Establishing the relationship between level of safety climate and safety performance is a current challenge. This work examines the relationship between level of safety climate and orientation toward safety in the decision making process and choice. Alternatively, this work seeks to answer the question of whether level of safety climate can predict safety-oriented decision making. A generalized safety climate questionnaire and a decision making simulation are utilized to examine this relationship. The results indicate that level of safety climate is not a significant predictor of the decision process; however, it was found to be a significant predictor of the selection of safer choices.  相似文献

In production plants, monitoring and maintaining industrial processes and emergency shutdowns are not straightforward tasks due to the large number of events and alarms which are triggered during the plant shutdown process. It is also vitally important to provide decision support to stakeholders for efficient and effective monitoring and maintenance of production process. This paper presents a novel framework and design to enhance maintenance decisions based on the knowledge gathered through the process of monitoring. This monitoring process is based on signals which are triggered during the plant safety shutdown process. We have designed and implemented a framework using an ontology and business rules to define the logical structure and operation of the petroleum plant with the objective of monitoring the cause and effect of the petroleum plant shutdown process. To enhance maintenance decisions, we have extended the ontology and the framework to ensure that decision makers have sufficient information to make the right decision at the right time. The proposed extended framework is designed, implemented and evaluated using an example petroleum production plant as a case study.  相似文献

The overall objective of the maintenance process is to increase the profitability of the operation and optimize the total life cycle cost without compromising safety or environmental issues. Risk assessment integrates reliability with safety and environmental issues and therefore can be used as a decision tool for preventive maintenance planning. Maintenance planning based on risk analysis minimizes the probability of system failure and its consequences (related to safety, economic, and environment). It helps management in making correct decisions concerning investment in maintenance or related field. This will, in turn, result in better asset and capital utilization.

This paper presents a new methodology for risk-based maintenance. The proposed methodology is comprehensive and quantitative. It comprises three main modules: risk estimation module, risk evaluation module, and maintenance planning module. Details of the three modules are given. A case study, which exemplifies the use of methodology to a heating, ventilation and air-conditioning (HVAC) system, is also discussed.  相似文献

This paper contributes to the discussion on contingencies of process innovations by focusing on and introducing organizational‐level constructs of climate for initiative and psychological safety. We argue that process innovations, defined as deliberate and new organizational attempts to change production and service processes, need to be accompanied by climates that complement the adoption and implementation of such innovations. Our study of 47 mid‐sized German companies examines the relation between process innovations, climates for initiative and psychological safety, and firm performance. Results show that climates for initiative and psychological safety were positively related to two measures of firm performance—longitudinal change in return on assets (holding prior return on assets constant) and firm goal achievement—and moderated the relation between process innovations and firm performance. The U.S. National Institute for Occupational Safety and Health (NIOSH) conducts research to improve and protect the health and safety of workers. This paper describes the experience of the NIOSH Construction Program with two recent program planning initiatives intended to improve the program: (a) an independent external review of work over the past decade and (b) the development of strategic goals organized into a “National Construction Agenda” to guide a decade of future work. These goals, developed with input from construction industry stakeholders and researchers, are a part of the NIOSH National Occupational Research Agenda (NORA) initiative. The NORA goals are intended to provide an ambitious set of goals for all construction stakeholders to work together on. Both efforts relate to insuring the relevance and impact of research, reflecting an emerging policy perspective that research programs should be judged not just by the quality and quantity of science produced, but by the industry impact and tangible benefit resulting from the research. This paper describes how views on research planning have evolved to incorporate lessons learned about how research leads to improved safety and health for workers. It also describes the process used to develop the goals and the resulting strategic and intermediate goals that comprise the National Construction Agenda.  相似文献

This study set out to develop a composite road safety indicator for benchmarking countries’ road safety performance, which would combine the main layers of the road safety pyramid which describes the complex nature of road safety activities, performance and outcomes. Four groups of basic safety indicators were considered, which refer to: policy performance (road safety programmes), final road safety outcomes (fatality rates, scope of traffic injury), intermediate outcomes (wearing rates of seat belts, crashworthiness and composition of vehicle fleet, alcohol-impaired driving), and background characteristics of countries (motorization level, population density). The analysis used the data collected for 27 European countries. Weights based on statistical models were used to combine the basic indicators into a composite one. Principal Component Analysis and Common Factor Analysis weighting were examined. The composite indicators, estimated by several methods, enabled us to rank and group the countries according to their safety performance.The analysis revealed that the countries’ ranking based on the composite indicators is not necessarily similar to the traditional ranking of countries based on fatality rates only. Furthermore, it was observed that the indicators belonging to the final outcomes and intermediate outcomes are not uniform in their behaviour. Indicators which were found to be more consistent and influential and termed a ‘core set of basic indicators’ are recommended for future uses. The general conclusion is that the design of a composite road safety indicator in which relevant information from the different components of the road safety pyramid has been captured and weighted is realistic and meaningful. Such an indicator gives a more enriched picture of road safety than a ranking based only on fatality rates, which is the common practice at present. Grouping countries in this process is promising and seems to be preferable to simply ranking countries.  相似文献

