Background
Falls are a common, serious, and often unrecognized problem facing older adults. The objective of this study was to provide an initial clinical and statistical validation for a public health strategy of fall risk self-assessment by older adults using a Fall Risk Questionnaire (FRQ).Methods
Adults age 65 + (n = 40) were recruited at a Los Angeles Veterans Affairs (VA) medical facility and at a local assisted living facility. Participants completed the FRQ self-assessment and results were compared to a “gold standard” of a clinical evaluation of risks using the American/British Geriatrics Society guidelines to assess independent predictors of falls: history of previous falls, fear of falling, gait/balance, muscle weakness, incontinence, sensation and proprioception, depression, vision, and medications. For the comparison, we used an iterative statistical approach, weighing items based on relative risk.Results
There was strong agreement between the FRQ and clinical evaluation (kappa = .875, p < .0001). Individual item kappa values ranged from .305-.832. After dropping one FRQ item (vision risk) because of inadequate agreement with the clinical evaluation (kappa = .139, p = .321), the final FRQ had good concurrent validity.Conclusions
The FRQ goes beyond existing screening tools in that it is based on both evidence and clinical acceptability and has been initially validated with clinical examination data. A larger validation with longitudinal follow-up should determine the actual strength of the FRQ in predicting future falls. 相似文献72.

Sex chromosome aneuploidy (SCA) can be detected on prenatal diagnostic testing and cell free DNA screening (cfDNA). High risk cfDNA results should be confirmed with diagnostic testing. This summary article serves as an update for prenatal providers and assimilates data from neurodevelopmental, epidemiologic, and registry studies on the most common SCA. From a conservation perspective, quantifying potential refugial capacity has been predominantly focused on climate refugia, which is critical for maintaining the persistence of species and ecosystems. However, protection from other stressors, such as human-induced changes in fire and hydrology, that cause habitat loss, degradation, and fragmentation is also necessary to ensure that conservation efforts focused on climate are not undermined by other threats. Thus, conceptual and methodological advances for quantifying potential refugia from multiple anthropogenic stressors are important to support conservation efforts. We devised a new conceptual approach, the domains of refugia, for assessing refugial capacity that identifies areas where exposure to multiple stressors is low. In our framework, patterns of environmental variability (e.g., increased frequency of warm summers), thresholds of resilience, and extent and intensity of stressors are used to identify areas of potential refugia from a suite of ongoing anthropogenic stressors (e.g., changes in fire regime). To demonstrate its utility, we applied the framework to a Southern California landscape. Sites with high refugial capacity (super-refugia sites) had on average 30% fewer extremely warm summers, 20% fewer fire events, 10% less exposure to altered river channels and riparian areas, and 50% fewer recreational trails than the surrounding landscape. Our results suggest that super-refugia sites (∼8200 km2) for some natural communities are underrepresented in the existing protected area network, a finding that can inform efforts to expand protected areas. Our case study highlights how considering exposure to multiple stressors can inform planning and practice to conserve biodiversity in a changing world. 相似文献

Two methods were used to assess the grazing impact of roving herbivorous fishes across a coral reef depth gradient within
Pioneer Bay, Orpheus Island, Great Barrier Reef. The first technique employed was a method traditionally used to quantify
herbivory on coral reefs via the (indirect) inference of herbivore impact from biomass estimates and reported feeding rates.
The second method (one of a range of direct approaches) used remote underwater video cameras to film the daily feeding activity
of roving herbivores in the absence of divers. Both techniques recorded similar patterns and relative levels of herbivore
biomass across five reef zones at the study site. Indirect estimates of the grazing impact across the reef depth gradient
of the three predominant species of herbivore broadly coincided with levels quantified directly by remote underwater video,
indicating that, to a large extent, presence does correspond to function. However, the video data suggested that, for individual
species in particular reef zones, the absolute level of impact may be less than that inferred from presence. In the case of
the parrotfish Scarus rivulatus, the video recordings suggested that, at the reef crest, an average of 52% (±18 SE) of each m2 area of reef would be grazed each month, compared with an area of 109% (±41 SE) suggested by inferring grazing activity from
presence alone. Potential biases associated with remote video recorders may explain some of the discrepancy between values.
Overall, the results suggest that, for some fish groups, the indirect method of inferring function from presence can provide
a good indication of relative levels of herbivore impact across a coral reef.

