泄漏、扩散、点火这3个因素,在液氨泄漏源附近一定的时空范围内,按不同时序可以演变成BLEVE、喷射火、闪火、蒸气云爆炸和大气中扩散等5种事故后果模式。该文针对液氨的独特理化性质,对液氨泄漏及其在大气中扩散物理过程进行了分析,并通过算例对比研究了5种液氨事故模式的危害范围和程度,为预防液氨泄漏事故发生和液氨泄漏事故预警提供参考。  相似文献

Fault tree analysis (FTA) is a logically structured process that can help identify potential causes of system failure before the failures actually occur. However, FTA often suffers from a lack of enough probabilistic basic events to check the consistency of the logic relationship among all events through linkage with gates. Sometimes, even logic relationship among all events is difficult to determine, and failures in system operation may have been experienced rarely or not at all. In order to address the limitations, this paper proposes a novel incident tree methodology that characterizes the information flow in a system instead of logical relationship, and the amount of information of a fuzzy incident instead of probability of an event. From probability statistics to fuzzy information quantities of basic incidents and accident, we propose an incident tree model and incident tree analysis (ITA) method for identification of uncertain, random, complex, possible and variable characteristic of accident occurrence in quantified risk assessment. In our research, a much detailed example for demonstrating how to create an incident tree model has been conducted by an in-depth analysis of traffic accident causation. The case study of vehicle-leaving-roadway accident with ITA illustrates that the proposed methodology may not only capture the essential information transformations of accident that occur in system operation, but also determine the various combinations of hardware faults, software failures and human errors that could result in the occurrence of specified undesired incident at the system level even accident.  相似文献

Abstract

Objective: The objective of this article is to describe the characteristics of fatal crashes with bicyclists on Swedish roads in rural and urban areas and to investigate the potential of bicycle helmets and different vehicle and road infrastructure interventions to prevent them. The study has a comprehensive approach to provide road authorities and vehicle manufacturers with recommendations for future priorities.

Methods: The Swedish Transport Administration’s (STA) in-depth database of fatal crashes was used for case-by-case analysis of fatal cycling accidents (2006–2016) on rural (n?=?82) and urban (n?=?102) roads. The database consists of information from the police, medical journals, autopsy reports, accident analyses performed by STA, and witness statements. The potential of helmet use and various vehicle and road infrastructure safety interventions was determined retrospectively for each case by analyzing the chain of events leading to the fatality. The potential of vehicle safety countermeasures was analyzed based on prognoses on their implementation rates in the Swedish vehicle fleet.

Results: The most common accident scenario on rural roads was that the bicyclist was struck while cycling along the side of the road. On urban roads, the majority of accidents occurred in intersections. Most accidents involved a passenger car, but heavy trucks were also common, especially in urban areas. Most accidents occurred in daylight conditions (73%). Almost half (46%) of nonhelmeted bicyclists would have survived with a helmet. It was assessed that nearly 60% of the fatal accidents could be addressed by advanced vehicle safety technologies, especially autonomous emergency braking with the ability to detect bicyclists. With regard to interventions in the road infrastructure, separated paths for bicyclists and bicycle crossings with speed calming measures were found to have the greatest safety potential. Results indicated that 91% of fatally injured bicyclists could potentially be saved with known techniques. However, it will take a long time for such technologies to be widespread.

Conclusions: The majority of fatally injured bicyclists studied could potentially be saved with known techniques. A speedy implementation of important vehicle safety systems is recommended. A fast introduction of effective interventions in the road infrastructure is also necessary, preferably with a plan for prioritization.  相似文献

近来,我国各类事故频繁发生,事故造成人员的伤亡或设备、装置、建筑物的破坏,给国家、企业和个人造成了很大的经济损失,也给社会造成了不安定因素.笔者认为事故发生后不仅要查清人员的伤亡情况、事故经过、原因分析、责任人处理、人员教育、措施制定,而且还要弄清事故经济损失的划分,进而对事故经济损失进行分析和统计,从而追究经济损失的承担者.本文介绍了国内外事故经济损失的划分、对比以及计算方法.参照国外的经验,根据我国的实际情况,对于直接损失和间接损失给出了详细的划分和计算的方法.  相似文献

FDA事故致因模型从安全信息视角出发,将海上船舶碰撞事故发生的宏观与微观因素系统地结合在一起,以管理科学和技术科学为依据,构建海上船舶碰撞FDA事故致因模型,从微观到宏观即个体船只、海运企业、海事局3个层面进行安全信息视域下的事故致因分析,通过逻辑推导定位最佳的安全管理方位。研究结果表明:FDA事故致因模型由3条事故子链及1条事故主链构成,能够从微观到宏观完整表达海上船舶碰撞事故发生过程及机理;破坏3条FDA事故子链或FDA事故主链的形成即可有效地预防海上船舶碰撞事故的发生,对促进海上船舶航行安全管理水平的提高具有重要的理论与现实意义。  相似文献

