危险化学品生产是当前的高危行业和安全管理的重点领域。面对事故频发的严峻形势,如何开展好危险化学品生产企业的安全管理,建立事故预防系统防止事故的发生非常关键。“纵深防御”原则作为核电厂实现核安全的一项基本原则,为核电厂长期稳定安全的发展奠定了坚实的基础,对危化企业提升事故预防能力,建立事故预防系统有较强的借鉴意义。借鉴“纵深防御”原则,文章分析危化企业应用“纵深防御”原则预防和减少事故发生的必要性,结合生产特点建立了事故纵深防御系统,对企业提高安全管理水平,预防事故的发生有重要的意义。  相似文献

Despite, the overwhelming amount of currently available safety management systems (and accompanying tools) accidents with hazardous substances still occur every now and then in the process industries.In this paper a large number of recently occurred accidents in the process industries are analysed showing that reoccurring disruptions during daily operation were present in the causal path of these accidents. The reoccurring disruptions can be seen as pre-warning signals. Their existence forms a gap with the common proactive safety indicators. This gap exists of information, already present and available in daily operation, of which it is UNKNOWN (to the local assessor) that it may lead to unsafe situations/accidents, e.g. maintenance backlogs, quality reports, etc.It is argued that these reoccurring events should be considered to be included in the safety indicators after assessment by an extended body of knowledge, because these events seem to become increasingly important regarding the prevention of accidents that still occur nowadays.  相似文献

针对零星危险化学品运输中的安全隐患问题,应用博弈理论构建了政府、快递企业、零星危险化学品托运人的三方博弈模型,对零星危险化学品安全运输问题进行了研究。在求得博弈模型均衡解的基础上,对均衡解进行了深入分析,研究结果表明:政府的最优检查概率与快递企业的安全管理成本正相关,与快递企业发生事故时的损失负相关;快递企业实施安全管理的概率与政府对快递企业的惩罚、快递企业发生事故时政府的损失正相关,与政府的检查成本、托运人选择托运且安全运输时的收益负相关;托运人选择托运的概率与快递企业实施安全管理的成本正相关。最后为有效杜绝零星危险化学品运输事故的发生提出了相应对策。  相似文献

为提升危化品事故应急救援能力,保障消防救援队员救援时安全,基于2015年天津港“8·12”瑞海公司危险品仓库特别重大火灾爆炸事故教训,从应急管理生命周期的角度构建危化品应急管理体系,研究危化品事故应急救援能力提升方法。结果表明:事故发生及特大伤亡与预防和应急准备工作不足有密切关系。构建基于应急管理生命周期的危化品企业、有关监管部门、技术服务机构、消防队联动应急管理体系是提升危化品应急救援能力的重要方法。危化品企业、有关监管部门、技术服务机构、消防队应做好危化品事故预防与应急准备工作,通过充足的预防和准备工作促使有效应急响应。事故发生后,危化品企业做好早期处置利于消防队采取针对性应急救援方案。对于复杂性危化品事故,及时成立抢险救援指挥部,科学应急响应,可保障消防救援的安全有效。  相似文献

说明目前乡镇工业企业中职业安全卫生问题十分突出。据调查,厂矿工伤事故发生率为8.23%,每千名职工的工伤发生率为2.66人,每十万名职工的工伤死亡率为7.02人。通过对事故的原因进行分析,提出了加强监督管理的建议。  相似文献

This article studies organizational assessment in complex sociotechnical systems. There is a practical need to monitor, anticipate and manage the safety and effectiveness of these systems. A failure to do so has resulted in various organizational accidents. Many theories of accidents and safety in industrial organizations are either based on a static and rational model of an organization or they are non-contextual. They are thus reactive in their search for errors and analysis of previous accidents and incidents, or they are disconnected from the actual work in the organization by their focus on general safety attitudes and values. A more proactive and predictive approach is needed, that is based on an accurate view on an organization and the demands of the work in question. This article presents and elaborates four statements: (1) the current models of safety management are largely based on either a rational or a non-contextual image of an organization, (2) complex sociotechnical systems are socially constructed and dynamic cultures, (3) in order to be able to assess complex sociotechnical systems an understanding of the organizational core task is required, and (4) effectiveness and safety depend on the cultural conceptions of the organizational core task. Finally, we will discuss the implications of the proposed concepts for safety research and development work in complex sociotechnical systems.  相似文献

