Chains of accidents (the domino effect) have been occurring with ever increasing frequency in chemical process industries. This is reflected in several accidents ‘J Loss Prevent Process Ind 12 (1999a) 361’; the world's worst industrial accident of the 1990s — the Vishakhpatnam disaster — also involved the domino effect ‘J Loss Prevent Process Ind 12 (1999a) 361; and Process Safety Prog 18 (1999b) 135’. Such chains of accidents have a greater propensity to cause damage than stand-alone accidents ‘Process Safety Prog 17(2) (1998a) 107; and J Loss Prevent Process Ind 12 (1999a) 361’.In order to assess the likelihood of occurrence of the domino effect and its damage potential, use of deterministic models in conjunction with probabilistic analysis is required. Recently we have proposed a systematic methodology called ‘domino effect analysis’ (DEA). A computer-automated tool, DOMIFFECT, has also been developed by us based on DEA ‘Process Safety Prog 17(2) (1998a) 107; Environment Model Software 13 (1998b) 163; and Risk assessment in chemical process industries: advanced techniques. Discovery Publishing House (1998c)’.This paper illustrates the application of DEA and DOMIFFECT to an industrial complex comprising 16 different industries. Out of 12 credible accident scenarios envisaged in three different industries — namely Madras Refineries Limited (MRL), UB Petrochemicals (UBP) and Indian Organic Chemicals Limited (IOCL), eight scenarios are likely to cause the domino effect. A further detailed analysis reveals that accidents in the storage of liquified petroleum gas and propylene and in the reflux drum units of MRL may cause domino effects. Similarly, propylene storage of UBP and monoethylene glycol storage of IOCL are also likely to cause domino effects. The impact of various chains of accidents has been forecast which reveals that in several cases the accidents may be catastrophic, harming the entire industrial complex of 16 industries. The study leads to the identification of ‘hot spots’ — units that pose the greatest risk — in turn forewarning the industries concerned and enabling them to prioritize and augment accident-prevention steps.  相似文献

Objective: On 2-lane roads, discomfort glare can cause annoyance, discomfort, inconvenience, stress, and fatigue to drivers, posing a risk of accidents. The aim of this study is to evaluate discomfort glare from the headlamps of cars widely used in Iran.

Methods: The discomfort glare of new vehicles including Pride Saba model GTX, Pride Model 131SL, Samand Soren, Peugeot 405, Megane, and Peugeot Pars was examined at distances of 5 to 100 m at a background luminance of 50 cd/m2 (late twilight/early dawn lighting) and 1 cd/m2 (nighttime) using Schmidt-Clausen and Bindels model and de Boer's subjective scale.

Results: According to the de Boer scale, at a background luminance of 50 cd/m2, the discomfort glare for all studied vehicles was between 1.98 and 4.05 in high-beam mode and between 3.5 and 5.4 in low-beam mode. At a background luminance of 1 cd/m2, discomfort glare was between 0.41 and 2.48 in high-beam mode and between 1.93 and 3.84 in low-beam mode. In high-beam mode, the average levels of discomfort glare of these vehicles gradually increased when the distance between cars was reduced by up to about 20 m. In low-beam mode, there was no discomfort glare up to a vehicle distance of 40 m. In addition, at an angle of 1.15°–5.73° between the line of sight and light of vehicles in high-beam mode, the level of discomfort glare was increased, but at an angle of 5.73°–22.9° the level of discomfort glare was reduced. In low-beam mode at an angle of 2.86°–22.9°, the level of discomfort glare was almost identical. The results show that in high-beam mode and with a 100-m distance between vehicles as well as in low-beam mode at intervals of less than 40 m between cars, discomfort glare is created.

