With the development of increasingly complex processes and technologies in chemical and manufacturing industries, Process Safety Management (PSM) has been globally recognized as the primary tool for operating companies to reduce process accidents on their industrial sites and the risks posed to their employees and surrounding communities. Yet, industrial facilities are often interdependent and collocated with others. Recognizing this, regional authorities are also applying PSM principles to reduce the cumulative incidents associated with high density industrial areas and the multiplicative risks posed to broader communities. This paper compares Strathcona County Emergency Service (SCES) in Alberta, Contra Costa County Health Service Hazard Material Programs (CCCHSHMP) in California, and Technical Standards & Safety Authority (TSSA) in Ontario and their PSM systems to provide practical recommendations to improve SCES's system. Four aspects of PSM are considered: regulation and guidance, auditing and inspection, annual performance indicators, and public participation. Based on the results of this comparison, we recommend that SCES develop comprehensive PSM regulations based on CSA Z767-17 PSM including clear instructions for assessing technologies and methodologies for consequence analysis. Both worst-case scenarios and alternative scenarios need to be considered as well as the domino effect of primary accidents. Furthermore, regular audits and inspections will ensure compliance with PSM regulations while helping the design of planning, performing, and following-up strategies to ensure effectiveness. In addition, we suggest the use of lagging and leading performance indicators to evaluate the performance of the PSM program. Finally, we recommend using advisory councils or commissions to increase public participation and ensure the representation of stakeholders' perspectives with the PSM system.  相似文献

Background: The objective of this study is to provide an up-to-date overview of the patterns of injuries, especially traumatic brain injury (TBI) caused by RTAs and to discuss some of the public health consequences. Methods: A scientific team was established to collect road traffic accidents occurring between 2013 and 2018 in Chongqing, Southwest China. For each accident, the environment-, vehicle-, and person- variables were analyzed and determined. The overall injury distribution and TBI patterns of four types of road users (driver, passenger, motorcyclist and pedestrian) were compared. The environmental and time distribution of accidents with TBI were shown by bar and pie chart. The risks of severe brain injury whether motorcyclist wearing helmets or not were compared and the risk factors of severe TBI in pedestrian were determined by odds ratio analysis. Results: This study enrolled 2131 accidents with 2741 persons of all kind of traffic participants, 1149 of them suffered AIS1+ head injury and 1598(58%) died in 7 days. The most common cause of deaths is due to head injury with 714(85%) and 1266(79%) persons died within 2 hours. Among 423 persons suffered both skull fracture and intracranial injury, 102 (24.1%) have an intracranial injury but no skull fractures, while none of the skull fractures without intracranial injury was found. Besides, motorcyclists without a helmet were at higher risks for all the brain injury categories. The risk of pedestrian suffering severe TBI at an impact speed of more than 70 km/h is 100 times higher than that with an impact speed of less than 40 km/h. Conclusion: It is urgently needed to develop a more reliable brain injury evaluation criterion for better protection of the road users. We believe that strengthening the emergency care to head injury at the scene is the most effective way to reduce traffic fatality.  相似文献

Abstract

Objective: It is well established within the traffic psychology literature that a distinction can be made between driving skill and driving style. The majority of self-report questionnaires have been developed for car drivers, whereas only limited knowledge exists on the riding skill and style of cyclists. Individual differences in cycling skills need to be understood in order to apply targeted interventions.

Methods: This study reports on a psychometric analysis of the Cycling Skill Inventory (CSI), a self-report questionnaire that asks cyclists to rate themselves from definitely weak to definitely strong on 17 items. Herein, we administered the CSI using an online crowdsourcing method, complemented with respondents who answered the questionnaire using paper and pencil (n?=?1,138 in total). Our analysis focuses on understanding the major sources of variance of the CSI and its correlates with gender, age, exposure, and self-reported accident involvement as a cyclist.

Results: The results showed that 2 components underlie the item data: Motor–tactical skills and safety motives. Correlational analyses indicated that participants with a higher safety motives score were involved in fewer self-reported cycling accidents in the past 3 years. The analysis also confirmed well-established gender differences, with male cyclists having lower safety motives but higher motor–tactical skills than female cyclists.

