The continuing high frequency of occupational accidents in the Swedish food industry calls for new approaches to better understand the underlying factors. In the present study, 54 accidents involving hand injuries were investigated from the operators’ perspective, to explore the organisational preconditions. In-depth interviews were conducted with operators and their supervisors, and 24 of these interviews were analysed using the grounded theory method. The core category ‘safety as a process’ was identified encompassing the perception of the process of the accident at operative level and organisational preconditions that increased the risk of occupational accidents. These preconditions were open factors: deficiencies in technical/physical environment and work organisation; and concealed factors: insufficient communication and learning, a high level of responsibility in combination with low control, conflicting goals and a gap between procedures and practice. These preconditions lead to risk acceptance, resignation towards improved safety and normalisation of risk. Through the analysis a five-step hypothesis was empirically generated.  相似文献

Management in the field of environmental protection and risk prevention has evolved to the increasing participation of all stakeholders in the decision-making process. It certainly results from the development of the Information society and the global increase of knowledge of the population, combined with the concerns of the populations related to a sustainable development of our civilisation. Our ‘risk society’, following the big industrial disasters (Flixborough,Tchernobyl, Bhopal, Challenger, and more recently Toulouse), has also developed a cautious attitude towards the role of the expertise when it comes to assessing risks, along with a question of the ability of science to give definite answers.

This has lead in particular to the adoption of the Aarhus convention in 1998 and the evolution of several regulations in the developed countries. For example, in France the new law no. 2003–699 of 30 July 2003 about the ‘prevention of the technological and natural risks and to the compensation for the damages’ has introduced an important innovation into the process of technological risks prevention.

This law has enabled the involvement of the stakeholders in the decision-making process related to risk prevention and has urged the development of specific tools to deal with the complexity of risk management issues, in particular for those related to land-use planning.

As technical support to decision-makers in risk management from both public and private sectors, INERIS has played an important role for the evolution of the French risk management system.

This paper describes an analysis on the difficulty to control major accident hazards in an evolving context where the industrial systems becomes more and more complex and where the expectations of the civil society has increased. Then, the authors describe how an integrated vision for industrial risk management has emerged in France and is being implemented in a new law adopted after the Toulouse disaster.  相似文献

In 1997 a workshop was held on the subject of how to cope with accelerating technologies. These are industries such as aviation, air traffic control and biotechnology, amongst others, where the rate of technological development is significant. A number of papers were presented from a range of viewpoints and in a variety of different industrial contexts. The papers ranged from theoretical models to help understand the process of acceleration and its impact on organisational learning, to practical analyses of future potential risks in specific accelerating industries such as air traffic control. The aim of the papers and the workshop as a whole was to provide insight into the problems associated with accelerating technologies, and thereby derive measures to control or cope with such acceleration. The problems arising from acceleration, as predicted by the theoretical models and evidenced by experiences in accelerating industries (e.g. aviation) are manifold. Two examples of particular problems are unforeseen risks in an industry (a lack of forward vision), and a failure to learn adequately (i.e. in time) from incidents occurring in an organisation (a lack of constructive hindsight). There is also an incipient danger of society being driven by technology rather than being led by social needs. Even if each technology ultimately becomes ‘ultra-safe’, it will nevertheless have its own ‘event horizon’, limiting useful further progress. Each of the papers from the workshop is summarised and integrated into a three-part synopsis of the workshop. This covers the context of accelerating technologies, modelling their impacts, and deriving coping strategies. Four of the papers are included in their entirety as separate papers in their own right in this special issue of Safety Science.  相似文献

Despite the increased understanding of how accidents occur in the chemical process industry, today's safety measures and indicators do not prevent these accidents from occurring. Finding a way to better, proactively, identifying precursors to imminent safety risks, could help organizations to focus their limited resources. The protocol proposed and demonstrated in this paper shows a way to do this. Basis is to examine usually low consequence, high likelihood reoccurring disruptions or deviations in the process (e.g. defects, disturbances, anomalies) for their relevance to process safety and define those possibly leading to accidents as ‘precursors’. As a next step ineffective supervising and managerial control processes and their underlying latent conditions, causing the persistence of these precursors, are identified and it was shown how these cause safety barriers to become ineffective and to open the way to accidents. In conclusion, this paper demonstrates that the proposed 7-stage protocol developed to this end can explicitly and proactively indicate safety risks, and find the controlling latent conditions causing the trouble. It will help organizations to direct their resources to improve safety, which will include their control structure and their normal way of working and will yield higher efficiency as a side benefit.  相似文献

