This work was developed with the support of MEMC, one of the most important producers of ultra-pure silicon wafers for electronic applications throughout the world. The availability of ultra-pure water is of prime importance in the silicon production process. In order to maximize the availability of UP water, MEMC has developed a preventive maintenance program and a detailed record of each maintenance intervention is recorded. This has allowed a complete failure rate data bank to be developed. In order to optimize the maintenance intervention, a Recursive Operability Analysis (ROA) has been used as a decision support tool. The results of the ROA, coupled with the failure rates, have made it possible to calculate the expected number of events (ENE) of various top events (TEs). The magnitude of each TE has been estimated on the basis of the monetary losses provoked by each event. The risk then has been calculated and the events ranked on this basis. Maintenance policies have been optimized with the aim of reducing the risk of the top ranked events.  相似文献

Since gas plants are progressively increasing near urban areas, a comprehensive tool to plan maintenance and reduce the risk arising from their operations is required. To this end, a comparison of three Risk-Based Maintenance methodologies able to point out maintenance priorities for the most critical components, is presented in this paper. Moreover, while the literature is mostly focused on probabilistic analysis, a particular attention is directed towards consequence analysis throughout this study. The first developed technique is characterized by a Hierarchical Bayesian Network to perform the occurrence analysis and a Failure Modes, Effects and Criticality Analysis to assess the magnitude of the adverse outcomes. The second approach is a Quantitative Risk Analysis carried out via a software named Safeti. Finally, another software called Synergi Plant is adopted for the third methodology, which provides a Risk-Based Inspection plan, through a semiquantitative risk analysis. The proposed study can assist asset manager in adopting the most appropriate methodology to their context, while highlighting priority components. To demonstrate the applicability of the approaches and compare their rankings, a Natural Gas Regulating and Measuring Station is considered as case study. The results showed that the most suited method strongly depends on the available data.  相似文献

在HAZOP原理的基础上,讨论了计算机辅助HAZOP技术的优势,并针对传统的HAZOP、基于深层知识模型SDG的计算机辅助HAZOP和应用PHA-Pro软件3种方法进行了分析和比较;介绍了研发成功的针对开、停车过程和应急阶段顺序颠倒、操作步骤遗漏的人工误操作危险与可操作性分析系统MO-HAZOP;该系统具有定量计算出所有的人工误操作顺序组合的发生概率值,结合现有知识和专家经验,判定事件发生的风险等级,从而有重点地给出预防危险、保障生产安全的建议的功能;MO-HAZOP分析系统对于石化企业的安全生产具有重要意义。  相似文献

Devastating vapour cloud explosions can only develop under appropriate (boundary) conditions. The record of vapour cloud explosion incidents from the past demonstrates that these conditions are readily met by the congestion by process equipment at (petro-) chemical plant sites. Therefore, the possibility of an accidental release of a flammable and a subsequent vapour cloud explosion is a major hazardous scenario considered in any risk assessment with regard to the process industries.If an extended flammable vapour cloud at a chemical plant site extends over more than one process unit, which are separated by lanes of sufficient width, the vapour cloud explosion on ignition develops the same number of separate blasts. If, on the other hand, the separation between the units is insufficient, the vapour cloud explosion develops one big blast. The critical separation distance (SD) is the criterion that allows discriminating in this matter for blast modelling purposes.This paper summarises some major results of an experimental research programme with the objective to develop practical guidelines with regard to the critical SD. To this end, a series of small-scale explosion experiments have been performed with vapour clouds containing two separate configurations of obstacles. Blast overpressures at various stations around have been recorded while the SD between the two configurations of obstacles was varied.The experimental programme resulted in some clear indications for the extent of the critical SD between separate areas of congestion. On the basis of safety and conservatism, these indications have been rendered into a concrete guideline. Application of this guideline would allow a greater accuracy in the modelling of blast from vapour cloud explosions.  相似文献