A method is presented here by which the actual numbers of individuals in the U.S. population who would be expected to suffer respiratory illness as a result of exposure to ambient nitrogen dioxide (NO2) concentrations in excess of the Federally-designated ambient air quality standard can be estimated. At the same time we have attempted to quantify how these adverse health effects would be alleviated by various degrees of reductions in current ambient concentrations. In addition, the health benefit expected to be derived by various NOx emission control strategies will be examined utilizing the health benefit estimation method presented.It is estimated that there were approximately 33.2×106 excess cases of respiratory illnesses in the U.S. in 1973 associated with ambient NO2 concentrations in excess of the national ambient standard. A reduction of approximately 50 to 60 percent below 1973 NO2 levels is needed to essentially eliminate excess respiratory illness associated with ambient NO2 concentration. 相似文献

Communication and advocacy approaches that influence attitudes and behaviors are key to addressing conservation problems, and the way an issue is framed can affect how people view, judge, and respond to an issue. Responses to conservation interventions can also be influenced by subtle wording changes in statements that may appeal to different values, activate social norms, influence a person's affect or mood, or trigger certain biases, each of which can differently influence the resulting engagement, attitudes, and behavior. We contend that by strategically considering how conservation communications are framed, they can be made more effective with little or no additional cost. Key framing considerations include, emphasizing things that matter to the audience, evoking helpful social norms, reducing psychological distance, leveraging useful biases, and, where practicable, testing messages. Team trust is gaining attention in research and practice due to its benefits for team performance, yet clarity about the intricacies of its measurement is needed. Therefore, we meta-analyzed 118 studies (N = 7,738) to untangle the role of measurement features by investigating the degree to which they influence the trust–performance relationship. Results showed that the trust–performance relationship is contingent upon time lag and source of measurement. Specifically, cross-sectional and single-source studies produced higher effect sizes than time-lagged and different-source studies. In contrast, the moderating roles of conceptualization–operationalization alignment and referent of trust measures were not supported. Post hoc analyses revealed that affective trust is more strongly related to global, versus specific team outcomes, and that mixed-referent items are particularly effective within intact teams, whereas the trust–performance relationship is constrained when direct consensus items are used within ad hoc teams. Furthermore, we provided a critical review that highlights the importance of composites, multilevel forces, and item content and wording. Finally, we clarified key gaps in the literature, calling for research where needed. This review serves as a bridge between conceptualization and measurement and lays the groundwork for advancing knowledge of team trust. 相似文献

The swimming abilities of larval fishes are important for their survival, potentially affecting their ability to avoid predators,
obtain food and control dispersal patterns. Near settlement swimming abilities may also influence spatial and temporal patterns
of recruitment. We examined Critical speed (U-crit) swimming ability in late stage larvae of 89 species of coral reef fishes from the Great Barrier Reef and the Caribbean.
Coefficients of variation in U-crit calculated at the individual level were high (28.4%), and this was not explained by differences in size or condition
factor of these same larvae. Among species U-crit ranged from 5.5 cm s−1 to 100.8 cm s−1 (mean=37.3 cm s−1), with 95% of species able to swim faster than the average current speed around Lizard Island, suggesting that most species
should be capable of influencing their spatial and temporal patterns of settlement. Inter-specific differences in swimming
ability (at both the family and species levels) were significantly correlated with size and larval morphology. Correlations
were found between swimming performance and propulsive area, fineness ratio and aspect ratio, and these morphological parameters
may prove useful for predicting swimming ability in other taxa. Overall, the swimming speeds of larvae from the same families
at the two locations were relatively similar, although the Lutjanidae and Acanthuridae from the Caribbean were significantly
slower than those from the great barrier reef. Differences in swimming speed and body form among late stage larvae suggests
that they will respond differently to factors influencing survival and transport during their pelagic phase, as well as habitat