Part 2 extends the analysis to show that it is possible to find the “permission point”, the value of (the coefficient of relative) risk-aversion, at which decisions to sanction environmental protection are most likely to be made. The mathematical model describes the process by which the decision maker varies his risk-aversion over a range of feasible values to find the risk-aversion that will give him the greatest desire to invest in the protection system under consideration. If he can find such a risk-aversion before losing discrimination (because the system is too expensive, given its performance), he will adopt it as his “permission point” and decide in favour of the expenditure. The permission point is, of course, bounded above by the point of indiscriminate decision. A maximum Risk Multiplier calculated at the point of indiscriminate decision may be applied to the protection expenditure at monetary break-even to give the maximum, rational outlay on protection. Moreover, it is possible to model how the average UK adult should take decisions on protection to maximise his utility. Different situations will call for different values of risk-aversion, which may explain why economists have come up with differing estimates of this parameter in the past. However, a central, average risk-aversion may be calculated for the average UK adult as 0.85, which is within 4% of the value, 0.82, found from the newly reported method based on a trade-off between income and future free time, and is consistent with several recent economic estimates. Worked examples assess how much an organisation should spend on a protection scheme to prevent accidents with very large environmental consequences.  相似文献

Personal initiative training is a promising way to increase entrepreneurial personal initiative, which is a key behavior for successful entrepreneurship. Although personal initiative training has been shown to promote personal initiative, little is known about how this proactive behavior can be maintained over time and what the consequences are. The training transfer literature suggests that training effects usually decline with time. It is not clear, however, which factors contribute to personal initiative maintenance and which benefits go along with it. In a randomized controlled field experiment with 912 microentrepreneurs in Lomé, Togo, we investigate the influence of need for cognition—a cognitive factor driving proactive behavior—on personal initiative maintenance after training. In addition, we examine the effect of need for cognition on the well‐being consequences of personal initiative maintenance. We show that people high in need for cognition tend to maintain posttraining personal initiative longer than those low in need for cognition. However, contrary to our predictions, need for cognition has no effect on the level of well‐being that results from personal initiative maintenance. Our findings contribute to a better understanding of personal initiative and its maintenance and could be used to increase training effectiveness.  相似文献

Modern process plants are complex engineering systems. While thorough reviews of system safeguards are performed, catastrophic events continue to occur, often unfolding in unforeseen ways. Success in process safety demands safe processes, and understanding rare, high consequence events is central to the traditional process safety approach. This philosophy is common to all high-hazard industries, offering the potential for sharing approaches, experience, and lessons learned. The problem, however, is that people (and organizations and entire industries) who fear failure (atychiphobia) sometimes obsess about failure so much that they miss opportunities to succeed.This paper examines selected risk management practices in the power generation and aerospace industries and how those practices have led to improved performance. Risk informed decision making (RIDM) has had widespread application in the nuclear and aerospace industries, and is undergoing enhancements to become a key framework for risk management. Additionally, rather than focusing on avoidance of loss, there are emerging approaches supporting achievement of success. This approach provides a more direct link of risk to business and operational objectives, but does challenge conventional risk approaches founded in a loss prevention-centric view. The paper reflects upon risk informed decision making and success modeling, and suggests how these methods may be applied in the field of process safety. Specific examples are drawn from the defense in depth approach from the nuclear power industry and mission success concepts developed for NASA.  相似文献

Turnaround maintenance (TAM) is a usual plant shutdown that is done by process industries to perform asset inspections, repairs, and overhauls. TAM engineering projects are needed on a periodic basis to optimize a plant's performance. Process industrial plants usually don't have enough in-house manpower to carry out needed TAM jobs; it is not uncommon for a process industrial plant to outsource its TAM manpower supply to external contractors. This will create a peculiar management challenge regarding how to assure orientation of safety practices for the newly hired labor. Although accidents during process plant shutdowns may have severe consequences, the safety management systems in place for many companies only cover normal operations and few explicitly address TAM projects. Usually, contractors are evaluated from a safety perspective after handling the project by monitoring safety indicators such as number of injuries, incidents, etc. This paper emphasizes safety attributes to be included in selecting contractors at the bidding stage to assure loss prevention and better safety orientation during TAM implementation. The paper starts with reviewing TAM literature and defining main needed safety attributes to prequalify contractors. It develops an analytic hierarchy process (AHP) model that is applied to rank TAM contractors' selection. A case study is presented for a petrochemical plant in Saudi Arabia to demonstrate the AHP model.  相似文献

