根据中国海上石油工业缺乏独立的风险评价体系的现状 ,笔者对极端海况进行了环境荷载的联合概率分析 ,利用随机模拟技术 ,求解结构的失效概率 ,提出结构失效分析的新方法 ,并利用DNV提供的历史数据 ,对结构失效的后果做了适当分析。以埕北 12 C井组平台作为实例 ,对平台甲板高程 (AirGap)失效分析进行了实例计算。结果表明 ,使用多维联合极值分布理论及相应的求解方法 ,是离岸工程结构物风险评估的重要手段。  相似文献

Offshore oil and gas platforms are well known for their compact geometry, high degree of congestion, limited ventilation and difficult escape routes. A small mishap under such conditions can quickly escalate into a catastrophe. Among all the accidental process-related events occurring offshore, fire is the most frequently reported. It is, therefore, necessary to study the behavior of fires and quantity the hazards posed by them in order to complete a detailed quantitative risk assessment. While there are many consequence models available to predict fire hazards-varying from point source models to highly complex computational fluid dynamic models—only a few have been validated for the unique conditions found offshore.

In this paper, we have considered fire consequence modeling as a suite of sub-models such as individual fire models, radiation model, overpressure model, smoke and toxicity models and human impact models. This comprehensive suite of models was then revised by making the following modifications: (i) fire models: existing fire models have been reviewed and the ones most suitable for offshore conditions were selected; (ii) overpressure impact model: a model has been developed to quantify the overpressure effects from fires to investigate the possible damage from the hot combustion gases released in highly confined compartments; (iii) radiation model: instead of a point/area model, a multipoint grid-based model has been adopted for better modeling and analysis of radiation heat flux consequences. A comparison of the performance of the revised models with the ones used in a commercial software package for offshore risk assessment was also carried out and is discussed in the paper.  相似文献

PROBLEM: Tragic marine and offshore accidents have caused serious consequences including loss of lives, loss of property, and damage of the environment. METHOD: A proactive, risk-based "goal setting" regime is introduced to the marine and offshore industries to increase the level of safety. DISCUSSION: To maximize marine and offshore safety, risks need to be modeled and safety-based decisions need to be made in a logical and confident way. Risk modeling and decision-making tools need to be developed and applied in a practical environment. SUMMARY: This paper describes both the offshore safety case approach and formal safety assessment of ships in detail with particular reference to the design aspects. The current practices and the latest development in safety assessment in both the marine and offshore industries are described. The relationship between the offshore safety case approach and formal ship safety assessment is described and discussed. Three examples are used to demonstrate both the offshore safety case approach and formal ship safety assessment. The study of risk criteria in marine and offshore safety assessment is carried out. The recommendations on further work required are given. IMPACT ON INDUSTRY: This paper gives safety engineers in the marine and offshore industries an overview of the offshore safety case approach and formal ship safety assessment. Risk assessment can be classified into two broad categories: traditional and modern. This paper is aimed at contrasting the functional resonance analysis method (FRAM) as a modern approach with the fault tree analysis (FTA) as a traditional method, regarding assessing the risks of a complex system. Applied methodology by which the risk assessment is carried out, is presented in each approach. Also, FRAM network is executed with regard to nonlinear interaction of human and organizational levels to assess the safety of technological systems. The methodology is implemented for lifting structures deep offshore. The main finding of this paper is that the combined application of FTA and FRAM during risk assessment, could provide complementary perspectives and may contribute to a more comprehensive understanding of an incident. Finally, it is shown that coupling a FRAM network with a suitable quantitative method will result in a plausible outcome for a predefined accident scenario.  相似文献

Rare events often result in large impacts and are hard to predict. Risk analysis of such events is a challenging task because there are few directly relevant data to form a basis for probabilistic risk assessment. Due to the scarcity of data, the probability estimation of a rare event often uses precursor data. Precursor-based methods have been widely used in probability estimation of rare events. However, few attempts have been made to estimate consequences of rare events using their precursors. This paper proposes a holistic precursor-based risk assessment framework for rare events. The Hierarchical Bayesian Approach (HBA) using hyper-priors to represent prior parameters is applied to probability estimation in the proposed framework. Accident precursor data are utilized from an information theory perspective to seek the most informative precursor upon which the consequence of a rare event is estimated. Combining the estimated probability and consequence gives a reasonable assessment of risk. The assessed risk is updated as new information becomes available to produce a dynamic risk profile. The applicability of the methodology is tested through a case study of an offshore blowout accident. The proposed framework provides a rational way to develop the dynamic risk profile of a rare event for its prevention and control.  相似文献

