The Health and Safety Executive (HSE) has considerable interest in predicting the consequence of releases of hazardous liquids from storage tanks (HSE, 1998), because of its responsibilities as a statutory adviser to local planning authorities on the siting of hazardous installations and on the control of development in their vicinity, and because of its responsibilities alongside the Environment Agencies as the Competent Authority in the assessment of COMAH safety reports. As a result, HSE has commissioned a number of studies, including: experimental studies of the behavior of liquid spills (Lee and Kountouris, 1992, Skitt and Wheeler, 1989, Bentinck and Crow, 1991, Clark and Savery, 1993, Law and Johnskareng, 1994, Ruddle and Widowson, 1985, Ruddle and Elms, 1985, Cleaver, Cronin, Evans and Hirst, 2001), a review of overtopping data and theoretical models used to study liquid flow (Thyer & Jagger, 1997), the characterization of the profile of the leading edge of the spreading liquid (Thyer, MacMillan, & Jagger, 1999), and the development and use of complex mathematical models to predict overtopping fractions and liquid spill behavior (Daish et al., 1998, Ivings and Webber, 2001).This paper summarizes the consequences of catastrophic tank failure, the current position on the availability of liquid spill data for model validation, and indicates how such data may be used to predict the often considerable fraction of liquid that could overtop retaining bunds around real tanks.  相似文献

Gas refineries have been continuously focusing on Health, Safety and Environment programs to improve maintenance activities. Several researches have studied on this area with different analysis methods. This study presents an integrated approach for optimization of factors contributing to the implementation of Health, Safety and Environment (HSE) in maintenance activities. HSE managers in each sector answered standard questionnaire whit respect to HES. The methodology is based on fuzzy data envelopment analysis (FDEA) and Deming's continuous improvement cycle. Also, this method is used to rank the relevant performance efficiencies in certain and uncertain conditions of each HSE sectors whit considering HSE in maintenance activities. It corresponds and integrates its registered HSE-MS with OHSAS 18001:2007 and ISO 14001:2004 to evaluate multiple inputs and outputs of over 36 subsidiary HSE divisions with parallel mission and objectives simultaneously. Also, it determines efficient target indices and could assure continuous improvement in the organization. This is the first study that introduces an integrated approach to improve HSE management programs in a gas refinery by a robust and continuous improvement approach.  相似文献

This paper describes part of a programme of work undertaken at the Health and Safety Laboratory (HSL) to investigate the behaviour of selected water-reactive chemicals. Following an accidental release, such substances react exothermically with any water present, generating acidic vapours. The STAWaRS (Source Term Assessment of Water Reactive Substances) software was developed for the Health and Safety Executive (HSE) by ESR Technology to model this complex process. The aims of the study described here were to provide experimental validation of the heats of hydrolysis used within STAWaRS, and to perform sensitivity studies on selected STAWaRS input parameters.The heat of hydrolysis of acetyl chloride was measured and showed good correlation with the value used within STAWaRS. Some of the variables that influence the severity of acetyl chloride spills are examined, with reference to predictions made by the STAWaRS model. The heats of hydrolysis of titanium tetrachloride previously measured at HSL are also discussed, and the effect of adopting these experimentally derived values for modelling spills is shown for a hypothetical land use planning case. This study demonstrates the importance of using experimentally validated values for STAWaRS input parameters.  相似文献

结合实际工程建设项目开展危险源辨识(HAZID)分析的情况,介绍了HAZID分析方法的广义和狭义概念内涵,主要分析步骤、每步主要工作内容、典型引导词、分析形式和特点,并将HA-ZID方法与常用的过程危险源方法、尤其是HAZOP分析方法进行了对比,以帮助读者更好地理解、运用此方法。本文拟推荐的HAZID分析方法将危险化学品行业的风险管理向前延伸到项目的早期阶段(如项目论证、可行性研究、基础工程设计或初步设计阶段),可以帮助项目组在项目建设早期认识与项目有关的健康、安全、环境(HSE)各方面问题的重要性和相关性,所辨识出的关键危险源及其危险,为下一步有重点、有目的地开展项目HSE风险管理确定了关注重点。在早期阶段进行HAZID分析,还可以有机会以最小的代价实现对项目厂址和总图布置的调整、工艺方案和设计原则的优化,可以避免后期再发现HSE隐患、必须整改时可能存在的不可实施性,或对项目费用、进度等产生严重负面影响。  相似文献

