Cell-free fetal DNA-based noninvasive prenatal testing (NIPT) could significantly change the paradigm of prenatal testing and screening. Intellectual property (IP) and commercialization promise to be important components of the emerging debate about clinical implementation of these technologies. We have assembled information about types of testing, prices, turnaround times, and reimbursement of recently launched commercial tests in the United States from the trade press, news articles, and scientific, legal, and business publications. We also describe the patenting and licensing landscape of technologies underlying these tests and ongoing patent litigation in the United States. Finally, we discuss how IP issues may affect clinical translation of NIPT and their potential implications for stakeholders. Fetal medicine professionals (clinicians and researchers), genetic counselors, insurers, regulators, test developers, and patients may be able to use this information to make informed decisions about clinical implementation of current and emerging noninvasive prenatal tests. © 2013 John Wiley & Sons, Ltd. 相似文献

In three phases, this study identifies and evaluates the major messages being used in communication campaigns focused on the ongoing drought in California. A literature review in Phase 1 resulted in a typology of 12 message strategies. Following this typology, trained coders in Phase 2 evaluated water conservation messages (N?=?100) to determine whether each message utilized one or multiple strategies. The results revealed various frequencies of strategy application; and a correlational analysis rendered a pattern of strategy use in combinations. Phase 3 focused on a controlled message experiment applying the three most relevant strategies: conservation tips, loss aversion, and evidence of drought. Analysing data sampled from California residents (N?=?180), conservation messages, regardless of the strategy, led to attitude change in a negative direction. Additional analyses revealed interesting patterns of combinatorial strategy effects. GOAL, SCOPE AND BACKGROUND: The issue of whether food miles are a relevant indicator for the environmental impacts associated with foods has received significant attention in recent years. It is suggested here that issues other than the distance travelled need to be considered. The argument is presented by illustrating the case for the provision of apples. MATERIALS AND METHODS: The effects of variability in primary energy requirements for apple cultivation and for other life cycle stages, seasonality (timing of consumption) and loss of produce during storage are studied in this paper, by comparing apples from different supplier countries for consumption in Europe. RESULTS: Data sources for primary energy use (PEU) of apple production are identified ranging from 0.4-3.8 MJ/kg apples for European and Southern American countries and 0.4-0.7 MJ/kg for New Zealand. This variability is related to different yields and producer management practices in the different countries. Storage loss may range from 5% to 40% for storage periods between 4 and 10 months, and this has a significant effect on the results (e.g. increasing the total PEU by 8-16% when stored for 5-9 months in Europe as compared with a no loss and no storage situation). The storage periods and related storage losses change markedly through the year for imported (i.e. non-European) versus European apples. DISCUSSION: The timing of consumption and related storage losses need to be included in the assessment, as this affects the order of preference for locally sourced versus imported apples. The variability in energy requirements in different life cycle stages, but particularly for the fruit production stage, is also significant in this comparative analysis. CONCLUSIONS: Overall, it seems that there are similarities in the total PEU ranges for European and New Zealand apples during the Southern Hemisphere's apple season (European spring and summer). However, during the European autumn and winter (Northern Hemisphere apple season) PEU values are generally higher for apples imported from the Southern Hemisphere compared with European apples consumed in Europe. However, this latter observation may not hold true where apples for consumption in one European country are imported from another European country, because energy use for road transportation has a significant influence on the result. RECOMMENDATIONS AND PERSPECTIVES: Future studies comparing alternative sources of fresh produce need to account for ranges of data for the fruit production and storage stages, which reflect the seasonality of production. 相似文献

Previous studies have found associations between traffic-related air pollution and asthma exacerbation in children, where exacerbations were measured according to emergency department visits and hospital admissions. Fewer studies have been undertaken that look at asthma exacerbations in a less severe primary care setting. Therefore, the authors sought to examine the associations between childhood asthma exacerbations, measured as acute visits to a primary care setting, and vehicular-traffic measures in a population of children aged 18 and under in the metropolitan Atlanta area. Statistical tests for differences of mean monthly visits for members with traffic measures above the median compared with below the median and for the upper quartile compared with the lower quartile were conducted. We also compared the odds of having one or more visits in a month for those who lived closer to a major roadway were compared with those who lived farther (greater than 300 m) from a major roadway. Poisson general linear modeling was used to determine associations between daily levels of acute visits for childhood asthma and traffic-related pollutants (zinc, EC [elemental carbon], and PM10 and PM2.5 [particulate matter with an aerodynamic diameter of ≤10 and ≤2.5 μm, respectively]) for different levels of traffic and distance measures. This analysis found that both larger traffic volumes and smaller distances to the nearest major roadway were positively and significantly associated with larger numbers of childhood asthma visits, when compared with less traffic and larger distances. Our findings point to motor vehicle traffic as an important contributor to childhood asthma exacerbations.
Implications: Previous studies have found associations between traffic-related air pollution and asthma exacerbation in children. However, these studies were mainly conducted in emergency department or hospital admission settings; little is known regarding less acute health effects. This analysis of the association between vehicular traffic measures and childhood asthma in a primary care setting suggests that motor vehicle traffic is a contributor to less acute asthma episodes in children. The present analysis of traffic-related air pollutants and childhood asthma were less conclusive, likely due to methods limitations outlined in the paper. The implication is that further evidence of adverse respiratory health effects in children due to motor vehicle traffic can be found in a primary care setting and similar studies should be considered. 相似文献