Natech accidents at industrial plants are an emerging risk with possibly serious consequences. For the mitigation of natech risk, authorities need to identify natech prone areas in a systematic manner. In order to facilitate probabilistic natech risk mapping, a unified methodology was developed that is based on the estimation of on-site natural hazard parameters, determination of damage probabilities of plant units, and assessment of probability and severity of possibly triggered natech events. The methodology was implemented as an on-line, extensible risk assessment and mapping software framework called RAPID-N, which allows rapid local and regional natech risk assessment and mapping with minimal data input. RAPID-N features an innovative data estimation framework to complete missing input data, such as on-site natural hazard parameters and plant unit characteristics. The framework is also used for damage assessment and natech consequence analysis, and allows easy modification of input parameters, dynamic generation of consequence models according to data availability, and extension of models by adding new equations or substituting existing ones with alternatives. Results are presented as summary reports and interactive risk maps, which can be used for land-use and emergency planning purposes by using scenario hazards, or for rapid natech consequence assessment following actual disasters. As proof of concept, the framework provides a custom implementation of the U.S. EPA's RMP Guidance for Offsite Consequence Analysis methodology to perform natech consequence analysis and includes comprehensive data for earthquakes. It is readily extendible to other natural hazards and more comprehensive risk assessment methods.  相似文献

The growing demand for natural gas has pushed oil and gas exploration to more isolated and previously untapped regions around the world where construction of LNG processing plants is not always a viable option. The development of FLNG will allow floating plants to be positioned in remote offshore areas and subsequently produce, liquefy, store and offload LNG in the one position. The offloading process from an FLNG platform to a gas tanker can be a high risk operation. It consists of LNG being transferred, in hostile environments, through loading arms or flexible cryogenic hoses into a carrier which then transports the LNG to onshore facilities. During the carrier's offloading process at onshore terminals, it again involves risk that may result in an accident such as collision, leakage and/or grounding. It is therefore critical to assess and monitor all risks associated with the offloading operation. This study is aimed at developing a novel methodology using Bayesian Network (BN) to conduct the dynamic safety analysis for the offloading process of an LNG carrier. It investigates different risk factors associated with LNG offloading procedures in order to predict the probability of undesirable accidents. Dynamic failure assessment using Bayesian theory can estimate the likelihood of the occurrence of an event. It can also estimate the failure probability of the safety system and thereby develop a dynamic failure assessment tool for the offloading process at a particular FLNG plant. The main objectives of this paper are: to understand the LNG offloading process, to identify hazardous events during offloading operation, and to perform failure analysis (modelling) of critical accidents and/or events. Most importantly, it is to evaluate and compare risks. A sensitivity analysis has been performed to validate the risk models and to study the behaviour of the most influential factors. The results have indicated that collision is the most probable accident to occur during the offloading process of an LNG carrier at berth, which may have catastrophic consequences.  相似文献

The significance of accidental releases has increased during the past few years. The reason for this is that the level of continuous process emission has gone down, partly due to more stringent environmental regulations and partly due to technical improvements. The aim of the study was to minimize accidental releases from a sulphate pulp mill. Some well-known methods of risk analysis were used as a tool. This article presents the application of risk analysis to reduce accidental releases of concentrated malodorous gases from a sulphate pulp mill. This particular objective was chosen not only because of the inconvenience caused by the smell but also the possible health hazards which have been of growing concern recently. In this study, risk analysis has proved to be a recommendable tool in environmental protection of sulphate pulp mills. However, only thorough advance planning before analysis begins can guarantee the most efficient and beneficial results of risk analysis. The aim of the examination and the resources available have an effect on the selection of the method and also on the level of examination.  相似文献

通过概括介绍危险与可操作性分析(HAZOP)、保护层分析(LOPA)和安全完整性等级分析(SIL)三种方法的特点,总结三种分析方法之间的关系.LOPA分析是HAZOP分析的继续,可以解决HAZOP分析中残余风险不能定量化的不足,是对HAZOP分析结果的丰富和补充;SIL分析则在LOPA分析的基础上,进一步对需要增加的安全仪表系统(SIS)进行设计,并对LOPA分析结果进行验证,即HAZOP、LOPA分析是SIL分析的前期准备工作.因此,在详细介绍SIS的组成、安全生命周期阶段、SIL的选择确定方法以及SIL分析流程之前,也简要介绍了HAZOP、LOPA分析方法,梳理了两种方法的分析流程.最后通过引入示例来展示三种分析方法之间的关系.  相似文献