This paper reports the first investigation of risk perception by workers on offshore oil and gas installations on the UK Continental Shelf, following changes in offshore safety legislation in the wake of the Piper Alpha disaster in 1988. The Offshore Safety Case regulations (Health and Safety Executive, 1992, A Guide to the Offshore Installations (Safety Case) Regulations) put the onus on the operator to identify the major hazards and to reduce the risks to As Low As is Reasonably Practicable (ALARP). The regulations specifically state that Quantitative Risk Assessments (QRA) must be used when preparing the Safety Case. However, people do not use QRA when making everyday judgements about risk; they make subjective judgements known as risk perceptions, which are influenced by a number of different factors. This study was designed to complement the extensive QRA calculations that have already been carried out in the development of Safety Cases. The aim was to measure subjective risk perception in offshore personnel and examine how this relates to the more objective risk data available, namely accident records and QRA calculations. This paper describes the Offshore Risk Perception Questionnaire developed to collect the data and reports on UK offshore workers' perceptions of the risks associated with major and minor hazards, work tasks and other activities aboard production platforms.  相似文献

INTRODUCTION: Focusing on people and organizations, this paper aims to contribute to offshore safety assessment by proposing a methodology to model causal relationships. METHOD: The methodology is proposed in a general sense that it will be capable of accommodating modeling of multiple risk factors considered in offshore operations and will have the ability to deal with different types of data that may come from different resources. Reason's "Swiss cheese" model is used to form a generic offshore safety assessment framework, and Bayesian Network (BN) is tailored to fit into the framework to construct a causal relationship model. The proposed framework uses a five-level-structure model to address latent failures within the causal sequence of events. The five levels include Root causes level, Trigger events level, Incidents level, Accidents level, and Consequences level. To analyze and model a specified offshore installation safety, a BN model was established following the guideline of the proposed five-level framework. A range of events was specified, and the related prior and conditional probabilities regarding the BN model were assigned based on the inherent characteristics of each event. RESULTS: This paper shows that Reason's "Swiss cheese" model and BN can be jointly used in offshore safety assessment. On the one hand, the five-level conceptual model is enhanced by BNs that are capable of providing graphical demonstration of inter-relationships as well as calculating numerical values of occurrence likelihood for each failure event. Bayesian inference mechanism also makes it possible to monitor how a safety situation changes when information flow travel forwards and backwards within the networks. On the other hand, BN modeling relies heavily on experts' personal experiences and is therefore highly domain specific. IMPACT ON INDUSTRY: "Swiss cheese" model is such a theoretic framework that it is based on solid behavioral theory and therefore can be used to provide industry with a roadmap for BN modeling and implications. A case study of the collision risk between a Floating Production, Storage and Offloading (FPSO) unit and authorized vessels caused by human and organizational factors (HOFs) during operations is used to illustrate an industrial application of the proposed methodology.  相似文献

This paper introduces the new approach of risk analysis established by the French Ministry of the Environment and develops the benefits of applying nuclear probabilistic safety assessment approaches to the chemical industry.In the aftermath of the AZF disaster in Toulouse on 21 September 2001, a new law was proposed by the French government asking for the investigation of all representative scenarios and the assessment of the probability of the resulting dangerous phenomena to demonstrate an acceptable level of safety. Therefore, any accident is investigated from a global point of view, according to its gravity and its probability.In 2000, the French Ministry of the Environment asked the Institute for Radiological Protection and Nuclear Safety (IRSN), and in particular its Systems and Risk Protection Assessment (SESPRI) and its Industrial Risks, Fire and Containment Assessment and Study (SERIC) departments, to conduct a Probabilistic Safety Assessment (PSA) study of an LPG distribution facility, specially for the BLEVE scenario. This study has showed the power of PSA for defining and prioritizing actions to be carried out to improve safety of facilities; however, it requires credible data for reliability and failure of the equipment, not available in generic failure databases.Since 2007, IRSN has taken several initiatives in collaboration with operators in order to provide more precise and representative failure rates for main safety equipment, ready to use in future PSA relative to LPG plants.  相似文献

针对含硫气井开发过程中井筒完整性失效风险,建立了含硫气井风险评价模型。采用Bow-tie模型将井筒完整性的失效形式分为水力屏障失效、实体屏障失效和操作管理屏障失效3个方面,确定了含硫气井井筒完整性失效的28个风险因素;基于层次分析法定量确定了各风险因素的权重,通过风险矩阵法评估了风险因素发生的可能性与严重程度,实现了对井筒完整性风险等级的划分。将以上方法对某含硫气井进行了应用,确定了该井的风险等级及主要风险因素。研究结果表明:建立的风险评价模型能够对含硫气井井筒完整性进行评价,确定风险等级。该方法的应用有助于降低含硫气井的事故风险,可为含硫气井完整性现场管理提供参考。  相似文献