Conclusion: It can be concluded that by providing solutions such as installing road lighting system, an increase in luminance of roads, separating or widening road lanes, increasing the lateral distance between vehicles, and increasing the angle between lighted vehicles and drivers can noticeably reduce discomfort glare.  相似文献

A new kinetic method has been developed for the determination of nitrite in fruit juice samples. The method is based on the catalytic effect of nitrite with the oxidation of Nile Blue A (NBA) by KBrO3 in the sulfuric acid medium. The optimum conditions obtained are 1.2 mM sulfuric acid, 0.034 mM of NBA, 2.8?×?10?3 M KBrO3, reaction temperature of 20 °C, and reaction time of 100 s at 595.5 nm. Under the optimized conditions, the method allowed the quantification of nitrite in a range of 0.2–800 μg/mL with a detection limit of 0.02 μg/mL. The method was applied to the determination of nitrite in 15 brands of fruit juice samples.  相似文献

The paper presents an inventory, perhaps the most comprehensive till date, of the major process-industry accidents involving ‘domino effect’. The inventory includes, among other relevant information, the sequence of accidents that had occurred in each domino episode. The information has been analyzed to identify several patterns which may be useful in further work on understanding domino effect and reducing the probability of its occurrence in future. A new electrochemical adsorptive stripping voltammetry method was developed for the determination of trace amounts of copper in food and water samples. The study of electrochemical behavior of Cu ion indicated that Cu(II) and Schiff base formed a complex in H3BO4–NaOH buffer solution (pH?=?7.25). An accumulation potential of ?100 mV (vs Ag/AgCl) was applied while the solution was stirred for 60 s. The response curve was recorded by scanning the potential, and the peak current of ?0.31 V (vs Ag/AgCl) was recorded. The peak current and concentration of copper accorded with linear relationship in the range of 0.04–120 ng mL?1. The relative standard deviation (for 12 ng mL?1 of copper) was 1.73 %, and the detection limit was 0.007 ng mL?1. The possible interference of some common ions was studied. The proposed method was applied to the determination of copper in water, rice, wheat, tea, milk, and tomato with satisfactory results.  相似文献

Quantification of spatial and temporal concentration profiles of vapor clouds resulting from accidental loss of containment of toxic and/or flammable substances is of great importance as correct prediction of spatial and temporal profiles can not only help in designing mitigation/prevention equipment such as gas detection alarms and shutdown procedures but also help decide on modifications that may help prevent any escalation of the event.The most commonly used models - SLAB (Ermak, 1990), HEGADAS (Colenbrander, 1980), DEGADIS (Spicer & Havens, 1989), HGSYSTEM (Witlox & McFarlane, 1994), PHAST (DNV, 2007), ALOHA (EPA & NOAA, 2007), SCIPUFF (Sykes, Parker, Henn, & Chowdhury, 2007), TRACE (SAFER Systems, 2009), etc. - for simulation of dense gas dispersion consider the dispersion over a flat featureless plain and are unable to consider the effect of presence of obstacles in the path of dispersing medium. In this context, computational fluid dynamics (CFD) has been recognized as a potent tool for realistic estimation of consequence of accidental loss of containment because of its ability to take into account the effect of complex terrain and obstacles present in the path of dispersing fluid.The key to a successful application of CFD in dispersion simulation lies in the accuracy with which the effect of turbulence generated due to the presence of obstacles is assessed. Hence a correct choice of the most appropriate turbulence model is crucial to a successful implementation of CFD in the modeling and simulation of dispersion of toxic and/or flammable substances.In this paper an attempt has been made to employ CFD in the assessment of heavy gas dispersion in presence of obstacles. For this purpose several turbulence models were studied for simulating the experiments conducted earlier by Health and Safety Executive, (HSE) U.K. at Thorney Island, USA (Lees, 2005). From the various experiments done at that time, the findings of Trial 26 have been used by us to see which turbulence model enables the best fit of the CFD simulation with the actual findings. It is found that the realizable k-? model was the most apt and enabled the closest prediction of the actual findings in terms of spatial and temporal concentration profiles. It was also able to capture the phenomenon of gravity slumping associated with dense gas dispersion.  相似文献
