A number of recent and highly publicised fatal aircraft accidents, within the Australian regional airline industry, has highlighted the need for operators of regular public transport aircraft to be more proactive in identifying and addressing aviation safety hazards. Despite this need, there are currently few proactive safety management programs that are practical, simple, cost effective and which reliably demonstrate improvements in airline safety performance. This paper outlines a new proactive airline safety program called INDICATE (Identifying Needed Defences In the Civil Aviation Transport Environment) that has been applied within the Australian regional airline industry. To evaluate the INDICATE program, a major Australian regional airline agreed to implement the program in one of its operational bases while another base was used as a control group. Five evaluation criteria were applied to determine whether the program would have a positive influence on the airline's safety performance. These criteria included airline safety culture, staff risk perception of aviation safety hazards, willingness of staff to report safety hazards, action taken on identified safety hazards and staff comments about safety management within the airline. Results from the trial suggest that the program can have a positive influence on airline safety performance, specifically: improving staff confidence in how safety is managed, increasing staff willingness to report safety hazards and incidents, improving organisational safety culture and reducing staff perceptions of the severity and likelihood of safety hazards occurring within the airline. The success of the trial has resulted in a number of Australian and International airlines adopting the program.  相似文献

Most of the adverse impacts on man and/or the environment result from routine human activities such as the process industry, electricity generation and use, transport and agriculture (hazards, i.e. sources of risk). Apart from such essentially technological hazards, possibly resulting in “accidents”, human health and the environment can also be affected by natural hazards, possibly resulting in “disasters”, such as earthquakes or floods. This paper examines current trends in the risk sources and occurrences of four classes of such types of undesired events, entailing largely involuntary risk (e.g. neither car-driving nor smoking):
  • •major accidents at fixed installations in the process industry,
  • •incidents/accidents at nuclear installations,
  • •marine transport and offshore installation accidents,
  • •disasters caused by natural hazards and their potential exacerbation by human activities.
It aims to provide an integrated overview of such events in Europe (≡ 15 EU Member States, 4 EFTA, 13 PHARE, 7 TACIS and 5 other South and South Eastern European countries) during the last decade, estimating and interpreting trends in the number of risk sources and accidental events. For each type of event, specific “accident” definitions are given, illustrating the differences in the perception of the respective risk.  相似文献

The aim of the present study is to explore the process dynamics of the safety compliance climate and work relationships, including their antecedents and consequences. After investigating the many dimensions of safety compliance climate and of work relationships, the study concludes that a multi-component measure is needed to fully assess all of their dimensions. With respect to the impacting positively on safety compliance legislation, the present study found the following factors: “contribution measures”, “concrete preventive measures” and “risk assessment process”. For risk assessment factors on safety compliance the study identified “planning, guidelines, policy, management”, “prioritization of proactive measures”, “results in cooperation and information” and “active use of risks assessment document”. In a regression model for the workers, both risk assessment process and contribution measures had a statistically highly significant (p < 0.001) effect on concrete preventive measures. In the regression analysis of the employers risk assessment process had a statistically significant (p < 0.01) effect and contribution measures had a statistically highly significant (p < 0.001) effect on concrete preventive measures.  相似文献

Introduction: Automobile manufacturers are developing increasingly sophisticated driving automation systems. Currently, the highest level of automation available on the market is SAE Level 2, which provides sustained assistance for both lateral and longitudinal vehicle control. The purpose of this study was to evaluate how drivers’ perceptions of what behaviors secondary to driving are safe while a Level 2 system is operating vary by system name. Methods: A nationally representative telephone survey of 2005 drivers was conducted in 2018 with questions about behaviors respondents perceived as safe while a Level 2 driving automation system is in operation. Each respondent was asked about two out of five system names at random for a balanced study design. Results: The name “Autopilot” was associated with the highest likelihood that drivers believed a behavior was safe while in operation, for every behavior measured. There was less variation observed among the other four SAE Level 2 system names when compared with each other. A limited proportion of drivers had experience with advanced driver assistance systems and fewer of these reported driving a vehicle in which Level 2 systems were available. Drivers reported that they would consult a variety of sources for information on how to use a Level 2 system. Conclusions: The names of SAE Level 2 driving automation systems influence drivers’ perceptions of how to use them, and the name “Autopilot” was associated with the strongest effect. While a name alone cannot properly instruct drivers on how to use a system, it is a piece of information and must be considered so that drivers are not misled about the correct usage of these systems. Practical Applications: Manufacturers, suppliers, and organizations regulating or evaluating SAE Level 2 automated driving systems should ensure that systems are named so as not to mislead drivers about their safe use.  相似文献

