The use of wood-fired steam baths, or temazcales, is a potentially dangerous source of CO exposure in Guatemalan Highland communities where adults and children use them regularly for bathing, relaxation, and healing purposes. Physical characteristics of children predispose them to absorb CO faster than adults, placing them at greater exposure and health risks. Efforts to quantify temazcal exposures across all age groups, however, have been hampered by the limitations in exposure measurement methods. In this pilot study we measured COHb levels in children and adults following use of the temazcal using three field-based, non-invasive CO measurement methods: CO-oximetry, exhaled breath, and by estimation of COHb using micro-environmental concentrations and time diaries. We then performed a brief comparison of methods. Average CO concentrations measured during temazcal use were 661 ± 503 ppm, approximately 10 times the 15 min WHO guideline. Average COHb levels for all participants ranged from 12-14% (max of 30%, min 2%), depending on the method. COHb levels measured in children were not significantly different from adults despite the fact that they spent 66% less time exposed. COHb measured by CO-oximetry and exhaled breath had good agreement, but precision of the former was affected substantially by random instrument error. The version of the field CO-oximeter device used in this pilot could be useful in screening for acute CO exposure events in children but may lack the precision for monitoring the burden from less extreme, but more day-to-day CO exposures (e.g. indoor solid fuel use). In urban settings, health effects in children and adults have been associated with chronic exposure to ambient CO concentrations much lower than measured in this study. Future research should focus on reducing exposure from temazcales through culturally appropriate modifications to their design and practices, and targeted efforts to educate communities on the health risks they pose and actions they can take to reduce this risk.  相似文献

Occupational exposure to inhalational anesthetics occurs routinely in operating rooms. It could induce serious health hazards and diseases. This exposure assessment is a crucial step in determining risks. In this study, a pen-shaped holder for solid-phase microextraction (SPME) sampler was successfully applied as a time-weighted average sampling tool for workshift exposure assessment of operation room staff to halothane. It proved to be very convenient for use in occupational environments such as operation rooms. Samples were analyzed by a gas chromatography-mass spectrometry. The validity of the SPME method was checked in real-world conditions with Occupational Safety and Health Administration (OSHA) 103 standard method for the determination of inhalational anesthetics. A good agreement between OSHA 103 and SPME methods was obtained and results demonstrated no statistically significant differences in anesthetic concentrations determined by the two analytical methods (p?≥?0.05). It is concluded that SPME in retracted mode could successfully be applied in occupational exposure assessment purposes.  相似文献

Organic emissions from a chemical weapons incinerator have been characterized with an improved set of analytical methods to reduce the human health risk assigned to operations of the facility. A gas chromatography/mass selective detection method with substantially reduced detection limits has been used in conjunction with scanning electron microscopy/energy dispersive X-ray spectrometry and Fourier transform infrared microscopy to improve the speciation of semi-volatile and non-volatile organics emitted from the incinerator. The reduced detection limits have allowed a significant reduction in the assumed polycyclic aromatic hydrocarbon (PAH) and aminobiphenyl (ABP) emission rates used as inputs to the human health risk assessment for the incinerator. A mean factor of 17 decrease in assigned human health risk is realized for six common local exposure scenarios as a result of the reduced PAH and ABP detection limits.  相似文献

United States environmental regulations, intended to protect human health, generally fail to address major sources of pollutants that endanger human health. These sources are surprisingly close to us and within our control, such as consumer products and building materials that we use within our homes, workplaces, schools, and other indoor environments. Even though these indoor sources account for nearly 90% of our pollutant exposure, they are virtually unregulated by existing laws. Even pollutant levels found in typical homes, if found outdoors, would often violate federal environmental standards. This article examines the importance of human exposure as a way to understand and reduce effects of pollutants on human health. Results from exposure studies challenge traditional thinking about pollutant hazards, and reveal deficiencies in our patchwork of laws. And results from epidemiological studies, showing increases in exposure-related diseases, underscore the need for new protections. Because we cannot rely solely on regulations to protect us, and because health effects from exposures can develop insidiously, greater efforts are needed to reduce and prevent significant exposures before they occur. Recommendations include the development and use of safer alternatives to common products, public education on ways to reduce exposure, systematic monitoring of human exposure to pollutants, and a precautionary approach in decision-making.  相似文献

