在复杂工业系统中,屏障是预防与减少人误的有效方法,一旦屏障失效,事故就有可能发生。笔者从屏障的概念与屏障分析入手,分析了实体屏障和管理屏障两类基本的屏障,介绍了屏障分析的6个步骤即辨识危险和目标、辨识屏障、评估屏障是否失效、弄清原因、评估与决定、制定方案及其分析案例,进行创建和维持屏障的方案分析,建立纠正行动计划的屏障方案,指出有效使用屏障分析技术来减少人误的几个场合以及屏障分析的局限性。  相似文献

OCI Nitrogen wants to gain knowledge of (leading) indicators regarding the process safety performance of their ammonia production process. This paper answers the question whether indicators can be derived from the barrier system status to provide information about the development and likelihood of the major accident processes in the ammonia production process.The accident processes are visualized as scenarios in bowties. This research focuses on the status of the preventive barriers on the left-hand side of the bowtie. Both the quality – expressed in reliability/availability and effectiveness – and the activation of the barrier system give an indication of the development of the accident scenarios and the likelihood of the central event. This likelihood is calculated as a loss of risk reduction compared to the original design. The calculation results in an indicator called “preventive barrier indicator”, which should initiate further action. Based on an example, it is demonstrated which actions should be taken and what their urgency is.  相似文献

An integrated approach for performance assessment and management of safety barriers in a systemic manner is needed concerning the prevention and mitigation of major accidents in chemical process industries. Particularly, the effects of safety barriers on system risk reduction should be assessed in a dynamic manner to support the decision-making on safety barrier establishments and improvements. A simulation approach, named Simulink-based Safety Barrier Modeling (SSBM), is proposed in this paper to conduct dynamic risk assessment of chemical facilities with the consideration of the degradation of safety barriers. The main functional features of the SSBM include i) the basic model structures of SSBM can be determined based on bow-tie diagrams, ii) multiple data (periodic proof test data, continuous condition-monitoring data, and accident precursor data) may be combined to update barrier failure probabilities and initiating event probabilities, iii) SSBM is able to handle uncertainty propagation in probabilistic risk assessment by using Monte Carlo simulations, and iv) cost-effectiveness analysis (CEA) and optimization algorithms are integrated to support the decision-making on safety barrier establishments and improvements. An illustrative case study is demonstrated to show the procedures of applying the SSBM on dynamic risk-informed safety barrier management and validate the feasibility of implementing the SSBM for cost-effective safety barrier optimization.  相似文献

Management of safety, and barriers in particular, includes using information expressing performance, i.e. use of safety performance indicators. For this information to be useful, the indicators should demonstrate adequate quality. In other words, they should satisfy some predefined set of quality criteria. Without showing adequate quality, the indicators are generally unable to provide sufficient support for barrier management, which could result in poor decisions. In this article, the use of the SMART criteria is considered to assess the quality of safety performance indicators in process industries. SMART being an acronym for ‘specificity’, ‘measurability’ or ‘manageability’, ‘achievability’, ‘relevancy’ and ‘time-based’, covering five key aspects and criteria for assessing the quality of an indicator. A discussion on whether the indicators are able to demonstrate adequate quality by satisfying these criteria has been conducted. The finding is that all of the SMART criteria should be satisfied for a safety performance indicator to demonstrate acceptable quality and to be regarded as useful to support barrier management decision-making. However, it has also been observed that including the ‘M’ criterion in the assessment of quality is not needed. When all the other criteria are satisfied there is no way the conclusions could be misleading as a result of measurability or manageability aspects. Hence, for safety performance indicator quality, only four of the criteria are assessed and suggested for such situations to shorten the acronym to ‘STAR’. A key safety indicator used in downstream process facilities, i.e. ‘dangerous fluid overfilling events’, motivated from the 2005 Texas City refinery accident, is used to illustrate the situation. The indicator is also applied to another incident, the Buncefield oil storage depot's accident in 2005, to provide a broader context for using it. The findings in this article could also be applied beyond the context studied. This means that, despite focusing on safety indicators in the process industries, the findings are considered as relevant and applicable to other types of performance indicators and to other energy industries.  相似文献

海上钻完井作业面临海洋环境恶劣、浅层地质灾害等复杂工况,极易发生油气泄漏、井喷等事故。为有效预防海上钻完井作业事故,提出基于瑞士奶酪模型的安全屏障模型。采用事故树和故障模式及影响分析相结合的方法,分析作业过程风险。该模型根据挪威标准D-010,建立完井作业关井阶段的物理安全屏障和安全屏障控制原理图,在此基础上构建油气泄漏事故树和失效模式与影响分析表,找出关井阶段可能的油气泄漏途径。通过对重要度计算和风险优先度值排序确定作业过程中最薄弱的安全屏障和关键故障模式。结果表明,作业过程中最薄弱的安全屏障是采油树、油管和地面控制井下安全阀(SCSSV),采油树腐蚀、密封失效、油管接头密封失效和SCSSV开关故障是影响作业过程的关键故障模式。  相似文献

