This experimental study was originally designed to quantify the electrostatic characteristics of offshore grade fiberglass reinforced plastic (FRP) pipes [Dastidar, A. G., Dahn, C. J., Cole, B. W., & Lo, K. H. (2005a). Electrostatic characteristics of FRP pipes. In Fourth international conference on composite materials for offshore operation, Houston, TX, Oct 4–6, 2005]. Discharge energies were measured from the sample surfaces for each test condition after an aggressive corona charging of up to −40 kV. While the measured values of total discharge energy were relatively high for some samples, the energy in the first peak was significantly lower. The first peak energy is thought to be the most significant measure in establishing the potential for incendive events.

To further quantify the incendive potential of discharges from FRP pipes, a unique test method was developed [Dastidar, A. G., Dahn, C. J., Cole, B. W., & Lo, K. H., (2005b). Incendiary nature and electrostatic discharge characteristics of FRP pipes. In Fourth international conference on composite materials for offshore operation, Houston, TX, Oct 4–6, 2005]. The methodology has been expanded to include other FRP components. This paper describes the test method and the test results obtained for testing several FRP structures and materials (pipes, grill work, railing etc.). The significance of the experimental results to industry is also discussed.  相似文献

PurposeInvestigation reports into the Deepwater Horizon drilling rig disaster identified issues with the drill crew's situation awareness (SA). The aim was to (1) apply the Driller's Situation Awareness (DSA) model to the cognitive data extracted from accident reports from this event to determine if it could help to explain why the crew erroneously concluded that the well was stable, which would (2) provide a preliminary evaluation of the model's validity.MethodThe DSA model was used for a content analysis of the SA components in the accounts of the crew's actions during two Negative Pressure Tests (NPT), in the hours before the blowout.ResultsThe analysis provided (1) insight into the crew's likely cognitive processes before the blowout. In particular, it revealed issues with their interpretation and mental models of the well state, as well as possible influencing factors including expectation, distraction and experience, emphasising the impact that SA can have on process safety. The categorisation has (2) initially suggested that the DSA model does contain the appropriate components.LimitationsThere are limited first hand reports of this event and thus cognitive processes have to be inferred with a degree of caution.Practical implicationsThe findings give a preliminary validation of the DSA model for further use in training and in investigation of well control events. Recommendations based on the findings are offered for assisting driller SA and consequently, for supporting safe and efficient drilling operations. There is also the opportunity to adapt the DSA model and apply the recommendations from the analysis to similar monitoring positions, where SA is essential, within the process industries.  相似文献

The oil and gas industry in Norway has taken into use and is developing ICT-based work processes to improve efficiency and safety. Protection of information resources and technology; ensuring safe and secure processing of information; and ensuring reliable and safe flow of information is essential for both production and protection in such ICT-intensive organizations. Information systems security (IS) thus matters for major accident prevention. In the ICT-based work processes in the oil and gas industry new challenges for IS security occur related to: increased need for availability and integrity of information; merge of process safety systems and ICT-systems; manual use of information resources; and flow of information between distributed actors. These challenges imply new requirements for IS management: More focus on availability and integrity as protection principles; resilience-based approaches as a supplement to formal management approaches; validation of information flows; and integration of loss prevention approaches.  相似文献

The Integrated Pollution Prevention and Control (IPPC 2008/1/EC) directive emphasises the use of a permit system to protect the environment as a whole from unwanted emissions in the specified industrial activities. The directive advocates the implementation of Best Available Techniques (BAT). Today, it is essential to select appropriate techniques, especially when an operating company is functioning in an environmentally sensitive area. On these grounds, companies encounter challenges to fulfill the IPPC guidelines as well as to qualify techniques for a specific purpose. The situation is challenging when applying new techniques or in the case of where a technique is considered new. However, technique qualification provides the evidence that BATs function within specific limits for an intended use whilst delivering an acceptable level of confidence. This paper explains how the concept of the Best Qualified Techniques (BQTs) is implemented using BAT approach. It also discusses a framework for broadening the BAT concept to include BQTs, drawing upon case studies of the offshore oil and gas industry.  相似文献

● The main direct seal up carbon options and challenges are reviewed. ● Ocean-based CO2 replacement for CH4/oil exploitation is presented. ● Scale-advantage of offshore CCS hub is discussed. Carbon capture and storage (CCS) technology is an imperative, strategic, and constitutive method to considerably reduce anthropogenic CO2 emissions and alleviate climate change issues. The ocean is the largest active carbon bank and an essential energy source on the Earth’s surface. Compared to oceanic nature-based carbon dioxide removal (CDR), carbon capture from point sources with ocean storage is more appropriate for solving short-term climate change problems. This review focuses on the recent state-of-the-art developments in offshore carbon storage. It first discusses the current status and development prospects of CCS, associated with the challenges and uncertainties of oceanic nature-based CDR. The second section outlines the mechanisms, sites, advantages, and ecologic hazards of direct offshore CO2 injection. The third section emphasizes the mechanisms, schemes, influencing factors, and recovery efficiency of ocean-based CO2-CH4 replacement and CO2-enhanced oil recovery are reviewed. In addition, this review discusses the economic aspects of offshore CCS and the preponderance of offshore CCS hubs. Finally, the upsides, limitations, and prospects for further investigation of offshore CO2 storage are presented.  相似文献

