The historical analysis of 828 accidents in port areas, which have been selected from a database, has been used to identify the sequences of the accidents. Processing these data has allowed the event trees and the probability of the various accident scenarios to be determined. By using these event trees and figures detailing the frequency of the events that initiated the accidents, as taken from various authors, the frequency of the accidents has been determined. 相似文献

The potential for major accidents is inherent in most industries that handle or store hazardous substances, for e.g. the hydrocarbon and chemical process industries. Several major accidents have been experienced over the past three decades. Flixborough Disaster (1974), Seveso Disaster (1976), Alexander Kielland Disaster (1980), Bhopal Gas Tragedy (1984), Sandoz Chemical Spill (1986), Piper Alpha Disaster (1988), Philips 66 Disaster (1989), Esso Longford Gas Explosion (1998), Texas City Refinery Explosion (2005), and most recently the Macondo Blowout (2010) are a few examples of accidents with devastating consequences.Causes are being exposed over time, but in recent years maintenance influence tends to be given less attention. However, given that some major accidents are maintenance-related, we intend to concentrate on classifying them to give a better insight into the underlying and contributing causes.High degree of technological and organizational complexity are attributes of these industries, and in order to control the risk, it is common to deploy multiple and independent safety barriers whose integrity cannot be maintained without adequate level of maintenance. However, maintenance may have a negative effect on barrier performance if the execution is incorrect, insufficient, delayed, or excessive. Maintenance can also be the triggering event.The objectives of this article are: (1) To investigate how maintenance impacts the occurrence of major accidents, and (2) To develop classification schemes for causes of maintenance-related major accidents.The paper builds primarily on model-based and empirical approaches, the latter being applied to reports on accident investigation and analysis. Carbon dioxide pipeline is an essential carrier in carbon capture, utilization, and storage (CCUS). Statistically revealing the accident rate and risk of carbon dioxide pipelines is conducive to integrity management. Based on 112 accident records collected from Pipeline & Hazardous Materials Safety Administration, this work analyzes the frequency, rate, and risk of accidents. In addition, the impact of relevant factors on risk is further discussed. Some primary conclusions are as follows: (1) For carbon dioxide pipelines, the leak is the leading form of accident. Most carbon emissions are generated in the form of leakage, but economic losses are mainly generated in other forms. (2) The pipelines that have been in service for 0–10 years have the highest frequency of accidents and the highest proportion of carbon emissions, but the pipelines that have been in service for 11–20 years have caused the most economic losses. (3) Among the accident causes, the number of accidents caused by equipment failure is the highest, while the economic loss caused by natural force damage is the highest, and the carbon emission caused by material failure is the highest. 相似文献

The purpose of this paper is to systematically analyse a typical planning process in the offshore industry from the perspective of causes of major accidents, with the ultimate aim of identifying factors that affect the risk for major accidents occurring. We first study and describe a typical planning process for offshore oil and gas operations in Norway. Then we analyse a number of theories of major accidents, to see how the different theories and their explanations of causes and contributing factors can be of relevance for future plans and planning processes. Finally, we review accident investigations to search for evidence of how weaknesses in planning processes can contribute to major accidents through the above identified factors. Also, we try to identify any additional factors that have not been recognised through the theoretical review. This provides empirical support for the theoretical basis. Thirteen factors which directly or indirectly can influence the planning process causing a major accident potential are identified. These are exemplified through a review of investigation reports. The paper suggests that planning process should focus more on increasing quality in the plans at an early phase, with examples from incidents, and illustrate the relation between planning quality and potential for major accidents. 相似文献

A historical survey was performed on 330 accidents involving domino effect, occurred in process/storage plants and in the transportation of hazardous materials; only accidents occurred after 1st-January-1961 have been considered. The main features – geographical location, type of accident, materials involved, origin and causes, consequences, domino sequences – were analyzed, with special consideration to the situation in the developing countries and compared to those from other previous surveys. Among the involved substances, LPG was the most frequent one, followed by liquid hydrocarbons. Process plants (38.5% of cases) and storage areas (33%) were the most common settings; 10.6% of past domino accidents occurred in transfer operations. The ratio between “two-step” and “three-step” domino accidents was found to be 6. A specific analysis of the accidents (84) occurred in the 21st century was performed, comparing them with the total set of accidents. Finally, a set of specific recommendations inferred from the results is provided. 相似文献

This article aims to demonstrate the need for changing the methods with which accidents are analyzed, if we truly wish to use what we uncover from them to learn and enrich our knowledge base of organizational management. The goal is to relinquish the broadly adopted and rather simplistic paradigm that accepts the search for human error and unsafe acts performed by workers, and produces “guilt diagnostics”. Instead, we use a systemic accident analysis methodology, based on the sociotechnical principle of understanding the real operating conditions in which accidents take place. In order to demonstrate the benefits of the theoretical framework, we compare the analyses of an Anhydrous Ammonia gas leakage accident in a fish processing plant using the traditional accident analysis model based on unsafe acts and the proposed systemic approach. The results favor the latter since it tends to be more reliable and offering useful recommendations to safety management processes, thus helping to prevent accidents, especially in complex systems. 相似文献