A significant gap exists between accident scenarios as foreseen by company safety management systems and actual scenarios observed in major accidents.The mere fact that this gap exists is pointing at flawed risk assessments, is leaving hazards unmitigated, threatening worker safety, putting the environment at risk and endangering company continuity. This scoping review gathers perspectives reported in scientific literature about how to address these problems.Safety managers and regulators, attempting to reduce and eventually close this gap, not only encounter the pitfalls of poor safety studies, but also the acceptance of ‘unknown risk’ as a phenomenon, companies being numbed by inadequate process safety indicators, unsettled debates between paradigms on improving process safety, and inflexible recording systems in a dynamic industrial environment.The immediacy of the stagnating long term downward major accident rate trend in the Netherlands underlines the need to address these pitfalls. A method to identify and systematically reduce unknown risks is proposed. The main conclusion is that safety management can never be ready with hazard identification and risk assessment.  相似文献

为遏制危险化学品重特大安全事故,解决传统安全监测手段的空间局限性问题,结合高分遥感与数值模拟技术,开展危险化学品重大危险源区域安全布局动态管控研究。利用数值模拟技术模拟并确定危险化学品重大危险源重点管控范围;基于高分遥感影像提出适用于危险化学品重大危险源安全布局动态管控的变化检测方法,动态监测安全防护范围内的建筑、道路等人工构筑物的变化情况,严控事故影响范围内的人工构筑物密度;以内江市隆桥化工为实例开展示范应用。结果表明:高分遥感技术能够高效监测危险化学品重大危险源安全防护范围内的工程建设情况,有助于危险化学品企业保持足够的外部安全防护距离,降低危险化学品重特大安全事故风险。  相似文献

统计分析了2008年1月~2010年5月我国发生的485起危险化学品公路运输事故。从事故发生的原因、事故涉及的化学品、事故造成的危害、事故发生的月份分布及年份变化等几个方面,分析了近年来危险化学品公路运输事故的新特点及变化规律。经统计分析,道路交通事故是引发危险化学品运输事故的主要原因之一;侧翻是危险化学品车辆最容易发生的道路交通事故;而随着我国高速公路的迅猛发展,追尾造成的危险化学品运输事故数量呈上升趋势;危险化学品公路运输过程中易燃液体事故起数最多,爆炸品和毒性物质事故造成的人员伤亡最严重;春节前后取代夏季,成为近两年危险化学品运输事故高发期。针对这些特点,对我国危险化学品公路运输安全管理与监控提出了建议。  相似文献

通过对国内城市轨道交通工程建设期间典型事故的统计分析,总结城市轨道交通工程建设期间可能存在的危险有害因素。分析结果表明,城市轨道交通工程建设期间事故多发的主要原因有:不同施工工法引起的地层坍塌和结构物破坏;不良地质体诱发的安全事故;地下管线引起的安全事故;沿线临近建筑、构筑物引起的安全事故;施工管理不当造成的安全事故等。  相似文献

为提高危险化学品安全管理水平,收集我国134起以人为主因的危险化学品事故,构建危险化学品事故的HFACS-BN模型,并基于贝叶斯网络对危险化学品事故中的人因路径及各因素灵敏度进行分析。研究结果表明:行为违规是导致事故的最主要不安全行为;在复杂路径中,环境因素和操作者状态具有较高的灵敏度,与其相关的风险认知与处理不当是阻碍事故预防的关键因素;在非复杂路径中,操作者状态与运行计划不当具有较高的灵敏度,其所涉及的监督违规、组织过程和组织氛围,是导致危险化学品事故的根本起点,并可依此分析事故直接原因和间接原因;操作者状态在2种路径中均表现出高灵敏度,因此化工企业在人员管理上要加强关注职工状态,减少不安全行为的出现。  相似文献