Process safety practices have undergone multiple refinements over the past few decades, but major accidents continue to occur. Most organizations strive to improve performance by strengthening existing methods or by adopting new and/or different approaches. Central to these continual improvement efforts is the practice of applying lessons learned as a means to drive out potential risk exposures. Often, lessons learned may be transferred from other industries; indeed, high-performing organizations regularly benchmark practices outside of their immediate industry.In pursuit of continual process safety improvement, this paper examines risk management practices in the Rail Industry, and explores how methods intended for managing passenger and public rail safety may be transferred to drive continual improvement of process safety. Rail safety has its roots in engineered safety solutions; modern practices have additionally embraced the human aspects of safety performance. A selection of approaches for rail safety assessment and risk management are described in three areas considered fundamental to safety management: management of systems, management of technology, and management of human elements. In light of these examples, the authors provide views regarding how the field of process safety management may leverage the rail experience.  相似文献

The European iNTeg-Risk project is a large-scale integrated project aimed at improving the management of emerging risks related to new technologies in European industry. The project aims to build a new management paradigm for emerging risks as a set of principles supported by a common language, agreed tools and methods, and key performance indicators, all integrated into a single framework. It is using a number of Emerging Risk Representative Applications (ERRAs), or case studies, to inform the development of the framework; one of which concerns the carbon capture and storage (CCS) process.This paper describes the iNTeg-Risk CCS ERRA. Relevant hazards and properties of carbon dioxide are described and the emerging risks from CCS are discussed. Three new tools have been developed or trialled within the ERRA. These are: the DyPASI methodology for taking account of atypical (not usually identified) events during hazard identification; a methodology for including the time dimension in a risk assessment; and life-cycle approaches for risk management and communication. For CCS, the risk assessment needs to include both short-term potential accidents from capture, transport or injection, as well as very long-term risks from storage. Knowledge gaps which are generic to emerging risks are also identified.  相似文献

In general, contexts of coal mining safety involve technology, administration and socioeconomic environment as well. This paper presents statistical analysis of China coal mine accidents in recent years and analyzes the reasons causing coal mining high risk from technical and socioeconomic viewpoints. Social and economic reform has been unleashing social, economical factors that are driving a fundamental transformation of new workplace safety problems and making China coal mining safety take on Chinese features. Compared with major state-owned and local state-owned coalmines, township and village coalmines are most dangerous coal mines with highest occupational risks. The incidence and death toll of ceiling accidents are higher than those of accidents such as gas, machinery, electricity, transportation, flood, and fire. New organizational risks appeared with rapid development of state-owned coal mines’ reorganization. Low skilled labors restrict both technical renovation and safety management. The Government has adopted a systematic arrangement to improve coal mining safety such as closing the township and village coalmines that cannot meet the standard of safety, reinforcing the supervision over coal mining safety, strengthening technological renovation and enhancing work safety input.  相似文献

The author presents a personal view that production of bulk chemicals and the attendant risks are being transferred from developed to developing nations. Some evidence is presented on the transfer of production. The transferred risk is increased because of the larger scale plants that are now built in locales that are less able to cope with the increased hazards. Bhopal was an example of an inherently unsafe plant, with major hazards that could have been avoided or drastically reduced by design. It behoves the industry to adopt the inherently safer philosophy and practice in the new plants that it builds, in order to minimise the opportunity for another accident like Bhopal and the threat to our industry that such an accident would pose.  相似文献

The nature of work has changed dramatically due to globalisation, migration, technological advances and the emergence of the knowledge-based economy. These changes have been accompanied by the increased prevalence of new and emerging types of risk to workers’ health and safety, such as psychosocial risks. This paper reviews the key regulatory and voluntary standards on occupational health and safety of relevance to the management of psychosocial risks in the workplace that are applicable to the European Union member states. The review indicates that regulatory standards set the minimum level of protection for workers and voluntary standards may further enable organisations to go beyond their legal obligations. However, the review also highlights that the terms ‘stress’ and ‘psychosocial risks’ are not mentioned explicitly in most pieces of legislation leading to lack of clarity on the terminology used. While voluntary standards seek to address this, very few provide specific guidance on the process of psychosocial risk management to enable organisations to manage psychosocial risks successfully. On the basis of the review, the case for the development of a voluntary standard for psychosocial risk management is presented. The paper further discusses the recent development of such a standard and its implications at the European level.  相似文献