Interactions between natural events and industrial installations may lead to dangerous phenomena. According to bibliographical research, the industrial sector is often unprepared for these joint natural and technological or Natech events mainly because of the lack of guidelines on how to apply Natech regulations and the lack of information on the dynamics of Natechs. In order to fill the gaps and provide guidance on Natech risk assessment to operators, a systematic risk analysis methodology was developed and resulted firstly in proposing general reference bow-ties that reconfigure accidental scenarios triggered by flood events. The validation of these scenarios was made in the surface treatment sector. Building on these bow-ties, the risk analysis methodology will be completed and a checklist simple to use, along with a list proposing preventive and protective measures, to be used by operators in order to decrease the vulnerability of their industrial facilities to technological accidents triggered by floods will be developed in future work. 相似文献

Overpressure is a major hazard in the process industry with the potential to lead to a major accident. Pressure Safety Valves (PSVs) are often used as the last layer of protection against such a hazard and require regular recertification in order to be dependable. The valve safely vents gas from a vessel when the pressure becomes excessive. It is often the practice in industry to apply one or two years as the normal recertification interval of PSV. However, experience from the Norwegian oil and gas industry is that the recertification process several times have caused leaks of gas. The process thus represents a certain risk in itself and the question is then whether the recertification intervals presently being used actually are optimal from a risk point of view? The objective of this paper is to look into this problem, applying typical data from an oil and gas installation. An optimal recertification interval will be calculated based on minimization of risk to personnel. 相似文献

This paper concerns a procedure to quantify the risk of occupational accidents based on Fuzzy Logic approach, named Fuzzy Application Procedure (FAP). FAP was developed as a support tool, after the method based on fuzzy logic was developed and validated. The methodology and FAP support tool were found to be able to quantitatively assess the risk of occupational accidents for different industrial and site activities and to identify the most efficient intervention measures that can be taken to reduce risks. The application of FAP to two Italian industrial plant allowed the validation of the procedure. It can be seen how FAP should be considered easy to use for any type of company, with the only requirement of having a sufficient and homogeneous number of accidents recorded, so as to be able to correctly tune the reference parameters of the fuzzy methodology. 相似文献

The transport of hazardous materials by pipeline is widely used for the transfer of significant quantities of oil and chemicals. Due to the extremely low frequency of spills, pipelines are considered the safest mode for the land transportation of hazardous substances. Accident records, while confirming that Loss of Containment (LOC) events are rare, also point out the major-accident hazard of pipelines, due to the extremely severe potential consequences of spills. Quantitative Risk Analysis (QRA) techniques have been applied to pipelines since many years with the aim of evaluating risk for workers or exposed population. However, releases of liquids, as oil and oil products, also create an hazard to the environment, due to the potential of extensive soil and groundwater contamination. An integrated model was developed for the environmental Risk Analysis of spills from pipelines. Specific environmental risk indexes were defined, expressing the risk of soil and groundwater contamination, both in physical and economic terms. A case-study is presented and discussed to illustrate the features of the methodology. The results confirmed that the proposed model may be considered an important tool within a comprehensive approach to the management of risk related to onshore pipelines. 相似文献

Statistical analysis of hazardous material accidents (HMAs) in China from 20013 to 2018 related to characteristics and consequences is discussed in this study, revealing different time volatility, spatial distribution and accident consequence features. Yearly, monthly, weekly and hourly levels of HMAs are analysed, and the corresponding factors are discussed. The trends of HMAs are affected by different factors, such as efforts toward safety-specific rectification by the government, holidays, five-day workweek systems and daily traffic peaks. The spatial distribution trends indicate that highway transportation of hazardous materials is mostly short-distance transport. The accidents that occurred on normal road sections account for 82.76% of the total HMAs during transportation. Leaking accidents caused by HMAs account for 79.35% of the total accidents. Countermeasures focusing on improving highway transportation safety for hazardous materials are recommended. 相似文献

Since the late 80s the application of quantitative risk assessment to the issue of land-use planning with respect to major accident hazards emerged as a topic to be addressed within the safety assessment of chemical and process plants. However, in the case of industrial clusters or complex industrial areas specific methodologies are needed to deal with high-impact low-probability (HILP) events. In the present study, innovative methodologies developed for the quantitative assessment of risk due to domino and NaTech scenarios are presented. In recent years a set of models for the calculation of equipment damage probability were developed. A specific effort was dedicated to the improvement of models for the calculation of equipment damage probability in these accident scenarios. In the present study, the application of these models to case-studies was analyzed. The results of the improved models obtained for NaTech quantitative assessment were compared to previous results in the literature. A specific innovative approach was developed to multi-level quantitative assessment of domino scenarios, and its potential was analyzed. The results were examined also evidencing the role and the progress with respect to the pioneering work started on these topics by Franco Foraboschi. 相似文献