Cyanide is a very toxic chemical that is used to extract gold from its ores. Wastewaters from gold mining companies such as Bogoso Gold Limited (BGL) contain cyanide and other potentially toxic chemicals that have adverse effects on human beings and aquatic organisms. This study was conducted to evaluate the human health risk assessment from exposure to free cyanide via oral and dermal contact of surface/underground water by resident adults within the concession of Bogoso Gold Limited. The chronic non-cancer health risk from exposure to cyanide in River Bogo Upstream is 230 and 43 (by Central Tendency Exposure (CTE) parameters respectively). This means that approximately 230 and 43 resident adults are likely to suffer diseases related to cyanide intoxication via oral and dermal contact respectively. For chronic exposure to River Bogo Downstream by resident adults, the non-cancer health risks are: 0.031 and 0.57 via oral and dermal contact for CTE parameters respectively, which also means that, the non-cancer health risks associated with cyanide intoxication is negligible as the hazard index is less than 1.0 via oral and dermal contacts respectively. The results showed that health risk for acute exposure to cyanide by the resident adults is very high. Hence the residents attribute most of the unexplained deaths in the communities to accidental ingestion and dermal contact of cyanide water.  相似文献

In both ambient air pollution epidemiology and health impact assessment an accurate assessment of the population exposure is crucial. Although considerable advances have been made in assessing human exposure outdoors, the assessments often do not consider the impact of individual travel behavior on such exposures.Population-based exposures to NO2 and O3 using only home addresses were compared with models that integrate all time-activity patterns—including time in commute—for Flanders and Brussels. The exposure estimates were used to estimate the air pollution impact on years of life lost due to respiratory mortality.Health impact of NO2 using an exposure that integrates time-activity information was on average 1.2% higher than when assuming that people are always at their home address. For ozone the overall estimated health impact was 0.8% lower. Local differences could be much larger, with estimates that differ up to 12% from the exposure using residential addresses only. Depending on age and gender, deviations from the population average were seen.Our results showed modest differences on a regional level. At the local level, however, time-activity patterns indicated larger differences in exposure and health impact estimates, mainly for people living in more rural areas. These results suggest that for local analyses the dynamic approach can contribute to an improved assessment of the health impact of various types of pollution and to the understanding of exposure differences between population groups.  相似文献

Assessing the health risk from lead (Pb) in potable water requires accurate quantification of the Pb concentration. Under worst-case scenarios of highly contaminated water samples, representative of public health concerns, up to 71–98 % of the total Pb was not quantified if water samples were not mixed thoroughly after standard preservation (i.e., addition of 0.15 % (v/v) HNO3). Thorough mixing after standard preservation improved recovery in all samples, but 35–81 % of the total Pb was still un-quantified in some samples. Transfer of samples from one bottle to another also created high errors (40–100 % of the total Pb was un-quantified in transferred samples). Although the United States Environmental Protection Agency’s standard protocol avoids most of these errors, certain methods considered EPA-equivalent allow these errors for regulatory compliance sampling. Moreover, routine monitoring for assessment of human Pb exposure in the USA has no standardized protocols for water sample handling and pre-treatment. Overall, while there is no reason to believe that sample handling and pre-treatment dramatically skew regulatory compliance with the US Pb action level, slight variations from one approved protocol to another may cause Pb-in-water health risks to be significantly underestimated, especially for unusual situations of “worst case” individual exposure to highly contaminated water.  相似文献

To determine whether a population has been affected by a chemical, evidence of exposure must be established. The mere presence of a chemical in the surroundings of a population may not, in all instances, result in actual exposure. Not all such exposures will cause health effects; nor is it always possible to establish that illness has or will result from exposure to chemicals. The inability to establish health effects in humans cannot a priori be translated to mean that a specific chemical is harmless. On the other hand, it must be determined whether health studies would be fruitful. If exposure was so minimal that no health effects are expected, then no health studies should be conducted.  相似文献