A new methodology for quantitative risk assessment (QRA) integrated with dynamic simulation and accident simulation is proposed. The objective of this study is to discover inherent risks that are undetectable by conventional risk analysis methods based on steady-state conditions. The target process is the reactor section in the heavy oil desulfurization (HOD) process, which is likely to pose vast potential risks due to the high operating conditions of pressure and temperature. First, a dynamic simulation of a shut-down procedure was performed to observe the behavior of process variables using Aspen HYSYS V10, which is a commercially available process software. Based on the results of the dynamic simulation, several blind spots indicating a higher operating pressure than that in the steady-state simulation were identified. To assess the risks of the detected blind spots, a QRA was performed using the commercial software of SAFETI V8.22, which performs risk calculation based on consequence and frequency data. As a result of applying the proposed method to the HOD process, the risk assessment outcome was identified as intolerably risky unlike that of steady-state conditions, thereby indicating that dynamic simulations can serve as a method to spot inherent risks that are undetectable in steady-state conditions. In addition, mitigation procedures that reduce the risk of the process to a tolerable level are performed, thereby enabling a safer and more reliable process.  相似文献

The growing demand for natural gas has pushed oil and gas exploration to more isolated and previously untapped regions around the world where construction of LNG processing plants is not always a viable option. The development of FLNG will allow floating plants to be positioned in remote offshore areas and subsequently produce, liquefy, store and offload LNG in the one position. The offloading process from an FLNG platform to a gas tanker can be a high risk operation. It consists of LNG being transferred, in hostile environments, through loading arms or flexible cryogenic hoses into a carrier which then transports the LNG to onshore facilities. During the carrier's offloading process at onshore terminals, it again involves risk that may result in an accident such as collision, leakage and/or grounding. It is therefore critical to assess and monitor all risks associated with the offloading operation. This study is aimed at developing a novel methodology using Bayesian Network (BN) to conduct the dynamic safety analysis for the offloading process of an LNG carrier. It investigates different risk factors associated with LNG offloading procedures in order to predict the probability of undesirable accidents. Dynamic failure assessment using Bayesian theory can estimate the likelihood of the occurrence of an event. It can also estimate the failure probability of the safety system and thereby develop a dynamic failure assessment tool for the offloading process at a particular FLNG plant. The main objectives of this paper are: to understand the LNG offloading process, to identify hazardous events during offloading operation, and to perform failure analysis (modelling) of critical accidents and/or events. Most importantly, it is to evaluate and compare risks. A sensitivity analysis has been performed to validate the risk models and to study the behaviour of the most influential factors. The results have indicated that collision is the most probable accident to occur during the offloading process of an LNG carrier at berth, which may have catastrophic consequences.  相似文献

International companies investing in emerging markets need to address the management of safety under varying conditions. The paper presents an evaluation of the approaches taken by an international hydropower company in two different construction projects, one in India and one in the Philippines. In the first project, the company had to intervene as a reaction to poor safety and overall performance. Based on previous experiences, the company selected a proactive approach in the second project, involving use of the contracting process to ensure adequate conditions from the start of construction and onwards. The paper accounts for the safety performance results in the two projects and interprets them in relationship to external and internal influencing factors in the respective project. It is concluded that a proactive approach has the potential of delivering satisfactory safety results at moderate costs for follow-up. A reactive approach risks exposing the company to ethical and reputation risks due to a poor safety performance before the corrective actions have taken full effect. It is also resource demanding. Results demonstrate the feasibility of turning around a project’s safety performance also when preconditions are challenging.  相似文献