Introduction: There has been a significant increase in online purchasing and product safety problems have been identified in e-commerce. This study examines consumer behavior and safety perceptions among parents purchasing child products online. Method: A mixed methods approach, including focus groups and a survey with parents, identified key characteristics and behaviors. Cluster analysis was used to determine different population segments (including “Informed,” “Uninformed,” and “Infrequent” consumer groups) based on their frequency of online shopping and search for product information. “Safe” and “Unsafe” behavior groups were identified related to their search for safety information on child products. Logistic regression analysis was used to study the effects of consumer type groups and demographic variables on the chances of being a safer consumer. Results: Findings indicate that child product safety considerations are not a priority for parents when shopping online. Only 62% of the survey respondents indicated that they search for information prior to buying a child product online, of which only 13% of the respondents noted that they search for information on product safety. Risky consumer behaviors were identified including the purchase of imitation products (counterfeit or knockoff products) and autonomous checks for product safety in lieu of safety standards. The logistic regression analysis found that being an “Uninformed Consumer” increases the odds of an individual being an “Unsafe Consumer” by 8.4 times (χ2(11) = 97.33, p < .001). Practical Applications: Design of a social marketing campaign that targets these different population segments to change perceptions and promote safe online purchasing is recommended.  相似文献

Risks to human beings arise from an inherent characteristic to make plans and try to make them happen, while external forces resist and tend to move our endeavours away from the plan. Any such “endeavour” is a complex ensemble of a bewildering variety of interacting elements which together form something “whole”, usually called a “system” (e.g. a chemical process plant, a nuclear power plant, the stock market, air traffic control). A system has a certain state in the present and subsequent states in the future. There are deterministic and probabilistic systems and corresponding approaches to analyse them, that is to make their current states apparent and predict their future behaviour. In this paper, it is shown in which ways both analyses appear in risk analysis and it is hypothesised that both approaches are modelling the same process, though probabilistic analysis may reveal more information since it explicitly incorporates uncertainty in the form of numbers (and “there is safety in numbers”). The overall objective of this paper is to make clear what the differences between the two approaches really are. Their respective main strengths and weaknesses are discussed. A more refined objective is to discuss the specific role of probabilities in risk analysis. Examples of both approaches are given from applications in the nuclear power and chemical process industries, and some of the main problems encountered thereby are identified. Further, the challenge to system analysis posed by “chaos theory” is discussed. Due to its non-linear dynamic character, the future behaviour of a “chaotic system” is difficult to predict over a long period of time because it depends on arbitrarily small and thus not observable variations in the current state. Such behaviour might have serious consequences for human operators involved in the control of such systems. The paper concludes that, although both approaches to risk analysis can provide adequate safety levels to systems if applied in a correct and non-biased way, probabilistic methods seem to be more cost-effective and the results easier to communicate to decision- and policy-makers.  相似文献

Introduction: Young workers are especially vulnerable to occupational injuries and illnesses. There is a continued need to investigate injury burden among young workers across demographics and industry to inform targeted interventions. Workers compensation (WC) claims are important for quantifying work-related injuries and illnesses, however published studies have focused on disabling claims. This study extended previous research on Oregon young workers by including the most recent WC claims data to identify patterns of injury and high risk industries. Methods: We obtained all accepted disabling claims (N = 13,360) and a significant portion of non-disabling claims (N = 24,660) on workers aged 24 years and under from 2013 to 2018. Claim count, rate and cost were calculated by year, age, gender, industry, and injury type. A prevention index (PI) method was used to rank industries in order to inform prevention efforts. Results: Average annual disabling and non-disabling claim rates were 111.6 and 401.3 per 10,000 young workers. Workers aged 19–21 (disabling: 119.0 per 10,000 and non-disabling: 429.3) and 22–24 years (115.7 and 396.4) and male workers (145.3 and 509.0) had higher claim rates than workers aged 14–18 (80.6 and 297.0) and female workers (79.8 and 282.9). The most frequent injury types were “struck by/against” (35.6%) and “work-related musculoskeletal disorders (WMSDs)” (19.5%). High risk industries included agriculture, construction, and manufacturing for both genders combined. For female young workers, the highest risk industry was healthcare. Conclusions: This study demonstrated the added value of non-disabling WC claims data. Using both disabling and non-disabling data and PI method, agriculture, construction, manufacturing and healthcare industries were identified as priority workplaces to prevent common and costly injuries among Oregon young workers. Practical Applications: While the industries identified are considered hazardous for all workers, findings in this study can guide targeted research and prevention efforts specific to young workers.  相似文献