高速铁路经过城市建成区两侧分布有高层住宅建筑,普通直立式、折角式声屏障对于较高楼层难以有效阻隔噪声,框架式声屏障具有更好的降噪效果,对其降噪效果的研究越来越成为工程设计中噪声治理选取的重要依据。在阐述框架式结构声屏障插入损失确定方法的基础上,分别采用边界元法、统计能量法预测框架式声屏障的降噪效果,通过实际案例加以类比验证,得出京沈客运专线框架式声屏障降噪效果。  相似文献

明确建设项目安全管理的意义;简述建设项目安全管理技术的国内外研究现状;指出当前的建设项目安全管理研究存在的主要不足;提出笔者研究的思路和研究的主要内容;收集详细的施工现场资料,参考相关历史文献资料,提取可能引起安全事故的原始不确定风险因素,建立安全管理风险评价指标体系;并利用人工神经网络(ANN)强大的非线性函数逼近能力,将BP神经网络引入建设项目风险控制,构建基于BP神经网络的建设项目安全控制系统,从而实现建设项目安全管理的智能化。通过工程实例验证:该方法具有可行性和有效性,为实现建设项目安全管理目标及工期、成本、质量目标打下坚实的基础。  相似文献

基于比较科学理论与安全科学学的原理,将比较方法应用于安全科学方法学研究;提出比较安全学的定义,分析其内涵,从安全科学的理论类别、灾害类别、安全管理类别、工程类别与安全学的纵向分支5个方面构建比较安全学的学科分支体系,并阐述了各分支体系研究内容,在此基础上构建了比较安全学学科体系框架;综合与比较安全学相关的文献资料,依据其应用的个案探讨比较学在安全科学中的应用,阐述比较安全学应用与研究的广阔前景。研究表明,多视角的比较安全学构建了诸多新的安全方法学思路和研究方向,为构建安全科学方法学体系提供了基础。  相似文献

In this theory-driven literature review we examine how leadership and emerging research in positive organizational behaviour (POB) may inform our understanding of human mechanisms that affect safety outcomes. According to authentic leadership theory, leader self-awareness and self-regulation processes are vital mechanisms in the leader-follower exchange. From emerging research on authentic leadership, we propose that production management values, attitudes, and behaviour are linked to safety climate and safety outcomes in safety critical organizations (SCOs). According to recent developments in management theory, “psychological capital” is comprised of four distinct aspects that can be linked to desirable organizational outcomes and sustained high quality performance in individual workers. From this we offer a research model and five research propositions implicating that authentic leadership directly affects safety outcomes via promoting positive safety climate perceptions. Furthermore, we propose a second path where psychological capital mediates the relationship between authentic leadership, safety climate and safety outcomes in SCOs.  相似文献

为了研究国际工程安全管理在国家(地区)、研究人员的合作现状,基于研究热点与研究前沿的分析结果,对工程安全管理科研合作研究未来的发展趋势进行预测,利用CiteSpace的可视化与统计功能对工程安全管理领域展开研究,得到可视化知识图谱。研究结果表明:国际上关于工程安全管理的研究主要集中在欧美等发达国家,许多国家均与美国建立了合作,而其他国家间的合作较为分散,呈现明显的“无标度”现象; 2003—2017年国际工程安全管理研究的热点领域集中在文化和技术层面; 2003—2017年工程安全管理的研究热点可以分为5个阶段分析,当前的研究热点为施工现场的安全管理及施工前的风险管控。  相似文献

Process safety is the common global language used to communicate the strategies of hazard identification, risk assessment and safety management. Process safety is identified as an integral part of process development and focuses on preventing and mitigating major process accidents such as fires, explosions, and toxic releases in process industries. Accident probability estimation is the most vital step to all quantitative risk assessment methods. Drilling process for oil is a hazardous operation and hence safety is one of the major concerns and is often measured in terms of risk. Dynamic risk assessment method is meant to reassess risk in terms of updating initial failure probabilities of events and safety barriers, as new information are made available during a specific operation. In this study, a Bayesian network model is developed to represent a well kick scenario. The concept of dynamic environment is incorporated by feeding the real-time failure probability values (observed at different time intervals) of safety barriers to the Bayesian network in order to obtain the corresponding time-dependent variations in kick consequences. This study reveals the importance of real-time monitoring of safety barrier performances and quantitatively shows the effect of deterioration of barrier performance on kick consequence probabilities. The Macondo blowout incident is used to demonstrate how early warnings in barrier probability variations could have been observed and adequately managed to prevent escalation to severe consequences.  相似文献

Introduction: The main objective of this research is to investigate the effect of traffic barrier geometric characteristics on crashes that occurred on non-interstate roads. Method: For this purpose, height, side-slope rate, post-spacing, and lateral offset of about 137 miles of traffic barriers were collected on non-interstate (state, federal aid primary, federal aid secondary, and federal aid urban) highways in Wyoming. In addition, crash reports recorded between 2008 and 2017 were added to the traffic barrier dataset. The safety performance of traffic barriers with regards to their geometric features was analyzed in terms of crash frequency and crash severity using random-parameters negative binomial, and random-parameters ordered logit models, respectively. Results: From the results, box beam barriers with a height of 27–29 inches were less likely to be associated with injury and fatal injury crashes compared to other barrier types. On the other hand, the likelihood of a severe injury crash was found to be higher for box beam barriers with a height taller than 31 inches. Both W-beam and box beam barriers with a post-spacing between 6.1 and 6.3 inches reduced the probability of severe injury crashes. In terms of the crash frequency, flare traffic barriers had a lower crash frequency compared to parallel traffic barriers. Non-interstate roads without longitudinal rumble strips were associated with a higher rate of traffic barrier crashes.  相似文献

