The theoretical understanding of road safety has improved in eight respects: recognizing that there can be no cure, abandoning the language of cause and blame, understanding that we need to go beyond crashes to address their consequences, admitting the importance of exposure, taking into account statistical pitfalls and trends, rejecting arguments based on what stands to reason, attempting project evaluation, and, finally, admitting that cost, not carnage, is the issue. A program is proposed with six components: reorganization of road safety under an independent public health agency, planning for the long range, disengagement of road safety from public concern and public relations, commitment to full truthful disclosure to the public and political leaders, reorganization of professional education and public information, and formulation of a coherent modern research program.  相似文献

System safety is of particular importance for many industries. Broadly speaking, it refers to the state or objective of striving to sustainably ensure accident prevention through actions on multiple safety levers (technical, organizational, and regulatory). While complementary to risk analysis, it is distinct in one important way: risk analysis is anticipatory rationality examining the possibility of adverse events (or accident scenarios), and the tools of risk analysis support and in some cases quantify various aspects of this analysis effort. The end-objective of risk analysis is to help identify and prioritize risks, inform risk management, and support risk communication. These tools however do not provide design or operational guidelines and principles for eliminating or mitigating risks. Such considerations fall within the purview of system safety.In this work, we propose a set of five safety principles, which are domain-independent, technologically agnostic, and broadly applicable across industries. While there is a proliferation of detailed safety measures (tactics) in specific areas and industries, a synthesis of high-level safety principles or strategies that are independent of any particular instantiation, and from which specific safety measures can be derived or related to, has pedagogical value and fulfills an important role in safety training and education. Such synthesis effort also supports creativity and technical ingenuity in the workforce for deriving specific safety measures, and for implementing these principles and handling specific local or new risks. Our set of safety principles includes: (1) the fail-safe principle; (2) the safety margins principle; (3) the un-graduated response principle (under which we subsume the traditional “inherently safe design” principle); (4) the defense-in-depth principle; and (5) the observability-in-depth principle. We carefully examine each principle and provide examples that illustrate their use and implementation. We relate these principles to the notions of hazard level, accident sequence, and conditional probabilities of further hazard escalation or advancement of an accident sequence. These principles are a useful addition to the intellectual toolkit of engineers, decision-makers, and anyone interested in safety issues, and they provide helpful guidelines during system design and risk management efforts.  相似文献

This paper considers the implementation and operation of provisions for employee representation in health and safety in a number of member states of the European Union. It describes the results of two studies undertaken in seven European countries, including the UK, between 1989 and 1993. These studies compared legislative provisions for employee representation in health and safety in different countries and analysed the information available on the implementation and operation of these provisions. With regard to the coverage and implementation of legislation, the research found a similar pattern in all of the countries studied, with Britain distinguished by a number of features, most notably the very specific rights on this subject given to trade unions. However, analysis of information on operation of the provisions in all of the countries studied pointed to the strong association between trade union workplace organization and support as a powerful influence and determinant of the effectiveness of employee representation in health and safety. Taking this finding into account, the paper discusses the present and future situation in Britain with special reference to the implications of the provisions on employee representation found in the CEC Framework Directive 89/391.  相似文献

This article introduces a method for assessing health and safety management systems (MAHS) that has two innovative characteristics: (a) it brings together the three main auditing approaches to health and safety (HS) – the structural approach (which assesses the system prescribed), the operational approach (which assesses what is really happening on the shop-floor) and the performance approach (which assesses the results of performance indicators); (b) it emphasizes the resilience engineering perspective on HS, which takes into consideration four major principles (flexibility, learning, awareness, and top management commitment). Such principles underlie seven major assessment criteria, which in turn are divided into items (e.g. hazard identification from a resilience perspective is an item that belongs to the criteria of production processes). The items are sub-divided into statements, which are the requirements that should be assessed based on interviews, analysis of documents and direct observations. Within the 112 requirements proposed, 38 of them have clear links with at least one out of the four resilience engineering principles adopted. The remaining requirements are based on traditional assumptions underlying the so-called best practices of HS management. The results of the assessment for each item are expressed by a score on a scale of compliance with the established requirements, ranging from 0% to 100%. The specific score within that scale is obtained from tables used to assess applications for the Brazilian national quality award. The MAHS was tested in a case study that was carried out in a factory that manufactures automobile exhaust systems, located in Brazil.  相似文献

IntroductionVirginia saw a 20% reduction in traffic fatalities in 2008, an unprecedented annual reduction since 1950, and safety stakeholders in Virginia were intrigued about what caused such large a reduction and more generally what affects traffic safety from a macroscopic perspective.MethodThis study attempted to find factors associated with such a reduction using historical data of Virginia. Specifically, the study related 18 factors to seven traffic safety measures.ResultsIn terms of annual changes, the study found that typical crash exposures were not generally associated with the seven measures, while two economic indicators (unemployment rate and U.S. Consumer Price Index [CPI]) were strongly associated with most of them.ConclusionsAnnual changes in the CPI and unemployment rate account for about half of the annual changes in total and fatal crash counts, respectively. On average, a 1 point increase in CPI and a 1% increase in the unemployment rate are associated with about 2,500 fewer traffic crashes and about 40 fewer fatal crashes annually in Virginia, respectively.  相似文献

