This paper discusses the framework methodology behind the proposed simulation-based HAZOP tool. Simulation-based approach is one of the many ways to support conventional HAZOP by its automation. Compared to knowledge-based and other approaches, a HAZOP software tool based on deviations simulation is able to examine the investigated process more into detail and so find root causes of hazardous consequences. Another advantage is the ability to identify also potential hazards which did not occur in the past and might be overlooked. The presented framework methodology uses a layer of protection analysis (LOPA) concept of independent protection layers (IPLs) testing. Control system integrated into the raw process design represents the first of various protection layers of the LOPA concept. As a case study, a CSTR chemical production with nonlinear behavior under Proportional-Integral-Derivative (PID) actions as the predominant type of classical feedback control strategy is used. The presented tool identifies hazardous regimes under conditions when control loop introduces hazardous consequences or even acts synergically with existing hazardous events. Risk derived from different consequences is ranked by the risk assessment matrix (RAM) as a part of the conventional quantitative HAZOP study. 相似文献

The root cause of most accidents in the process industry has been attributed to process safety issues ranging from poor safety culture, lack of communication, asset integrity issues, lack of management leadership and human factors. These accidents could have been prevented with adequate implementation of a robust process safety management (PSM) system. Therefore, the aim of this research is to develop a comparative framework which could aid in selecting an appropriate and suitable PSM system for specific industry sectors within the process industry. A total of 21 PSM systems are selected for this study and their theoretical frameworks, industry of application and deficiencies are explored. Next, a comparative framework is developed using eleven key factors that are applicable to the process industry such as framework and room for continuous improvement, design specification, industry adaptability and applicability, human factors, scope of application, usability in complex systems, safety culture, primary or secondary mode of application, regulatory enforcement, competency level, as well as inductive or deductive approach. After conducting the comparative analysis using these factors, the Integrated Process Safety Management System (IPSMS) model seems to be the most robust PSM system as it addressed almost every key area regarding process safety. However, inferences drawn from study findings suggest that there is still no one-size-fits-all PSM system for all sectors of the process industry. 相似文献

Like all hazardous installations, inherently safer design (ISD) is one of the key tools in offshore oil and gas projects to minimize risks in offshore facilities. As the life cycle of offshore facilities is relatively short compared with onshore counterparts and there are many projects running every year, the potential is high for raising inherent safety standards and lowering safety risks throughout the offshore industry as old facilities are phased out. This paper gives an overview of offshore facilities and examples of implementation of ISD. Good examples of ISD are numerous. Industry guidance on ISD implementation abound. Yet, the systematic implementation of it in the industry is patchy. There are many reasons for factors which impede the effective, efficient and consistent implementation of ISD in projects. This paper describes some of them and proposes solution to address them. They include (a) the effective integration of ISD into hazard management systems with appropriate language to engage all disciplines in projects, (b) the phasing of resources to enable the project to capture ISD measures which are only available during early phases, (c) application of appropriate ISD goals and ISD performance metrics at various stages and (d) the appropriate use of quantified risk assessment to support ISD. 相似文献

The polymerization reaction can lower the threshold of the required energy by the initiator to improve the efficiency of the overall process reaction. Emerging polymerization initiators are also a major focus of process improvement and technological progress. Azo compounds (azos), which used in dyeing applications, are subsequently used in polymerization reactions due to their highly exothermic reaction characteristics. Although higher heat release can promote polymerization and modify the product, heat generation may also cause process hazards.These thermal hazard parameters were studied by selecting dimethyl 2,2′-azobis(2,4-dimethylvaleronitrile) (ABVN), 2,2′-azobis(2-methyl propionate) (AIBME), 2,2′-azobis(2-methylpropionamide) dihydrochloride (AIBA), and 2,2′-azobis(isobutyronitrile) (AIBN), which are common azo initiators at present. Thermal hazards are closely related to the reaction kinetics of the substance itself. The form of the reaction, the apparent activation energy and the thermodynamic parameters of the exothermic mode were also obtained.Kinetic analysis of the actual process using the experimental data of the isothermal calorimetry model is rarely used in the evaluation of related thermal hazard characteristics. The numerous formulated products which are introduced to the market consist of chemical ingredients that may cause various safety and health hazards to the consumers. Therefore, it is extremely important to practice a systematic methodology to formulate products with acceptable safety and health performances. This work presents an index-based methodology to assess the safety and health hazards of the ingredients during the early formulation stage of product design. Hence, new inherent safety and health sub-indexes are introduced to improve the current safety and health hazards that are needed in formulated product design. The inherent safety and health sub-indexes are assigned with scores based on the degree of potential hazards. A higher score indicates a higher safety risk or severe health effect, and vice versa. The proposed methodology will greatly assist the users to identify the adverse safety and health effects caused by the ingredients. Hence, it is pivotal to eliminate or reduce the safety and health impacts from product usage. Process hazard analysis (PHA) studies (e.g. HAZOP) identify hazard scenarios and each scenario is examined individually to decide whether risk reduction measures are needed. However, much more can be done to benefit from the contents of studies.PHA studies contain so much data that a manual review is precluded from yielding insights into their results. Fortunately, valuable information can be extracted using metrics and analytics that consider all the contents of a PHA study. The management of safety, operability, reliability, utilization, and loss prevention can all be improved.Metrics evaluate the contents of PHA studies to provide insights into the safety of processes and the quality of studies. Measures of process safety and study quality can be compared with norms to identify possible departures that may need to be addressed.Rather than focusing decision making on the risks of individual hazard scenarios, as is common practice, analytics enable a deeper analysis of the entire set of scenarios for a process. This enables better decisions to be made on where and how risk reduction resources should be allocated. Analytics can also be used to prioritize maintenance, training, and other activities.Various analytics and metrics for PHA studies are described together with examples that illustrate their use. 相似文献