为了有效预测海上气井环空带压油套管柱潜在失效风险及安全管理环空压力,提出1种基于井口压力、温度和液位多参数监测的海上气井环空带压油套管柱动态安全评价技术。首先,结合气井生产状态参数和完井信息,建立气井井筒压力分布计算方法;然后,根据力学知识分析气井环空带压油套管柱应力场特点,进而提出利用环空带压井油套管柱安全系数作为油套管柱失效评价依据;最终,开发海上气井环空带压安全监测系统并开展现场案例井分析。研究结果表明:监测系统能够准确获取环空带压状态参数,通过环空压力状态变化分析阐述“A”环空带压机理为泄漏点处油套管之间压力平衡的保持;结合实际井口环空压力监测数据,井筒安全系数能够实时表征油套管柱安全状态。  相似文献

Corrosion is a major cause of structural deterioration in marine and offshore structures. It affects the life of process equipment and pipelines, and can result in structural failure, leakage, product loss, environmental pollution and the loss of life. Pitting corrosion is regarded as one of the most hazardous forms of corrosion for marine and offshore structures. The total loss of the structure might be very small, but local rate of attack can be very large and can lead to early catastrophic failure. Pitting corrosion is a localized accelerated dissolution of metal that occurs as a result of a breakdown in the protective passive film on the metal surface. It has been studied for many years; however, the structural failure due to pit characteristics is still not fully understood. Accurate pit depth measurements, precise strength assessment techniques, risk analysis due to pitting, and the mathematical relationship of the environmental factors that causes pitting failure are also factors, which need further understanding. Hence this paper focuses on these issues. It reviews and analyses the current understanding of the pitting corrosion mechanism and investigates all possible factors that can cause pitting corrosion. Furthermore, different techniques employed by scientists and researchers to identify and model the pitting corrosion are also reviewed and analysed. Future work should involve an in-depth scientific study of the corrosion mechanism and an engineering predictive model is recommended in order to assess failure, and thereby attempt to increase the remaining life of offshore assets.  相似文献

Failure of Leak Detection System (LDS) to detect pipeline leakages or ruptures may result in drastic consequences that could lead to excessive financial losses. To minimize the occurrence of such failure, the functionality of the LDS and the integrity of the pipeline should be assessed on a priority basis. This paper presents an integrated risk-based assessment scheme to predict the failure and the failure consequences of offshore crude oil pipelines. To estimate risk, two important quantities have to be determined, the joint probability of failure of the pipeline and its LDS and the consequences of failure. Consequences incorporate the financial losses associated with environmental damage, oil spill cleanup and lost production. The assessment provides an estimate of the risk in monetary value and determines whether the estimated risk exceeds a predefined target risk. Moreover, the critical year for the asset can be determined. In essence, the outcome of the assessment facilitates an informed decision-making about the future of the asset.  相似文献

Operational safety is receiving more and more attention in the Norwegian offshore industry. Almost two thirds of all leaks on offshore installations in the period 2001–2005, according to the Risk Level Project by the Petroleum Safety Authority in Norway, resulted from manual operations and interventions, as well as shut-down and start-up. The intention with the Risk OMT (risk modelling – integration of organisational, human and technical factors) program has been to develop more representative models for calculation of leak frequencies as a function of the volume of manual operations and interventions. In the Risk OMT project a generic risk model has been developed and is adapted to use for specific failure scenarios. The model considers the operational barriers in event trees and fault trees, as well as risk influencing factors that determine the basic event probabilities in the fault trees. The full model, which applies Bayesian belief networks, is presented more thoroughly in a separate paper. This paper presents the evaluation of the model. The model has been evaluated through some case studies, and one important aspect is the evaluation of the importance of each risk influencing factor. In addition some risk-reducing measures have been proposed, and the paper presents how the effect of these measures has been evaluated by using the model. Finally, possible applications and recommendations for further work are discussed.  相似文献