Blowout is one of the most serious accidents in the offshore oil and gas industry. Accident records show that most of the offshore blowouts have occurred in the drilling phase. Efficient measures to prevent, mitigate, and control offshore drilling blowouts are important for the entire offshore oil and gas industry. This article proposes a new barrier-based accident model for drilling blowouts. The model is based on the three-level well control theory, and primary and secondary well control barriers and an extra well monitoring barrier are established between the reservoir and the blowout event. The three barriers are illustrated in a graphical model that is similar to the well-known Swiss cheese model. Five additional barriers are established to mitigate and control the blowout accident, and event tree analysis is used to analyze the possible consequence chains. Based on statistical data and literature reviews, failures of each barrier are presented. These failures can be used as guidance for offshore drilling operators to become aware of the vulnerabilities of the safety barrier system, and to assess the risk related to these barriers. The Macondo accident is used as a case study to show how the new model can be used to understand the development of the events leading to the accident. The model can also be used as an aid to prevent future blowouts or to stop the escalation of events.  相似文献

Occupational health and safety is established as a factor in international competitiveness, but we question the relevance of this economic argument at the organisational level. On this basis we assess the place of health and safety in the senior management agenda, based on findings from recent market research. Out of this we take a brief perspective on the literature on performance assessment and corporate reputation, and their relationship with health and safety in the workplace. We then report original in-depth research on directors' attitudes to and practice in workplace health and safety. Accident investigations indicate that inadequate barrier management has been a main cause of many accidents in the process industry. In 2013, the Norwegian Petroleum Safety Authority issued a barrier management framework for the offshore oil and gas industry. The framework describes principles related to barrier management and may be a valuable guide for the entire process industry. However, the offshore industry faces several challenges when implementing the framework. This paper discusses these challenges and clarifies the central concepts and steps of barrier management. A key message is the need for clarity and integration in a systematic approach to risk and barrier management.  相似文献

The main objective of this paper is to present and discuss a set of scenarios that may lead to hydrocarbon releases on offshore oil and gas production platforms. Each release scenario is described by an initiating event (i.e., a deviation), the barrier functions introduced to prevent the initiating event from developing into a release, and how the barrier functions are implemented in terms of barrier systems. Both technical and human/operational safety barriers are considered. The initiating events are divided into five main categories: (1) human and operational errors, (2) technical failures, (3) process upsets, (4) external events or loads, and (5) latent failures from design. The release scenarios may be used as basis for analyses of: (a) the performance of safety barriers introduced to prevent hydrocarbon releases on specific platforms, (b) the platform specific hydrocarbon release frequencies in future quantitative risk analyses, (c) the effect on the total hydrocarbon release frequency of the safety barriers and risk reducing measures (or risk increasing changes).  相似文献

安全系统工程学是近几年发展起来的新兴学科,作为我国安全工程专业专业课设置中的一个重要内容,其地位显得越来越重要。为了了解和掌握近年我国安全系统工程学的研究进展,用文献统计的方法对近十年(2000-2009年)中国期刊全文数据库、中国优秀硕士学位论文全文数据库、中国博士学位论文全文数据库等收录的关于安全系统工程学方面的论文进行检索;分析、比较统计的结果后发现,近十年我国安全系统工程学发展迅速并有逐年上升趋势,期间安全系统工程学自身内容也逐步发展完善,其研究领域广泛分布于其他各领域学科中,该阶段安全系统工程学的研究内容主要集中于系统安全分析和安全评价上。根据统计结果以及现阶段我国安全系统工程学的发展现状,结合安全科学和系统工程的知识对今后学科发展提出建议。  相似文献

The performance assessment of safety barriers is essential to find vulnerable elements in a safety barrier system. Traditional performance assessment approaches mainly focus on using several static indicators for quantifying the performance of safety barriers. However, with the increasing complexity of the system, emerging hazards are highly uncertain, making it challenging for the static indicators to assess the performance of safety barriers. This paper proposes a resilience−based performance assessment method for safety barriers to overcome this problem. Safety barriers are classified according to their functions first. The dynamic Bayesian network (DBN) is then introduced to calculate the availability function under normal and disruption conditions. The ratio of the system's availability, when affected by the disruption, to the initial availability, is used to determine the absorption capacity of the system. The ratio of the quantity of availability recovery to the total quantity of system represents the adaptation and restoration capacity of the system. The system's resilience is represented by the sum of absorption, adaptation, and restoration capacities. The wax oil hydrogenation process is used to demonstrate the applicability of the proposed methodology.  相似文献