This study investigated workers' perceptions of workplace safety in an African work environment, specifically in Ghanaian work places. Workers' safety perceptions were examined with Hayes et al.'s. (1998) Work Safety Scale. Comparative analyses were done between high- and low-accident groups, and t tests were employed to test for differences of statistical significance. Relative to their colleagues in the low-accident category, workers in the high-accident category exhibited negative perceptions on safety. They had negative perceptions regarding work safety, safety programmes, supervisors, and co-workers' contributions. Besides, they expressed less job satisfaction and were less committed to safety management policies. Perceptions regarding management's attitude towards safety between the 2 groups were not of statistical significance. The analyses provided an explanation for the cause of a substantial portion of the high rate of industrial accidents in Ghana's work environment. Introduction: The objective of this study was to determine the reciprocal relationship between safety professionals perceived organizational support (POS) and perceived safety climate. Safety professionals are most effective when they perceive support from management and employees and they also attribute most of their success to support from the organization. Their work directly improves safety climate, and organizations with a high safety climate show a higher value for the safety professional. The causal direction of this relationship is, however, unclear. Method: Using a sample of 162 safety professionals, we conducted a cross-lagged panel study over one year to examine whether safety professionals’ POS improves their perceived safety climate and/or whether safety climate also increases POS over time. Data were collected at two points and, after testing for measurement invariance, a cross-lagged SEM was conducted to analyze the reciprocal relationship. Results: Our findings show that safety professionals’ POS was positively related to perceived safety climate over time. Perceived safety climate, however, did not contribute to safety professionals’ POS. Conclusions: This study significantly adds to the discussion about the factors influencing safety professionals’ successful inclusion in organizations, enabling them to perform their work and, thus, improve occupational safety. Practical Applications: Since safety climate increases in organizations in which safety professionals feel supported, this study points out the kind of support that contributes to improved organizational safety. Support for safety professionals may come in classical forms such as approval, pay, job enrichment, and information on or influence over organizational policies.  相似文献

The potential contribution of Advanced Driver Assistance Systems (ADAS) to traffic safety policies is currently unknown. In this article we first investigate how different ADAS might contribute to public goals on road traffic safety, based on accident statistics in the Netherlands. Next, the societal costs of ADAS implementation are qualitatively assessed. Intelligent speed adaptation (ISA) appears to be one of the most promising devices in terms of cost-effectiveness, as compared to other ADAS applications. Finally, the acceptance among potential ISA users is explored based on a survey among potential ISA users. The study points out that although the advantages of ISA are recognised by car drivers, their willingness to purchase and use ISA is strictly conditioned by the costs and functionality of the system.  相似文献

Introduction

Research in human error has provided useful tools for designing procedures, training, and intelligent interfaces that trap errors at an early stage. However, this “error prevention” policy may not be entirely successful because human errors will inevitably occur. This requires that the error management process (e.g., detection, diagnosis and correction) must also be supported. Research has focused almost exclusively on error detection; little is known about error recovery, especially in the context of safety critical systems. The aim of this paper is to develop a research framework that integrates error recovery strategies employed by experienced practitioners in handling their own errors.

Method and Results

A control theoretic model of human performance was used to integrate error recovery strategies assembled from reviews of the literature, analyses of near misses from aviation and command & control domains, and observations of abnormal situations training at air traffic control facilities. The method of system dynamics has been used to analyze and compare error recovery strategies in terms of patterns of interaction, system affordances, and types of recovery plans. System dynamics offer a promising basis for studying the nature of error recovery management in the context of team interactions and system characteristics.

Impact on industry

The proposed taxonomy of error recovery strategies can help human factors and safety experts to develop resilient system designs and training solutions for managing human errors in unforeseen situations; it may also help incident investigators to explore why people's actions and assessments were not corrected at the time.  相似文献

Occupational health and safety represents a set of technical, medical, legal, psychological, pedagogical and other measures with the aim to detect and eliminate hazards that threaten the lives and health of employees. These measures should be applied in a systematic way. Therefore, the aim of this study is to review occupational health and safety legislation in Serbia and the requirements that airports should fulfill for Occupational Health and Safety Assessment Series certification. Analyzing the specificity of airport activities and injuries as their outcomes, the article also proposes preventive measures for the health and safety of employees. Furthermore, the airport activities which are the most important from the standpoint of risks are defined, as the goals for occupational health and safety performance improvement.  相似文献