澳洲能源公司在生产、经营的全过程中非常重视健康、安全及环境(HSE)工作,HSE管理体系得到了广泛的推广和应用。本文介绍了必和必拓、加德士等公司的可持续体系、HSE规划,论述了澳洲能源公司的减碳计划、碳排放公开、节能项目的落实及安全与风险管理等具体措施。分析了HSE管理体系的重要作用以及我国能源企业所面临的问题,包括安全事故多发、环境保护政策的落实不够充分、人员健康意识有待提高等。以中国国家电网公司、中国石油天然气集团公司及中国神华集团有限责任公司为例,分析其HSE体系,并从规范HSE建设、推动HSE可持续发展及责任落实等方面借鉴澳洲先进经验,提出明确我国HSE发展方向、战略,稳固HSE根基,完善我国能源企业HSE政策,保证体制全面、协调运转的具体建议。  相似文献

The aim of this study is to examine and compare all publicly accessible data on occupational injuries in Slovenia in the period from 1948 to 2008, obtained either from archives or websites. The data obtained from five different sources which have been formally collecting and presenting data on occupational injuries in Slovenia (Labour Inspectorate of the Republic of Slovenia – LIRS, Institute of Public Health of the Republic of Slovenia – IPHRS, International Labour Organization – ILO, Statistical Office of the Republic of Slovenia – SORS, and issues of the Journal of Occupational Safety and Fire Safety) were reviewed and managed. Since the early 1960s, there has been a significant decrease of fatal and non-fatal occupational injuries in Slovenia. Problems related to gathering and interpreting data on occupational injuries are also discussed with special emphasize on diversity of models used for occupational injuries data collecting. The results of the 60-year-long retrospective study can also be of assistance for professionals involved in planning, implementing and controlling the national policy and/or regional policies on safety and health at work.  相似文献

In this study, an integrated “Health, Safety, and Environment (HSE)” and maintenance systems are presented. Multivariate analysis is used for continuous performance assessment and improvement of these systems. The two subjects of “Maintenance System” and “HSE” have been individually investigated several times in different studies. However, few studies have been done to integrate these two systems and provide an integrated system for their implementation. This study evaluates current maintenance and HSE systems of a Gas Transmission Unit by Data Envelopment Analysis (DEA) and Principal Component Analysis (PCA). Moreover, decision making units (DMUs) are examined and ranked. Employees are considered as DMUs. This is done through investigating and measuring their efficiencies and identifying the inefficient and less-efficient units. Since an increase in the number of inputs does not lead to an increase in the number of outputs with the same scale, an output-oriented DEA with a Variable Returns to Scale (VRS) is used. The Fuzzy DEA (FDEA) is also used in this research to decrease uncertainty existing in qualitative indicators and human error. Finally, suggestions are given to improve those DMUs. The managers and employees of the gas transmission unit constitute the statistical population of the study. To achieve the objectives of this study, standard questionnaires with respect to HSE and Maintenance system are completed by operators. The proposed approach would help policy makers and top managers of Gas Transmission Company to have a more comprehensive and thorough understanding the working conditions with respect to the maintenance and HSE features.  相似文献

Performance evaluation of Health, Safety and Environment (HSE) is the measurement of a company's achievement in HSE management. In order to receive a comprehensive and objective evaluation result, it is necessary to consider all evaluation factors and experts at different levels when HSE performance assessment is conducted. To improve conventional HSE performance evaluation, where weighted average method was used, a Fuzzy Comprehensive Evaluation (FCE) method is used in this study by taking experts' weights into account. Further, an HSE operating performance assessment system is designed to simplify manual and complex assessment process and generate charts and analysis reports automatically. Finally, a case of petrochemical enterprise is used to illustrate the effectiveness of the method and system.  相似文献