The CA Department of Pesticide Regulation (CDPR) developed methods to select pesticides and a community to fulfill criteria for an ambient air monitoring study it conducted as part of the CA Environmental Protection Agency's Environmental Justice Action Plan. Using a scoring system, CDPR evaluated 100 pesticides based on statewide-reported pesticide use, volatility, and priority in CDPR's risk assessment process (indicators of exposure and toxicity) to produce a list of pesticides to consider as candidates for monitoring. The CDPR also evaluated and scored 83 communities based on demographics and health factors, availability of cumulative impacts data, and reported pesticide use to create a list of community candidates. The scores provide relative rankings to distinguish more highly impacted communities from less impacted ones and to identify which pesticides might contribute most to potential adverse health effects. These methods use criteria that can be quantified, validated, and verified in order to provide a transparent and fair selection process. Based on public comments and highest scores, CDPR recommended 40 pesticides (including some of their degradation products) and one community for its yearlong monitoring study. The CDPR then further refined its list of pesticides by soliciting input from local and technical advisory groups. The CDPR plans to use these methods to select pesticides and communities in future monitoring activities.  相似文献

The temporal pattern of exposure to a specific compound may affect health in several ways. Exposure to pollution can have short-term effects or long-term effects. For some compounds there is a threshold under which there is no presumed measurable effect, whereas for other compounds, there is no presumed threshold. For short-term effects, the exposure to a high concentration of a compound one day may either increase or decrease the response if values of the same compound become high again the next day. Adaptation to effects of short-term exposure to ozone, for example, is reported. Similarly, health response to sudden high peaks of concentration may also possibly differ in effect from those to peaks attained more gradually. For long-term effects of some compounds, the cumulative exposure may be more decisive in influencing health. This paper proposes and describes in detail several air quality indicators that reflect the time variability and the episodic nature of air pollution exposure, as an attempt to represent the temporal aspects of pollution exposure that may have important effects on health. Mean concentrations, 98th percentile and maximum values are the traditional indicators for estimating exposure. The temporal variability of particulate matter (PM10) and NO2, however, is here described by means of: (1) the rate of change of pollution as the difference between two consecutive hourly or daily values, and of (2) episodes, described in terms of number, duration and inter-episode period, maximum concentration in the episode, and integrated episode exposure.  相似文献

Selection of an appropriate species is a key element of effective ecological risk assessments (ERA), especially when site-specific field studies are to be employed. Great blue herons (GBH) possess several ideal characteristics of a receptor species for the assessment of bioaccumulative compounds in the environment, such as ease of study, high potential for exposure, widespread distribution, and territorial foraging behavior. Methodologies for assessing exposure and population health are described herein. As outlined, the collection of GBH eggs, GBH nestling blood, and adult GBH blood allows for the determination of contaminant concentrations in various GBH tissues, a top-down assessment, which can be done in conjunction with predicted dietary exposure, a bottom-up assessment, to support a multiple lines of evidence approach. Additionally, population parameters, such as productivity and survival, can also be measured to elucidate if the contaminant exposure may be causing population level effects. Over the course of two years, three GBH rookeries were monitored for productivity and nestling exposure. Nests were monitored from blinds and individually accessed at multiple time points to obtain measures of nestling health, band nestlings, and collect eggs and nestling plasma. Multiple nests could frequently be accessed by climbing one tree, resulting in minimal effort to obtain the necessary sample size. Additionally, 51 adult GBH, captured in their foraging areas, were banded, and provided a blood sample. With these samples, a statistical difference in tissue based exposure was identified between the reference and target area. Statistically significant differences were also identified between the upper and lower reaches of the target area, thereby identifying a range of doses geographically which could be correlated to specific measurement endpoints. The ability to identify a dose response greatly increases the ability of the dataset to determine causation, a key goal of such studies. Overall, the use of the described methods allowed for the collection of a statistically sufficient and ecologically relevant dataset with reasonable effort and minimal impact on GBH.  相似文献

