The outcome of the pregnancy following (a) a mid-trimester termination of pregnancy (TOP) for fetal neural tube defect (NTD) (77 women=group 1); (b) mid-trimester TOP for fetal Down's syndrome (13 women=group 2); (c) delivery of a baby with NTD (119 women=group 3) was studied. The prenatal fetal loss was relatively high in all groups. In group 1 it was similar to that found in other studies after first trimester TOP, in group 2 it was associated with advanced maternal age and the unexpected finding in group 3 was not attributable to advanced maternal age. It is suggested that a previous NTD per se might increase the risk of fetal loss in the next pregnancy. A previous mid-trimester TOP for NTD was not associated with an increase in premature labour, small for dates babies or congenital abnormality in the next pregnancy, but there was a slight increase in the number of babies weighing less than 2500 g. 相似文献

Parental decisions concerning the continuation of pregnancy following prenatal detection of abnormal chromosomes were evaluated for 80 patients whose diagnosis and prenatal counselling were performed in our centre. Twenty-two anomalies were diagnosed by chorionic villus sampling (CVS) and 58 by amniocentesis. The severity of the chromosome anomaly and associated ultrasound findings in the first vs. second trimester were correlated with patients' decisions. No difference was found in the likelihood of parental decisions to interrupt or continue a pregnancy between CVS and amniocentesis for either the‘severe’ or the‘questionable’ group of chromosome anomalies. Ninety-three per cent of patients with severe prognosis and 27 per cent with questionable prognosis opted for pregnancy termination (p <0·0001). The association of ultrasound anomalies and termination was highly significant (p< 0·001). The severity of the chromosome anomaly, and, to a lesser extent, the visualization of anomalies on ultrasound were the major determinants of parental decisions to terminate the pregnancy. An amniocentesis was performed at 13.3 weeks' gestation for advanced maternal age. A mosaic sex chromosome pattern was found: of 50 cells examined, 34 had a 45,X karyotype. In 14 cells with a modal number of 46, a recognizable Y was substituted by a small non-fluorescent marker. C-banding identified the marker as an isodicentric in 12 cells. In two cells, the non-fluorescent marker appeared to be monocentric and looked like a non-fluorescent del (Yq), but could have been an isodicentric Y with inactivation of one of the centromeres. Two cells with a modal number of 47 showed two copies of the monocentric marker. Fluorescent in situ hybridization with an alpha satellite Y-specific centromeric probe confirmed the Y-chromosome origin of the markers and allowed for more accurate prenatal diagnostic information. 相似文献

Risk governance of GM plants and GMfood products is presently subject to heatedscientific and public controversies. Scientistsand representatives of the biotechnologyindustry have dominated debates concerningsafety issues. The public is suspicious withregard to the motives of scientists, companies,and political institutions involved. Thedilemmas posed are nested, embracing valuequestions, scientific uncertainty, andcontextual issues. The obvious lack of data andinsufficient information concerning ecologicaleffects call for application of thePrecautionary Principle (PP). There are,however, divergent opinions among scientistsabout the relevance of putative hazards,definition of potential ``adverse effects,' andwhether actions should be taken to preventharm. The reliance on the concept ofsubstantial equivalence in safety evaluation ofGM food is equally controversial. Consequently,value assumptions embedded in a scientificframework may be a barrier for employment ofthe PP. One of our major conclusions is thatprecautionary GMP usage requires riskassessment criteria yet undeveloped, as well asbroader and more long-term conceptions of risk,uncertainty, and ignorance. Conflicts ofinterest and public participation are otherissues that need to be taken intoconsideration. GMP governance regimes that arejustifiable from a precautionary and ethicalpoint of view must transcend traditionalscientific boundaries to include alternativescientific perspectives as well as publicinvolvement. 相似文献

