Authority (INAIL), refer to accidents in the industry and artisan sectors and in the service industry between 1994 and 1999. We selected the data on accidents among 633,735 persons 25–55 years of age with a first accident in 1996–1997, and we determined the occurrence of further accidents within two years in the same economic activity, taking into account job mobility and factors associated with underreporting (geographic area, age, and size of the enterprise). The gender-specific relative risk was computed for each economic activity, given by the ratio between the risk of repeat accidents in a given economic activity and the risk in all of the other activities.ResultsIndividuals with a single accident differed from those with a repeat accident in terms of the distribution by gender, age-class, and severity of injury. The economic activities with the highest relative risk were generally those known to represent a risk of accidents in general: metal ore and coal mining, shipbuilding, the manufacturing of railroad equipment, building construction, road and railway construction, the primary iron and steel industry, foundry work, logging/wood manufacturing, slaughtering, and agriculture. However, certain activities not generally considered as hazardous showed a high risk of repeat accidents (e.g., public hygiene, and the manufacturing of bricks, pottery, and glass).ConclusionsThe epidemiological pattern of repeat accidents by economic activity reflects that of accidents in general, though with exceptions. Results suggest that factors associated with specific technical aspects and production processes are important determinants of safety. The differences between single accidents and repeat accidents by gender, age class, and severity of injury suggest that studying repeat accidents separately from first accidents is quite useful.  相似文献

The present study aimed to investigate differences in risk perception, the demand for risk mitigation, priorities and worry related to transport among the Norwegian public in 2004 and 2008. Age, gender, education, driver licence and number of injuries in transport were controlled for. The comparison was carried out with questionnaire surveys among representative samples of the Norwegian public in 2004 (n = 1730) and 2008 (n = 1864). The results showed that the perceived probabilities of transport accidents decreased, whereas the perceived severity of consequences increased during this period. Both samples estimated higher probabilities of accidents by private means of transport. The demand for risk mitigation and priorities related to transport safety increased significantly in the same period. The 2008 sample also reported more worry regarding accidents by private transport. Results were discussed in relation to transport safety campaigns, safety measures and significant accidents which occurred in this period.  相似文献

Objective: The aim of the study was to compare the safety effects of studded and unstudded winter tires based on fatal road accidents.

Methods: The data included 958 road accidents involving a passenger car or van that occurred in Finland from November to March between 1997 and 2012.

Results: Comparing the proportions of winter tire type in accidents and in general traffic showed that the overall effect of tire type on the number of accidents was not significant, although studded tires reduced fatal accidents by 10–15%. Compared to unstudded tires, studded tires reduced accidents significantly only on bald ice in 2005–2012. Drivers using unstudded tires were more experienced and their profession was more frequently related to driving. In addition, the vehicle age was lower for vehicles with unstudded tires. On the other hand, the state of repair was less pertinent for unstudded than for studded tires. These confounding factors offset their effects to some degree.

Conclusions: The risk of fatal road accidents in winter between studded and unstudded tires does not differ significantly. However, the accident risk has recently been substantially higher on bald ice for unstudded than for studded tires. The magnitude of this risk difference is difficult to determine without specific information on exposure by road surface.  相似文献

Objective: Although it is important to assess the factors associated with traffic accident fatalities to decrease them as a matter of public health, such factors have not been fully identified.

Methods: Using a large-scale data set of ambulance records in Osaka City, Japan, we retrospectively analyzed all traffic accident patients transported to hospitals by emergency medical service personnel from 2013 to 2014. In this study, prehospital death was defined as that occurring at the scene or in the emergency department immediately after hospital arrival. We assessed prehospital factors associated with prehospital death due to traffic accidents by logistic regression models.

