Previously published reports have indicated that idiopathic polyhydramnios may be associated with trisomies 18 and 21 and that chromosomal analysis is indicated. Furthermore, the natural history and fetal outcome of polyhydramnios diagnosed in early gestation have not been well delineated. We identified 138 pregnancies with polyhydramnios prior to 26 weeks' gestation. Of 131 complete cases, 21 were diagnosed as severe, 18 as moderate, and 92 as mild polyhydramnios. Congenital abnormalities were noted in 18 of 21 severe cases (86 per cent). Two of the remaining three cases were twin-to-twin transfusion. Thirteen of 18 cases with moderate polyhydramnios (72 per cent) were associated with anomalies; six of the remaining cases were twin-to-twin transfusion. Sixteen of 92 cases of mild polyhydramnios (17 per cent) were associated with congenital abnormalities. In 69 of 76 cases of mild hydramnios not associated with anomalies (91 per cent), the hydramnios resolved prior to delivery. Only 2 of 16 (13 per cent) associated with anomalies resolved. In 4 of 5 cases (80 per cent) with moderate hydramnios and no anomalies, the amniotic fluid volume was normal on subsequent ultrasound. No case of moderate polyhydramnios associated with anomalies or maternal conditions nor any case of severe polyhydramnios resolved. There were seven cases of chromosomal abnormalities in this series; all were associated with sonographic findings in addition to the presence of polyhydramnios. On the basis of these data, we doubt the benefit of amniocentesis following the early diagnosis of idiopathic polyhydramnios in the absence of other ultrasound findings. 相似文献

Addressing environmental problems requires sophisticated approaches to complexity and uncertainty. Conceptual models are increasingly used to improve understanding of complex system interactions. However, cursory treatment of governance limits their analytical potential. This study included governance considerations in a social-ecological system model of biodiversity conservation in the Tasmanian Midlands (Australia). Effectiveness of engagement processes and conservation programme longevity were identified as critical governance influences. The conceptual representation of this system enabled exploration of how governance influences interact with social drivers (e.g. landholder engagement in conservation practices) to modify the effect of biophysical drivers (e.g. land use) on biodiversity outcomes. This article explores social innovation as a tool for the promotion of place-based sustainable development. The literature highlights the satisfaction of basic needs and situations of crises as two major drivers of socially innovative actions. We use these insights to explore the conditions under which social innovation can lead to social transformation and sustainable place making. We also recognise that social processes occur through, and are shaped by, the material forms that constitute and are constituted in place-specific settings. This highlights the deep interconnections that exist between place making and the resources, attributes and characterises – the materiality (such as rivers, soil, trees) – that exist within that locality. It is here that a close tie can be discerned between understanding the adaptive process in complex socio-ecological systems and the role of social innovation in such adaptation. Socially innovative initiatives at the community level can also be scaled upwards through the co-ordinating role of the state, while at the same time act as a pressure for more participatory forms of governance. Governance processes that enhance the role of both economic and social actors in the steering of social change help to infuse more open, democratic practices into social steering. With social, economic and state actors co-mingling as agents of social change, social innovation can come to play a key role in enhancing sustainable human–environment interactions. 相似文献

Headwater streams are the primary sources of water in a drainage network and serve as a critical hydrologic link between the surrounding landscape and larger, downstream surface waters. Many states, including North Carolina, regulate activity in and near headwater streams for the protection of water quality and aquatic resources. A fundamental tool for regulatory management is an accurate representation of streams on a map. Limited resources preclude field mapping every headwater stream and its origin across a large region. It is more practical to develop a model for headwater streams based on a sample of field data that can then be extrapolated to a larger area of interest. The North Carolina Division of Water Quality has developed a cost‐effective method for modeling and mapping the location, length, and flow classification (intermittent and perennial) of headwater streams. We used a multiple logistic regression approach that combined field data and terrain derivatives for watersheds located in the Triassic Basins ecoregion. Field data were collected using a standard methodology for identifying headwater streams and origins. Terrain derivatives were generated from digital elevation models interpolated from bare‐earth Light Detection and Range data. Model accuracies greater than 80% were achieved in classifying stream presence and absence, stream length and perennial stream length, but were not as consistent in predicting intermittent stream length. 相似文献

The global biodiversity crisis requires an engaged citizenry that provides collective support for public policies and recognizes the consequences of personal consumption decisions. Understanding the factors that affect personal engagement in proenvironmental behaviors is essential for the development of actionable conservation solutions. Zoos and aquariums may be some of the only places where many people can explore their relations with wild animals and proenvironmental behaviors. Using a moderated‐mediation analysis of a survey of U.S. zoo and aquarium visitors (n = 3588), we explored the relationship between the sense of connection to animals and self‐reported engagement in proenvironmental behaviors related to climate change and how this relationship is affected by certainty that climate change is happening, level of concern about climate change, and perceptions of effectiveness in personally addressing climate change. We found a significant, directional relationship between sense of connection to animals and self‐reported proenvironmental behaviors. Political inclination within the conservative to liberal spectrum did not affect the relationship. We conclude that a personal sense of connection to animals may provide a foundation for educational and communication strategies to enhance involvement in proenvironmental actions. 相似文献

We report the prenatal diagnosis of a fetus with tetrasomy 5p mosaicism resulting from a supernumerary isochromosome for the short arm of chromosome 5. The pregnancy was terminated and fetal autopsy revealed minor facial abnormalities (long philtrum, up-slanting palpebral fissures and a broad nasal bridge). Cultures were established from fetal skin, kidney, and pancreas for cytogenetic analysis; the i(5p) was observed only in the skin fibroblasts. The significance of choroid plexus cysts in fetuses at 18–20 weeks. An indication for amniocentesis?
Over a period of 25 months, all antenatal patients were offered a detailed ultrasound scan at 18–20 weeks' gestation. The lateral cerebral ventricles were scanned for the presence of choroid plexus cysts. Fifty-one patients found to have choroid plexus cysts were offered amniocentesis to exclude chromosomal abnormalities. One pregnancy, in which the only abnormality found was bilateral choroid plexus cysts, was terminated after trisomy 18 was detected on amniocentesis at 19 weeks. The other 50 pregnancies had normal fetal outcomes. The significance of the isolated finding of choroid plexus cysts is reviewed. 相似文献

Field samples and a 9-week glasshouse growth trial were used to investigate the accumulation of mining derived arsenic (As) and antimony (Sb) in vegetable crops growing on the Macleay River Floodplain in Northern New South Wales, Australia. The soils were also extracted using EDTA to assess the potential for this extractant to be used as a predictor of As and Sb uptake in vegetables, and a simplified bioaccessibility extraction test (SBET) to understand potential for uptake in the human gut with soil ingestion. Metalloids were not detected in any field vegetables sampled. Antimony was not detected in the growth trial vegetable crops over the 9-week greenhouse trial. Arsenic accumulation in edible vegetable parts was <10 % total soil-borne As with concentrations less than the current Australian maximum residue concentration for cereals. The results indicate that risk of exposure through short-term vegetable crops is low. The data also demonstrate that uptake pathways for Sb and As in the vegetables were different with uptake strongly impacted by soil properties. A fraction of soil-borne metalloid was soluble in the different soils resulting in Sb soil solution concentration (10.75?±?0.52 μg L–1) that could present concern for contamination of water resources. EDTA proved a poor predictor of As and Sb phytoavailability. Oral bioaccessibility, as measured by SBET, was <7 % for total As and <3 % total Sb which is important to consider when estimating the real risk from soil borne As and Sb in the floodplain environment. 相似文献