This research introduces a proactive methodology for accident prevention, called Continuous Safety Sampling Methodology, by utilizing the principles of work sampling and control charting. Sampling is performed to observe the occurrence of conditions that may become hazardous in a given system. These conditions, known as dendritics, may become hazards and could result in an accident or occupational disease. Continuous Safety Sampling Methodology performs a random sampling for the occurrence of these dendritics. The collected data are then used to generate a control chart. Based on the pattern of the control chart, a system “under control” is not disturbed whereas a system “out of control” is investigated for potential conditions becoming hazardous. Appropriate steps are then taken to eliminate or control these conditions to maintain a desired safe system.  相似文献

IntroductionSafety management in construction is complicated due to the complex “nature” of the construction industry. The aim of this research was to identify safety management factors (e.g., risk management and site management), contextual factors (e.g., organisational complexity) and combinations of such factors connected to safety performance. Method: Twelve construction projects were selected to compare their safety management and safety performance. An analytical framework was developed based on previous research, regulations, and standards where each management factor was defined. We employed qualitative comparative analysis (QCA) to produce case knowledge, compare the cases, and identify connections between the factors and safety performance. The material collected and analyzed included, for example, construction planning documents, reports from OHS-inspections, safety indicators, and interviews with project leaders and OHS experts. Results and conclusions: The research showed that: (a) the average score on 12 safety management factors was higher among projects with high safety performance compared to projects with low safety performance; (b) high safety performance can be achieved with both high and low construction complexity and organizational complexity, but these factors complicate coordination of actors and operations; (c) it is possible to achieve high safety performance despite relatively poor performance on many safety management factors; (d) eight safety management factors were found to be “necessary” for high safety performance, namely roles and responsibilities, project management, OHS management and integration, safety climate, learning, site management, staff management, and operative risk management. Site management, operative risk management, and staff management were the three factors most strongly connected to safety performance. Practical implications: Construction stakeholders should understand that the ability to achieve high safety performance in construction projects is connected to key safety management factors, contextual factors, and combinations of such factors.  相似文献

IntroductionCurrently, there is a lack of specific analytical tools for general aviation accidents (GAAs). This has led to loopholes in the prevention of GAAs.MethodsA Swiss Cheese model for general aviation (SCM-GA) is proposed to identify the human and organizational factors involved in GAAs. In the proposed SCM-GA, 5 categories, 45 subcategories, a general aviation safety management system (GA-SMS) and safety culture were developed based on the classic accident causation models combined with the laws and regulations and safety management practices in the general aviation industry.ResultsOne GAA was analyzed using SCM-GA. The human and organizational causes revealed by SCM-GA were more complete than the causes revealed through the accident report. The identification results of the deficiencies in the subcategories of GA-SMS and the safety culture were more consistent with the requirements in the general aviation laws and regulations than the organizational factors in the accident report. Based on the subcategories of SCM-GA, 41 GAAs that occurred between 1996 and 2010 in China were statistically analyzed and χ2 test analyses were performed to estimate the statistical strength of the association between two adjacent subcategories of SCM-GA. The results showed that two adjacent subcategories of SCM-GA were significantly associated. They helped to determine the hidden problems in the accident report based on the path of accident.ConclusionsSCM-GA is an accident analysis tool that can comprehensively analyze the human and organizational deficiencies involved in GAAs. The accident causes revealed by SCM-GA were more consistent with the general aviation safety management practices.Practical applicationsGeneral aviation companies should establish their own GA-SMS and safety culture based on the subcategories developed herein. Using SCM-GA for routine safety inspection and accident investigation will help the management and the staff make effective safety decisions to effectively prevent GAAs.  相似文献

