A significant number of pipeline operators use pipeline integrity management (PIM) to improve pipeline safety and reliability. Risk assessment is a critical step in PIM, because it determines the necessity of conducting the following steps in PIM for certain pipelines. Risk acceptance criteria are required in the process of risk assessment. Individual risk and societal risk are most frequently adopted as the two indicators of the risk acceptance criteria. To the best of the authors’ knowledge, quantitative societal risk acceptance criteria, especially for gas distribution pipelines, do not exit. The aim of this paper is to establish the societal risk acceptance criteria for gas distribution pipelines. Hence, FN curves were established using historical incident data from 2002 to 2017 provided by the U.S. Department of Transportation (DOT). Linear regression and the ALARP principle are used in evaluating the limits of the negligible line and intolerable line to obtain a graphical societal risk acceptance criterion for gas distribution pipelines. A line having a slope of −1.224, and an anchor point of (1, 8.413 × 10−7) is proposed as the negligible line. Further, the intolerable line has a slope of −1.224, and an anchor point of (1, 2.524 × 10−6). Both the negligible risk and the intolerable risk for the gas distribution pipeline are lower than the current societal risk acceptance criteria for hazardous installations. The reasons for these relatively lower risk acceptance criteria are discussed.  相似文献

Standards and industry guidelines for Safety Instrumented Systems (SISs) describe the use of hazard and risk analysis to determine the risk reduction required, or Safety Integrity Levels (SILs), of Safety Instrumented Functions (SIFs) with reference to hazardous events and risk tolerance criteria for them. However, significant problems are encountered when putting this approach into practice. There is ambiguity in the meaning of the term hazardous event. Notably, even though it is a key concept in the process-sector-specific SIS standard, IEC 61511/ISA 84, it is not defined in the standard. Consequently, risk tolerance criteria for hazardous events are ill-defined and, therefore, they are not the most appropriate criteria to use. Most current approaches to SIL determination use them and therefore they are flawed fundamentally.An informed decision on the tolerability of risk for a facility cannot be made by determining only the tolerability of risk for individual hazardous events. Rather, the tolerability of the cumulative risk from all hazard scenarios and their hazardous events for a facility must be determined. Such facility risk tolerance criteria are the type used by regulators. This issue applies to all per event risk tolerance criteria. Furthermore, determining the tolerability of risk for a facility based only on the risks of single events, be they hazard scenarios or hazardous events, and comparing them to risk tolerance criteria for the events is not meaningful because there is no consideration of how many such events can actually occur and, therefore, no measure of the total risk. The risks from events should be summed for a facility and compared with overall facility risk tolerance criteria.This paper describes and illustrates SIL determination using a risk model implemented within the framework of Layers of Protection Analysis (LOPA) that overcomes these problems. The approach allows the allocation of risk across companies, facilities, processes, process units, process modes, etc. to be managed easily.  相似文献

Employees' beliefs about how promotions are awarded within their organizations can have important consequences. We conducted two studies that focus on perceptions of the criteria used to make promotion decisions. In Study 1, we identified two types of perceived promotion criteria, performance‐based and nonperformance‐based. Then we use justice and social exchange theories to develop a model linking employee perceptions of promotion criteria to performance via their relationships with promotional justice and organizational commitment. In a sample of 305 employee–supervisor pairs, we found that both promotional justice and organizational commitment mediated between perceptions of promotion criteria and supervisor rated in‐role and extra‐role performance, and that having received a promotion in the past predicted attributions that promotions were based relatively more on performance or nonperformance criteria. Study 2 further examined the role of promotions themselves in the formation of perceptions of promotion criteria. Drawing from image and attribution theories, we hypothesized that the relationship between having received a promotion or not and perceptions of promotion criteria depends on ego defensiveness. In a sample of 145 employees, we found that those who scored high on ego defensiveness and who had not been promoted were especially likely to attribute promotion decisions to nonperformance criteria. In this work we present a method for risk-informed decision-making in the physical asset management context whereby risk evaluation and cost-benefit analysis are considered in a common framework. The methodology uses quantitative risk measures to prioritize projects based on a combination of risk tolerance criteria, cost-benefit analysis and uncertainty reduction metrics. There is a need in the risk and asset management literature for a unified framework through which quantitative risk can be evaluated against tolerability criteria and trade-off decisions can be made between risk treatment options. The methodology uses quantitative risk measures for loss of life, loss of production and loss of property. A risk matrix is used to classify risk as intolerable, As Low As Reasonably Practicable (ALARP) or broadly tolerable. Risks in the intolerable and ALARP region require risk treatment, and risk treatment options are generated. Risk reduction benefit of the treatment options is quantified, and cost-benefit analysis is performed using discounted cashflow analysis. The Analytic Hierarchy Process is used to derive weights for prioritization criteria based on decision-maker preferences. The weights, along with prioritization criteria for risk reduction, tolerance criteria and project cost, are used to prioritize projects using the Technique for Order Preference by Similarity to Ideal Solution. The usefulness of the methodology for improved decision-making is illustrated using a numerical example.  相似文献