Conclusions: The nomological network of the CSI for cyclists is similar to that of the Driving Skill Inventory for car drivers. Safety motives are a predictor of self-reported accident involvement among cyclists.  相似文献

INTRODUCTION: The urban road traffic accident (RTA) risks for the city of Zagreb, Croatia, from 1999 through 2000 were analyzed with the aim of reducing the increasing injury incidence. METHOD: Simple and bivariate analysis using chi(2), odds ratio, and confidence interval of 95% was used to determine risks in three outcome groups: killed, severely, and mildly injured. RESULTS: There were 528 RTA victims consisting of 260 severely, 213 mildly injured, and 55 killed at the scene of an accident and during transportation. More fatal accidents occurred during night hours (OR=3.78; 95% CI, 2.08-6.85), on urban road links (OR=2.33; 95% CI, 1.30-4.19), and at exceeding speed limit (OR=2.56; 95% CI, 1.43-4.61). More people were injured than killed on urban junctions (OR=5.27; 95% CI, 2.21-12.57). The highest combined risk of dying or being severely injured was found in males, driving at excessive speed, on urban links, and during bad visibility (OR=16.15; 95% CI, 3.901-66.881). CONCLUSION: These results will influence the urban traffic police enforcement measures, which will change inappropriate behavior of drivers and protect the least experienced road users.  相似文献

Incidents at U.S. onshore hazardous liquid pipeline systems were analyzed with an emphasis on natural hazards. Incidents triggered by natural hazards (natechs) were identified by keyword-based data mining and expert review supplemented by various data sources. The analysis covered about 7000 incidents in 1986–2012, 3800 of which were regarded as significant based on their consequences. 5.5% of all and 6.2% of the significant incidents were found to be natechs that resulted in a total hazardous substance release of 317,700 bbl. Although there is no trend in the long-term yearly occurrence of significant natechs, importance is found to be increasing due to the overall decreasing trend of the incidents. Meteorological hazards triggered 36% of the significant natechs, followed by geological and climatic hazards with 26% and 24%. While they occurred less frequently, hydrological hazards caused the highest amount of release which is about 102,000 bbl. The total economic cost of significant natechs was 597 million USD, corresponding to about 18% of all incident costs in the same period. More than 50% of this cost was due to meteorological hazards, mainly tropical cyclones. Natech vulnerabilities of the system parts vary notably with respect to natural hazard types. For some natural hazards damage is limited possibly due to implemented protection measures. The geographical distribution of the natechs indicated that they occurred more in some states, such as Texas, Oklahoma, and Louisiana. About 50% of the releases was to the ground, followed by water bodies with 28%. Significant consequences to human health were not observed although more than 20% of the incidents resulted in fires. In general, the study indicated that natural hazards are a non-negligible threat to the onshore hazardous liquid pipeline network in the U.S. It also highlighted problems such as underreporting of natural hazards as incident causes, data completeness, and explicit data limitations.  相似文献

Road planning is based on technical rules, know-how acquired from experience and professional expertise. A more ergonomics approach to road planning and safety analyses the psychological procedures used by the drivers. Every intervention on road network has an influence on safety. Increasing the coherency of all the interventions in the same area is the only possible way of producing areas with a better level of security. This paper presents the results of a survey on the procedures for reporting occupational accidents in the EU countries. It focuses particularly on the reporting and registration systems, as well as on the main characteristics of National Databases being used for the production of official statistics. The survey shows that many European countries have different methods and procedures for collecting and compiling data on occupational accidents, which inevitably pose problems when comparing accident statistics between the EU member states.The paper includes a brief review of the main goals and achievements of the European Project ESAW (European Statistics on Accidents at Work), which was launched in 1990 by the Eurostat (European Statistics), for the harmonisation of occupational accidents’ statistics among the member states. It is believed that the new variables being introduced by the harmonised methodology will quickly affect the way in which managers and insurance companies carry out their current procedures in several EU member states.The International Labour Organisation (ILO) recommendations and the future trends are also analysed. In conclusion, the study identifies opportunities for further development and research in the reporting and registration systems of occupational accidents.  相似文献