This paper reviews how aerosol exposure assessment, for people in both working and living environments, has evolved over the years. It charts the main scientific developments that led to progressively improved ways of thinking and methods to assess exposure to airborne particulate matter in a manner more relevant to human health. It has been a long scientific journey as one generation of pioneering contributors has handed off to the next. In the process a consistent rationale has emerged, producing aerosol sampling criteria--and in turn exposure standards--which have been increasingly relevant to actual human exposures. The journey continues as a new generation of scientists steps up to deal with the new challenges that are emerging. An appreciation of the history of what went before is essential to charting the most effective path looking forward.  相似文献

Beryllium is widely used in industry for its unique properties; however, occupational exposure to beryllium particles can cause potentially fatal disease. Consequently, exposure limits for beryllium particles in air and action levels on surfaces have been established to reduce exposure risks for workers. Field-portable monitoring methods for beryllium are desired in order to facilitate on-site measurement of beryllium in the workplace, so that immediate action can be taken to protect human health. In this work, a standardized, portable fluorescence method for the determination of trace beryllium in workplace samples, i.e., air filters and dust wipes, was validated through intra- and inter-laboratory testing. The procedure entails extraction of beryllium in 1% ammonium bifluoride (NH(4)HF(2), aqueous), followed by fluorescence measurement of the complex formed between beryllium ion and hydroxybenzoquinoline sulfonate (HBQS). The method detection limit was estimated to be less than 0.02 microg Be per air filter or wipe sample, with a dynamic range up to greater than 10 microg. The overall method accuracy was shown to satisfy the accuracy criterion (A< or = +/-25%) for analytical methods promulgated by the US National Institute for Occupational Safety and Health (NIOSH). Interferences from numerous metals tested (in >400-fold excess concentration compared to that of beryllium) were negligible or minimal. The procedure was shown to be effective for the dissolution and quantitative detection of beryllium extracted from refractory beryllium oxide particles. An American Society for Testing and Materials (ASTM) International voluntary consensus standard based on the methodology has recently been published.  相似文献

This article questions the basis for benzene as the carcinogenic surrogate in deriving health risk-based 'clean-up levels' for gasoline-impacted soil and groundwater at leaking underground storage tank properties. The epidemiological evidence suggests that acute myelogenous leukemia (AML) associated with chronic occupational benzene exposure can be best described by sigmoid dose-response relationships. A review of the molecular toxicology and kinetics of benzene points to the existence of threshold mechanisms in the induction of leukemia. The toxicological and epidemiological literature on chronic exposure to unleaded gasoline indicates that the benzene exposures required to induce a measurable carcinogenic response are substantially greater than exposures likely to be encountered from exposure to gasoline at contaminated properties. Thus, assuming that theoretical cancer risks associated with exposure to benzene from gasoline reflect actual health risks associated with such environmental exposures to gasoline and using these theoretical cancer risks and cancer potency factors for benzene to dictate soil and groundwater clean up of gasoline are not scientifically defensible.  相似文献

调查了某垃圾填埋场地下水和周边居民饮用水中的As、Hg等重金属和挥发酚等有机物的污染状况,运用美国环保局的健康风险计算模型,评估了该垃圾填埋场地下水对周边居民的潜在健康风险。结果表明,各类人群对4种有阈化合物的饮食和皮肤暴露的终身健康危险度在8.2×10^-11~1.3×10^-10之间,均低于可以接受的风险水平10^-6;对As的终身超额健康危险度在3.1×10^-7~4.9×10^-7之间,表明地下水中砷污染对居民潜在的致癌风险不明显。各类人群通过皮肤对Hg、As、Cd和挥发酚的暴露剂量比通过饮水暴露的剂量高0.7倍~14.5倍,皮肤暴露将是地下水影响人体健康并构成潜在风险的重要途径。  相似文献