对安全评价方法在危险废物处置建设项目环境风险评价中的运用进行初步探索。主要用"工艺过程风险因素分析表"对工艺过程潜在风险性识别;用蒙德法进行源项分析;用池火灾模型、蒸气云爆炸伤害模型对易燃、易爆物质的火灾、爆炸等重大事故后果进行计算,得出人员的伤亡半径和财产损失半径等参数,以便于判断风险的可接受水平。分析结果表明:采用安全评价方法对危险废物处置建设项目进行环境风险评价是适用的、可行的。  相似文献

A number of chemical accidents have occurred in China over the past two decades with significant impact on humans and the environment. It is expected that lessons will have been learned from these accidents that will help industries to reduce the risk that catastrophic chemical accidents occur in future. In fact, to some extent there is evidence that lessons have been learned, to the extent that the Chinese government has substantially strengthened legislation and regulatory standards. Nonetheless, there remains a concern that much more still needs to be done to reduce chemical accidents risks in China. Important progress in this area requires not only government support but a commitment across all hazardous industries to learn from past accidents that may in many cases require establishment or considerable improvement of their safety management systems. To assist small and medium-sized enterprises (SMEs), in this effort, results of an analysis of common causes of the chemical accidents reported in the Major Accident Information (MAI) website of Chinese State Administration of Work Safety (SAWS) are presented in this paper In particular, inadequate process hazard analysis (PHA), training and emergency response planning (ERP) were identified as the top three process safety management (PSM) elements that contribute to most of the SMEs accidents in China. Seven recommendations are proposed in order to improve the effectiveness of lesson learning for government agencies and SMEs.  相似文献

Statistical analysis of hazardous material accidents (HMAs) in China from 20013 to 2018 related to characteristics and consequences is discussed in this study, revealing different time volatility, spatial distribution and accident consequence features. Yearly, monthly, weekly and hourly levels of HMAs are analysed, and the corresponding factors are discussed. The trends of HMAs are affected by different factors, such as efforts toward safety-specific rectification by the government, holidays, five-day workweek systems and daily traffic peaks. The spatial distribution trends indicate that highway transportation of hazardous materials is mostly short-distance transport. The accidents that occurred on normal road sections account for 82.76% of the total HMAs during transportation. Leaking accidents caused by HMAs account for 79.35% of the total accidents. Countermeasures focusing on improving highway transportation safety for hazardous materials are recommended.  相似文献

The photoelectric, semiconductor and other high-tech industries are Taiwan's most important economic activities. High-tech plant incidents are caused by hazardous energy, even when that energy is confined to the inside of the process machine. During daily maintenance procedures, overhauling or troubleshooting, engineers entering the interior of the machines are in direct contact with the source of the energy or hazardous substances, which can cause serious injury. The best method for preventing such incidents is to use inherently safer design strategies (ISDs); this approach can fully eliminate the dangers from the sources of hazardous energy at a facility.This study first conducts a lithography process hazard analysis and compiles a statistical analysis of the causes of the fires and losses at high-tech plants in Taiwan since 1996, the aim being to establish the necessary improvement measures by using the Fire Dynamics Simulation (FDS) to solve relevant problems. The researchers also investigate the lithography process machine in order to explore carriage improvement measures, and analyse the fires' causes and reactive materials hazardous properties, from 1996 to 2012. The effective improvement measures are established based on the accident statistics. The study site is a 300 mm wafer fabrication plant located in Hsinchu Science Park, Taiwan.After the completion of the annual maintenance jobs improvement from September 2011 to December 2012, the number of lithography process accidents was reduced from 6 to 1. The accident rate was significantly reduced and there were no staff time losses for a continuous 6882 h. It is confirmed that the plant safety level has been effectively enhanced. The researchers offer safety design recommendations regarding transport process appliances, chemical storage tanks, fume cupboard devices, chemical rooms, pumping equipment, transportation pipelines, valve manual box (VMB) process machines and liquid waste discharge lines. These recommendations can be applied in these industries to enhance the safety level of high-tech plants, facilities or process systems.  相似文献