The river Rhine has been in humanity's use for many centuries for a variety of activities. However, in our time, considerable changes in the course and the natural conditions of the river and the increasing use of the river for the discharge of wastewater has caused serious floods and major ecological problems. Since 1950, the International Commission for the Protection of the Rhine (ICPR) acts as the coordination point between the states bordering the Rhine for the development of programmes for river protection. The Sandoz disaster in 1986 was a turning point in the approach of the ICPR and the starting point for the present strategy of integrated riverbasin management. The Muggah Creek estuary in Sydney, Nova Scotia, received liquid and solid wastes from a steel mill and its associated coke ovens for approximately 100 years. This resulted in pollution of soils and sediments with polycyclic aromatic hydrocarbons (PAHs), polychlorinated biphenyls (PCBs), heavy metals, and other pollutants, including those in untreated domestic wastewaters. The Canadian federal and Nova Scotia provincial governments organized the Sydney Tar Ponds Agency (STPA) to develop a remediation approach for the Coke Ovens site soils and Sydney Tar Ponds sediments. The STPA developed a remediation approach for the Sydney Tar Ponds sediments, involving solidification/stabilization (S/S) through mixing cement and other materials into the sediments, and then capping them as a waste pile. High‐density polyethylene (HDPE) plastic sheeting vertical barriers are proposed to be used to divert groundwater and surface water from entering into the S/S‐treated sediments and to collect any water and associated pollutants released from the S/S‐treated sediments. The Coke Ovens site soils are proposed to be landfarmed to reduce some of the PAHs and other pollutants and then capped with a layer of soil. This remediation program is estimated to cost on the order of $400 million (CAN). This article discusses the appropriateness of using landfills as part of remediating hazardous chemical and Superfund sites, with particular emphasis on providing for true long‐term public health and environmental protection from the wastes and contaminated soils that are placed in the landfills. On‐site landfilling or capping of existing wastes is typically the least expensive approach for gaining some remediation of existing hazardous chemical/Superfund sites. The issues of the deficiencies in US EPA and state landfilling approaches discussed herein are also applicable to the landfilling of municipal and industrial solid “nonhazardous” wastes. These deficiencies were presented in part as “Problems with Landfills for Superfund Site Remediation” at the US EPA National Superfund Technical Assistance Grant Workshop held in Albuquerque, New Mexico, in February 2003. They are based on the author's experience in investigating the properties of landfill liners and the characteristics of today's landfills, relative to their ability to prevent groundwater pollution and to cause other environmental impacts. Discussed are issues related to both solid and hazardous waste landfills and approaches for improving the ability of landfills to contain wastes and monitor for leachate escape from the landfill for as long as the wastes in the landfill will be a threat. © 2004 Wiley Periodicals, Inc. 相似文献

Many Superfund/hazardous chemical sites include waterbodies whose sediments contain hazardous chemicals. With the need to assess, rank, and remediate contaminated sediments at such sites, as well as in other waterways, regulators seek a simple, quantitative assessment approach that feeds easily into a decision‐making scheme. Numeric, co‐occurrence‐based “sediment quality guidelines” have emerged with the appearance of administrative simplicity. However, the very foundation of the co‐occurrence approach, based on the total concentrations of a chemical(s) in sediment, is technically invalid; its application relies on additional technically invalid presumptions. Use of technically invalid evaluation approaches renders any assessment of the significance of sediment contamination unreliable. This article reviews the technical roots and assumptions of the co‐occurrence‐based SQGs, the fundamental flaws in the rationale behind their development and application, and their misapplication for sediment quality evaluation. It also reviews concepts and approaches for the more reliable evaluation, ranking, and cleanup assessment of contaminated sediments at Superfund sites and elsewhere. © 2005 Wiley Periodicals, Inc. 相似文献

A disturbing trend among governmental agencies is the remediation of so‐called “nonhazardous” contaminated sediments/soils by deposition in minimum‐design Subtitle D municipal solid waste (MSW) landfills or landfills with equivalent design. This is done despite the fact that, in terms of protection of public health and environmental quality, the designation “nonhazardous” is misleading at best, and the fact that minimum‐design Subtitle D landfills as being allowed will not ensure protection of groundwater quality for as long as the buried wastes remain a threat. Although acknowledged in the regulatory documentation and exposed in the writings of a few in the scientific/engineering community, the environmental and public health issues that will inevitably be faced at minimum‐design Subtitle D landfills are underplayed, and even misrepresented, to the public. Discussion of relevant issues, as well as remarkable omissions, characterized the October 2004 United States Army Corps of Engineers (US ACE)/United States Environmental Protection Agency (US EPA)/Sediment Management Work Group (SMWG) conference,” Addressing Uncertainty and Managing Risk at Contaminated Sediment Sites.” This article addresses many of those neglected issues. © 2005 Wiley Periodicals, Inc. 相似文献