综述了二噁英对人体健康的危害特性, 以及我国生活垃圾焚烧厂二噁英排放浓度水平,及其周边环境空气和土壤中二噁英污染浓度水平、人群暴露途径、剂量和健康影响。指出,现有研究显示,我国生活垃圾焚烧排放的二噁英浓度在合理范围之内,垃圾焚烧厂周边人群二噁英暴露量普遍低于世界卫生组织(WHO)推荐的容许标准,但也有研究发现,垃圾焚烧会导致周边环境介质中二噁英浓度水平增高。提出,应重点关注垃圾焚烧排放二噁英的长期低剂量暴露对人群健康的影响。  相似文献

Metal contamination of urban soils and homegrown products has caused major concern. In Part 1, we investigated the long-term effects of a former smelter on the degree of kitchen garden-soil contamination and the quality of the homegrown vegetables from these gardens. The results showed that the soils retained a high level of contamination and that a large proportion of the vegetables produced did not comply with the legislation on the levels of metals allowed for human consumption. The present study aims to assess the associated potential health risk to local inhabitants through consumption of homegrown vegetables and ingestion of soil particles using a land use-based approach. For lead (Pb), the standard hazard quotient (HQ)-based risk assessment method was used to determine the HQ. For cadmium (Cd), the approach consisted of calculating the HQs and then deriving site-specific assessment criteria (SSAC) using the SNIFFER method. The results suggested that the exposure pathways considered should not engender any form of deleterious health effects for adults. For children, Pb was the main concern and induced a relatively high health risk through soil particle ingestion, and most total soil Cd concentrations exceeded the derived SSAC, in particular, through consumption of vegetables. The metal bioaccessibility in soils was incorporated into the methods to establish more realistic risk assessment measures. This study proposes an approach to integrate different human health risk assessment methods. Further investigations should complete the assessment to improve risk determination, e.g., the determination of metal bioaccessibility in vegetables.  相似文献

Mercury contamination in fish is a serious public health concern that contrasts with other health benefits of eating fish. Like most US states, Illinois has monitored fish mercury contamination for decades to warn the public of mercury exposure risks by consuming fish. Has this monitoring program been effective in detecting public mercury exposure risks? I analyzed fish mercury contamination data from Illinois inland lakes (1974–1998; >?2,300 samples, 18 fish species, 149 lakes) and found that: (a) sampling and analyses have been severely limited since 1985; (b) sampling effort varied widely among lakes and species, and (c) trends and spatial patterns were confused by this variability. As a result of a severely limited and nonstrategic monitoring program, public mercury exposure risks via Illinois fish consumption remain unclear, despite much effort over many years. Illinois monitors fewer fish per angler than many US states, but is not alone in this regard. Illinois should resurrect and redesign its fish contaminant monitoring program to one that strategically and systematically assesses human mercury exposure risk. Other US states and nations may also benefit from similar retrospective examinations of monitoring programs intended to protect public health.  相似文献

Diesel pollution of groundwater poses great threat to public health, mainly as a result of the constituent polycyclic aromatic hydrocarbons (PAHs). In this study, the human health risk exposure to polycyclic aromatic hydrocarbons (PAHs) in diesel contaminated groundwater used by several families at Ring Road, Jos, Nigeria (as caused by diesel spill from a telecom base station) was assessed. Prior to the groundwater being treated, the residents were using the water after scooping off the visible diesel sheen for purposes of cooking, washing, and bathing. Until this study, it is not clear whether the groundwater contamination had resulted in sub-chronic exposure of the residents using the water to polycyclic aromatic hydrocarbons (PAHs) to the extent of the PAHs posing a health risk. The diesel contaminated groundwater and uncontaminated nearby groundwater (control) were collected and analyzed for PAHs using gas chromatography-mass spectrometry (GC-MS). The dosage of the dermal and oral ingestion entry routes of PAHs was determined. The estimation of the non-carcinogenic health risk was via hazard quotients (HQ) and the associated hazard index (HI), while the estimation of the carcinogenic health risk was via lifetime cancer risks (LCR) and the associated risk index (RI). Obtained results indicate that the exposure of the residents to the PAHs may have made them susceptible to the risk of non-carcinogenic health effects of benzo(a)pyrene and the carcinogenic health effects of benzo(a)anthracene and benzo(a)pyrene.  相似文献
