The increase in the number of chronic kidney disease (CKD) patients from the north central region of Sri Lanka has become
a environmental health issue of national concern. Unlike in other countries where long-standing diabetes and hypertension
are the leading causes of renal diseases, the majority of CKD patients from this part of Sri Lanka do not show any identifiable
cause. As the disease is restricted to a remarkably specific geographical terrain, particularly in the north central dry zone
of the country, multidisciplinary in-depth research studies are required to identify possible etiologies and risk factors.
During this study, population screening in the prevalent region and outside the region, analysis of geoenvironmental and biochemical
samples were carried out. Population screening that was carried out using a multistage sampling technique indicated that the
point prevalence of CKD with uncertain etiology is about 2–3% among those above 18 years of age. Drinking water collected
from high-prevalent and non-endemic regions was analyzed for their trace and ultratrace element contents, including the nephrotoxic
heavy metals Cd and U using ICP-MS. The results indicate that the affected regions contain moderate to high levels of fluoride.
The Cd contents in drinking water, rice from affected regions and urine from symptomatic and non-symptomatic patients were
much lower indicating that Cd is not a contributing factor for CKD with uncertain etiology in Sri Lanka. Although no single
geochemical parameter could be clearly and directly related to the CKD etiology on the basis of the elements determined during
this study, it is very likely that the unique hydrogeochemistry of the drinking water is closely associated with the incidence

ABSTRACT: Nutrient data from all available sources for the lower Mississippi River were examined for potential differences among sampling agencies and geographic locations for the period between 1960 and 1998. Monthly means grouped by parameter, sampling location and agency, were calculated and compared as paired sets, excluding those months where data were not available for both sets. Some significant differences were found between various agencies collecting nutrient data on the river, as well as between various stretches of river, especially in the case of phosphorus nutrient data. Results were used to synthesize data sets from which a history of nutrient loading in the Mississippi River was determined. General trends in nitrate+nitrite, total Kjeldahl nitrogen, orthophosphate, total phosphorus and silica loads, as well as changes in nutrient proportions and the specific limiting nutrient (by month) are reported. This study provides a useful summary of contemporary and historical nutrient data that may assist in the evaluation of Mississippi River water quality and its potential effect on the Gulf of Mexico. 相似文献

The present study elucidated the biomagnification profiles of persistent organic pollutants (POPs) through a tropical aquatic food web of Vietnam based on trophic characterization using stable nitrogen analysis. Various biological samples collected from the main stream of the Mekong Delta were provided for the analysis for both POPs, and stable nitrogen and carbon isotope ratios. Of the POPs analyzed, dichlorodiphenyltrichloroethane and its metabolites (DDTs) were the predominant contaminants with concentrations ranging from 0.058 to 12 ng/g wet weight, followed by polychlorinated biphenyls (PCBs) at 0.017-8.9 ng/g, chlordane compounds (CHLs) at 0.0043-0.76 ng/g, tris-4-chlorophenyl methane (TCPMe) at N.D.-0.26 ng/g, hexachlorocyclohexane isomers (HCHs) at N.D.-0.20 ng/g and hexachlorobenzene (HCB) at 0.0021-0.096 ng/g. Significant positive increases of concentrations in DDTs, CHLs, and TCPMe against the stable nitrogen ratio (delta(15)N) were detected, while, concentrations of HCHs and HCB showed no significant increase. The slopes of the regression equations between the log-transformed concentrations of these POPs and delta(15)N were used as indices of biomagnification. The slopes of the POPs for which positive biomagnification was detected ranged from 0.149 to 0.177 on a wet weight basis. The slopes of DDTs and CHLs were less than those reported for a marine food web of the Arctic Ocean, indicating that less biomagnification had occurred in the tropical food web. Of the isomers of CHLs, unlike the studies of the Arctic Ocean, oxychlordane did not undergo significant biomagnification through the food web of the Mekong Delta. This difference is considered to be due to a lack of marine mammals, which might metabolize cis- and trans-chlordane to oxychlordane, in the Mekong Delta ecosystem. ABSTRACTEasy-to-use commercial kit-based enzyme-linked immunosorbent assays (ELISAs) have been used to detect neonicotinoid dinotefuran, clothianidin and imidacloprid in Chinese chives, which are considered a troublesome matrix for chromatographic techniques. Based on their high water solubility, water was used as an extractant. Matrix interference could be avoided substantially just diluting sample extracts. Average recoveries of insecticides from spiked samples were 85–113%, with relative standard deviation of <15%. The concentrations of insecticides detected from the spiked samples with the proposed ELISA methods correlated well with those by the reference high-performance liquid chromatography (HPLC) method. The residues analyzed by the ELISA methods were consistently 1.24 times that found by the HPLC method, attributable to loss of analyte during sample clean-up for HPLC analyses. It was revealed that the ELISA methods can be applied easily to pesticide residue analysis in troublesome matrix such as Chinese chives. 相似文献

Prenatal paternity testing was evaluated by DNA analysis in chorionic villus biopsies obtained during the 7th-22nd weeks of gestation. Using highly polymorphic variable number of tandem repeats (VNTR) probes, we analysed four cases consisting of mother/child/alleged father trios. In all cases, we were able to detect maternal and paternal alleles and could establish or exclude paternity. The prenatal diagnosis of an echogenic fetal lung (EFL) is now often made in the early second trimester using high-resolution ultrasound. This ultrasound appearance is usually caused by a congenital cystic adenomatoid lung malformation (CCAM), an intrapulmonary lung sequestration or obstruction of a major airway. In order to provide prognostic guidelines to parents who may be considering termination of a fetus with these findings, we have analysed a series of 11 cases diagnosed in our centre over the past 2 years in conjunction with 60 cases from major published series. The data suggest that in the absence of non-immune hydrops fetalis (NIHF) or other anomalies, the outcome for the fetuses is excellent, with over 90 per cent survival. Neither early diagnosis (24 weeks) nor the presence of mediastinal shift is a poor prognostic indicator. In addition, it appears that if NIHF is absent at diagnosis, the chance that it will develop as the pregnancy continues is small (6 per cent). Furthermore, there is a significant (up to 30 per cent) chance that this ultrasound finding will resolve in utero. The development of in utero fetal surgical techniques may be the only hope for those hydropic fetuses who appear to have a dismal prognosis. 相似文献

Magnetic resonance (MR) imaging was performed at 29 weeks of pregnancy after ultrasonographic detection of an abnormal cleft in the fetal brain. Fetal neuromuscular blockade was induced by pancuronium bromide injected into the umbilical vein under continuous ultrasound (US) guidance. MR images supported the echotomographic diagnosis of schizencephaly improving the visualization of symmetrical broad clefts connecting the lateral ventricles with the subarachnoid space. In order to gain more insight into the association between alpha-fetoprotein (AFP) and fetal chromosomal disorders, especially Down's syndrome, we measured AFP in fetal serum, amniotic fluid, and maternal serum at cordocentesis. We compared the concentration and gradient of AFP in these three compartments. Our data confirm earlier findings on second-trimester fetal serum AFP concentration. The results indicate that low maternal serum AFP in pregnancies with fetal chromosomal disorders could result from an impaired fetal kidney function as well as from impaired membrane or placental passage of AFP, rather than from reduced fetal AFP production. 相似文献