In Great Britain, advice on land-use planning decisions in the vicinity of major hazard sites and pipelines is provided to Local Planning Authorities by the Health and Safety Executive (HSE), based on quantified risk assessments of the risks to the public in the event of an accidental release. For potential exposures to toxic substances, the hazard and risk is estimated by HSE on the basis of a “toxic load”. For carbon dioxide (CO2), this is calculated from the time-integral of the gas concentration to the power eight. As a consequence of this highly non-linear dependence of the toxic load on the concentration, turbulent concentration fluctuations that occur naturally in jets or plumes of CO2 may have a significant effect on the calculated hazard ranges. Most dispersion models used for QRA only provide estimates of the time- or ensemble-averaged concentrations. If only mean concentrations are used to calculate the toxic load, and the effects of concentration fluctuations are ignored, there is a danger that toxic loads and hence hazard ranges will be significantly under-estimated.This paper explores a simple and pragmatic modification to the calculation procedure for CO2 toxic load calculations. It involves the assumption that the concentration fluctuates by a factor of two with a prescribed square-wave variation over time. To assess the validity of this methodology, two simple characteristic flows are analysed: the free jet and the dense plume (or gravity current). In the former case, an empirical model is used to show that the factor-of-two approach provides conservative estimates of the hazard range. In the latter case, a survey of the literature indicates that there is at present insufficient information to come to any definite conclusions.Recommendations are provided for future work to investigate the concentration fluctuation behaviour in dense CO2 plumes. This includes further analysis of existing dense gas dispersion data, measurements of concentration fluctuations in ongoing large-scale CO2 release experiments, and numerical simulations.  相似文献

A study of a small sample of construction fatal accidents was used to pilot a potential method of analysis for the UK Health and Safety Executive (HSE) Construction Division. The work was conducted in the context of a Governmental Inquiry into construction fatalities in 2009, but has been developed further since. The sample of 26 accidents (28 fatalities) was drawn from the 211 fatal accidents in the years ending 2006–2008, to be broadly illustrative of the range of accident characteristics. The accidents were analysed on the basis of available inspectorate reports and structured interviews with the investigating inspectors. A standard method of classification on four levels was developed, based on the Human Factors Analysis and Classification System (HFACS) classification of errors and task level factors, with additional categories covering the organisational and regulatory/market levels of the system. The results showed a concentration of underlying factors associated with inadequacies in planning and risk assessment, competence assurance, hardware design, purchase and installation, and contracting strategy. These findings were partially validated by comparing them with another sample of 50 accidents analysed earlier by the HSE. This paper describes the development and testing of the investigation and classification method and how it is being further developed since the initial study. It also provides a summary of the findings relating to underlying causes.  相似文献

Most occupational safety and health (OSH) professionals would agree that having a good OSH promotion programme is a challenge, and problems can be encountered at almost every stage in the planning and delivery of the programme.The first obstacle which may be encountered is programme justification. How does one convince employers and employees of the need for OSH promotion, and does competition from “Wellness Programmes” reduce available resources? Programme planning, deciding what constitutes a programme, and what to include in the OSH promotion programme, may be a dilemma. The programme implementation, deciding who implements the activities and how a programme is sustained, may be other difficult issues. Finally, programme evaluation can be problematic: How is it done properly? and Who should evaluate the programme?Possible solutions to these problems may include the costing of work accidents and occupational ill health to justify the need and benefits of an OSH programme, and riding on the bandwagon of “wellness promotion” instead of competition for limited resources. In programme planning, knowing what is wanted and employing a professional to plan the programme is essential. The programme coverage should encompass health and safety issues, and preventive strategies should be directed towards both environment and lifestyle factors. There should be a prioritization of topics according to the needs of workers. The choice of OSH professionals versus “wellness promoters” or human resource officers as programme co-ordinators may affect the programme outcome. Training supervisors and line workers to be trainers would involve those directly in contact with work hazards in the programme. Programme evaluation is crucial. There should be provision for both process and outcome evaluations, and pains should be taken to ensure independence and objectivity of evaluations. The idea of linking programme results to the OSH personnel assessments is worth considering.The delivery of OSH promotion programmes in a relevant and effective manner poses a real challenge. While some obstacles and problems appear to be insurmountable, these challenges have to be faced and overcome in order to achieve the goals of protecting and promoting the health of the worker.  相似文献

PROBLEM: Accidents involving temporary access systems, particularly temporary scaffolds, account for a large proportion of injuries in the construction industry worldwide. METHOD: This paper outlines the development of a prototype decision aid (SCAFPASS, an abbreviation for Scaffold Planning Aid for System Safety) to promote access scaffold safety. It was guided by an examination of the root-causes and management deficiencies apparent in: (a) paper-based files of 186 access-related incidents held by the Health and Safety Executive (HSE) in the UK; and (b) computer-based files of 2,910 incidents appearing in the HSE database between 1997 to 2000. RESULTS: The more frequent root-causes included the fitting of defective components, unauthorized modification of the structure, omission of barriers, and errors resulting in simple, readily detectable structural faults. Common managerial deficiencies included failure to control risk, unsafe methods and procedures, and inadequate training and supervision. IMPACT ON INDUSTRY: SCAFPASS aims to avoid these root-causes by improving safety management from the outset and throughout all phases of a project.  相似文献