Results: This study enrolled 28,903 emergency patients involved in traffic accidents, of whom 68 died prehospital. In a multivariate model, elderly patients aged ≥75 years (adjusted odds ratio [AOR] = 4.34; 95% confidence interval [CI], 2.29–8.23), nighttime (AOR = 2.75; 95% CI, 1.65–4.70), and type of injured person compared to bicyclists such as pedestrians (AOR = 9.58; 95% CI, 5.07–17.99), motorcyclists (AOR = 2.75; 95% CI, 1.21–6.24), and car occupants (AOR = 2.98; 95% CI, 1.39–6.40) were significantly associated with prehospital death due to traffic accidents. In addition, the AOR for automobile versus nonautomobile as the collision opponent was 4.76 (95% CI, 2.30–9.88).

Conclusions: In this population, the factors associated with prehospital death due to traffic accidents were elderly people, nighttime, and pedestrian as the type of patient. The proportion of prehospital deaths due to traffic accidents was also high when the collision component was an automobile.  相似文献

Introduction

Fatalities from traffic accidents in less-motorized societies are an important global issue. We aimed to characterize the geographic differences of fatalities in such societies to facilitate the development of targeted interventions.

Method

This study linked police reports, hospital data, and vital registration data from Taiwan with special reference to accident factors in pre-hospital deaths and medical care in hospital deaths.

Results

A higher percentage of pre-hospital deaths were observed following rural as compared to urban traffic accidents. The deaths due to rural accidents can be attributed to lower use of restraints (i.e., helmets or seat belts), lower percentage of motorcyclists, and more highway accidents. A higher percentage of victims in rural accidents were transported to distant medical centers rather than to local hospitals.

Conclusion

Specific interventions, such as intelligent emergency medical systems, campaigns for helmets and seat belt usage, enforcement of helmets and seat belt use, and speed control measures should be targeted to rural areas.

Impact on industry

Cooperation between the vehicle industry and emergency medical providers in rural traffic accident rescue teams may decrease the numbers of deaths in these regions.  相似文献

In this study, a methodology is proposed towards development of an uncertainty model that includes randomness in the occurrence of days-lost accidents in a coal mine. The accident/injury data consists of 1390 days-lost accident cases recorded at GLI-Tuncbilek underground lignite mine from January 1994 to December 2002. In the first step of proposed methodology, the frequency and the severity of the accidents have been modeled statistically by fitting appropriate distributions. The test done by BestFit software yields a chi-square value of 21.53 (p = 0.089) with 14 degrees of freedom and estimates the parameter of lambda for Poisson distribution as 12.87 accidents/month. For the severity component, a lognormal distribution is fitted to days-lost data and chi-square goodness-of-fit test calculates a value of 40.44 (p = 0.097) with 30 degrees of freedom. The parameters of lognormal distribution are estimated as a mean of 14.3 days and standard deviation of 23.1 days, respectively. Then, two distributions are basically combined by Monte Carlo simulation in order to construct relative risk levels in yearly base referring to the final cumulative distribution. Finally, a simple forecasting modeling is carried out in order to quantitatively predict the expected risk levels by using decomposition technique in time series analysis. Stochastic model estimates that although, there would be substantial reduction in the expected number of accidents in the near future, the higher level of risks still should be a concern for the mine management.  相似文献

IntroductionThis article looks into how perceptions of experience-based analysis (EBA) influence causal explanations of accidents given by managers and workers in the chemical industry (n = 409) and in the nuclear industry (n = 222).MethodThe approach is based on the model of naive explanations of accidents (Kouabenan, 1999, 2006, 2009), which recommends taking into account explanations of accidents spontaneously given by individuals, including laypersons, not only to better understand why accidents occur but also to design and implement the most appropriate prevention measures. The study reported here describes the impact of perceptions about EBA (perceived effectiveness, personal commitment, and the feeling of being involved in EBA practices) on managers' and workers' explanations of accidents likely to occur at the workplace.ResultsThe results indicated that both managers and workers made more internal explanations than external ones when they perceived EBA positively. Moreover, the more the participants felt involved in EBA, were committed to it, and judged it effective, the more they explained accidents in terms of factors internal to the workers. Practical Applications: Recommendations are proposed for reducing defensive reactions, increasing personal commitment to EBA, and improving EBA effectiveness.  相似文献

Introduction

The concept of knowledge translation as defined by the Canadian Institutes for Health Research and the Knowledge to Action Cycle, described by Graham et al (Graham et al., 2006), are used to make a case for the importance of using a conceptual model to describe moving knowledge into action in the area of falls prevention.