Some recent accidents involving the bioenergy production and feedstock supply chain raised concern on the safety of such technologies. A survey of major accidents related to the production of bioenergy (intended as biomass, bioliquids/biofuels and biogas) was carried out, and a data repository was built, based on past accident reports available in the open literature and in specific databases. Data analysis shows that major accidents are increasing in recent years and their number is growing faster than bioenergy production. The results obtained represent an early warning concerning the major accident hazard of bioenergies, and suggest the importance of risk awareness and safety culture in bioenergy production, in the perspective of a safe and sustainable exploitation of renewable resources.  相似文献

Automated controlled systems are vulnerable to faults. Faults can be amplified by the closed loop control systems and they can develop into malfunction of the loop. A control loop failure will easily cause production stop or malfunction at a petrochemical plant. A way to achieve a stable and effective automated system is to enhance equipment dependability. This paper presents a standard methodology for the analysis and improvement of pump performance to enhance total operational effectiveness and stability in offshore industry based on dependability. Furthermore, it is shown how a reliability–safety analysis can be conducted through equipment dependability indicators to facilitate the mitigation of hazard frequency in a plant. The main idea is to employ principle component analysis (PCA) and importance analysis (IA) to provide insight on the pumps performance. The pumps of offshore industries are considered according to OREDA classification. The approach identifies the critical pump and their fault through which the major hazards could initiate in the process. At first PCA is used for assessing the performance of the pumps and ranking them. IA is then performed for the worst pump which could have most impact on the overall system effectiveness to classify their components based on the component criticality measures (CCM). The analysis of the classified components can ferret out the leading causes and common-cause events to pave a way toward improving pump performance through design optimization and online fault detection which ultimately enhance overall operational effectiveness.  相似文献

Global demand for natural gas may double by 2030, with Liquefied Natural Gas (LNG) growing perhaps fivefold - driven by continued cost reduction. Propane Pre-cooled Mixed Refrigerant (PPMR) of Air Products and Chemicals Inc. (APCI) currently holds 78% of the liquefaction plants on the market. Since the PPMR process has the largest production capacity, its operational reliability needs to be high as any failure may result in catastrophic consequences. Therefore, operational reliability has a key importance in LNG plants. To achieve an acceptable reliability, usually, a constant interval of a preventive maintenance method is used. The limitation of such a method is that the shutdown strategy is not optimum. This study focuses on determining a risk-based shutdown management strategy for APCI plants with capacity of 4.5 million tons per annum. To achieve the minimum risk for the expected life of an LNG plant, a combination of preventive maintenance, active redundancy and standby redundancy is considered. Results of this study reveal that such a combination can significantly reduce the operational risk. This combination improves the plant reliability and maintains it above a minimum operational reliability.  相似文献

根据我国法规、标准与美国核管委对核电厂核事故应急疏散的要求,考虑我国核电厂特殊的厂址条件与社会环境,分析核事故应急疏散的可行性。通过应急疏散行为研究、厂区的交通需求估计和路网分析,对不同情境,考虑核事故应急疏散的特殊问题,分别采用人工容量分析与宏观仿真模拟得到应急疏散时间。针对某核电厂,给出核事故应急疏散分析的一般步骤、方法与计算结果。对宏观仿真模型的主要参数进行灵敏度分析,找出影响应急疏散时间的主要因素,提出缩短应急疏散时间的针对性措施。  相似文献

加氢装置属甲类火灾、爆炸危险生产装置。为了在设计阶段尽可能消除或控制潜在风险,本文总结了多套加氢装置HAZOP分析报告中的设备类别及其分析内容,提出了基于危险与可操作性(HAZOP)分析的加氢装置工程设计方法。在传统工程设计方法的基础上增加了参数敏感性工程设计方法,依据分析报告中的设计建议,利用ASPEN软件计算过程参数变化对目标参数的影响程度,确定参数稳定操作区域;建立了数据库管理界面实现了加氢装置工程安全设计经验知识的有序管理。应用基于HAZOP分析的加氢装置工程设计方法,有助于将安全隐患问题在设计阶段消除或加以控制,可为降低石化装置改造成本和提高装置的安全水平提供方法依据。  相似文献