European Standard EN 14491 (2006): “Dust explosion venting protective systems”, based on German Standard VDI 3673 (2002): “Pressure venting of dust explosions”, Verein Deustcher Ingenieure, specifies the basic design requirements for the selection of dust explosion venting protection systems.In this study an investigation into the efficiency of venting surfaces in accordance with Standard EN 14491 has been undertaken to check the validation of the same. Thus, the application of the standard to an actual case of an explosion in a milk spray dryer has been studied, taking into consideration the explosive characteristics of products treated in this type of installation and their venting requirements.The results obtained indicate that, in specific situations – such as the effects of turbulence, particularly in the cone, or dust layer self-ignition – the European Standard could underestimate the venting surface requirements needed to alleviate possible excess internal pressure in order to avoid injury or structural damage to spray dryers.  相似文献

Introduction: This study performed a path analysis to uncover the behavioral pathways (from contributing factors, pre-crash actions to injury severities) in bicycle-motor vehicle crashes. Method: The analysis investigated more than 7,000 bicycle-motor vehicle crashes in North Carolina between 2007 and 2014. Pre-crash actions discussed in this study are actions of cyclists and motorists prior to the event of a crash, including “bicyclist failed to yield,” “motorist failed to yield,” “bicyclist overtaking motorist,” and “motorist overtaking bicyclist.” Results: Model results show significant correlates of pre-crash actions and bicyclist injury severity. For example, young bicyclists (18 years old or younger) are 23.5% more likely to fail to yield to motor traffic prior to the event of a crash than elder bicyclists. The “bicyclist failed to yield” action is associated with increased bicyclist injury severity than other actions, as this behavior is associated with an increase of 5.88 percentage points in probability of a bicyclist being at least evidently injured. The path analysis can highlight contributing factors related to risky pre-crash actions that lead to severe injuries. For example, bicyclists traveling on regular vehicle travel lanes are found to be more likely to involve the “bicyclist failed to yield” action, which resulted in a total 44.38% (7.04% direct effect + 37.34% indirect effect) higher likelihood of evident or severe injuries. The path analysis can also identify factors (e.g., intersection) that are not directly but indirectly correlated with injury severity through pre-crash actions. Practical Applications: This study offers a methodological framework to quantify the behavioral pathways in bicycle-motor vehicle crashes. The findings are useful for cycling safety improvements from the perspective of bicyclist behavior, such as the educational program for cyclists.  相似文献

Objectives: National fatality rates for commercial tow truck operators exceed those of other first responders who also perform traffic incident management services. The objectives of the current study are to (1) characterize causal factors associated with injuries among commercial tow truck operators engaged in roadside assistance through analysis of coded and free text data obtained from U.S. Occupational Safety and Health Administration (OSHA) investigation files, and (2) utilize supplemental data sources to analyze environmental factors for injuries in which commercial tow truck operators were struck by roadway traffic. Methods: Searches of OSHA’s online IMIS database were performed to identify investigations of incidents in which tow truck operators were injured while performing roadside assistance duties. Freedom of Information Act (FOIA) requests were submitted to obtain full investigation files for each case. Coded and narrative text analyses were performed to identify causal themes across the identified cases. Results: One-hundred and six cases of tow truck operators being killed or severely injured were identified in IMIS; 41 FOIA requests for related investigation documents were fulfilled. Two major event type themes were identified which accounted for 9 in 10 of the cases identified. These were (1) ‘struck-by’ incidents, which were primarily injuries resulting from contact with roadway traffic, rolling vehicles and equipment or other non-motorized objects; and (2) ‘caught-in or -between’ incidents, which were primarily injuries resulting from being pinned beneath and between vehicles and being caught in moving parts. Conclusions: The towing industry should provide initial and refresher safety training on vehicle loading and unloading, defensive techniques when exposed to traffic on roadways, and proper wheel chocking and braking procedures. States should include tow trucks as a first responder vehicle type in their “Move Over” laws and implement public awareness campaigns to protect all first responders, including tow truck operators.  相似文献

Some basic principles for philosophical definition work are introduced and then applied to safety and related concepts. Definitions are provided first for comparative safety concepts such as “safer than” and then for the monadic “safe”. It is shown that “safe” is an inherently inconsistent concept, i.e. it cannot be restored to consistency without giving up what we perceive as some of its central elements. The reason for this is that both absolute and relative conceptions of safety are entrenched in common usage of the term. In order to avoid the inconsistency a strategy of terminological ramification is proposed: We should distinguish between the two concepts “reasonably safe” and “absolutely safe”. Any usage of “safe” or “safety” simpliciter should be seen as an abbreviated reference to one of these two closely related, remarkably confusable, but still unmergable concepts.  相似文献