Natural gas pipeline construction is developing rapidly worldwide to meet the needs of international and domestic energy transportation. Meanwhile, leakage accidents occur to natural gas pipelines frequently due to mechanical failure, personal operation errors, etc., and induce huge economic property loss, environmental damages, and even casualties. However, few models have been developed to describe the evolution process of natural gas pipeline leakage accidents (NGPLA) and assess their corresponding consequences and influencing factors quantitatively. Therefore, this study aims to propose a comprehensive risk analysis model, named EDIB (ET-DEMATEL-ISM-BN) model, which can be employed to analyze the accident evolution process of NGPLA and conduct probabilistic risk assessments of NGPLA with the consideration of multiple influencing factors. In the proposed integrated model, event tree analysis (ET) is employed to analyze the evolution process of NGPLA before the influencing factors of accident evolution can be identified with the help of accident reports. Then, the combination of DEMATEL (Decision-making Trial and Evaluation Laboratory) and ISM (Interpretative Structural Modeling) is used to determine the relationship among accident evolution events of NGPLA and obtain a hierarchical network, which can be employed to support the construction of a Bayesian network (BN) model. The prior conditional probabilities of the BN model were determined based on the data analysis of 773 accident reports or expert judgment with the help of the Dempster-Shafer evidence theory. Finally, the developed BN model was used to conduct accident evolution scenario analysis and influencing factor sensitivity analysis with respect to secondary accidents (fire, vapor cloud explosion, and asphyxia or poisoning). The results show that ignition is the most critical influencing factor leading to secondary accidents. The occurrence time and occurrence location of NGPLA mainly affect the efficiency of emergency response and further influence the accident consequence. Meanwhile, the weight ranking of economic loss, environmental influence, and casualties on social influence is determined with respect to NGPLAs. 相似文献

基于国家化学事故应急咨询专线2002年-2011年10年间接到的1100起化学品事故咨询电话,从事故发生时间、事故发生地域、事故类型、事故发生环节、事故引发物质等进行统计分析,结果表明,自2007年起应急电话的使用频率呈现逐年递增的趋势,5月-8月是化学品事故高发季节,危险化学品企业分布越多的地域发生事故的机率越大,人身伤害事故占的比例最高,使用环节的事故咨询电话起数最多;农药是引发事故最多的物质。从加强化学品安全知识普及教育、建立完善的立法和执法机制、继续大力推行危险化学品登记制度、进一步加强应急电话宣传等方面提出了预防和减少化学品事故的对策建议。 相似文献

The paper presents the implementation of the SEVESO II Directive in Poland. Particularly, a systematic approach to the realization of MAPP, Safety Report and Emergency Plan is shown. It takes into account the real technical and organizational standards of the Polish major hazard industry as well. The impact of the implementation of SEVESO II Legislation on safety performance changes in particular major hazard companies was assessed using the questionnaires method of data collection. The fundamental question of that survey was: did that exercise improve safety performance or was it just a ‘paper work’? The majority of survey reported more positive than negative comments. The conclusions were drawn and some suggestions were made to improve major accident control in Poland. 相似文献

Natech events and domino accidents happen worldwide and usually lead to severe consequences, especially in hazardous areas such as Oil-Gas depots. With the continuous development of the Oil-Gas industry and information technology, it is essential to realize the three-dimensional management and monitoring of hazardous substances. To evaluate the consequences caused by Natech events triggered domino accidents (accident chain), a matrix loop method was proposed to calculate the probability and evolution path of the accident chain. The actual layout and the real-time data of the Oil-Gas depot are input into the evaluation method in matrices, thus making the results reliable and updated. Besides, a B/S architecture system is developed to present the evaluation consequences of the proposed method. The three-dimensional visualization effects of natural disasters, technological accidents and the dynamic propagation process of the accident chain are also realized in the system to enhance the user's experience. A system application regarding lightning-triggered domino accidents was carried out to demonstrate the feasibility and rapidity of the proposed evaluation method and perform the system's operation process and visualization effects. The application results show that the system can provide effective decision-making assistance to safety management workers before the accident and guidance for emergency rescue operations during the accident. 相似文献

This paper is based on a review of 183 detailed, major accident investigation and analysis reports related to the handling, processing and storage of hydrocarbons and hazardous chemicals over a decade from 2000 to 2011. The reports cover technical, human and organizational factors. In this paper, the Work and Accident Process (WAP) classification scheme is applied to the accident reports with the intention of investigating to what extent maintenance has been a cause of major accidents and what maintenance-related causes have been the most frequent.The main objectives are: (1) to present more current overall statistics of maintenance-related major accidents, (2) to investigate the trend of maintenance-related major accidents over time, and (3) to investigate which maintenance-related major accident causes are the most frequent, requiring the most attention in the drive for improvement.The paper presents statistical analysis and interpretation of maintenance-related major accidents’ moving averages as well as data related to the types of facility, hazardous substances, major accidents and causes. This is based on a thorough review of accident investigation reports.It is found that out of 183 major accidents in the US and Europe, maintenance was linked to 80 (44%) and that the accident trend is decreasing. The results also show that “lack of barrier maintenance” (50%), “deficient design, organization and resource management” (85%) and “deficient planning/scheduling/fault diagnosis” (69%) are the most frequent causes in terms of the active accident process, the latent accident process and the work process respectively. 相似文献