A methodology for maintenance planning is developed which helps in improving the reliability of the components and safety performance in process facilities. This methodology helps design an optimum safety maintenance investment plan by integrating the optimization techniques and a fuzzy dynamic risk-based method. Intuitionistic Fuzzy Analytic Hierarchy Process (IFAHP) is applied to deal with uncertain data. The proposed approach employs multi-experts’ knowledge which helps to optimize the maintenance investments. A separator system in an offshore process facility platform is selected as a case study to demonstrate the application of the proposed methodology. A practical example in the separator system is surveyed and potential failures and Basic Events (BEs) are identified. Finally, a risk-based maintenance plan is provided for future safety investment analysis. The results indicate that the developed methodology estimates the risk more accurately, which enhances the reliability of future process operations.  相似文献

Safety at Finnish work places is mainly considered to be the task of a separate safety organization, e.g., a safety manager and safety delegates. However, it is the supervisor who is responsible for the daily safety in the departments. The Finnish paper industry has developed a supplementary system for the safety organization to carry out safety work in the factories. The monitoring and improving of the daily safety has been delegated to small groups in the production departments. These small groups consist of the supervisor and the safety representative (worker member) of the work area. Sometimes also a department manager or a member of the maintenance department may participate in the work group. A training program was designed to give basic knowledge and skills for the groups. The course has helped the participants to better outline their duties and activities within the safety field.  相似文献

航空装备维修安全是确保装备飞行安全的重要因素,为提高航空装备飞行的安全性,需要对航空装备维修安全进行准确评价。根据航空装备维修安全评价涉及的不确定性问题,采用模糊数学和熵理论,提出一种基于模糊贴近度和熵权的航空装备维修安全综合评价方法。在建立航空装备维修安全评价指标体系的基础上,采用熵确定各指标的权重值、加权平均算子确定综合评价集,利用非对称贴近度进行决策分析。通过对某型飞机调查数据的算例分析,确定航空装备维修安全的主要影响因素,得出该型飞机维修安全水平的评价值。  相似文献

Thirteen small groups were employed in an intervention program to improve housekeeping and safety conducted at a shipyard. Two main tasks were given to the small groups, which were formed specifically for the one-year program. First, the groups had to solve practical problems in housekeeping to eliminate the obstacles of good housekeeping. Then they had to establish new work practices for the maintenance of a good housekeeping level by using the elements of the behavior modification technique. The groups were responsible for all the actions taken in their own department. Comparative material was gathered about the process and the outcome of the program. The program was successful in improving housekeeping and safety at the shipyard. Differences were noted between the departments, however. In the best departments the small groups worked very actively and succeeded in involving the other personnel into the program. With careful design and coordination, the program using small groups can successfully be applied for improving safety performance in industry.  相似文献

PROBLEM: Falls represent a significant occupational hazard, particularly in industries with dynamic work environments. This paper describes rates of noncompliance with fall hazard prevention requirements, perceived safety climate and worker knowledge and beliefs, and the association between fall exposure and safety climate measures in commercial aircraft maintenance activities. METHODS: Walkthrough observations were conducted on aircraft mechanics at two participating facilities (Sites A and B) to ascertain the degree of noncompliance. Mechanics at each site completed questionnaires concerning fall hazard knowledge, personal safety beliefs, and safety climate. Questionnaire results were summarized into safety climate and belief scores by workgroup and site. Noncompliance rates observed during walkthroughs were compared to the climate-belief scores, and were expected to be inversely associated. RESULTS: Important differences were seen in fall safety performance between the sites. The study provided a characterization of aircraft maintenance fall hazards, and also demonstrated the effectiveness of an objective hazard assessment methodology. Noncompliance varied by height, equipment used, location of work on the aircraft, shift, and by safety system. DISCUSSION: Although the expected relationship between safety climate and noncompliance was seen for site-average climate scores, workgroups with higher safety climate scores had greater observed noncompliance within Site A. Overall, use of engineered safety systems had a significant impact on working safely, while safety beliefs and climate also contributed, though inconsistently. IMPACT ON INDUSTRY: The results of this study indicate that safety systems are very important in reducing noncompliance with fall protection requirements in aircraft maintenance facilities. Site-level fall safety compliance was found to be related to safety climate, although an unexpected relationship between compliance and safety climate was seen at the workgroup level within site. Finally, observed fall safety compliance was found to differ from self-reported compliance.  相似文献

以空管政事分离改革后民航空管质量安全监察人员胜任事业单位内部质量安全监察职责为目标,从教育、培训、技能和经验四个方面分析构成民航空管质量安全监察人员胜任能力的因素,并构建了胜任能力指标体系.运用网络分析法(ANP)构建了民航质量安全监察人员胜任能力的网络结构模型;利用三角模糊数(FUZZY)对网络结构模型中各个因素进行重要度比较,得到权重向量.鉴于评价指标之间存在的较强的关联性,利用非线性加权综合法,构建了民航空管质量安全监察人员胜任能力模型,该模型对评价指标中取值较小的指标反应灵敏,能够更好地体现不同监察人员之间胜任能力的差异.利用该模型对某空管单位来自于不同背景的5名监察人员进行评价,结果与上一年度该空管单位的绩效考核结果一致,证明了模型的合理性.  相似文献