In this work we present a method for risk-informed decision-making in the physical asset management context whereby risk evaluation and cost-benefit analysis are considered in a common framework. The methodology uses quantitative risk measures to prioritize projects based on a combination of risk tolerance criteria, cost-benefit analysis and uncertainty reduction metrics. There is a need in the risk and asset management literature for a unified framework through which quantitative risk can be evaluated against tolerability criteria and trade-off decisions can be made between risk treatment options. The methodology uses quantitative risk measures for loss of life, loss of production and loss of property. A risk matrix is used to classify risk as intolerable, As Low As Reasonably Practicable (ALARP) or broadly tolerable. Risks in the intolerable and ALARP region require risk treatment, and risk treatment options are generated. Risk reduction benefit of the treatment options is quantified, and cost-benefit analysis is performed using discounted cashflow analysis. The Analytic Hierarchy Process is used to derive weights for prioritization criteria based on decision-maker preferences. The weights, along with prioritization criteria for risk reduction, tolerance criteria and project cost, are used to prioritize projects using the Technique for Order Preference by Similarity to Ideal Solution. The usefulness of the methodology for improved decision-making is illustrated using a numerical example.  相似文献

综述了微藻培养技术研究进展,并分析了沼液作为资源制备微藻生物质可行性,提出了沼液资源化利用与高价值生物质生产耦合工艺技术系统。结果表明,沼液预处理技术、目标藻种筛选技术、微藻规模化培养技术和藻渣高值利用技术是耦合系统的四大关键技术。基于微藻培养技术,该耦合系统促使沼液处理系统从"处理工艺"向"生产工艺"转化,实现了多元功能耦合,即沼液除磷脱氮耦合氮磷回收,沼液净化处理耦合生物能源生产以及CO2的固定耦合沼气净化。  相似文献

对传统的HAZOP分析中偏差原因发生可能性进行量化。对于有统计数据的,根据行业数据、公司经验及企业事故建立HAZOP风险分析统计数据库;对于没有统计数据的HAZOP分析偏差原因发生概率,通过专家主观评判,用模糊数理论将专家自然语言转换为模糊数,采用左右模糊排序法将模糊数转换为模糊失效概率值。研究了偏差后果严重程度的划分标准,并根据偏差原因概率和偏差后果严重程度确定风险等级,利用风险矩阵得出偏差风险的大小。从而把HAZOP分析方法从定性改进为半定量的分析方法。据此对石油化工装置进行了HAZOP风险分析。  相似文献

Fire is the most prevalent accident in natural gas facilities. In order to assess the risk of fire in a gas processing plant, a fault tree analysis (FTA) and event tree analysis (ETA) has been developed in this paper. By utilizing FTA and ETA, the paths leading to an outcome event would be visually demonstrated. The framework was applied to a case study of processing plant in South Pars gas complex. All major underlying causes of fire accident in a gas processing facility determined through a process hazard analysis (PHA). Fuzzy logic has been employed to derive likelihood of basic events in FTA from uncertain opinion of experts. The outcome events in event tree has been simulated by computer model to evaluate their severity. In the proposed methodology the calculated risk has the unit of cost per year which allows the decision makers to discern the benefit of their investment in safety measures and risk mitigation.  相似文献

The inherent risks associated with accidental releases of hazardous materials during transport have drawn attention and concerns in the recent decades. The aim of this study is to propose a tool for evaluation and comparison of the transportation networks which can be used to assess the routing options between origins and destinations of the cargos for their suitability for transporting hazardous material cargos by tanker trucks and to identify routes which provide lower accidental release risks, lower public exposure risks, and offer economical benefits. Each route segment of transportation networks were evaluated using specific criteria which included health risk and cost of delay in case of an accidental release of materials, trucking cost and proximity to vulnerable areas. Since, the health impact of hazardous materials differ depending on the characteristics of the material being transported as well as release quantities and atmospheric conditions; this paper aimed in providing a tool that can be used to estimate the impact radius (for health risks) after accidental release of hazardous materials by taking into account different atmospheric conditions based on the meteorological data and solar elevation angle. The Gaussian air dispersion model paired with ArcGIS using Python programming were employed to estimate the health risk impact zones by considering the meteorological data, and accordingly to analyze road segments for cost impacts (delay and trucking costs), and the proximity to vulnerable areas. The route assessment tool was demonstrated with a case study. The results of this study can efficiently aid decision makers for transportation of hazardous materials.  相似文献