The old saying, “what you don't know can't hurt you,” implies that ignorance is bliss. “A little knowledge is a dangerous thing,” may be closer to the truth; however, it is not the little that we know that is dangerous, but that which is not known. By design, the processes used in the chemical industry are reactive, and the intended reaction receives much scrutiny. However, other reactions occur, often unexpectedly, and possibly with severe consequences. The lessons we learn from these reactions must drive the improvement of our process development and technology management processes and the culture that shapes those processes, a culture of Technical Discipline.Technical Discipline, analogous to Operating Discipline in the manufacturing organization, is a culture committed to fully identifying and characterizing chemical and reaction hazards, and properly documenting and communicating those hazards to create a permanent knowledge and understanding within the organization operating that process.A culture of Technical Discipline will reveal reaction hazards that might otherwise remain unknown until being unveiled in a dramatic and unexpected fashion. The research questions posed in this study highlight the importance of valuing proactivity in both prompting an individual to engage in proactive behavior and encouraging the expression of that behavior. We integrate a variety of constructs from the proactivity literature to gain a deeper understanding of proactive behavior as it relates to proactive motivation and supervisory performance evaluations. First, we draw upon self‐determination theory, expectancy–value theory, and the recent integration of the proactive motivation literature to hypothesize that proactive behavior is predicted by the interaction of “can do” and “reason to” proactive motivational states. Second, on the basis of performance theory, we hypothesize that the relationship between proactive behavior and performance depends upon the extent to which the supervisor values proactivity. Specifically, we argue that supervisors with proactive personalities are more likely to value and reward subordinate proactive behavior than passive supervisors. Results provide support for both of our hypotheses. Interestingly, results show that proactive behavior did not result in negative consequences but rather that there was a cost (i.e., lower performance rating) for not taking charge for employees with proactive supervisors. In-vehicle navigation systems use an auditory interface to avoid redirecting drivers’ visual attention when interaction with the devices is required. Since most of these systems emit sounds from the installation location of the devices, there are situations when the sound-source location conflicts with the message’s semantics. This research studied the effect of congruency on recall accuracy and response time after a distraction period during which participants had to perform a modified Brown–Peterson task. Three conditions of verbal message presentation methods were tested: to left-ear only (PL), to right-ear only (PR), and to one of the ears according to the message semantics (PS), in which message content was either “left” or “right”. Three distraction intervals were also tested. The results indicated that the length of the distraction interval significantly reduced response time, suggesting an expectancy effect in the presence of distractions. The presentation condition did not have significant effect on error rate. The data were further divided into two conditions: the congruent condition, which was equivalent to PS, and the conflicting condition, for PL and PR. The analysis showed that the congruent/conflicting condition significantly affected the error rate; the data revealed that congruent condition had a lower error rate. The findings have implications for the design of verbal messages with semantics that are spatial in nature.  相似文献

Introduction: Controlled Flight Into Terrain (CFIT) account for a considerable amount of fatalities when compared to other accident categories. Human factors are deemed significant contributory causes in these accidents. This paper aims to identify the human factors involved with aviation accidents that resulted in CFIT. Method: The study used the Human Factors Analysis and Classification System (HFACS) framework to determine the factors involved in 50 CFIT accidents from 24 counties over a 10 year period, i.e. 2007–2017. Interviews with five senior aviation safety experts were used to provide a better comprehension of the human factors affecting the flight safety. Results: The study identified 1289 individual causal and contributory human factors with unsafe actions and preconditions for unsafe actions being the main subcategories of the accidents. The study found that CFIT occur across a range of pilot experience and 44% of accidents occurred in cruise flight. Distraction, complacency and fatigue are all elements that flight crews may experience as contributors to CFIT during cruising. Conclusions: Human factors represent a major component of CFIT accidents. The analysis revealed a similar pattern of contributory and causal human factors across the various flight categories, with some noteworthy isolated variations. The prevalent factors were decision and skill-based errors along with communication, coordination and planning issues. Practical applications: Provision of specific CFIT awareness, pilot training focusing on improved decision-making and revision of basic flight skills, development of specific Global Positioning System routes for transiting high terrain areas are necessary to prevent CFIT accidents. Installation of Terrain Avoidance and Warning System and Ground Proximity Warning System and appropriate equipment training, specific CFIT Crew Resource Management training and improvement of organizational knowledge on the elements involved in CFIT are also recommended.  相似文献
