Major hazards risk indicators are proposed for offshore installations, based on what has been used by the Petroleum Safety Authority Norway for the Risk Level approach in the Norwegian offshore petroleum industry. Since 2002 also leading indicators are used, in the sense that indicators for barrier performance are included together with the lagging indicators. There are individual indicators for a number of barrier functions, including fire and gas detection, a number of safety valves, active fire protection and mustering of personnel. The performance of the leading indicators during several years of data collection as well as lessons learned from the project are discussed. This is followed by a review of how indicators may be used by individual companies, and how the lessons may be utilised by individual companies to develop risk indicators for individual installations as well as company wide indicators. 相似文献

The context and habits of accident investigation practices were explored by means of questionnaire data obtained from accident investigators in the healthcare, transportation, nuclear and rescue sectors in Sweden. Issues explored included; resources, training, time spent in different phases of an investigation, methods and procedures, beliefs about causes to accidents, communication issues, etc. Examples of findings were: differences in the extent to which the ‘human factor’ was perceived as a dominant cause to accidents; manning resources to support investigations were perceived as rather scarce; underutilization of data from safety related processes such as risk analysis and auditing data; the phase of suggesting remedial actions (recommendations) were comparatively brief and generally not well supported. A majority of the investigators thought that the investigations were free from pressures to follow a specific direction; the investigators also thought that performing an investigation in itself (regardless of the specific results) had positive influences on safety. A majority of the investigators thought that upper management had a relatively strong influence on safety in the organizations. The results are discussed in terms of suggestions for strategies to strengthen investigation practices – particularly those conducted as part-time work in organizations. 相似文献

The transboundary Georgia Basin Puget Sound ecosystem is situated in the southwest corner of British Columbia and northwest comer of Washington State. While bountiful and beautiful, this international region is facing significant threats to its marine and freshwater resources, air quality, habitats and species. These environmental challenges are compounded by rapid population growth and attendant uiban sprawl. As ecosystem stresses amplified and partnerships formed around possible solutions, it became increasingly clear that the shared sustainability challenges in the Georgia Basin and Puget Sound required shared solutions. Federal, state and provincial institutional arrangements were made between jurisdictions, which formalized small scale interest in transboundary management of this ecosystem. Formal agreements, however, can only do so much to further management of an ecosystem that spans international boarders. A transboundary regional research meeting, the 2003 GB/PS Research Conference, opened the doors for large-scale informal cross-boarder cooperation and management. In recognition of the need to estimate biosorption for natural microbial populations, the variability of partition coefficients for two hydrophobic pollutants to natural populations from a variety of aquatic systems was investigated. Biosorption partition coefficents for pyrene [2.46(±0.6) × 104] and phenanthrene [6.34(±1) × 103] were nearly constant over 14 different microbial sources, consisting of sediments and soils from eight states. For these condensed ring aromatics, semi-empirical equations were developed relating biosorption partition coefficients to octanol/water partition coefficients and to water solubility and melting point. A model for the accumulation of mercury in four compartments of woodland stream has been set up. The four components considered are sediments, detritus, invertebrates and fish. The model inputs are concentrations of dissolved monomethyl mercury and dissolved inorganic mercury (Hg2+) in the water. Choices of where to carry out everyday activities in large-scale environments were conceptualized as a process of forming ‘travel plans’, and, to test a model of how such plans are formed, three experimental simulations of a planning task were performed in the laboratory. In Experiment 1, subjects (high school students) were found to choose a shortest route to travel between a number of actual, familiar locations in a town by first choosing the order between the locations that minimized straight-line (Euclidean) distances, then choosing the shortest paths between the locations in the constrained order. The order choices were, in Experiment 2, found to be made by minimizing distance locally rather than globally, except in some cases when ‘spatial configurations’ of the locations were discovered. Both the results of this experiment and of Experiment 3 suggested that such discoveries were facilitated by a simultaneous representation of the locations which was possible when they were positions on a display, committed to short-term memory or available for perceptual inspection, but to a less extent when they were actual locations. 相似文献
