Fetoscopic endotracheal occlusion (FETO) is a prenatal treatment that may increase survival in severe congenital diaphragmatic hernia (CDH). In the USA, FETO is offered for isolated severe left-sided CDH in the context of an FDA-approved feasibility study. FETO in non-isolated cases of severe CDH is only performed with a compassionate use exemption from US regulatory bodies. Anomalies frequently associated with CDH include congenital cystic lesions of the lung and cardiac defects. We describe two cases of non-isolated severe left-sided CDH that underwent prenatal FETO, survived after birth and underwent postnatal surgical repair. This study tested the hypothesis that stable C and N isotope values in surface soil and litter would be increased by fire due to volatilization of lighter isotopes. The hypothesis was tested by: (1) performing experimental laboratory burns of organic and mineral soil materials from a watershed at combinations of temperature ranging 100 to 600 degrees C and duration ranging from 1 to 60 min; (2) testing field samples of upland soils before, shortly after, and 1 yr following a wildfire in the same watershed; and (3) testing field soil samples from a down-gradient ash/sediment depositional area in a riparian zone following a runoff event after the wildfire. Muffle furnace results indicated the most effective temperature range for using stable isotopes for tracing fire impacts is 200 to 400 degrees C because lower burn temperatures may not produce strong isotopic shifts, and at temperatures>or=600 degrees C, N and C content of residual material is too low. Analyses of field soil samples were inconclusive: there was a slightly significant effect of the wildfire on delta15N values in upland watershed analyses 1 yr postburn, while riparian zone analyses results indicated that delta13C values significantly decreased approximately 0.71 per thousand over a 9 mo post-fire period (p=0.015), and ash/sediment layer delta13C values were approximately 0.65 per thousand higher than those in the A horizon. The lack of field confirmation may have been due to overall wildfire burn temperatures being <200 degrees C and/or microbial recovery and vegetative growth in the field. Since the completion of the Management of Myelomeningocoele Study, maternal-fetal surgery for spina bifida has become a valid option for expecting parents. More recently, multiple groups are exploring a minimally invasive approach and recent outcomes have addressed many of the initial concerns with this approach. Based on a previously published framework, we attempt to delineate the developmental stage of the surgical techniques. Furthermore, we discuss the barriers of performing randomized controlled trials comparing two surgical interventions and suggest that data collection through registries is an alternative method to gather high-grade evidence. 相似文献

In order to examine the early life-history characteristics of tropical eels, otolith microstructure and microchemistry were examined in leptocephali of Anguilla bicolor pacifica (27.6-54.1 mm TL, n=20) and A. marmorata (22.0-47.3 mm TL, n=8) collected during a cruise in the western Pacific. A. bicolor pacifica occurred between 10°N and 15°N in the west and between 5°S and 10°N farther to the east. A. marmorata also occurred in two different latitudinal ranges in the Northern (15-16°N) and Southern Hemispheres (3-15°S) of the western Pacific. The increment widths in the otoliths of these leptocephali increased between the hatch check (0 days) and about an age of 30 days in both species, and then gradually decreased toward the otolith edge. Otolith Sr:Ca ratios showed a gradual increase from the otolith center to the edge. The ages of A. bicolor pacifica and A. marmorata leptocephali ranged from 40 to 128 days and from 38 to 99 days, respectively. Growth rates of A. bicolor pacifica and A. marmorata leptocephali ranged from 0.33 to 0.71 mm day-1 and from 0.45 to 0.63 mm day-1, respectively. These leptocephali had estimated growth rates that were spread out throughout most of the reported range of growth rates of the leptocephali of the temperate species, the Japanese eel and the Atlantic eels. Differences in the spatial distribution in relation to current systems, and the age and size compositions of the leptocephali of A. bicolor pacifica and A. marmorata suggested different spawning locations for these two species. 相似文献