This paper examines socialization and adjustment processes of newcomers to organizations. While scholars in this area agree that the socialization process is a stressful event, few have empirically approached the process from this perspective. In addressing this problem, a theoretical synthesis of the literatures of organizational socialization and organizational stress is proposed. From this integration, a model is developed which facilitates an examination of the task, role, and interpersonal demands which individuals face in the stages of socialization and an analysis of the interrelationships of these demands and their effects upon individual and organizational outcomes of the socialization process. Finally, directions for future research are proposed for extending understanding of this crucial part of organizational entry.  相似文献

We argue that attempts to extrapolate moral motives for non‐egoistic behavior in organizational behavior often interpret results empathically or deontically, while leaving other moral motivational frames, such as the utilitarian and virtue ethical, under‐examined. The education of safety professionals shows a high variety in their level of approved qualifications, ranging from a technician level up to university master courses, and more recently, doctoral programs. In European countries, it seems that there are some differences in what regards the characteristics of the courses. These may be due to national particularities and legal issues, but also to the nature of the institutions and people ‘behind’ the courses.This paper presents the results of a survey carried out in the scope of an European research project and it aims to provide a basic understanding of the range and diversity of the OHS post-graduation courses. With an estimated average answer rate of 50%, the survey has only included courses with more than 120 teaching hours, from a post-graduation level, and with complete programs. Results are presented for 90 courses, from 18 countries, mainly (84.4%) from universities.It is possible to highlight the fact that, as expected, the majority of the courses (59%) are Masters (or equivalent), and are organised primarily by Engineering, Applied Sciences and Management schools/faculties, which together accounted for nearly 65% of the courses. In what regards the adopted quality systems reported by respondents, there is a predominance (65.8%) of the use of “internal” tools, such as the students and teachers evaluations and internal audits.One of the main conclusions is that there is a large variability amongst the analysed courses. However, it should be emphasised that these results are not representative of the situation all around Europe, as it was not possible to obtain information from all OHS courses. Considering the identified differences within all the European countries, the harmonisation of post-graduation courses on OHS, if it is to occur, has a long way to go.  相似文献

This paper reviews 23 studies that have examined safety climate within commercial and military aviation. The safety climate factors identified in the aviation safety climate questionnaires were found to be consistent with the literature examining safety climate in non-aviation high reliability organizations. Therefore, it was concluded that the aviation safety climate tools had some construct validity (the extent to which the questionnaire measures what it is intended to measure). However, the majority of the studies made no attempt to establish the discriminate validity (the ability of the tool to differentiate between organizations or personnel with different levels of safety performance) of the tools. It is recommended that rather than constructing more aviation safety climate questionnaires, researchers should focus on establishing the construct and discriminate validity of existing measures by correlating safety climate with other metrics of safety performance. It is recognized that the accident rate in commercial aviation is too low to provide a sufficiently sensitive measure of safety performance. However, there are other measures of safety performance, collected as part of a company’s Aviation Safety Action Program or Flight Operational Quality Assurance, which could be used to assess the discriminate validity of an aviation safety climate tool.  相似文献

This paper proposes a competency-based view of careers, derived from competency-based models of employer firms. The implications for boundaryless careers are explored by reference to changing organizational, occupational and industry community contexts. All of these contexts are seen as likely to promote boundaryless career behaviors. Introduction: Compared to other types of occupational training, safety training suffers from several unique challenges that potentially impair the engagement of learners and their subsequent application or “transfer” of knowledge and skills upon returning to the job. However, existing research on safety training tends to focus on specific factors in isolation, such as design features and social support. The aim of this research is to develop an overarching theoretical framework that integrates factors contributing to training engagement and transfer. Method: We conducted a comprehensive qualitative review of safety training research that was published between 2010 and 2020. We searched Web of Science, Scopus, and Google Scholar, yielding 147 articles, and 38 were included. We content analyzed article summaries to arrive at core themes and combined them with contemporary models of general occupational training to develop a rich model of safety training engagement and transfer. Results: We propose that training engagement is a combination of pre-training factors such as individual, organizational, and contextual factors, that interact with design and delivery factors. Safety training engagement is conceptualized as a three-component psychological state: affective, cognitive, and behavioral. Organizations should prioritize pre-training readiness modules to address existing attitudes and beliefs, optimize the safety training transfer climate, and critically reflect on their strategy to design and deliver safety training so that engagement is maximized. Conclusions: There are practical factors that organizations can use before training (e.g., tailoring training to employees’ characteristics), during training (e.g., ensuring trainer credibility and use of adult learning principles), and after training (e.g., integrating learned concepts into systems). Practical Applications: For safety training to ‘stick,’ workers should be affectively, cognitively, and behaviorally engaged in learning, which will result in new knowledge and skills, improvements in attitudes, and new safety behaviors in the workplace. To enable engagement, practitioners must apply adult learning principles, make the training relevant, and tailor the training to the job and individual needs. After training, ensure concepts are embedded and aligned with existing systems and routines to promote transfer.  相似文献