To ensure the safety of a process system, engineers use different methods to identify the potential hazards that may cause severe consequences. One of the most popular methods used is quantitative risk assessment (QRA) which quantifies the risk associated with a particular process activity. One of QRA's major disadvantages is its inability to update risk during the life of a process. As the process operates, abnormal events will result in incidents and near misses. These events are often called accident precursors. A conventional QRA process is unable to use the accident precursor information to revise the risk profile. To overcome this, a methodology has been proposed based on the work of Meel and Seider (2006). Similar to Meel and Seider (2006) work, this methodology uses Bayesian theory to update the likelihood of the event occurrence and also failure probability of the safety system. In this paper the proposed methodology is outlined and its application is demonstrated using a simple case study. First, potential accident scenarios are identified and represented in terms of an event tree, next, using the event tree and available failure data end-state probabilities are estimated. Subsequently, using the available accident precursor data, safety system failure likelihood and event tree end-state probabilities are revised. The methodology has been simulated using deterministic (point value) as well as probabilistic approach. This Methodology is applied to a case study demonstrating a storage tank containing highly hazardous chemicals. The comparison between conventional QRA and the results from dynamic failure assessment approach shows the significant deviation in system failure frequency throughout the life time of the process unit.  相似文献

Major hazards risk indicators are proposed for offshore installations, based on what has been used by the Petroleum Safety Authority Norway for the Risk Level approach in the Norwegian offshore petroleum industry. Since 2002 also leading indicators are used, in the sense that indicators for barrier performance are included together with the lagging indicators. There are individual indicators for a number of barrier functions, including fire and gas detection, a number of safety valves, active fire protection and mustering of personnel. The performance of the leading indicators during several years of data collection as well as lessons learned from the project are discussed. This is followed by a review of how indicators may be used by individual companies, and how the lessons may be utilised by individual companies to develop risk indicators for individual installations as well as company wide indicators.  相似文献

Like all hazardous installations, inherently safer design (ISD) is one of the key tools in offshore oil and gas projects to minimize risks in offshore facilities. As the life cycle of offshore facilities is relatively short compared with onshore counterparts and there are many projects running every year, the potential is high for raising inherent safety standards and lowering safety risks throughout the offshore industry as old facilities are phased out. This paper gives an overview of offshore facilities and examples of implementation of ISD. Good examples of ISD are numerous. Industry guidance on ISD implementation abound. Yet, the systematic implementation of it in the industry is patchy. There are many reasons for factors which impede the effective, efficient and consistent implementation of ISD in projects. This paper describes some of them and proposes solution to address them. They include (a) the effective integration of ISD into hazard management systems with appropriate language to engage all disciplines in projects, (b) the phasing of resources to enable the project to capture ISD measures which are only available during early phases, (c) application of appropriate ISD goals and ISD performance metrics at various stages and (d) the appropriate use of quantified risk assessment to support ISD.  相似文献

Quantitative risk assessment (QRA) is a powerful and popular technique to support risk-based decisions. Unfortunately, QRAs are often hampered by significant uncertainty in the frequency of failure estimation for physical assets. This uncertainty is largely due to lack of quality failure data in published sources. The failure data may be limited, incompatible and/or outdated. Consequently, there is a need for robust methods and tools that can incorporate all available information to facilitate reliability analysis of critical assets such as pipelines, pressure vessels, rotating equipment, etc. This paper presents a novel practical approach that can be used to help overcome data scarcity issues in reliability analysis. A Bayesian framework is implemented to cohesively integrate objective data with expert opinion with the aim toward deriving time to failure distributions for physical assets. The Analytic Hierarchy Process is utilized to aggregate time to failure estimates from multiple experts to minimize biases and address inconsistencies in their estimates. These estimates are summarized in the form of informative priors that are implemented in a Bayesian update procedure for the Weibull distribution. The flexibility of the proposed methodology allows for efficiently dealing with data limitations. Application of the proposed approach is illustrated using a case study.  相似文献

针对海上延期服役平台静设备的安全风险评估与控制问题,应用RBI风险评估技术的评估理论和方法对海上延期服役平台静设备的定量化安全管理方法进行了实践和探索,并选择某延期服役平台进行风险分析相关结论的工程验证。其具体过程是运用定量RBI技术,首先根据工艺流程和介质特点对海上平台静设备分别进行了物流回路和损伤回路的划分,并以此为基础对延期服役平台静设备的安全风险进行定量评估,进而得出各静设备单元的失效可能性、失效后果及风险等级,最后通过得出的损伤模式、损伤机理及对应的腐蚀速率,从而制定针对性的检测策略。将利用这些检验策略所检测出的真实情况与分析结论进行对比,最终验证了RBI技术在延期服役平台静设备风险评估方面具有很强的适用性。  相似文献