Laboratory hydrogen generators, medical oxygen, and micro-breweries are examples of modular and micro technologies that are commercial successes. Researchers, patients, and unskilled workers operate these facilities but more complex processes require highly qualified personnel to ensure they operate safely. Modular-micro processes in isolated locations meet economic objectives when operated remotely thereby minimizing labor costs. Mitigating the risk requires a comprehensive hazards analysis with advanced control systems particularly for explosive and toxic compounds. Here, we propose a method called Failure Mode Risk Decision (FMRD) to review the inherent hazards of a micro-refinery unit (MRU) that converts flared and wasted natural gas to long chain hydrocarbons. This approach combines the Process Flow Failure Mode (PFFM) methodology as a systematic and reliable technique with a novel numerical risk assessment to improve the analytical evaluation of hazardous conditions. The objective is to combine causes and consequences in a single metric, where scaled probability of evident causes and severity of consequences are used to derive a risk level measure. With the proposed metric, the magnitude of a potential hazard is directly correlated with the risk level. This mechanism identifies extra risk scenarios compared to the classical hazard analysis method and provides a straightforward comprehensive numerical assessment to represent the inherent and residual risks to facilitate justifying the hazardous scenarios. Accordingly, we design a safety loop and supply all the required facilities to remove the potential risks at the process plant. Not only the proposed methodology clarifies the risks of the MRU presented in this study, but can be extended to review the hazards of other chemical process plants.  相似文献

The occupational risk model (ORM) developed by the Dutch workgroup occupational risk model WORM has been transferred to a Danish context, with the aim of creating a more simple system, particularly for SMEs. The ORM identifies the activities in a person’s daily work that contribute most to the person’s risk and also identifies which conditions need to be changed in order to reduce that risk. Our investigation seeks to determine whether we can use the ORM method to collect information about risks in SMEs and, if so, whether we can present this information in a way that allows SMEs to use it constructively. Finally, we seek to evaluate the impact of this method on occupational safety in SMEs, as the project also focuses on management factors that can motivate the SMEs to heighten their risk-awareness and expand their own initiatives. The present paper describes the methodological approaches applied and some of the preliminary findings obtained during field observations.  相似文献

To simulate the heat transfer process between lithium-ion batteries (LIBs), an electric heater with the same size and shape as LIB in this work is used to trigger thermal runaway event. The effect of state of charge (SOC), the power of heater, the cell spacing on thermal behavior of LIB was investigated as well the amount of transferred heat between the heater and LIB was calculated. The results indicate that 50% SOC is an unstable state for LIB, that a stronger jet flame becomes more likely when the SOC of LIB is higher than 50%. Additionally, the increased spacing, lower heating power and SOC can contribute to mitigate the severity of thermal runaway behavior. Further, the dominant path of heat transfer between the heater and LIB will also vary with operating conditions. The heat conduction through air is the main heat transfer path in tests with lower heating power. However, heat radiation will replace heat conduction as the primary heat transfer mode when there is a large temperature difference between the heater and LIB in tests with higher heating power. Understanding the leading heat transfer path between LIBs can provide valuable guidelines for the safety design of lithium-ion battery modules.  相似文献

箱体零件是矿井机械中重要的基础零件。它的加工质量直接关系矿井设备的寿命、机器的性能和安全性。针对煤矿井下煤尘和瓦斯存在的环境特性、煤矿井用箱体防爆面加工的特殊要求,采用控制理论的信息流图,对工艺过程中传统方块图进行简化,使繁杂的方块图解采用信息流图表示工艺过程的工序(变量)之间关系。进而讨论箱体零件加工工艺过程误差转换模型、形状精度特征、传递图、描述方程、误差转换结构图;提出防爆面磨削工艺系统的数学模型;求解出振动特性、幅值和相位,得出了形状误差与工艺参数的关系曲线。实践证明,合理选择磨削参数,能够保证煤矿井下工作机械具有高的防爆性能,达到矿井安全生产的技术要求。  相似文献

In attitudinal studies safety often appears as an important attribute desired by consumers when buying a new car (Ben-Akiva and Lerman, 1985). However, economic models of vehicle choice usually neglect the role of safety. On the one hand, to capture the qualitative nature of safety and variables related to safety one should consider safety as an underlying construct in a context of latent variable models. The problem is that psychometric models that make use of latent variables do not necessarily provide a complete understanding of agent behavior and may lead to poor predictive power. On the other hand, discrete choice models - although a powerful tool to explain decision making based on utility maximization behavior - fail to include qualitative factors as explanatory variables of the decision process.In this paper, we explore how to model safety through a new generation of discrete choice models which simultaneously consider both a standard discrete choice model and latent causal variables. Using stated preference data concerning purchase intentions of low-emission vehicles in Canada, we test a hybrid choice model to explain consumers’ preferences for safety. Based on the results as well as on the hybrid choice modeling approach, we outline a general framework for the correct modeling of the adoption of safer vehicles and appreciation of safety equipment.  相似文献
