Since the completion of the Management of Myelomeningocoele Study, maternal-fetal surgery for spina bifida has become a valid option for expecting parents. More recently, multiple groups are exploring a minimally invasive approach and recent outcomes have addressed many of the initial concerns with this approach. Based on a previously published framework, we attempt to delineate the developmental stage of the surgical techniques. Furthermore, we discuss the barriers of performing randomized controlled trials comparing two surgical interventions and suggest that data collection through registries is an alternative method to gather high-grade evidence. 相似文献
Estuarine ecosystems of the Bay of Bengal, India, are considered as the most productive environment, which have been persistently threatened by substantial anthropogenic activity. This study aims to investigate the metal contamination in the sediment of two estuaries and possible biomagnifications in the indigenous edible oyster Saccostrea cucullata and related health hazards due to its consumption. The accumulative ecological risks indicated that the sediment is moderate to strongly contaminated with cadmium and lead. The sediment pollution index and pollution load index suggested that the sediment possesses a little ecological stress on the exposed flora and fauna. The statistical interpretation highlights the most metals which have a similar source of origin and are bound to the finer fractions of the sediment, except nickel. Bioaccumulation of sediment-associated Cu and Zn in oyster reflects their potential biomagnifications through aquatic food chain. HPI range was below the critical limit of safe human consumption. The non-carcinogenic (THQ) and carcinogenic (CR) health hazards were estimated from the PTDI provided by USEPA. Except Cr, Hg and Zn, the THQ of all other metals was > 1 suggesting detrimental non-carcinogenic health effects on humans. The TCR of Cr and Cd above safety limit indicates the exposed population might be under severe carcinogenic threat due to those metals.
This paper investigates the impact of Fairtrade and organic certification on household income of smallholder coffee farmers in the Jinotega Municipality of Nicaragua. Using a sample of 233 coffee farming households and employing endogenous switching regression model and propensity score matching method, the results found that Fairtrade and organic certification standards have different effects on the certified farmers; while Fairtrade farmers had experienced yield gains, organic farmers had the price advantage. However, the overall impact of these certification standards on the total household income is found to be statistically not significant. While some of the Fairtrade-certified cooperatives have used the social premium in creating community-level infrastructure, there is a need for more investment. The major constraint the organic-certified farmers face is lack of availability of adequate organic inputs such as manures and organic herbicides. 相似文献

Sub-lethal effects of mercury exposure (110th of LC(50), i.e. 0.045 mg l(-1)) for 5, 10 and 15 d was investigated on oxidative stress parameters and antioxidant defences in digestive gland of Perna viridis. In addition to this an in vitro effect of mercury single and supplemented with reduced glutathione on lipid peroxidation was studied. Increased lipid peroxidation (during first 10 days and also during in vitro exposures), protein carbonyl and hydrogen peroxides (from 5th till last day of exposure) indicate the resultant oxidative stress in the mercury exposed specimen. DNA damage (F-value) response although less distinct on 5th and 15th d, its low values on 10th d and significant correlation with hydrogen peroxide suggests the toxic role of free radicals towards DNA integrity. Superoxide dismutase, which remains low initially (5th d) and increases later suggests its immediate response against superoxide radical. Higher activities of catalase, glutathione peroxidase and glutathione reductase on 15th d and glutathione-S-tranferase from 10th d onwards suggests the adaptive behaviour of the tissue against oxyradicals. Increasing levels of non-enzymatic antioxidant molecules, such as reduced glutathione and ascorbic acid indicated its involvement in counteracting oxidative damage. Further role of reduced glutathione in reducing Hg toxicity is evident in in vitro experiments where lipid peroxidation remains low in mercury concentrations supplemented with reduced glutathione. The elevated levels of metallothionein from 5th to 10th d suggest involvement of this protein in detoxification of reactive oxygen species and toxic metal. The above results suggest that both enzymatic and non-enzymatic antioxidants play an important role in protecting cell against Hg toxicity, which can be used as a biomarker of metal contamination in aquatic environment. 相似文献
