Chorionic villus sampling (CVS) with either transcervical catheters or transabdominal needles is a widely-accepted method for prenatal diagnosis. However, there exists a small subset of patients in whom sampling is difficult or impossible with either route because of individual anatomic variations. A new method of chorionic villus biopsy has been developed to circumvent these problems, utilizing transvaginal chorionic needle aspiration guided by an intravaginal ultrasound probe. This technique was performed successfully in 15 patients in whom villi could not be obtained by either of the conventional methods. This method now makes CVS possible in essentially all women regardless of their uterine anatomy or placental placement; it may also prove useful for very early chorionic sampling. 相似文献

We studied the validity of mid-trimester amniotic fluid testosterone and FSH in the diagnosis of fetal sex in abnormal fetuses and in twins. It was found that the testosterone/FSH ratio, but not the level of the individual hormones, was an excellent criterion for fetal sex diagnosis. In a control group of 32 female and 34 male fetuses, the testosterone/FSH ratio was diagnostic. In two cases the values were indeterminate and fetal sex could not be predicted, but there were no false diagnoses. The abnormalities of the fetuses were diagnosed by karyotype analyses or raised levels of α-fetoprotein. It was found that neither autosomal trisomies nor X-chromosome aneuploidy or neural tube defect interfered with the diagnostic value of the hormonal approach. The twin study included 14 pairs, three of whom were discordant for sex. In all twins the fetal sex diagnosis was appropriate. In co-twins of the same sex the hormone levels were very close to each other. 相似文献

This paper reports eight cases of non-mosaic, rare, and typically lethal trisomies diagnosed in chorionic villi and not confirmed by amniocentesis. Four cases were 47,XX, + 16; two cases were 47,XX, +2; one was 47,XX, + 12; and one was 47,XY, +7. There have been no known complications in any of these gestations. These eight cases were found in a series of approximately 12 000 samples processed in our laboratory (0.07 per cent). We conclude that (1) rare non-mosaic trisomy not reflecting the fetal condition is an occasional source of diagnostic ambiguity in chorionic villus sampling; and (2) when encountered, a follow-up amniocentesis should be recommended to the patient to confirm or rule out the abnormality. We propose the term ‘confined placental abnormality’ to describe non-mosaic trisomies and other related abnormalities found only in chorionic tissue. 相似文献

A fish‐consumption advisory is currently in effect in a seven‐mile stretch of the Grasse River in Massena, New York, due to elevated levels of PCBs in fish tissue. One remedial approach that is being evaluated to reduce the PCB levels in fish from the river is in situ capping. An in‐river pilot study was conducted in the summer of 2001 to assess the feasibility of capping PCB‐containing sediments of the river. The study consisted of the construction of a subaqueous cap in a seven‐acre portion of the river using various combinations of capping materials and placement techniques. Optimal results were achieved with a 1:1 sand/topsoil mix released from a clamshell bucket either just above or several feet below the water surface. A longer‐term monitoring program of the capped area commenced in 2002. Results of this monitoring indicated: 1) the in‐place cap has remained intact since installation; 2) no evidence of PCB migration into and through the cap; 3) groundwater advection through the cap is not an important PCB transport mechanism; and 4) macroinvertebrate colonization of the in‐place cap is continuing. Additional follow‐up monitoring in the spring of 2003 indicated that a significant portion of the cap and, in some cases, the underlying sediments had been disturbed in the period following the conclusion of the 2002 monitoring work. An analysis of river conditions in the spring of 2003 indicated that a significant ice jam had formed in the river directly over the capping pilot study area, and that the resulting increase in river velocities and turbulence in the area resulted in the movement of both cap materials and the underlying sediments. The pilot cap was not designed to address ice jam–related forces on the cap, as the occurrence of ice jams in this section of the river had not been known prior to the observations conducted in the spring of 2003. These findings will preclude implementation of the longer‐term monitoring program that had been envisioned for the pilot study. The data collected immediately after cap construction in 2001 and through the first year of monitoring in 2002 serve as the basis for the conclusions presented in this article. It should be recognized that, based on the observation made in the spring of 2003, some of these conclusions are no longer valid for the pilot study area.The occurrence of ice jams in the lower Grasse River and their importance on sediments and PCBs within the system are currently under investigation. © 2003 Wiley Periodicals, Inc. 相似文献

ABSTRACT: Discrete cold water patches within the surface waters of summer warm streams afford potential thermal refuge for cold water fishes during periods of heat stress. This analysis focused on reach scale heterogeneity in water temperatures as influenced by local influx of cooler subsurface waters. Using field thermal probes and recording thermistors, we identified and characterized cold water patches (at least 3°C colder than ambient streamflow temperatures) potentially serving as thermal refugia for cold water fishes. Among 37 study sites within alluvial valleys of the Grande Ronde basin in northeastern Oregon, we identified cold water patches associated with side channels, alcoves, lateral seeps, and floodplain spring brooks. These types differed with regard to within floodplain position, area, spatial thermal range, substrate, and availability of cover for fish. Experimental shading cooled daily maximum temperatures of surface waters within cold water patches 2 to 4°C, indicating a strong influence of riparian vegetation on the expression of cold water patch thermal characteristics. Strong vertical temperature gradients associated with heating of surface layers of cold water patches exposed to solar radiation, superimposed upon vertical gradients in dissolved oxygen, can partially restrict suitable refuge volumes for stream salmonids within cold water patches. 相似文献

ABSTRACT: Pesticides in stormwater runoff, within the Sacramento River Basin, California, were assessed during a storm that occurred in January 1994. Two organophosphate insecticides (diazinon and methidathion), two carbamate pesticides (molinate and carbofuran), and one triazine herbicide (simazine) were detected. Organophosphate pesticide concentrations increased with the rising stage of the hydrographs; peak concentrations were measured near peak discharge. Diazinon oxon, a toxic degradation product of diazinon, made up approximately 1 to 3 percent of the diazinon load. The Feather River was the principal source of organophosphate pesticides to the Sacramento River during this storm. The concentrations of molinate and carbofuran, pesticides applied to rice fields during May and June, were relatively constant during and after the storm. Their presence in surface water was attributed to the flooding and subsequent drainage, as a management practice to degrade rice stubble prior to the next planting. A photo-degradation product of molinate, 4-keto molinate, was in all samples where molinate was detected and made up approximately 50 percent of the total molinate load. Simazine, a herbicide used in orchards and to control weeds along the roadways, was detected in the storm runoff, but it was not possible to differentiate the two sources of that pesticide to the Sacramento River. 相似文献