Past accident analysis (PAA) is one of the most potent and oft-used exercises for gaining insights into the reasons why accidents occur in chemical process industry (CPI) and the damage they cause. PAA provides invaluable ‘wisdom of hindsight’ with which strategies to prevent accidents or cushion the impact of inevitable accidents can be developed.A number of databases maintain record of past accidents in CPI. The most comprehensive of the existing databases include Major Hazard Incident Data Service (MHIDAS), Major Accident Reporting System (MARS), and Failure and Accidents Technical Information Systems (FACTS). But each of these databases have some limitations. For example MHIDAS can be accessed only after paying a substantial fee. Moreover, as detailed in the paper, it is not infallible and has some inaccuracies. Other databases, besides having similar problems, are seldom confined to accidents in chemical process industries but also cover accidents from other domains such as nuclear power plants, construction industry, and natural disasters. This makes them difficult to use for PAA relating to CPI. Operational injuries not related to loss of containment, are also often included. Moreover, the detailing of events doesn’t follow a consistent pattern or classification; a good deal of relevant information is either missing or is misclassified.The present work is an attempt to develop a comprehensive open-source database to assist PAA. To this end, information on about 8000 accidents, available in different open-source clearing houses has been brought into a new database named by us PUPAD (Pondicherry University Process-industry Accident Database). Multiple and overlapping accident records have been carefully eliminated and a search engine has been developed for retrieval of the records on the basis of appropriate classification. PUPAD doesn’t aim to replace or substitute the well established databases such as MHIDAS and MARS but, rather, aims to compliment them.  相似文献

Complete accident reports are important for diagnosing and ameliorating safety problems and for evaluating the effectiveness of intervention programs. Often, however, these reports are found to lack vital information. Baseline data at a paper mill revealed that accident reports generally failed to supply all requested information. In an effort to improve the situation, instructions and feedback notes were sent to personnel soon after they submitted their reports. These commented on the number of items completed, improvements over previous submissions, and other meritorious features. To evaluate the impact of the intervention, the feedback was withdrawn after 7 weeks and then reinstated. Following termination of feedback, completeness of reports continued to be monitored for about a year. It was found that reporting performance improved under feedback and deteriorated when it was withdrawn. These findings suggest that a simple feedback note can effectively contribute toward solving the problem of incomplete accident reports.  相似文献

Accident prediction models, the vast majority of which are negative binomial regression models, are of considerable importance to highway agencies since they can be used to conduct many traffic safety studies. However, not every agency possesses sufficient accident statistics that enable it to develop reliable models of its own. This problem gives rise to interest in the transferability of accident prediction models in time and space. It would save time, effort, and money if accident prediction models developed for one region in one period of time could be applied in different time periods and regions to produce reliable safety studies.This paper presents methods for recalibrating negative binomial accident models before transferring them for use in different time periods and regions of space. The paper emphasizes that the recalibration of the shape parameter of a transferred model using local data is absolutely necessary. It explains that it is also desirable to recalibrate the constant term of the transferred model in order to allow the model to better suit local conditions. A moment method is presented for recalibrating the shape parameter of a transferred model when its constant term is not recalibrated. However, a maximum likelihood method is presented for recalibrating both the shape parameter and the constant term of the transferred model and is shown to be superior to the recalibration methods existing in the traffic safety literature.  相似文献

Introduction: With a growing interest in increasing parental involvement in teen driving, it is important to find out what parents are already doing. Method This study assessed the content, delivery, rigidity, and consequences of 143 driving rules reported by 24 parent-teen dyads. Results: Strengths included that driving rules covered the full range of concerns, especially night driving limits and passenger limits, and most parents and teens reported that violations would be followed by consequences, especially talk/warn or no driving. Weaknesses included that many rules were not very strict and only half showed parent-teen agreement on content. Conclusions: These findings suggest that teen driving rules are not clearly defined.  相似文献

Accident investigation manuals are influential documents on various levels in a safety management system, and it is therefore important to appraise them in the light of what we currently know – or assume – about the nature of accidents. Investigation manuals necessarily embody or represent an accident model, i.e., a set of assumptions about how accidents happen and what the important factors are. In this paper we examine three aspects of accident investigation as described in a number of investigation manuals. Firstly, we focus on accident models and in particular the assumptions about how different factors interact to cause – or prevent – accidents, i.e., the accident “mechanisms”. Secondly, we focus on the scope in the sense of the factors (or factor domains) that are considered in the models – for instance (hu)man, technology, and organization (MTO). Thirdly, we focus on the system of investigation or the activities that together constitute an accident investigation project/process. We found that the manuals all used complex linear models. The factors considered were in general (hu)man, technology, organization, and information. The causes found during an investigation reflect the assumptions of the accident model, following the ‘What-You-Look-For-Is-What-You-Find’ or WYLFIWYF principle. The identified causes typically became specific problems to be fixed during an implementation of solutions. This follows what can be called ‘What-You-Find-Is-What-You-Fix’ or WYFIWYF principle.  相似文献

The purpose of this paper is to reflect on accident causation models and accident investigation methods. Theories on accident causation and the modelling of accident mechanisms, as well as a number of methods for accident investigation have been developed and described in the literature. The evolution of accident causation models over time shows a shift from the sequence of events to the representation of the whole system. Respectively, the evolution of accident investigation methods over time reveals a gradual shift from searching for a single immediate cause, to the recognition of multiple causes. In order to evaluate the accident investigation methods, specific plausible requirements were established in order to verify that a specific accident investigation method fulfils the principles of a specific accident causation model or give evidence to the degree of alignment between them. Since different models approach accident causation in different ways, methods linked to these models provide fragmentary information regarding the accident. It is therefore expected that using a combination of model-method pairs could provide a more reliable platform for accident analysis.  相似文献
