Intrauterine fetal growth restriction (IUGR) is an important pregnancy complication associated with significant adverse clinical outcome, stillbirth, perinatal morbidity and cerebral palsy. To date, no uniformly accepted management protocol of Doppler surveillance that reduces mortality and cognitive morbidity has emerged. Aortic isthmus (AoI) evaluation has been proposed as a potential monitoring tool for IUGR fetuses. In this review, the current knowledge of the relationship between AoI Doppler velocimetry and preterm fetal growth restriction is reviewed. Relevant technical aspects and reproducibility data are reviewed as we discuss AoI Doppler and its place within the existing repertoire of Doppler assessments in placental insufficiency. The AoI is a link between the right and left ventricles which perfuse the lower and upper body, respectively. The clinical use of AoI waveforms for monitoring fetal deterioration in IUGR has been limited, but preliminary work suggests that abnormal AoI impedance indices are an intermediate step between placental insufficiency-hypoxemia and cardiac decompensation. Further prospective studies correlating AoI indices with arterial and venous Doppler indices and perinatal outcome are required before encorporating this index into clinical practice. Originally prenatal diagnosis was confined to the diagnosis of metabolic disorders and depended on assaying enzyme levels in amniotic fluid. With the development of recombinant DNA technology, molecular diagnosis became possible for some genetic conditions late in the 1970s. Here we briefly review the history of molecular prenatal diagnostic testing, using Duchenne muscular dystrophy as an example, and describe how over the last 30 years we have moved from offering testing to a few affected individuals using techniques, such as Southern blotting to identify deletions, to more rapid and accurate PCR-based testing which identifies the precise change in dystrophin for a greater number of families. We discuss the potential for safer, earlier prenatal genetic diagnosis using cell free fetal DNA in maternal blood before concluding by speculating on how more recent techniques, such as next generation sequencing, might further impact on the potential for molecular prenatal testing. Progress is not without its challenges, and as cytogenetics and molecular genetics begin to unite into one, we foresee the main challenge will not be in identifying the genetic change, but rather in interpreting its significance, particularly in the prenatal setting where we frequently have no phenotype on which to base interpretation. This paper discusses the findings of a small scale research project which explored the possibility of adopting eco-design techniques. The paper focuses on identifying how eco-design techniques can be determined as being compatible with new product development processes. Via the development of a five stage ‘applicability framework’, this study demonstrates how a compatible suite of tools can be identified for application to product development processes. Testing and validation of this ‘applicability framework’, which was used to identify three key eco-design techniques; namely checklists, guidelines, and a material, energy and toxicity (MET) matrix, is shown to have taken place in relation to the development of a lightweight chemical detector product. It is established that checklists, guidelines and the MET matrix can be used both on a specific product, and also more generally in the design process. In particular, the MET matrix is shown as being used to successfully identify key environmental aspects of the product during its lifetime. The paper concludes by arguing that eco-design techniques may not have been more widely adopted by businesses because such methods are not necessarily generic and immediately applicable, but instead require some form of process-specific customisation prior to use, which can in turn act as a barrier to adoption. It is also highlighted that the shear diversity of pressures that come to bear during the product development process can also act as a barrier to adoption, and that the full integration of eco-design techniques will have to encompass approaches which overcome such pressures.  相似文献

Previous studies show that conservation actions have prevented extinctions, recovered populations, and reduced declining trends in global biodiversity. However, all studies to date have substantially underestimated the difference conservation action makes because they failed to account fully for what would have happened in the absence thereof. We undertook a scenario‐based thought experiment to better quantify the effect conservation actions have had on the extinction risk of the world's 235 recognized ungulate species. We did so by comparing species’ observed conservation status in 2008 with their estimated status under counterfactual scenarios in which conservation efforts ceased in 1996. We estimated that without conservation at least 148 species would have deteriorated by one International Union for Conservation of Nature (IUCN) Red List category, including 6 species that now would be listed as extinct or extinct in the wild. The overall decline in the conservation status of ungulates would have been nearly 8 times worse than observed. This trend would have been greater still if not for conservation on private lands. While some species have benefited from highly targeted interventions, such as reintroduction, most benefited collaterally from conservation such as habitat protection. We found that the difference conservation action makes to the conservation status of the world's ungulate species is likely to be higher than previously estimated. Increased, and sustained, investment could help achieve further improvements.  相似文献

A survey was conducted at eight U.S. drinking water plants, that spanned a wide range of water qualities and treatment/disinfection practices. Plants that treated heavily-wastewater-impacted source waters had lower trihalomethane to dihaloacetonitrile ratios due to the presence of more organic nitrogen and HAN precursors. As the bromide to total organic carbon ratio increased, there was more bromine incorporation into DBPs. This has been shown in other studies for THMs and selected emerging DBPs (HANs), whereas this study examined bromine incorporation for a wider group of emerging DBPs (haloacetaldehydes, halonitromethanes). Moreover, bromine incorporation into the emerging DBPs was, in general, similar to that of the THMs. Epidemiology studies that show an association between adverse health effects and brominated THMs may be due to the formation of brominated emerging DBPs of heath concern. Plants with higher free chlorine contact times before ammonia addition to form chloramines had less iodinated DBP formation in chloraminated distribution systems, where there was more oxidation of the iodide to iodate (a sink for the iodide) by the chlorine. This has been shown in many bench-scale studies (primarily for iodinated THMs), but seldom in full-scale studies (where this study also showed the impact on total organic iodine. Collectively, the THMs, haloacetic acids, and emerging DBPs accounted for a significant portion of the TOCl, TOBr, and TOI; however, ∼50% of the TOCl and TOBr is still unknown. The correlation of the sum of detected DBPs with the TOCl and TOBr suggests that they can be used as reliable surrogates.  相似文献

In ecosystems network, structure determines adjacent (direct) and non-adjacent (indirect) pathways over which energy, matter, and information can flow. The more pathways, the more possible ways the conservative substance can move in zero-sum transactions between network nodes that the pathways interconnect, and the more possible non-conservative, nonzero-sum relations can be secondarily derived from these. Structural analysis is a tool we employ, from a family of input–output methods for exploring zero- and nonzero-sum attributes of environmental networks, to count pathways of varying length between network nodes. In this paper, we examine the relationship between pathway length (k) and number (Pk) as determined by system size (n, number of nodes) and extent and pattern of connectance (C). We develop a measure (ma) of pathway growth in numbers with increasing length, and then normalize this to the maximum rate possible (ma/mc) for a given system size. These measures apply to two pathway types—paths, ma(0) and ma(0)/mc(0), which forbid adjacent node repetitions, and walks, ma(1) and ma(1)/mc(1), which allow them. We find that network size has a curvilinear effect on the pathway number versus length relationship, and extent and pattern of connectance are convolved. Values computed for the paths and walks of three ecosystem models (oyster reef, freshwater marsh, and reservoir cove) are used to compare their pathway structure.  相似文献

The diagnosis of congenital nephrosis is difficult during the antepartum period. The combination of an elevated amniotic fluid alpha-fetoprotein, a negative acetylcholinesterase, and a negative ultrasound examination is highly indicative of congenital nephrosis; however, these findings can also be associated with a normal gestation.