In nature reserves and habitat conservation areas, monitoring is required to determine if reserves are meeting their goals for preserving species, ecological communities, and ecosystems. Increasingly, reserves are established to protect multiple species and communities, each with their own conservation goals and objectives. As resources are always inadequate to monitor all components, criteria must be applied to prioritize both species and communities for monitoring and management. While methods for prioritizing species based on endangerment or risk have been established, approaches to prioritizing ecological communities for monitoring are not well developed, despite a long-standing emphasis on communities as target elements in reserve design. We established guidelines based on four criteria derived from basic principles of conservation and landscape ecology--extent, representativeness, fragmentation, and endangerment--to prioritize communities in the San Diego Multiple Species Conservation Plan (MSCP). The MSCP was one of the first multiple-species habitat conservation areas established in California, USA, and it has a complex spatial configuration because of the patterns of surrounding land use, which are largely urbanized. In this case study, high priority communities for monitoring include coastal sage scrub (high endangerment, underrepresented within the reserve relative to the region, and moderately fragmented), freshwater wetlands, and coastal habitats (both have high fragmentation, moderate endangerment and representativeness, and low areal extent). This framework may be useful to other conservation planners and land managers for prioritizing the most significant and at-risk communities for monitoring. 相似文献

Every species occupies a limited geographic area, but how spatiotemporal environmental variation affects individual and population fitness to create range limits is not well understood. Because range boundaries arise where, on average, populations are more likely to go extinct than to persist, range limits are an inherently population-level problem for which a demographic framework is useful. In this study, I compare demographic parameters and population dynamics between central and marginal populations of monkeyflowers, Mimulus cardinalis and M. lewisii, along an elevation gradient spanning both species' ranges. Central and marginal populations of both species differed in survival and fecundity. For M. lewisii, these components of fitness were higher in central than in marginal populations, but for M. cardinalis the converse was true. To assess spatiotemporal variation in population dynamics, I used transition matrix models to estimate asymptotic population growth rates (lambda) and found that population growth rates of M. lewisii were highest at the range center and reduced at the range margin. Population growth rates of M. cardinalis were highest at the range margin and greatly reduced at the range center. Life table response analysis decomposed spatiotemporal variation in lambda into contributions from each transition between life stages, finding that transitions from large nonreproductive and reproductive plants to the seed class and stasis in the reproductive class made the largest contributions to spatial differences in lambda. These transitions had only low to moderate sensitivities, indicating that differences in projected population growth rates resulted mainly from observed differences in transition matrix parameters and their underlying vital rates. 相似文献

An awareness campaign decreases distracted driving among hospital employees at a rural trauma center

AbstractObjective: To evaluate whether an educational campaign on distracted driving will have an impact in a given community.Methods: Investigators were stationed in an employee parking lot of a 256-bed hospital to determine baseline distracted driving followed by a 4-week hospital-wide distracted-driving awareness campaign. The campaign included signs/posters in the hospital, a booth outside of the cafeteria with flyers, a large banner in the employee lot and an opportunity for people to sign a pledge form to drive distraction free. The same employee lot was observed at the same time of the day to re-assess distracted driving immediately following the campaign. The observations were repeated again one year later to evaluate the short and long-term impact of the campaign.Results: A total of 485 vehicles were observed pre-campaign, identifying 170 (35%) distracted drivers at baseline. The awareness campaign resulted in 525 people pledging to drive distraction free. Following the campaign, 495 vehicles were observed and the number of distracted drivers was 64 (12.9%), showing a significant decrease in the number of distracted drivers by 22.1% (p?<?0.01). One year later, 530 drivers were observed with 150 (28%) displaying one form of distraction.Conclusions: A local distracted driving educational campaign resulted in a significant decrease in the number of distracted drivers immediately following the campaign. However, one year after the campaign, there was an increase in distracted driving. The proportion of distracted drivers was still significantly lower than the initial rate of distracted-drivers. 相似文献