A failure of a Liquefied Natural Gas (LNG) tanker can occur due to collision or rupture in loading/unloading lines resulting in spillage of LNG on water. Upon release, a spreading liquid can form a pool with rapid vaporization leading to the formation of a flammable vapor cloud. Safety analysis for the protection of public and property involves the determination of consequences of such accidental releases. To address this complex pool spreading and vaporization phenomenon of LNG, an investigation is performed based on the experimental tests that were conducted by the Mary Kay O'Connor Process Safety Center (MKOPSC) in 2007. The 2007 tests are a part of medium-scale experiments carried out at the Brayton Fire Training Field (BFTF), College Station. The dataset represents a semi-continuous spill on water, where LNG is released on a confined area of water for a specified duration of time. The pool spreading and vaporization behavior are validated using empirical models, which involved determination of pool spreading parameters and vaporization rates with respect to time. Knowledge of the pool diameter, pool height and spreading rate are found to be important in calculating the vaporization rates of the liquid pool. The paper also presents a method to determine the vaporization mass flux of LNG using water temperature data that is recorded in the experiment. The vaporization rates are observed to be high initially and tend to decrease once the pool stopped spreading. The results of the analysis indicated that a vaporization mass flux that is varying with time is required for accurate determination of the vaporization rate. Based on the data analysis, sources of uncertainties in the experimental data were identified to arise from ice formation and vapor blocking.  相似文献

With the development of increasingly complex processes and technologies in chemical and manufacturing industries, Process Safety Management (PSM) has been globally recognized as the primary tool for operating companies to reduce process accidents on their industrial sites and the risks posed to their employees and surrounding communities. Yet, industrial facilities are often interdependent and collocated with others. Recognizing this, regional authorities are also applying PSM principles to reduce the cumulative incidents associated with high density industrial areas and the multiplicative risks posed to broader communities. This paper compares Strathcona County Emergency Service (SCES) in Alberta, Contra Costa County Health Service Hazard Material Programs (CCCHSHMP) in California, and Technical Standards & Safety Authority (TSSA) in Ontario and their PSM systems to provide practical recommendations to improve SCES's system. Four aspects of PSM are considered: regulation and guidance, auditing and inspection, annual performance indicators, and public participation. Based on the results of this comparison, we recommend that SCES develop comprehensive PSM regulations based on CSA Z767-17 PSM including clear instructions for assessing technologies and methodologies for consequence analysis. Both worst-case scenarios and alternative scenarios need to be considered as well as the domino effect of primary accidents. Furthermore, regular audits and inspections will ensure compliance with PSM regulations while helping the design of planning, performing, and following-up strategies to ensure effectiveness. In addition, we suggest the use of lagging and leading performance indicators to evaluate the performance of the PSM program. Finally, we recommend using advisory councils or commissions to increase public participation and ensure the representation of stakeholders' perspectives with the PSM system.  相似文献

Safety instrumented systems (SIS) are becoming increasingly complex, and form a growing proportion of programmable electronic parts. The IEC 61508 global standard was established to ensure the functional safety of SIS; however, it was expressed in highly macroscopic terms. The safety integrity level (SIL) is a criterion describing whether a component meets the safety requirements of a SIS. The safety requirements give a target SIL for the expected risks using hazard analysis and risk assessment (HARA). The SIL must correspond to the safety requirements. This study introduces an evaluation process for determining the hardware SIL through failure modes, effects, and diagnostic analysis (FMEDA). First, the components of the SIS subsystem are defined in terms of failure modes and effects, and then the failure rate and failure mechanism distribution are assigned to each component. The safety mode and detectability of each failure mode are determined for each component and, finally, the hardware SIL is evaluated. We perform a case study to evaluate the hardware SIL of the flame scanner system using HARA and FMEDA, where the safety requirement of the flame scanner was determined using the risk graph method. We verified that the hardware SIL of the flame scanner corresponded to the safety requirement.  相似文献

从1991年国际石油工程协会把健康安全与环境(简称HSE)作为一个完整概念提出以来,各国石油公司都进行了积极探索与实践,并逐步发展成国际石油届开展合作业务的基本运行规则.中国石油天然气集团公司结合企业生产实际和传统管理方法,在HSE管理体系探索、实践中,发展了一整套既符合中国石油企业管理实际的情况,又满足与国际接轨要求的HSE运行模式,对企业实施HSE管理体系有很好指导意义.  相似文献
