The purpose of this research was to develop practical care procedures to help maintain the protective quality of flame resistant workwear laundered by workers in the field. Based on observed field conditions, experiments were conducted that simulated domestic laundry procedures. The first experiment involved two flame resistant (FR) fabrics, contaminated or not contaminated with oil. Independent variables also included detergent type and laundry pre-treatment. Other laundry parameters were controlled. Results indicated that it is easier to maintain the FR performance of the FR-treated blend than it is for the aramid fabric. It is hypothesized that energy generated by initial ignition of oil on the specimens triggers the FR mechanism of the treatment, which in turn inhibits further combustion. The Bhopal tragedy was a defining moment in the history of the chemical industry. On December 3, 1984, a runaway reaction within a methyl isocyanate storage tank at the Union Carbide India Limited pesticide plant released a toxic gas cloud that killed thousands and injured hundreds of thousands. After Bhopal, industrial chemical plants became a major public concern. Both the public and the chemical industry realized the necessity of improving chemical process safety.
Bhopal served as a wake-up call. To prevent the same event from occurring in the United States, many legislative and industrial changes were invoked—one of which was formation of the U.S. Chemical Safety and Hazard Investigation Board (CSB). The ultimate goal of CSB is to use the lessons learned and recommendations from its investigations to achieve positive change within the chemical industry—preventing incidents and saving lives.
Although it seems clear that the lessons learned at Bhopal have improved chemical plant safety, CSB investigations indicate that the systemic problems identified at Bhopal remain the underlying causes of many incidents. These include:
• Lack of awareness of reactive hazards.
• Lack of management of change.
• Inadequate plant design and maintenance.
• Ineffective employee training.
• Ineffective emergency preparedness and community notification.
• Lack of root cause incident investigations and communication of lessons learned.
The aim of this paper is to present common themes from recent cases investigated by CSB and to discuss how these issues might be best addressed in the future.

The performance of a wetland system in treating lead (Pb)/zinc (Zn) mine drainage was evaluated by using the polyurethane
foam unit (PFU) microbial community (method), which has been adopted by China as a standardized procedure for monitoring water
quality. The wetland system consisted of four cells with three dominant plants: Typha latifolia, Phragmites australis and Paspalum distichum. Physicochemical characteristics [pH, EC, content of total suspended solid (TSS) and metals (Pb, Zn, Cd, and Cu)] and PFU
microbial community in water samples had been investigated from seven sampling sites. The results indicated that the concentrations
of Pb, Zn, Cd, Cu, and TSS in the mine drainage were gradually reduced from the inlet to the outlet of the wetland system
and 99%, 98%, 75%, 83%, and 68% of these metals and TSS respectively, had been reduced in concentration after the drainage
passed through the wetland system. A total of 105 protozoan species were identified, the number of protozoa species and the
diversity index (DI) gradually increased, while the heterotrophic index (HI) gradually decreased from the inlet to the outlet
of the wetland system. The results indicated that DI, HI, and total number species of protozoa could be used as biological
indicators indicating the improvement of water quality. 相似文献