Method

There is a large body of research in the area of falls prevention. It would seem that in many areas it is clear what is needed to prevent falls and further syntheses can determine where the evidence is sufficiently robust to warrant its implementation as well as where the gaps are that require further basic research.

Conclusion

The phases of the action cycle highlight seven areas that should be paid attention to in order to maximize chances of successful implementation.  相似文献

Occupational Health and Safety in Spain has improved considerably over the last decade, most likely due to a new concept where an overall concept of safety culture is defined. Important changes in industrial safety, hygiene, and psychosocial factors present an optimistic panorama for the future of Spain. Despite this general improvement, according to the European Convergence Program, Spanish statistics still offer far from good safety results. In fact, according to 1997 official statistics, Spain had the highest incidence rate for nonfatal occupational accidents of all European Union (EU) countries, and occupied third place for fatal accidents. This paper summarizes the organizational structure of the Spanish National System of Health & Safety at Work, its effective health and safety laws, and statistics on the Spanish work environment obtained from III Spanish National Survey on Work Conditions (1997). The researchers hope that the findings of this work will have an impact on Spanish industry that will subsequently bring about improvements in work conditions and develop assessment and intervention models in occupational health and safety, from a theoretical position integrating environmental, human, and organizational factors.  相似文献

Objective: It is estimated that road traffic accidents are globally responsible for approximately 1.2 million deaths and 20 to 50 million injuries. About 70% of traffic incidences (TIs) occur in developing countries and among countries with high TI rates; Iran is the first. The aim of this study was to measure the association between being responsible for a traffic accident and some vehicle, human; and environmental related factors in Yasuj, a city with a high incidence of road traffic injuries and deaths in Iran.

Methods: This is a time-, date-, and place-matched case–control study conducted in 2015 using all traffic accidents registered and investigated by police during 2012. In total, 194 drivers were considered the at-fault driver in a traffic accident and the 194 drivers in the same collisions were included in the analysis.

Results: Based on the results from multivariate conditional logistic regression, significant associations between vehicle maneuver (ORTurn to right or left/Moving forward = 11.10, 95% confidence interval [CI], 1.77–69.58, P = .01) and age (odds ratio [OR] = 1.11, 95% CI, 1.004–1.22, P = .04) and the chance of being an at-fault driver were found.

Conclusion: Driver behavior–related interventions including training and law enforcement seem to be more effective in reducing road traffic accidents in Iran.  相似文献

Hazard and operability (HAZOP) studies constitute an essential step in the risk analysis of any chemical process industry and involve systematic identification of every conceivable abnormal process deviation, its causes and abnormal consequences. These authors have recently proposed optHAZOP as an alternative procedure for conducting HAZOP studies in a shorter span of time than taken by conventional HAZOP procedure, with greater accuracy and effectiveness [Khan, F. I. and Abassi, S. A., optHAZOP. An effective and efficient technique for hazard identification and assessment Journal of Loss Prevention in the Process Industries, 1997, 10, 191–204]. optHAZOP consists of several steps, the most crucial one requires use of a knowledge-based software tool which would significantly reduce the requirement of expert man-hours and speed up the work of the study team. TOPHAZOP (Tool for OPTmizing HAZOP) has been developed to fulfil this need.

The TOPHAZOP knowledge-base consists of two main branches: process-specific and general. The TOPHAZOP framework allows these two branches to interact during the analysis to address the process-specific aspects of HAZOP analysis while maintaining the generality of the system. The system is open-ended and modular in structure to make easy implementation and/or expansion of knowledge. The important features of TOPHAZOP and its performance on an industrial case study are described.  相似文献