The evolution of safety performance evaluation, from incident-based to prospective and recently to holistic approaches, follows the similar evolutionary path of accident causation and safety management. Holistic approaches are characterized by an integrated use of technical, organisational and human factors for the development of safety performance indicators. The aim of this paper is to propose a set of assessment criteria for safety performance evaluation methods which will also give emphasis to their holistic nature. Specific criteria related to conceptual, methodological and practical characteristics of the methods have been initially selected and six safety performance evaluation methods are evaluated in a trial implementation of these criteria. Results indicate that the proposed criteria can be applied in different types of safety performance evaluation methods in order to extract useful information in regards to their holistic character as well as to their appropriateness and usefulness.  相似文献

Ethics are concerned with distinguishing between what actions are “right” and “wrong” and what values are “good” and “bad”, etc. and there is a long academic tradition in discussing ethics and ethical theories. Risk acceptance criteria, on the other hand, distinguish between levels of risks that are acceptable and levels that are intolerable. In some sense, one may say that risk acceptance criteria distinguishes between “good” and “bad” systems and activities with regards to the risk they expose the society or elements of a society to and there is thus an obvious link between ethics and risk acceptance criteria or to risk management at large. However, there are few references in the literature that explores this link, and in this paper, the ethical foundation of fundamental principles of risk acceptance criteria will be elaborated upon.This paper considers some important principles for establishing risk acceptance criteria for safety critical systems and activities. The various principles and the philosophies behind them might at first sight seem contradictory and exclusive, but it is demonstrated how they may coexist in one and the same regulatory regime; They may complement each other in order to achieve the overall safety objectives of society. Then, some brief considerations of the ethical foundations for the principles will be given and some relevant examples of actual risk acceptance criteria will be given from the maritime industries. However, it is believed that the principles and discussions are of general interest and apply to all areas of technical risk and to safety regulations in a broader perspective.  相似文献

The value of research on the accuracy of job analysis is questioned. It is argued that the traditional criteria employed to evaluate job analysis accuracy (i.e., interrater agreement and deviations from proxy true scores) provide information of little practical value. Alternative criteria focusing on the consequences of job analysis data are suggested. Consequence‐oriented criteria are clarified through a review of the various inferential leaps or decision points that job analysis supports. In addition, the consequences of job analysis are also thought to be a function of the rules governing the making of job‐analysis‐based inferences which, unfortunately, are sometimes unspecified in even the most molecular job analysis methodologies. Two exemplary exothermic processes, synthesis of nitroglycerine in a continuous stirred tank reactor (CSTR) and synthesis of the explosive RDX in a CSTR, are used to demonstrate the dangers of ignoring the system dynamics when defining criteria for thermal criticality or runaway. Stability analyses are necessary to prescribe such criteria, and for these systems prove the presence of dangerous oscillatory thermal instability which cannot be detected using the steady state thermal balances.  相似文献

Following the indications of earlier work, the use of a closed pressure vessel test (a mini-autoclave system) to estimate the explosive properties of organic compounds has been investigated. The dependent variable was an explosivity ranking derived from the results of a propagation of detonation test, a heating under confinement test and a deflagration test. Tentative criteria have been derived to allow the following conclusions:• Not detonable• Not Class 1• No explosive properties with respect to transport classificationThe criteria are based upon the maximum rate of pressure rise observed in the mini-autoclave and the temperature at which this occurs. They offer a more efficient screen than the decomposition energy criteria which are currently used for initial assessment of explosive properties. The importance of event temperature suggests that existing classification tests may be underestimating heating under confinement and deflagration hazards. In this case, maximum rates of pressure rise from a closed pressure vessel test might be a better basis of classification, at least for supply if not transport. Further work is necessary to test the proposed screening criteria and to assess the use of a closed pressure vessel test as a means of classification.  相似文献

