A major chemical company established a formal incident investigation and reporting system several years ago. The original system focused heavily on worker-related injuries, illnesses, and near-misses and was used primarily to track statistics reportable to the Occupational Safety and Health Administration (OSHA). This Occupational Injury and Illness (OII) approach has been recognized to be an ineffective tool for measuring, predicting, and preventing process safety incidents. The Center for Chemical Process Safety (CCPS) recently published guidelines on how to establish safety metrics for the measurement and reduction of process safety incidents. The process safety metrics approach relies upon leading and lagging metrics to improve organization process safety. This paper is a case study of the analysis of one organization’s incident database, which represents approximately five years of data from over a dozen facilities. The aim of this investigation was to extract useful process safety metrics from the incident investigation and reporting system, which would be pertinent to the types of process units and process functions at these facilities. This paper will discuss the approach taken to extract process incident information from an OII-based database and present the difficulties of performing an analysis on such a database. This paper provides guidance on how to migrate an existing OII-based reporting system to a program that includes process safety metrics in accordance with industry best practices. 相似文献
The flammability of vapors above aqueous solutions of ethanol and acetonitrile was studied experimentally in a 20-L combustion apparatus. No liquid was present in the apparatus, but the vapor concentrations were adjusted to correspond to the vapor in equilibrium with a specified aqueous solution. The experimental results for these two systems show that
• As water is added to the vapor, the lower boundary of the flammability zone decreases. For ethanol, the lower flammability limits (LFL) decreases from 3.7% for pure vapor to 3.2% with saturated water vapor. For acetonitrile, the decrease is from 4.2% to 3.8%. Thus, to a good approximation, the water vapor can be treated as an inert, enabling the data to be displayed on a single flammability triangle diagram. This provides a very simplified method for estimating the flammable behavior for aqueous solutions.
• The upper boundary of the flammability zone is unchanged with the addition of water.
• The limiting oxygen concentration (LOC) is essentially constant for all concentrations of aqueous solutions. The LOC for the pure solvent may be used as a universal LOC for all solvent concentrations.
• The vapor mixture above the aqueous solution is not flammable below a certain liquid mol fraction of flammable. The flammable concentration at which this occurs can be called the maximum safe solvent concentration (MSSC). A method is presented to determine the MSSC from experimental flammability data.
• The oxygen concentration defining the flammable boundary for the vapor decreases rapidly from the MSSC and then increases as the liquid solvent concentration increases.
The calculated adiabatic flame temperature (CAFT) method qualitatively predicts the same behavior as the experimental data. 相似文献
OBJECTIVE: In developing countries, motorcycle use has grown in popularity in the past decades. Commensurate with this growth is the increase in death and casualties among motorcyclists in these countries. One of the strategic programs to minimize this problem is to reduce motorcyclists exposure by shifting them into safer modes of transport. This study aims to explore the differences in the characteristics of bus and motorcycle users. It identifies the factors contributing to their choice of transport mode and estimates the probability that motorcyclists might change their travel mode to a safer alternative; namely, bus travel. METHODS: In this article, a survey of 535 motorcycle and bus users was conducted in seven districts of Selangor state, Malaysia. A binary logit model was developed for the two alternative modes, bus and motorcycle. RESULTS: It was found that travel time, travel cost, gender, age, and income level are significant in influencing motorcyclists' mode choice behavior. The probability of motorcycle riders shifting to public transport was also examined based on a scenario of a reduction in bus travel time and travel cost. CONCLUSIONS: Reduction of total travel time for the bus mode emerges as the most important element in a program aimed at attracting motorcyclists towards public transport and away from the motorcycle mode. 相似文献
This paper presents an agent-based approach for error detection in incident management organizations. The approach consists of several parts. First, a formal approach for the specification and hierarchical verification of both traces and properties. Incomplete traces are enriched by enrichment rules. Furthermore, a classification mechanism is presented for the different properties in incident management that is based on psychological literature. Classification of errors provides insight in the functioning of the agents involved with respect to their roles. This insight enables the provision of dedicated training sessions and allows software support to give appropriate warning messages during incident management. 相似文献
This special issue of the Journal of Organizational Behavior was developed to further extend attribution theory and its application to the field of organizational behavior. In this introduction, we provide a brief overview of the motivation for this special issue, a concise summary of the papers included in it, a discussion about progress toward the field's goals, and suggestions for how future research can move the field forward. In doing so, we describe how applying attribution theory to studies that utilize a broad range of study designs, research contexts, and focal topics results in insights that enhance our understanding of how attributional processes predict and explain individuals' emotions and behaviors. We also provide suggestions for how to further develop attribution theory so we can more clearly describe the generalizability of relationships across a wide variety of organizational contexts. 相似文献
This paper reports the first investigation of risk perception by workers on offshore oil and gas installations on the UK Continental Shelf, following changes in offshore safety legislation in the wake of the Piper Alpha disaster in 1988. The Offshore Safety Case regulations (Health and Safety Executive, 1992, A Guide to the Offshore Installations (Safety Case) Regulations) put the onus on the operator to identify the major hazards and to reduce the risks to As Low As is Reasonably Practicable (ALARP). The regulations specifically state that Quantitative Risk Assessments (QRA) must be used when preparing the Safety Case. However, people do not use QRA when making everyday judgements about risk; they make subjective judgements known as risk perceptions, which are influenced by a number of different factors. This study was designed to complement the extensive QRA calculations that have already been carried out in the development of Safety Cases. The aim was to measure subjective risk perception in offshore personnel and examine how this relates to the more objective risk data available, namely accident records and QRA calculations. This paper describes the Offshore Risk Perception Questionnaire developed to collect the data and reports on UK offshore workers' perceptions of the risks associated with major and minor hazards, work tasks and other activities aboard production platforms. 相似文献
Objectives: This paper quantifies pediatric thoracoabdominal response to belt loading to guide the scaling of existing adult response data and to assess the validity of a juvenile porcine abdominal model for application to the development of physical and computational models of the human child. Methods: Table-top belt-loading experiments were performed on 6, 7, and 15 year-old pediatric post-mortem human subjects (PMHS). Response targets are reported for diagonal belt and distributed loading of the anterior thorax and for horizontal belt loading of the abdomen. Results: The pediatric PMHS exhibited abdominal response similar to the swine, including the degree of rate sensitivity. The thoraces of the PMHS were as stiff as, or slightly more stiff than, published adult corridors. Conclusions: An assessment of age-related changes in thoracic stiffness suggests that the effective stiffness of the chest increases through the fourth decade of life and then decreases, resulting in stiffness values similar for children and elderly adults. 相似文献