INTRODUCTION: Focusing on people and organizations, this paper aims to contribute to offshore safety assessment by proposing a methodology to model causal relationships. METHOD: The methodology is proposed in a general sense that it will be capable of accommodating modeling of multiple risk factors considered in offshore operations and will have the ability to deal with different types of data that may come from different resources. Reason's "Swiss cheese" model is used to form a generic offshore safety assessment framework, and Bayesian Network (BN) is tailored to fit into the framework to construct a causal relationship model. The proposed framework uses a five-level-structure model to address latent failures within the causal sequence of events. The five levels include Root causes level, Trigger events level, Incidents level, Accidents level, and Consequences level. To analyze and model a specified offshore installation safety, a BN model was established following the guideline of the proposed five-level framework. A range of events was specified, and the related prior and conditional probabilities regarding the BN model were assigned based on the inherent characteristics of each event. RESULTS: This paper shows that Reason's "Swiss cheese" model and BN can be jointly used in offshore safety assessment. On the one hand, the five-level conceptual model is enhanced by BNs that are capable of providing graphical demonstration of inter-relationships as well as calculating numerical values of occurrence likelihood for each failure event. Bayesian inference mechanism also makes it possible to monitor how a safety situation changes when information flow travel forwards and backwards within the networks. On the other hand, BN modeling relies heavily on experts' personal experiences and is therefore highly domain specific. IMPACT ON INDUSTRY: "Swiss cheese" model is such a theoretic framework that it is based on solid behavioral theory and therefore can be used to provide industry with a roadmap for BN modeling and implications. A case study of the collision risk between a Floating Production, Storage and Offloading (FPSO) unit and authorized vessels caused by human and organizational factors (HOFs) during operations is used to illustrate an industrial application of the proposed methodology.  相似文献

The Offshore Installation Manager (OIM) performs one of the most critical roles within the UK offshore Oil and Gas Industry. The OIM is responsible for the safety and well-being of all persons on-board and within 500 m of the installation whilst operating within a complex environment where multiple organisations and stakeholders interact. The persistence of major incidents in the industry highlights the importance of OIM competence in controlling emergencies, consequently the research aims to identify the potential barriers that can limit or prevent the proficient competence assessment of an OIM in this situation. The OIM role is investigated by qualitative analysis of key documents using hermeneutics to interpret text and to identify the potential barriers to effective competence assessment. Such analysis has historically been used within the domains of theology, business and law. Hermeneutics takes into account the cultural, historical and social environment at the time of issue of the document, and semiotics, i.e. the study of signs and symbols, their use and interpretation.Analysis is ongoing, however the six potential barriers to the effective competence assessment of OIMs in controlling emergencies identified to date are discussed. These barriers are associated with the multiple definitions of competence that exist in the UK and worldwide; the current predominance of competence assessment by observation; the lack of definitive competence requirements for trainers and assessors; the complexity in UK legislation and the interaction between UK Government departments and concerns associated with goal setting legislation when applied to competence assessment. The output from the research will assist industry Duty Holders improve the competence assessment of their appointed OIMs and allow safety engineers to consider the probable competence of an OIM in a given environment at a given point in time.  相似文献

Capping stack is an emergency shut-in technique that can effectively control offshore blowout accidents. This technique stemmed the spilled oil well (Macondo) in 2010 Deepwater Horizon blowout accident. However, few efforts have been devoted to studying the way to efficiently and safely conduct the operation of capping stack. In this paper, program evaluation and review technique (PERT) was employed to quantitatively design the operation procedure of capping stack. A mechanical model was established to determine the configuration scheme of capping stack. And a sensitivity analysis of operational factors of capping stack was conducted through orthogonal tests. Experimental results recommend to optimize the operation process by reducing the working time of diamond cutting tool down-pass, Lower Marine Riser Package (LMRP) upper riser incision, LMRP and original Blowout Preventer (BOP) separation, and LMRP recovery, in order to guarantee that the whole operation can be completed as scheduled. The results show that the number of ram BOPs has little effect on the equivalent stress of conductors. To improve system reliability, a five-ram capping stack is recommended to be applied in the defined accident scenario. The maximum equivalent stress of conductors rates the drilling vessel offset as the primary factor, followed by shut-in pressure, ocean current and top tension. The drilling vessel offset needs to be given the priority control and the shut-in pressure should be jointly monitored.  相似文献