Process hazard analysis (PHA) is a cornerstone of process safety management programs. The quality of the PHA performed directly affects the level of risk tolerated for a process. The lower the quality of a PHA, the more likely higher risk will be tolerated. There are few requirements for PHA team members in the U.S. Occupational Safety and Health Administration's process safety management regulations. More detailed requirements for participation in a PHA are desirable.A competency management program should be used to ensure PHA practitioners and teams are appropriately qualified. Criteria for selecting PHA team leaders, or facilitators, and other team members are key to such a program and are described in this paper. The criteria cover both technical and personal attributes. Application of the criteria is described and team performance metrics, which can be used to correlate performance with the assessment of competency to validate the criteria and methods used, are discussed.Owing to the importance of the role played by team leaders, certification of their competency is desirable. Management of safety, and barriers in particular, includes using information expressing performance, i.e. use of safety performance indicators. For this information to be useful, the indicators should demonstrate adequate quality. In other words, they should satisfy some predefined set of quality criteria. Without showing adequate quality, the indicators are generally unable to provide sufficient support for barrier management, which could result in poor decisions. In this article, the use of the SMART criteria is considered to assess the quality of safety performance indicators in process industries. SMART being an acronym for ‘specificity’, ‘measurability’ or ‘manageability’, ‘achievability’, ‘relevancy’ and ‘time-based’, covering five key aspects and criteria for assessing the quality of an indicator. A discussion on whether the indicators are able to demonstrate adequate quality by satisfying these criteria has been conducted. The finding is that all of the SMART criteria should be satisfied for a safety performance indicator to demonstrate acceptable quality and to be regarded as useful to support barrier management decision-making. However, it has also been observed that including the ‘M’ criterion in the assessment of quality is not needed. When all the other criteria are satisfied there is no way the conclusions could be misleading as a result of measurability or manageability aspects. Hence, for safety performance indicator quality, only four of the criteria are assessed and suggested for such situations to shorten the acronym to ‘STAR’. A key safety indicator used in downstream process facilities, i.e. ‘dangerous fluid overfilling events’, motivated from the 2005 Texas City refinery accident, is used to illustrate the situation. The indicator is also applied to another incident, the Buncefield oil storage depot's accident in 2005, to provide a broader context for using it. The findings in this article could also be applied beyond the context studied. This means that, despite focusing on safety indicators in the process industries, the findings are considered as relevant and applicable to other types of performance indicators and to other energy industries.  相似文献

根据航空运输企业安全生产系统的特征,制定相对科学的、适用的安全文化评价指标体系。将粗糙集、人工神经网络方法应用于航空安全文化评价研究。首先,通过问卷调查与粗糙集方法相结合,对安全文化评价指标信息表进行设定。利用粗糙集的属性约简功能,确定航空安全文化的核心要素;其次,利用人工神经网络对约简后指标权重进行计算;经过仿真实验,训练后的神经网络较好地获取并保存了安全文化评价的知识和经验,能够较为准确地对航空安全文化进行评价。  相似文献

Substantial efforts have been devoted to accident investigations, but do we gain a reasonable benefit from these efforts? The current study explores multilevel (individual, company, sector/trade, authorities) learning from major accidents and serious incidents. Specifically, this involves identifying a set of learning criteria, i.e. factors or indicators that potentially support and contribute to multilevel learning processes. Identifying such criteria is based on: (i) the accident investigation process, (ii) the follow-up efforts, and (iii) contextual aspects. Three Norwegian accident investigations in transportation (aviation, marine, rail) represent the empirical foundation for the study. Learning criteria were derived by both document analyses from the accidents (e.g. investigation reports) and literature reviews on organizational learning. These were the basis for a workshop regarding learning from accidents, with participants from the aviation, marine, and rail sector. Key results from this workshop are presented and discussed. The study concludes by presenting a set of criteria for learning from accidents.  相似文献

Introduction: Speed cameras have been implemented to improve road safety over recent decades in the UK. Although the safety impacts of the speed camera have been estimated thoroughly, the criteria for selecting camera sites have rarely been studied. This paper evaluates the current speed camera sites selection criteria in the UK based on safety performance. Method: A total of 332 speed cameras and 2,513 control sites with road traffic accident data are observed from 2002 to 2010. Propensity score matching method and empirical Bayes method are employed and compared to estimate the safety effects of speed cameras under different scenarios. Results: First, the main characteristics of speed cameras meeting and not meeting the selection criteria are identified. The results indicate that the proximity to school zones and residential neighborhoods, as well as population density, are the main considerations when selecting speed camera sites. Then the official criteria used for selecting camera sites are evaluated, including site length (a stretch of road that has a fixed speed camera or has had one in the past), previous accident history, and risk value (a numerical scale of the risk level). The results suggest that a site length of 500 m should be used to achieve the optimum safety effects of speed cameras. Furthermore, speed cameras are most effective in reducing crashes when the requirement of minimum number of historical killed and seriously injured collisions (KSIs) is met. In terms of the risk value, it is found that the speed cameras can obtain optimal effectiveness with a risk value greater than or equal to 30, rather than the recommended risk value of 22.  相似文献

Maintenance policy selection is a multiple criteria decision making. The criteria often considered are cost and reliability of maintenance. There has been a growing interest in using risk of accidents as a criterion for maintenance selection. This paper presents an approach of maintenance selection based on risk of equipment failure and cost of maintenance. Analytic hierarchy process (AHP) and goal programming (GP) are used for maintenance policy selection. A case study in a benzene extraction unit of a chemical plant was done. The AHP results show that considering risk as a criterion, condition based maintenance (CBM) is a preferred policy over time-based maintenance (TBM) as CBM has better risk reduction capability than TBM. Similarly, considering cost as a criterion, corrective maintenance (CM) is preferred. However, considering both risk and cost as multiple criteria, the AHP–GP results show that CBM is a preferred approach for high-risk equipment and CM for low risk equipment.  相似文献
